Abstract

Background: The COVID-19 pandemic heightened risks for individuals with hematological malignancies due to compromised immune systems, leading to more severe outcomes and increased mortality. While interventions like vaccines, targeted antivirals, and monoclonal antibodies have been effective for the general population, their benefits for these patients may not be as pronounced. Methods: The EPICOVIDEHA registry (National Clinical Trials Identifier, NCT04733729) gathers COVID-19 data from hematological malignancy patients since the pandemic's start worldwide. It spans various global locations, allowing comprehensive analysis over the first three years (2020–2022). Findings: The EPICOVIDEHA registry collected data from January 2020 to December 2022, involving 8767 COVID-19 cases in hematological malignancy patients from 152 centers across 41 countries, with 42% being female. Over this period, there was a significant reduction in critical infections and an overall decrease in mortality from 29% to 4%. However, hospitalization, particularly in the ICU, remained associated with higher mortality rates. Factors contributing to increased mortality included age, multiple comorbidities, active malignancy at COVID-19 onset, pulmonary symptoms, and hospitalization. On the positive side, vaccination with one to two doses or three or more doses, as well as encountering COVID-19 in 2022, were associated with improved survival. Interpretation: Patients with hematological malignancies still face elevated risks, despite reductions in critical infections and overall mortality rates over time. Hospitalization, especially in ICUs, remains a significant concern. The study underscores the importance of vaccination and the timing of COVID-19 exposure in 2022 for enhanced survival in this patient group. Abstract

Since the beginning of the COVID-19 pandemic, there has been an overall improvement in patient mortality. However, haematological malignancy patients continue to experience significant impacts from COVID-19, including high rates of hospitalization, intensive care unit (ICU) admissions, and mortality. In comparison to other haematological malignancy patients, individuals with chronic myeloid leukemia (CML) generally have better prognosis. This study, conducted using a large haematological malignancy patient database (EPICOVIDEHA), demonstrated that the majority of CML patients experienced mild infections. The decline in severe and critical infections over the years can largely be attributed to the widespread administration of vaccinations and the positive response they elicited. Notably, the mortality rate among CML patients was low and exhibited a downward trend in subsequent years. Importantly, our analysis provided confirmation of the effectiveness of vaccinations in CML patients.

Abstract

Introduction: Molnupiravir and nirmatrelvir/ritonavir are antivirals used to prevent progression to severe SARS-CoV-2 infections and decrease hospitalisation and mortality rates. Nirmatrelvir/ritonavir was authorised in Europe in December 2021, whereas molnupiravir is not yet licensed in Europe as of February 2022. Molnupiravir may be an alternative to nirmatrelvir/ritonavir because it is associated with fewer drug-drug interactions and contraindications. A caveat for molnupiravir is the mode of action induces viral mutations. Mortality rate reduction with molnupiravir was less pronounced than that with nirmatrelvir/ritonavir in patients without haematological malignancy. Little is known about the comparative efficacy of the two drugs in patients with haematological malignancy at high-risk of severe COVID-19. Thus, molnupiravir and nirmatrelvir/ritonavir were compared in a cohort of patients with haematological malignancies. Methods: Clinical data from patients treated with molnupiravir or nirmatrelvir/ritonavir monotherapy for COVID-19 were retrieved from the EPICOVIDEHA registry. Patients treated with molnupiravir were matched by sex, age (±10 years), and severity of baseline haematological malignancy to controls treated with nirmatrelvir/ritonavir. Results: A total of 116 patients receiving molnupiravir for the clinical management of COVID-19 were matched to an equal number of controls receiving nirmatrelvir/ritonavir. In each of the groups, 68 (59%) patients were male; with a median age of 64 years (interquartile range [IQR] 53-74) for molnupiravir recipients and 64 years (IQR 54-73) for nirmatrelvir/ritonavir recipients; 56.9% (n=66) of the patients had controlled baseline haematological malignancy, 12.9% (n=15) had stable disease, and 30.2% (n=35) had active disease at COVID-19 onset in each group. Abstract

