4. Selektivnost doktora medicine i doktora dentalne medicine u kontaktu s pacijentima na društvenim mrežama

Profesionalizam liječnika temelji se na deset načela, pri čemu se jedno od načela odnosi na pravednu distribuciju resursa. E-profesionalizam definiran je kao primjena načela „tradicionalnog“ profesionalizma u online aktivnostima, stoga se načelo pravednosti distribucije resursa odnosi i na resurse doktora na društvenim mrežama (DM). Osobno vrijeme doktora je također resurs, a ako doktor bira s kojim pacijentima će komunicirati na DM, a s kojima neće, to otvara problem nejednakosti u distribuciji resursa. Cilj ovog izlaganja je istražiti postoji li selektivnost liječnika/doktora dentalne medicine pri kontaktu s pacijentima na DM, te koji prediktori tumače takvo ponašanje. Istraživanje je provedeno 2021. godine, metodom ankete na neprobabilističkom prigodnom uzorku doktora medicine i doktora dentalne medicine u Hrvatskoj. Ukupno je prikupljeno 1013 odgovora, od čega 753 koriste DM, a njih 489 prihvatilo je odgovoriti na pitanje selektivnosti u komunikaciji s pacijentima, što je uzorak od interesa u ovom istraživanju. Rezultati pokazuju da 42, 9% ispitanika bira s kojim pacijentima će stupiti u kontakt na DM. Binarna logistička regresija pokazala je da doktori koji rade u privatnom sektoru imaju 2, 5 puta veću šansu da će birati pacijente s kojima će komunicirati na DM, doktori dentalne medicine imaju 2, 3 puta veću šansu da da će raditi taj odabir u odnosu na doktore medicine, te oni koji imaju pozitivniji stav prema etičnosti korištenja DM u komunikaciji s pacijentima imaju 1, 3 puta veću šansu da će napraviti odabir. Ovi rezultati otvaraju raspravu o utjecaju DM na tumačenje postojećih načela profesionalizma i pokazuje potrebu za istraživanjem i edukacijom e-profesionalizma.

All of these urge for new and novel conceptualizations of medial professionalism, those built around and within digital landscapes, such as those currently commonly encompassed within the notion of e-professionalism.

Uvod: Društvene mreže (DM) mogu se definirati kao online mediji i sučelja osmišljena za povezivanje ljudi te olakšavanje interakcije i razmjene sadržaja među njima. Razvoj društvenih mreža utječe na sve aspekte društva u cjelini, a zdravstveni djelatnici i zdravstvene institucije nisu izuzetak. Interakcija između zdravstvenih djelatnika i pacijenata neizbježna je putem društvenih mreža gdje se profesionalne i osobne granice zamagljuju i potencijalno prelaze. Tradicionalni koncepti profesionalizma u medicini zahtijevaju od zdravstvenih djelatnika ponašanje u skladu s profesionalnim i etičkim standardima profesije prema pacijentu, zdravstvenom osoblju i društvu u cjelini. Pojavom društvenih mreža definiran je termin e-profesionalizma zdravstvenih djelatnika kao stavovi i ponašanja koji odražavaju tradicionalne paradigme profesionalizma zdravstvenih djelatnika manifestirane kroz digitalne medije. Cilj: Cilj ovog istraživanja bio je istražiti razlike u stavu o e-profesionalizmu između doktora medicine i doktora dentalne medicine u Republici Hrvatskoj. Metode: Kvantitativno presječno istraživanje provedeno je korištenjem online upitnika na prigodnom neprobabilističkom uzorku doktora medicine i doktora dentalne medicine članova Hrvatske liječničke komore i Hrvatske komore dentalne medicine. U ovom radu su prikazani podaci jednog od sedam instrumenata korištenih u upitniku koji se sastoji od 16 čestica, a mjeri stav o e- profesionalizmu. Za statističku analizu razlika u stavovima o e-profesionalizmu korišten je Mann- Whitney U test. 7. Utjecaj edukacijske intervencije na izmjenu postavki privatnosti profila na društvenim mrežama studenata medicine i dentalne medicine

UVOD Studenti medicinskih profesija učestali su korisnici društvenih mreža (DM). Cilj ovoga rada bio je istražiti učestalost i razloge zbog kojih studenti medicine Medicinskog fakulteta i dentalne medicine Stomatološkog fakulteta Sveučilišta u Zagrebu, polaznici jednosemestralnih izbornih/elektivnih predmeta, izmjenjuju postavke privatnosti svojih profila na DM te utjecaj edukacijske intervencije na promjenu postavki privatnosti profila na DM. MATERIJALI I METODE Istraživanje „pred-test post-test zatečenih grupa“ provedeno je korištenjem online upitnika na uzorku studenata medicine i dentalne medicine na početku i na kraju izvođenja nastave (T1 i T2). Upitnik je sadržavao instrument o postavkama privatnosti. Deskriptivna statistika i McNemar test provedeni su prilikom obrade podataka. REZULTATI Oba puta upitnik je ispunilo 53 studenata (16 studenata medicine i 37 studenata dentalne medicine). Od svih ispitanika, samo jedan nije koristio DM, te je 52 ušlo u finalni uzorak. Nakon odslušane nastave statistički značajno više studenata izmijenilo je postavke privatnosti na DM koju koriste, najviše u odnosu na one koji to nisu učinili ili ne znaju jesu li to učinili (T1 61, 5 % vs. T2 84, 6 %, p=.012). Od 32 studenta koji su odgovori pozitivno na pitanje o promjeni postavki, ne postoji statistički značajna razlika u razlozima promjene postavki privatnosti. Najčešće su studenti mijenjali postavke privatnosti zbog „zaštite osobnih podataka od nepoznatih osoba” te zbog “nepovjerenja u sigurnost početnih postavki privatnosti društvenih mreža”. Iako nema statistički značajne razlike , u česticama „zbog savjeta kolega i prijatelja“ (T1 15, 6 % vs. T2 28, 1 %, p=.289) i „rizika da bi njihov profil mogao vidjeti trenutni ili budući poslodavac“ (T1 21, 9 % vs. T2 43, 8 %, p=.289) vidi se porast nakon nastave. ZAKLJUČAK Edukacijska intervencija je imala statistički značaj utjecaj na aktivnost promjene postavki privatnosti profila na DM. Najzastupljeniji razlozi promjene bili su nepovjerenje u sigurnost početnih postavki te zaštita osobnih podataka od nepoznatih osoba. 8. Attitudes of doctors of medicine and dental medicine towards contacting patients via social networks

Background As the use of social networking sites (SNSs) has greatly increased among health professionals, it is necessary to investigate their use of SNSs. The aim of this study was to identify the patterns of SNS use for contact with patients between medical doctors (MD) and doctors of dental medicine (DMD) in Croatia. Methods In collaboration with the Croatian Medical Chamber and the Croatian Chamber of Dental Medicine, a quantitative cross-sectional study was conducted on the use of SNSs and the attitudes of MDs and DMDs towards e-professionalism. Data were collected using online questionnaires. The data were analyzed using descriptive statistics. Results A total of 753 responses were processed, 507 (67,3%) MDs and 246 (32,7%) DMDs. DMDs were significantly more likely to visit patients or their family members’ profiles on SNSs (60,6% vs 33,3%, P < 0.001). The main reason for visiting a patient’s or family member’s profile is social communication, which is significantly more common among DMDs (46,3% vs 29,0%, P < 0.002). DMDs are significantly more likely to send friend requests from private SNS profiles to patients or their family members (15,0% DMDs vs 3,4% MDs, P < 0.001). Patients are much more likely to send friendship and connection requests on SNS to DMDs (91,1% DMDs vs 62,7% MDs, P < 0.001) who are also significantly more likely to accept these requests (76,8% DMDs vs 41,5% MDs, P < 0.001). MDs and DMDs would find the presence of e-professionalism guidelines useful (72,2% MDS vs 78,0% DMDs, P = 0.103). Conclusions The results show that DMDs are more open to communicating with patients via SNSs. Results showed a difference between MDs and DMDs in both directions - in sending requests to patients or their family members and in positive responses to their requests for connection. The findings point to the need to develop guidelines for health professionals regarding e-professionalism. Key messages • There are differences between MDs and DMDs toward communicating with patients using SNSs, DMDs being more open to the communication with patients. • There is a need to develop guidelines for health professionals on e-professionalism with emphasis regarding how to professionally communicate with patients on SNSs.

10. Differences between doctors of medicine and dental medicine in the perception of professionalism on social networking sites: the development of the e-professionalism assessment compatibility index (ePACI)

Background Social networking sites (SNSs) have penetrated all aspects of health care professionals’ (HCPs’) professional and private lives. A new term, e-professionalism, has emerged, which describes the linking of traditional values with this new dynamic online environment for HCPs. The four aims of this study were: (1) to examine their SNS prevalence and usage habits, (2) to examine their perception of e-professionalism, (3) to develop an e-professionalism assessment compatibility index and (4) to investigate their tendencies and differences in values of the e-professionalism assessment compatibility index (ePACI). Methods A cross-sectional study was conducted among MDs and DMDs in Croatia via email using a questionnaire. The questionnaire was distributed to all MDs and DMDs who were members of their respective chambers. In addition to descriptive statistics, Chi-square or Fisher’s exact test when appropriate, t-test, ANOVA and Mann–Whitney U tests were used to determine differences between groups. A principal component analysis (PCA) with varimax rotation was used to investigate dimensionality. Results of the PCA were compared to the coding based on the Social Media e-Professionalism rubric in order to create the ePACI. Results Of the 1013 gathered responses, 753 were from valid SNS users and suitable for further analysis. Facebook (91.6%) and Instagram (63.1%) were the predominant SNSs used. Both groups have a good understanding of e-professionalism. The newly developed ePACI deviates slightly in the “conservative” direction in the cases of both, MDs (t506 = 19.033, p 245 = 12.991, p p s = −0.142, p p Conclusions MDs and DMDs in Croatia are frequent SNS users, with Facebook and Instagram being the main SNSs used. Both groups have a good understanding of e-professionalism. The newly developed ePACI deviates slightly towards the conservative side regarding perception of the e-professionalism content for both groups. The development of the ePACI, and its subsequent usage in further research, will have a direct influence in its validation, gathering comparable data, and be able to direct efforts in oversight, regulation and education.

Uvod Društvene mreže (DM) zadiru u privatnost pojedinca i ostavljaju opasnost miješanja privatnog i profesionalnog. Održavanje postavka privatnosti profila te više profila pod raznim imenima omogućava višu razinu zaštite i privatnosti. Cilj ovog rada bio je istražiti razlike u vrsti profila doktora medicine (MD) i doktora dentalne medicine (DMD) na Facebooku i Instagramu. Materijali i metode Presječno istraživanje provedeno je korištenjem online upitnika na prigodnom neprobabilističkom uzorku svih MD i DMD članova Hrvatske liječničke komore i Hrvatske komore dentalne medicine. Deskriptivna statistika i hi kvadrat test provedeni su prilikom obrade podataka. Rezultati Uzorak se sastojao od 753 ispitanika (246 DMD i 507 MD) koji koriste barem jednu DM. Na obje DM i MD i DMD imali su predominantno samo jedan profil s pravim imenom (Facebook MD 84.9% vs. DMD 79%, Instagram MD 74.1% vs DMD 85%). Statistički značajna razlika između dvije skupine postoji na Facebooku gdje 17.6% DMD ima i privatan i profesionalan profil za razliku od 5.5% MD, te značajno više MD (9%) ima samo jedan profil pod drugačijim imenom za razliku od samo 2.5% DMD (x2=34.432, df=3, P