Objectives: Elderly patients with hematologic malignancies face the highest risk of severe COVID-19 outcomes. The infection's impact on different age groups remains unstudied in detail. Methods: We analyzed elderly patients (age groups: 65-70, 71-75, 76-80, and >80 years old) with hematologic malignancies included in the EPICOVIDEHA registry between January 2020 and July 2022. Univariable and multivariable Cox regression models were conducted to identify factors influencing death in COVID-19 patients with hematological malignancy. Results: The study included data from 3,603 elderly patients (aged 65 or older) with hematological malignancy, with a majority being male (58.1%) and a significant proportion having comorbidities. The patients were divided into four age groups, and the analysis assessed COVID-19 outcomes, vaccination status, and other variables in relation to age and pandemic waves. The 90-day survival rate for patients with COVID-19 was 71.2%, with significant differences between groups. The pandemic waves had varying impacts, with the first wave affecting patients over 80 years old, the second being more severe in 65-70, and the third being the least severe in all age groups. Factors contributing to 90-day mortality included age, comorbidities, lymphopenia, active malignancy, acute leukemia, less than three vaccine doses, severe COVID-19, and using only corticosteroids as treatment. Conclusion: These data underscore the heterogeneity of elderly hematological patients, highlight the different impacts of COVID-19 waves and the pivotal importance of vaccination, and may help in planning future healthcare efforts.

Abstract

Limited data are available on breakthrough COVID-19 in patients with hematologic malignancy (HM) after anti–severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination. Adult patients with HM, ≥1 dose of anti-SARS-CoV-2 vaccine, and breakthrough COVID-19 between January 2021 and March 2022 were analyzed. A total of 1548 cases were included, mainly lymphoid malignancies (1181 cases, 76%). After viral sequencing in 753 cases (49%), the Omicron variant was prevalent (517, 68.7%). Most of the patients received ≤2 vaccine doses before COVID-19 (1419, 91%), mostly mRNA-based (1377, 89%). Overall, 906 patients (59%) received COVID-19-specific treatment. After 30-day follow-up from COVID-19 diagnosis, 143 patients (9%) died. The mortality rate in patients with the Omicron variant was 7.9%, comparable to other variants, with a significantly lower 30-day mortality rate than in the prevaccine era (31%). In the univariable analysis, older age (P < .001), active HM (P < .001), and severe and critical COVID-19 (P = .007 and P < .001, respectively) were associated with mortality. Conversely, patients receiving monoclonal antibodies, even for severe or critical COVID-19, had a lower mortality rate (P < .001). In the multivariable model, older age, active disease, critical COVID-19, and 2-3 comorbidities were correlated with a higher mortality, whereas monoclonal antibody administration, alone (P < .001) or combined with antivirals (P = .009), was protective. Although mortality is significantly lower than in the prevaccination era, breakthrough COVID-19 in HM is still associated with considerable mortality. 11. Sensitivity of Modeled Microphysics to Stochastically Perturbed Parameters

Abstract

This study examines the characteristics of several model parameter perturbation methodologies for ensemble simulations of cloud microphysical processes in convection. A simplified 1D model is used to focus the results on cloud microphysics without the complication of feedbacks to the dynamics and environment. Several parameter perturbation methods are tested, including non-stochastic and stochastic with various distributions and parameter covariance. We find that an ensemble comprised of different time-invariant parameters (non-stochastic) exhibits little bias, but small spread. In addition, its behavior does not respect the time evolution of convection through its various phases. Stochastic parameter (SP) methods in which no inter-parameter covariance is applied produce greater spread, but significant bias. The bias is particularly large for lognormal parameter perturbation distributions. The ensemble spread is retained and the bias reduced when time-varying parameter covariance is applied. In this case, the SP scheme is able to adapt to the time and state-dependent covariance structures and produce ensemble characteristics that are consistent with the specific microphysical processes operating at any given time. The results suggest that SP schemes would benefit from inclusion of parameter covariances, and specifically those that vary with the state of the system. It also suggests that a Normal or LogNormal SP scheme with no covariance may significantly impact the ensemble bias. Finally, the results indicate that high temporal and spatial resolution observations may be needed to characterize the variability in parameter values and covariance.

Abstract

Patients with lymphoproliferative diseases (LPD) are vulnerable to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Here, we describe and analyze the outcome of 366 adult patients with chronic lymphocytic leukemia (CLL) or non-Hodgkin Lymphoma (NHL) treated with targeted drugs and laboratory-confirmed COVID-19 diagnosed between February 2020 and January 2022. Median follow-up was 70.5 days (IQR 0-609). Most used targeted drugs were Bruton-kinase inhibitors (BKIs) (N= 201, 55%), anti-CD20 other than rituximab (N=61, 16%), BCL2 inhibitors (N=33, 9%) and lenalidomide (N=28, 8%).Only 16.2% of the patients were vaccinated with 2 or more doses of vaccine at the onset of COVID-19. Mortality was 24% (89/366) on day 30 and 36%(134/366) on the last day of follow-up. Age >75 years (p<0.001, HR 1.036), active malignancy (p<0.001, HR 2.215), severe COVID-19 (p=0.017, HR 2.270) and admission to ICU (p<0.001, HR 5.751) were risk factors for mortality at last day of follow up. There was no difference in OS rates in NHL vs CLL patients (p=0.306), nor in patients treated with or without BKIs (p=0.151). Mortality in ICU was 66% (CLL 61%, NHL 76%). Overall mortality rate decreased according to vaccination status, being 39% in unvaccinated patients, 32% and 26% in those having received one or two doses, respectively, and 20% in patients with a booster dose (p=0.245). Overall mortality rate dropped from 41% during the first semester of 2020 to 25% at the last semester of 2021. These results show increased severity and mortality from COVID-19 in LPDs patients treated with targeted drugs.

Abstract

Background: The outcome of patients with simultaneous diagnosis of haematological malignancies (HM) and COVID-19 is unknown and there are no specific treatment guidelines. Methods: We describe the clinical features and outcome of a cohort of 450 patients with simultaneous diagnosis of HM and COVID-19 registered in the EPICOVIDEHA registry between March 2020 to February 2022. Results: Acute leukaemia and lymphoma were the most frequent HM (35.8% and 35.1%, respectively). Overall, 343 (76.2%) patients received treatment for HM, which was delayed for longer than one month since diagnosis in 57 (16.6%). An overall response rate was observed in 140 (40.8%) patients after the first line of treatment. After a median follow-up of 35 days, overall mortality was 177/450 (39.3%); 30-day mortality was significantly higher in patients not receiving HM treatment (42.1%) than in those receiving treatment (27.4%, p = 0.004), either before and/or after COVID-19, or compared to patients receiving HM treatment at least after COVID-19 (15.2%, p < 0.001). Age, severe/critical COVID-19, ≥2 comorbidities, and lack of HM treatment were independent risk factors for mortality, whereas a lymphocyte count >500/mcl at COVID-19 onset was protective. Conclusions: HM treatment should be delivered as soon as possible for patients with simultaneous diagnosis of COVID-19 and HM requiring immediate therapy.

Abstract

BackgroundInformation on tuberculosis (TB) and coronavirus disease 2019 (COVID-19) is still limited. The aim of this study was to describe the features of the TB/COVID-19 co-infected individuals from a prospective, anonymised, multicountry register-based cohort with special focus on the determinants of mortality and other outcomes.MethodsWe enrolled all patients of any age with either active TB or previous TB and COVID-19. 172 centres from 34 countries provided individual data on 767 TB-COVID-19 co-infected patients, (>50% population-based).ResultsOf 767 patients, 553 (74.0%) out of 747 had TB before COVID-19 (including 234 out of 747 with previous TB), 71 (9.5%) out of 747 had COVID-19 first and 123 (16.5%) out of 747 had both diseases diagnosed within the same week (n=35 (4.6%) on the same day). 85 (11.08%) out of 767 patients died (41 (14.2%) out of 289 in Europe and 44 (9.2%) out of 478 outside Europe; p=0.03): 42 (49.4%) from COVID-19, 31 (36.5%) from COVID-19 and TB, one (1.2%) from TB and 11 from other causes. In the univariate analysis on mortality the following variables reached statistical significance: age, male gender, having more than one comorbidity, diabetes mellitus, cardiovascular disease, chronic respiratory disease, chronic renal disease, presence of key symptoms, invasive ventilation and hospitalisation due to COVID-19. The final multivariable logistic regression model included age, male gender and invasive ventilation as independent contributors to mortality.ConclusionThe data suggest that TB and COVID-19 are a “cursed duet” and need immediate attention. TB should be considered a risk factor for severe COVID disease and patients with TB should be prioritised for COVID-19 preventative efforts, including vaccination.