16. E-profesionalizam zdravstvenih djelatnika – kako pacijenti prate online reputaciju liječnika

Online reputacija postaje najvažniji izvor informacija o pojedinim osobama i prije prvog susreta. Liječnici se tu ne izdvajaju, ali zbog specifičnosti posla, zaštite ugleda profesije i čuvanja liječničke tajne pod posebnim su povećalom. U sklopu uspostavnog istraživačkog projekta HRZZ „Opasnosti i prednosti društvenih mreža: E- profesionalizam zdravstvenih djelatnika“ provedeno je kvalitativno istraživanje primjenom metode fokus grupa. Ukupno su s općom populacijom/pacijentima provedene četiri fokus grupe u razdoblju od 20. travnja do 2. svibnja 2022. Provedba je fokus grupa uključivala grupnu raspravu i konsenzualno grupiranje potencijalno neprihvatljivog profesionalnog ponašanja prikazanih fotografijama prema unaprijed utvrđenim kategorijama. Svi su ispitanici punoljetni građani RH, korisnici društvenih mreža (DM), većinom pasivni promatrači koji ne kreiraju sadržaj. DM koriste u poslovne svrhe – umrežavanje, pronalazak klijenata, informiranje o struci ili reklamiranje. Svi koriste DM pod svojim stvarnim imenima i svjesni su echo chamber efekta DM. O liječnicima se informiraju na internetskim stranicama, a vrlo rijetko na DM – samo ako postoji službeni profil ordinacije/institucije ili poslovni profil liječnika. Informacije koje ih zanimaju su: iskustva drugih pacijenata, usluge koje liječnik nudi, životopis i fotografija liječnika, njegova profesionalna dostignuća (napredak u karijeri, sudjelovanja na konferencijama, tečajevi, edukacije) te nova oprema u ordinacijama.. Sadržaj koji je dobrodošao, ali nema presudnu ulogu u donošenju odluke o odabiru liječnika su savjeti iz struke, znanstveni radovi, preporuke znanstvenih časopisa i materijala. Većina pacijenata smatra da na poslovnom profilu liječnika ne smije biti sadržaja privatnog karaktera. Manjem dijelu prihvatljive su i objave privatnog sadržaja, ali isključivo u poslovnom kontekstu. Fotografije ne/provokativnog sadržaja na DM ne utječu na dojam o profesionalnosti liječnika ili ordinacije ako nije riječ o sadržaju koji je povezan sa zloupotrebom droga, uvredljivog političkog, vjerskom ili rasnog karaktera ili objavama koje su uvredljive prema kolegama iz struke ili pacijentima. Liječnika vide kao intelektualca, odnosno nositelja kulture u društvu i smatraju kako liječnici ne bi trebali komentirati društveno političke događaje. Treba postojati jasna distinkcija privatnih i profesionalnih profila liječnika. Pacijenti imaju očite razlike u očekivanjima od prisutnosti liječnika na društvenim mrežama s obzirom na to radi li u liječnik u javnom ili privatnom sektoru.

U okviru UIP-2017-05-2140 HRZZ projekta Opasnosti i pred nosti društvenih mreža: e-profesionalizam zdravstvenih djelat nika (SMePROF projekt) provedeno je kvantitativno presječno istraživanje o uporabi društvenih mreža, stavovima i etičkim vrijednostima doktora medicine i dentalne medicine u Hrvat skoj u svezi s e-profesionalizmom. Istraživanje je provedeno u partnerstvu s Hrvatskom liječničkom komorom i Hrvatskom komorom dentalne medicine. Javna prezentacija rezultata istraživanja projekta SMePROF održana je 18. svibnja 2022. godine na Školi narodnog zdravlja „Andrija Štampar“ Medicinskog fakulteta Sveučilišta u Zagrebu, kao hibridni događaj.

Uvod: Privatni i profesionalni život isprepleću se na društvenim mrežama (DM). Pove ćanje vlastite i percepcije javnosti prema razini e-profesionalizma doktora dentalne me dicine (DMD) i medicine (MD), shvaćanje i kontrola privatnosti na DM je važna. Cilj ovog rada bio je istražiti stavove i postupke prema vlastitoj razini privatnosti na društve nim mrežama DMD i MD. Materijali i metode: Provedeno je presječno istraživanje korištenjem online upitnika na prigodnom uzorku svih liječnika MD i DMD u Hrvatskoj. Za obradu podataka korištena je deskriptivna statistika i hi kvadrat test. Rezultati: Od 1013 ukupno prikupljenih odgovora, u konačni uzorak ušlo je ukupno 753 ispitanika (246 DMD i 507 MD) koji koriste barem jednu DM. Facebook je DM u ko joj se obje skupine najbolje snalaze, dok za Instagram, YouTube, LinkedIn i Twitter posto je statistički značajne razlike između skupina. Obje skupine su najliberalnije u „prihvaća nju prijateljstava“ na LinkedIn-u. DMD su u toj kategoriji statistički značajno liberalniji od MD na LinkedInu i na Facebooku. Postavke privatnosti izmijenilo je 69, 9% DMD i 66, 9% MD, a najčešći razlog bio je „Zaštita mojih osobnih podataka od nepoznatih oso ba“ i „Nisam vjerovao u sigurnost početnih postavki privatnosti DM“. Statistički značaj no više MD je bilo zabrinuto zbog „Rizika da bi moj profil mogao vidjeti trenutni ili bu dući poslodavac“ (29, 2% vs 19, 8%, P=0, 028). Zabrinjavajuće je da u obje skupine nešto manje od trećine ispitanika nisu ili ne znaju jesu li promijenili osnovne postavke privat nosti na DM. Zaključak: DMD i MD pokazuju zabrinutost i spremnost za kontrolu vlastite privatnosti na DM. Potrebna je daljnja edukacija o načinima kontrole privatnosti na DMD.

Introduction: The digital revolution has had a profound impact on medicine and patient care. As the use of social media (SM) has greatly increased among health professionals, it is necessary to investigate their use of SM. New standards are forming in the context of e-professionalism. The term “eprofessionalism” is defined as “attitudes and behaviors (some of which may occur in private settings) reflecting traditional professionalism paradigms that are manifested through digital media”. The development of technology, novel communication, and social networking can positively or negatively affect the therapeutic alliance between patients and psychiatrists. The objectives of this study were to assess the level of and to characterize e- professionalism on publicly available Facebook (FB) profiles of psychiatrist in Croatia. Methods: Content analysis of FB profiles of psychiatrist was performed. A search was performed via a new FB account using a publicly available Directory of physicians in Croatia, available at the web-site of the Croatian Chamber of Physicians. All psychiatrists listed in the directory were included in the analysis. Each profile was subsequently assessed with regard to e-professionalism based on previously published criteria and compared using the SMePROF coding scheme. Results and discussion: The sample of psychiatrist's list of names included 175 names. FB profiles were identified for 30.9% (54/175) psychiatrists. Displayed relationship status was found for 13 % (7/54). Among psychiatrists with identifiable FB accounts, potentially unprofessional content was observed for 5.6% (3/54). None of the accounts had any unprofessional content. The level of e- professionalism on Facebook profiles of Croatian psychiatrists available for public viewing has shown a high level of understanding of e- professionalism. Uvod: Porastom korištenja društvenih mreža (DM) među općom populacijom postoji i porast u korištenju među nastavnicima u zdravstvu. Cilj ovog istraživanja bio je istražiti navike korištenja DM i razlike u stavovima o e- profesionalizmu među nastavnicima Medicinskog i Stomatološkog fakulteta. Materijali i metode: Provedeno je presječno istraživanje korištenjem online upitnika za nastavnike Medicinskog i Stomatološkog fakulteta Sveučilišta u Zagrebu u ak. god. 2018./2019. Za obradu podataka korištena je deskriptivna statistika. Rezultati: U završni uzorak ušlo je 93 odgovora: 68 s Medicinskog (RR 13.6%) i 25 sa Stomatološkog fakulteta (RR 10.8%) Facebook je najšire korišten (82.2%) a na drugom mjestu LinkedIn (77.4%). Instagram je statistički značajno češće korišten od nastavnika Stomatološkog (64% stom vs. 39.7% med, P=0.037) a Twitter od nastavnika Medicinskog (23.5% med vs. 4% stom, P=0.035) fakulteta. YouTube (65.6%) je najčešće korišten u edukacijske svrhe. Većina nastavnika oba fakulteta ne bi prihvatilo (88.2%) niti bi poslalo (97.8%) zahtjev za prijateljstvo pacijentu. Percepcija neprofesionalnog ponašanja na značajno se razlikovala u stavovima prema postavljanju slika pacijenata (97.1% med vs 80% stom P= 0.006), dok su obje skupina neprofesionalnim smatrali postavljanjem povjerljivih informacija (94.6%), kriminalnih aktivnosti (90.3%), korištenje alkohola (82.8%) i droge (89.2%) te eksplicitni seksualni sadržaj (88.2%). Većina nastavnika (84.8%) smatra dobrodošlim postojanje smjernica o e- profesionalizmu na DM. Zaključak: Facebook i LinkedIn najčešće su korištene DM. Nastavnici Medicinskog fakulteta osjetljiviji su na postavljanje fotografija pacijenata. Potrebno je razviti smjernice o e-profesionalizmu na DM.

Uvod: Cilj ovog istraživanja bio je procijeniti i usporediti razinu online profesionalizma na Facebook profilima dostupnim za javno gledanje među studentima i nastavnicima Medicinskog (MEF) i Stomatološkog (SFZG) fakulteta Sveučilišta u Zagrebu. Materijali i metode: Konačni uzorci za analizu sadržaja Facebook profila izrađeni su slučajnim uzorkom od 16, 7% svih nastavnika i studenata sa oba fakulteta. Pretraživanje je izvršeno putem novog Facebook računa, koji nije povezan ni s jednim drugim profilom. Profili su kategorizirani kao "privatni" ili "javni". Svaki je profil naknadno bodovan s obzirom na profesionalnost na temelju prethodno objavljene sheme kodiranja. Rezultati: Konačni uzorak studenata činilo je 325 od 1951 (16, 7%) studenata MEF-a i 94 od 565 (16, 7%) studenata SFZG-a (n = 419). Konačni uzorak nastavnika činilo je 86 od 516 (16, 7%) nastavnika MEF-a i 28 od 166 (16, 8%) SFZG-a (n = 114). - Abstract
Aim To describe and compare social media (SM) use hab - its, and attitudes of medical and dental students toward e-professionalism and to determine their opinion on po - tentially unprofessional behavior and posts. Methods In this quantitative cross-sectional question - naire study, students of the University of Zagreb School of Medicine and those of the School of Dental Medicine com - pleted a survey-specific questionnaire on the use of SM, SM habits, and attitudes toward e-professionalism. Results Of the 714 collected questionnaires, we analyzed 698 (411 from medical and 287 from dental students). The most commonly used SM were Facebook (99%) and Ins - tagram (80.7%). Unprofessional content was recognized by both groups. Medical students significantly more fre - quently considered the posts containing patient pho - tos (61% vs 89.8%; P <0.001), describing interaction with a patient not revealing any personal identifiable informa - tion (23% vs 41.8%; P <0.001), and containing critical com - ments about faculty (53% vs 39.7%; P =0.001) to be un - professional. Dental medicine students were significantly more open to communication through SM (39.7% vs 16.3%; P <0.001), more often reported that they would ac - cept (41.5% vs 12.2%; P <0.001), and had accepted (28.2% vs 5.6%; P <0.001) friend requests/follows/tracks from pa - tients, and sent friend requests/follows/tracks to their pa - tients (5.2% vs 1.2%; P =0.002). Conclusion Both groups were highly aware of e-profes - sionalism. Dental students were more desensitized to vi - sual representations of patients, and more prone to SM in - teractions with patients, which might expose them to the risk of unprofessional behavior.

Aim: The aim of this study was to examine the use of the Internet among psychiatric patients at the University Psychiatric Hospital Vrapče diagnosed with depression and psychotic spectrum disorders. Methods: A survey was conducted between December 2018 and February 2020 at the University Psychiatric Hospital Vrapče among adult inpatients and day hospital patients treated under primary diagnoses of Depressive episode and Recurrent depressive disorder (F32 and F33), and Schizophrenia, schizotypal and delusional disorders (F20-F29), according to the ICD-10 manual. Background: As we are witnessing the evolution of social media (SM) use worldwide among the general population, the popularity of SM has also been embraced by health care professionals (HCPs). In the context of SM evolution and exponential growth of users, this scoping review summarizes recent findings of the e-professionalism of HCPs.Objective: The purpose of this scoping review is to characterize the recent original peer-reviewed research studies published between November 1, 2014, to December 31, 2020, on e-professionalism of HCPs; to assess the quality of the methodologies and approaches used; to explore the impact of SM on e-professionalism of HCPs; to recognize the benefits and dangers of SM; and to provide insights to guide future research in this area.Methods: A search of the literature published from November 1, 2014, to December 31, 2020, was performed in January 2021 using 3 databases (PubMed, CINAHL, and Scopus). The searches were conducted using the following defined search terms: "professionalism" AND "social media" OR "social networks" OR "Internet" OR "Facebook" OR "Twitter" OR "Instagram" OR "TikTok." The search strategy was limited to studies published in English. This scoping review follows the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews) guidelines.Results: Of the 1632 retrieved papers, a total of 88 studies were finally included in this review. Overall, the quality of the studies was satisfactory. Participants in the reviewed studies were from diverse health care professions. Medical health professionals were involved in about three-quarters of the studies. Three key benefits of SM on e-professionalism of HCPs were identified: (1) professional networking and collaboration, (2) professional education and training, and (3) patient education and health promotion. For the selected studies, there were five recognized dangers of SM on e-professionalism of HCPs: (1) loosening accountability, (2) compromising confidentiality, (3) blurred professional boundaries, (4) depiction of unprofessional behavior, and (5) legal issues and disciplinary consequences. This scoping review also recognizes recommendations for changes in educational curricula regarding e-professionalism as opportunities for improvement and barriers that influence HCPs use of SM in the context of e-professionalism.Conclusions: Findings in the reviewed studies indicate the existence of both benefits and dangers of SM on e-professionalism of HCPs. Even though there are some barriers recognized, this review has highlighted existing recommendations for including e-professionalism in the educational curricula of HCPs. Based on all evidence provided, this review provided new insights and guides for future research on this area. There is a clear need for robust research to investigate new emerging SM platforms, the efficiency of guidelines and educational interventions, and the specifics of each profession regarding their SM potential and use. Simpozij „Mediji i zdravlje“ održava se već 17 godina te okuplja glavne dionike u zdravstvu. Ove je godine na simpoziju sudjelovalo 80 zdravstvenih profesionalaca i novinara. Skup je počeo izlaganjima ministra zdravstva i predsjednice Hrvatskog liječničkog sindikata, koji su iznijeli različite stavove o stanju u zdravstvenom sustavu, problemima liječnika i Prijedlogu zakona o zdravstvenoj zaštiti. Nastavak prvog dana bio je posvećen rizicima zdravlja. Teme drugoga dana bile su, između ostalih, pacijenti te zdravstvena komunikacija. Tijekom skupa obilježeno je 25 godina Motovunske škole, a članovi Hrvatske mreže zdravih gradova predstavili su primjere svoga dugogodišnjeg rada.

The majority of people uses Internet every day to acquire needed information. A wide range of information regarding mental health can be found on the Internet and there are various ways of communication, one of them are Internet forums. Internet forums have become the source of information for any health or psycho- social problem. Participation and communication in the forums are anonymous, which greatly contributes to honest and immediate communication, especially on sensitive topics such as mental illness. The aim of this study was to examine the motives for using depression Internet forums in Croatia. The study sample consists of 297 user-generated posts on the largest Croatian depression Internet sub-forum over a period of one year, analysed using qualitative methodology-grounded theory. The results showed that the majority of depression Internet forums user in Croatia use Internet forums to receive emotional support from others with the same illness. Other important reasons were to exchange information about medications, symptoms and prognosis of the illness. Users of depression Internet forums in Croatia, mostly use those forums to receive emotional support from others with the same ill-ness. For some of the users, Internet forums are the first place where they seek help to cope with symptoms of depression. Because of this reason, Internet forums could be used to detect untreated individuals who could then be provided earlier with psychiatric treatment.

Introduction: Social networks are defined as web sites and applications that allow users to create, share, comment on content and present platforms that enable social networking. With the emergence of digital media and social networks, the term e- professionalism of physicians is defined as attitudes and behaviours that reflect the traditional paradigms of professionalism of health professionals manifested through digital media. Methods: Aim of this presentation is to explore the impact of social networks on e-professionalism of healthcare professionals in the Republic of Croatia. We’ll present the impact of social networks on e-professionalism from the prospective of a specialist in psychiatry, and compare it with the prospective from a different healthcare professional, dentist. Results and discussion: Digital media influence a wide range of medical professional behaviour, and present both dangers and challenges to opportunities. The opportunities are: 1) the use of social networks as a tool to improve information sharing ; 2) the presence of doctors in area with inadequate health care ; 3) the process of life-long learning is supported by social networks ; 4) mentoring student activities on social networks. The most common limitations are insufficient reliability, questionable quality of information, lack of privacy and confidentiality, inadequate use of social networks for communicating with patients and social networks may be an obstacle to realizing "real" contact with a healthcare worker.

Program okrugloga stola započeo je uvodnim obraćanjima akademika Zvonka Kusića, predsjednika Hrvatske akademije znanosti i umjetnosti, akademika Marka Pećine, tajnika Razreda za medicinske znanosti, i prof. dr. sc. Marijana Klarice, dekana Medicinskoga fakulteta Sveučilišta u Zagrebu, uslijedile su panel rasprave o dvjema važnim temama – sinergiji financiranja znanosti iz europskih strukturnih fondova (ESIF) i okvirnoga programa za istraživanje i inovacije Obzor 2020 (H2020) te udjelu rada u znanosti sveučilišnih biomedicinskih nastavnika. Uvodna izlaganja na okruglom stolu u okviru teme sinergija financiranja znanosti iz europskih strukturnih fondova (ESIF) i okvirnoga programa za istraživanje i inovacije Obzor 2020 (H2020), koja su održali eminentni profesori i stručnjaci u ovom području, obuhvatila su analizu uspješnosti Hrvatske u programima Obzor 2020. Predstavljena je analiza dodijeljenih sredstava iz europskih strukturnih fondova (Operativni program konkurentnost i kohezija za istraživanje, razvoj i inovacije, područje biomedicina i zdravstvo) te uspješnost Hrvatske u programima Obzor 2020, posebno u području biomedicine. Hrvatska, kao i ostale „manje učinkovite zemlje“, tzv. EU13, zaostaje za zemljama EU15 te je na 25. mjestu u cjelokupnom programu Obzor 2020, te na 19. u tematskom području Zdravlje, demografske promjene i kvaliteta života. Pokazalo se da količina sredstava dodijeljena zemljama iz programu H2020 direktno korelira s nacionalnim ulaganjem u znanost. Svi panelisti su se suglasili da je potreban napredak u vrednovanju i nagrađivanju izvrsnih znanstvenika i njihovih projekata te bi uz regionalno jačanje trebalo pronaći sredstva da se potaknu i oni najbolji, jer trenutačno rezultati hrvatske znanosti ovise o pojedincima ; sustav bi morao pomoći da se širi krug znanstvenika uključi u programe EU poticanjem potpore istraživačima, jačanjem infrastrukture, financiranjem nabave i održavanja opreme i većim financiranjem znanstvenih istraživanja. Uime Ministarstva znanosti i obrazovanja dr. sc. Marko Košiček je najavio mjere poticanja sudjelovanja u programima Obzor 2020 te mogućnost financiranja projekata u području izvrsnosti (Marie Sklodowska Curie i projekte Europskog vijeća za znanost) koji su uspješno evaluirani, ali nisu odabrani za financiranje. Tijekom izlaganja i rasprave u okviru druge teme udio rada u znanosti sveučilišnih biomedicinskih nastavnika zaključeno je da su sveučilišni biomedicinski nastavnici preopterećeni nastavom, otvaraju se novi studijski programi, a zbog zabrane zapošljavanja u javnim službama ne mogu se otvoriti nova radna mjesta. Samo na Medicinskom fakultetu Sveučilišta u Zagrebu nedostaje 100 nastavnika s obzirom na povećanu satnicu posljednjih 5 godina. Poseban problem, istaknut u raspravi, neravnopravan je popoložaj nastavnika kliničara koji teško usklađuju rad u klinici s nastavnim radom na fakultetu i znanstvenoistraživačkim radom. Prema preporukama Agencije za znanost i visoko obrazovanje, koja je provela analizu rada svih visokih učilišta u Hrvatskoj, potrebno je dodatno zapošljavanje znanstveno-nastavnog kadra i bolje definirana raspodjela znanstvenog, nastavnog i kliničkog rada. Dekan Medicinskog fakulteta Sveučilišta u Zagrebu prof. dr. sc. Marijan Klarica predložio je da se stvori nova paradigma financiranja biomedicinskih istraživanja s obzirom na dosadašnje rezultate znanstvene produktivnosti Medicinskog fakulteta koja čini 25% kompletne znanstvene produktivnosti u Hrvatskoj, a financiranje fakulteta iz državnog proračuna određuje se samo temeljem upisne kvote broja studenata. Dekan Klarica smatra da se na taj način kroz državna tijela financira samo nastavna djelatnost te je prikazao da je u posljednje 3 godine Medicinski fakultet izdvojio 6 milijuna kuna vlastitih sredstava koja su podržala infrastrukturu potrebnu za znanstvenu djelatnost. I dekan Medicinskog fakulteta Sveučilišta u Rijeci prof. dr. sc. Tomislav Rukavina osvrnuo se na neuređenost sustava u kojem rade biomedicinski sveučilišni nastavnici, nedostatnoj koordinaciji i komunikaciji dvaju ministarstava koja su ključna za Medicinske fakultete, Ministarstvo znanosti, i obrazovanja te Ministarstvo zdravstva. Dekan Rukavina upozorio je da nemamo strategiju sinergije znanstvenog razvoja biomedicinske znanosti, te se postojeći problemi rješavaju ad hoc financiranjem iz vlastitih sredstava fakulteta. Prof. dr. sc. Ksenija Geršak, prodekanica za znanost i istraživanje Medicinskog fakulteta Sveučilišta u Ljubljani prikazala je razvojni put Sveučilišta u Ljubljani koje je još 2008. donijelo strategiju razvoja znanosti i započela s njezinom implementacijom, a rezultati se već vide u njihovoj znanstvenoj produktivnosti koja nadmašuje Sveučilište u Zagrebu. Sveučilište u Ljubljani osnovalo je Centralni sveučilišni razvojni fond kojem je prvenstvena uloga financiranje znanstvene djelatnosti sveučilišta. Okrugli je stol održan je u okviru europskoga projekta Alliance4Life, koji financira Europska unija u sklopu programa Obzor 2020. Nositelj je projekta Sveučilište Masaryk u Brnu, sa svojim institutom CEITEC, a partneri na projektu devet vodećih istraživačkih institucija iz manje razvijenih europskih država. Uz Medicinski fakultet Sveučilišta u Zagrebu, partneri su na projektu: Sveučilišna bolnica svete Ane (International Clinical Research Center – ICRC) u Brnu (Češka), Sveučilište Tartu (Estonija), Latvijski institut za organsku sintezu (Latvija), Sveučilište Vilnius (Litva), Sveučilište Semmelweis u Budimpešti (Mađarska), Medicinsko sveučilište u Lodzu (Poljska), Biomedicinski istraživački centar SAS u Bratislavi (Slovačka) i Sveučilište u Ljubljani (Slovenija). Projekt Alliance4Life rezultat je inicijative deset vodećih biomedicinskih institucija iz devet EU13 – država članica. Cilj je projekta smanjivanje razlika i podjela u istraživanju i inovacijama u području biomedicine između EU15 i EU13 zemalja. Europska komisija poduprla je tu novu inicijativu u sklopu programa Obzor 2020. za zdravlje s obzirom na to da rezultati i preporuke projekta Alliance4Life mogu dati korisne prijedloge kojima se može doprinijeti znanstvenoj politici država članica EU-a, kao i drugim europskim državama.

One of the defining phenomena of the present times reshaping the world as we know it, is the worldwide accessibility to the internet. The lovechild of the World Wide Web is social media. The power of social networking is such that, the number of worldwide users is expected to reach some 2.95 billion by 2020, around a third of Earth’s entire population. Social network penetration worldwide is ever-increasing. In 2017, 71 percent of internet users were social network users and these figures are expected to grow. Social media has become ubiquitous, with more people accessing Web-based content by following links on social media than through direct searches. Thus, as a platform used by the public and by health care professionals, it presents an ideal opportunity for health promotion. Social media also brings substantial change to the way organizations and individuals can communicate. Researches postulated that social media had direct public health relevance because social networks could have an important influence on health behaviors and outcomes. However, public health agencies have not yet harnessed the full potential of social media. There is a wealth of opportunity to use social media for health promotion, through targeted messages, the ability to interact with the public, target hard-to-reach groups, and create dynamic campaigns. There is a great deal of enthusiasm for, and interest in, using social media for public health communications, but there is little understanding of the connection between online engagement and behavior change. Few research studies have examined the success of social media in influencing health promoting behaviors. Internationally, public health organizations are keenly experimenting with using social media as tools for both information sharing and behavior change. What remains to be seen is how best to reach out to social media users and what types of messages cut through the online clutter. Strong evidence indicates that public health social marketing campaigns conducted through mainstream media can have a direct and positive effect on behavior. Building the evidence base for conducting health promoting campaigns through social media will require both applying what is known to work in traditional media channels and developing new methods that incorporate the unique features of social media. To begin to understand how best to develop effective online social marketing campaigns, this presentation provides a summary of success factors and key lessons learnt from selected social media campaign case studies.

With the emergence of social media and social networks, the term e-professionalism of physicians is defined as attitudes and behaviors that reflect the traditional paradigms of professionalism of health professionals manifested through social media. Some health institutions and medical universities have developed guidelines for students, teachers and physicians regarding the use of social media and social networks, since there is unquestionable fact that nowadays everyone is using social media. Social media guidelines and policies are based on pre-existing guidelines about the concept of health professionalism, which have been developed for decades and adopted according to ethical principles, enabling to make decisions about what is right and what is wrong or what will we decide. Even though these ethical principles in its fundamental basis are still applicable today, they are not specific enough to guide psychiatrists in this brave new social world that is overwhelmed with technology and instant availability. Aim of this presentation is to explore the impact of social networks on e-professionalism from the prospective of a specialist in psychiatry. Social media influence a wide range of medical professional behaviour, and present both dangers and challenges to opportunities. The opportunities are: 1) the use of social networks as a tool to improve information sharing ; 2) the presence of psychiatrists in area with inadequate health care ; 3) the process of life-long learning is supported by social networks ; 4) mentoring student activities on social networks. The most common limitations are insufficient reliability, questionable quality of information, lack of privacy and confidentiality and inadequate use of social networks for communicating with patients

To review the evidence now being employed to evaluate mental health. To analyze mental health policy in Croatia and compare to best practices in the European Union. To define what is to be considered effective and efficient mental health policy and mental health treatment in Croatia. Summary of Materials and Methods We made an effort to compare National Strategy for Protection of Mental Health in Croatia 2011-2016 to Green Paper Improving the mental health of the population: Towards a strategy on mental health for the European Union. The idea was to establish the relationship between normative policies and instruments employed to measure impact evidence. The Evidence-Based Mental Health policy is sometimes seen as oversimplifying complex problems and offering only limited help in the gray zones of medicine, where scientific evidence is incomplete or conflicting. However, although medical interventions play a central role in tackling challenges, they alone cannot address and change social determinants. The method that has been proven as reliable and empirical is the number needed to treat (NNT) and its counterpart, the number needed to harm (NNH). These two methods are commonly used measures of effect in studies of mental health treatments. The question is what would be the results of employing NNT method to the analysis of different policies and strategic mental health measures in Croatia. Results In line with the WHO strategy, a comprehensive approach is needed. Public policy should cover the provision of treatment and care for individuals, but also the action for the whole population to promote mental health, to prevent mental ill health and to address the challenges associated with stigma and human rights. Different treatment methods such as Cognitive behavioral interventions ; psychological support for those at risk ; training health care professionals in prevention, recognition, and treatment of mental health problems including but not limited to pharmaceutical related policies. The measurable outcomes among others should be a level of wellbeing, physical health, new knowledge & skills, quality of relationships, sexual satisfaction, use of services, productivity, and public safety. Conclusion There is an evident need in Croatia to employ methods that are reliable for comparison of different international and national strategies and their impact on the mental health outcomes. The analytic instruments may vary from analysis of the mental health status of the population to similar health care policies, analysis of best practices and comparative methods and techniques such as NNT. U Grožnjanu je od 10. do 12. lipnja 2016. održan simpozij Mediji i zdravlje „Duh/g vremena“. Misija simpozija je jednostavna: unaprijediti komunikaciju između medija i zdravstvene struke, a cilj se definira svake godine ovisno o trenutačnoj situaciji u zdravstvu i širem društvenom okruženju. Teme ovogodišnjeg, 15. po redu simpozija bile su aktualne reforme i promjene koje planira provesti Ministarstvo zdravlja, transparentnost suradnje farmaceutske industrije sa zdravstvenim radnicima i ustanovama te sve teže djelovanje novinara kao pojedinaca ili zaposlenika medijskih kuća u želji da prenesu važne informacije ili komentiraju društvenu zbilju.

Introduction International students are diverse and increasing population at a numerous universities, including University of Zagreb. Because of demands for cultural adjustments than students in general, they are frequently placed at greater risk for psychological problems. At University of Zagreb School of Medicine, multidisciplinary counselling service was established for international students since 2014. The aim of this study was to analyze reasons for utilization of existing counselling service by international students because of mental health issues. Methods Data were obtained retrospectively from medical records of international students (28 boys, 59 girls) who visited multidisciplinary counselling service because of mental health issues in years 2014 and 2015. Data were analyzed by descriptive statistic. Results Due to mental and behavioural disorders, help in counselling service was demand more by girls than boys (59 vs. 28). The most common reasons were adjustment disorder, present in 15 students, followed by nonorganic insomnia and specific phobias, present in seven students, mixed anxiety and depressive disorder, present in six students, and acute stress reaction, predominantly obsessional thoughts or ruminations and unspecified anxiety disorder, present in five students. The majority of most common reasons of visiting counselling service because of mental health concerns, were present more in girls than in boys: adjustment disorders (10 vs. five), specific phobias (six vs. one), mixed anxiety and depressive disorder (five vs. one), predominantly obsessional thoughts or ruminations, present only in five girls and unspecified anxiety disorder (four vs. one). Nonorganic insomnia had four boys and three girls and acute stress reaction had only five boys. Discussion The results highlighted the importance of sufficient and readily accessible counselling health services with holistic, preventive and optimistic approach. Such approach is offered at University of Zagreb School of Medicine due to work of school medicine specialist, family medicine specialist and psychiatrist in multidisciplinary counselling service for international students.

Tečaj Mediji i zdravlje je zamišljen kao vodeće mjesto za predstavljanje gorućih tema u zdravstvu te otvorene rasprave o budućnosti zdravstvenog sustava u Hrvatskoj. Prvi dio ovogodišnjeg tečaja bio je posvećen zdravstvenoj komunikaciji, drugi dio temeljnim principima socijalnog marketinga, a završni likvidnosti sustava zdravstva i izlasku HZZO-a iz riznice. O toj temi održan je i okrugli stol na kojem su uz novog ministra Sinišu Vargu i v.d. ravnateljice HZZO-a Tatjanu Prenđa Trupec sudjelovali predsjednik HLK Hrvoje Minigo, predsjednik HLZ-a Željko Krznarić, bivši ravnatelj HZZO-a Tihomir Strizirep, Ante Ćorušić, te Ana Stavljenić Rukavina, bivša ministrica zdravlja.

The aims of this paper are to investigate the change in prevalence of psychological distress (PD), to investigate the cumulative incidence of the psychological distress and to indicate are there any differences between genders and various age groups in Croatian adult population. Psychological distress status was measured by the five-item Mental Health Scale (MHI-5) of the Short Form questionnaire (SF-36).Psychological distress was present in 28.5% (95% CI 25.7%-31.2%) of men and 32.1% (95% CI 30.1%-34.0%) of women in 2003. In 2008 PD was present in 33.0% (95% CI 30.1%- 35.9%) of men and 34.1% (95% CI 32.1% - 36.0%) of women. The highest incidence of PD, both for men and women, was in the oldest age group (>65 years). Results of this study demonstrating the fact that every third adult men and woman in Croatia are in psychological distress, the fact that there is an increase in PD prevalence for men and women should be taken into account in the future mental health policy planning.

- Abstract
This study aimed to assess levels of stress in Croatian adult population using PSS, in a population study (Croatian Adult Cohort Health Study - CroHort). Our results show that the levels of stress were 17.46 (SD = 6.73) for men and 18.32 (SD = 6.46) for women in Croatia. The lowest levels of stress experienced men living in urban area while women living in rural area had the highest level. Men and women who had university degree had significantly lower level of stress. The lowest levels of stress experienced participants who had much better financial condition than average. In men, stress was associated to weak heart, lower back pain, poor financial condition of the household and high alcohol consumption. In women, stress was associated to poor mental health, poor social functioning, poorer financial condition of the household, higher age, lower education, low monthly income of the household and poor general health.

This study provides an overview of the incidence of smoking, the socio-demographic characteristics of Croatian smokers during a five-year period and an assessment of predictors of the desire/decision to quit smoking. Analyses were performed separately for 2003 and 2008. A total of 3,229 subjects were included in the survey. There is a significant trend of a decreasing number of smokers in all age groups in 2008, compared to 2003. Almost half of the smokers included in the study expressed desire to quit smoking. Factors contributing significantly to decision to quit smoking were different in 2003 and 2008, except one. Concern about the harmful effects of tobacco smoking on health was a significant predictor in both models. Very worried respondents were more likely to decide to quit smoking (OR 17.6, 95% CI 9.41 to 33.17 vs. OR 12.54; 95% CI 6.0 to 26.2) than those who were not worried at all.

- Abstract
Objective of this paper is to estimate interim risk factors (INTF) proportions and changes within 5-years of groups with at least one risk health behavior (BEHF) in CroHort population. Results show that CroHort 2008 group has higher proportions of excess abdominal fat and overweight. Men older than 65 without any BEHF have smaller proportions of INTF in 2008 than in 2003. Proportion of people with high blood pressure is smaller in 2008 for all groups except for young women who show increase. Analysis of middle age group shows significant increase in all INTF in women smokers while men smokers have the highest increase in abdominal fat. Physical inactivity in women is associated with increase of all INTF, while men have decrease in overweight INTF. Alcohol intake has protective effect on middle aged men, except for increase in waist circumference. Women show constant increase in all INTF with heavy alcohol intake.