Abstract

This paper describes the novel concept of dual-type temperature sensors, that enables in-situ monitoring of thermocouple drift. The concept aims to provide users, who are performing temperature measurements by thermocouples, a relatively cheap and simple solution for monitoring its drift under conditions of typical use, including environments with varying temperatures. For industrial users, thermocouple drift causes many problems such as higher energy costs and lost time through process disruption due to the replacement or recalibration of failed thermocouples. This usually requires processes to be shut down when the measurement errors become too large. Such process disruptions can further lead to loss of raw material or the production of semi-finished products. For these reasons, users requiring high accuracy temperature measurements tend to replace thermocouples regularly as a precautionary measure, sometimes even if they have been used for only a few days. The main idea of the dual-type thermometers is that a drift of a thermocouple can be determined by monitoring the difference in its temperature readings and the readings of an additional (reference) thermometer if the reference sensor experiences a lower drift than the thermocouple under test. In many cases, the monitored thermocouple sensor cannot be simply replaced with a thermometer of lower drift, since it is connected to the electronic unit that is, in general, not suitable for measurement with a different type of temperature sensor. Even if changing the thermocouple type would be feasible, the replacement thermocouple would still experience drift that would be impossible to track in-situ. The concept of dual-type temperature sensors can solve this problem as a drift of a reference sensor can be estimated after every replacement of the monitored thermocouple. Also, in many industrial scenarios, thermocouples are not calibrated before being put in operation and users rely on their stated tolerance classes. In such cases, the methods described in this paper can be used for checking if a monitored thermocouple has departed outside the specified tolerance limits in time. Together with the description of the concept, this paper provides schematic diagrams of related measurement setups, drawings of dual-type temperature sensors (utilizing noble metal thermocouples and platinum resistance thermometers as the reference sensors) as well as an algorithm for drift determination. This work is performed within the framework of the European Metrology Programme for Innovation and Research (EMPIR) project 18RPT03 MetForTC. Abstract

Within the work package one of the ongoing EMPIR project „Developing Traceable Measurement Capabilities for Monitoring Thermocouple Performance” (MetForTC), the novel concept of dual-type temperature probes was developed. By using dual-type temperature probes, users would be able to continuously monitor the thermocouple drift in-situ, including environments with varying temperatures. The concept is developed mainly for use in industry, where the drift of thermocouples can lead to various problems, such as increased energy consumption, loss of raw material, or decrease in quality of products. To mitigate the problems related to thermocouple drift, industrial users tend to replace thermocouples regularly, in some cases even after only a few days of usage. Using this approach, thermocouples can be occasionally replaced too early as well as too late. Dual-type temperature probe consists of a monitored thermocouple sensor and an additional temperature sensor, accommodated within the same protective sheath. The use of an additional (reference) sensor, which experiences a lower drift than the monitored thermocouple (e.g. higher grade thermocouple wires, platinum resistance sensors, or fiber-optic sensors), enables monitoring of thermocouple drift by continuous comparison of the differences in their temperature readings. While the drift of the monitored thermocouple can be tracked continuously, the concept also enables occasional tracking of drift of the reference sensor.

21. Analysis of the impact of model nonlinearities in inverse problem solving

Abstract

In this study, the relationship between nonlinear model properties and inverse problem solutions is analyzed using a numerical technique based on the inverse problem theory formulated by Mosegaard and Tarantola. According to this theory, the inverse problem and solution are defined via convolution and conjunction of probability density functions (PDFs) that represent stochastic information obtained from the model, observations, and prior knowledge in a joint multidimensional space. This theory provides an explicit analysis of the nonlinear model function, together with information about uncertainties in the model, observations, and prior knowledge through construction of the joint probability density, from which marginal solution functions can then be evaluated. The numerical analysis technique derived from the theory computes the component PDFs in discretized form via a combination of function mapping on a discrete grid in the model and observation phase space and Monte Carlo sampling from known parametric distributions. The efficacy of the numerical analysis technique is demonstrated through its application to two well-known simplified models of atmospheric physics: damped oscillations and Lorenz's three-component model of dry cellular convection. The major findings of this study include the following: (i) Use of a nonmonotonic forward model in the inverse problem gives rise to the potential for a multimodal posterior PDF, the realization of which depends on the information content of the observations and on observation and model uncertainties. (ii) The cumulative effect of observations over time, space, or both could render the final posterior PDF unimodal, even with the nonmonotonic forward model. (iii) A greater number of independent observations are needed to constrain the solution in the case of a nonmonotonic nonlinear model than for a monotonic nonlinear or linear forward model for a given number of degrees of freedom in control parameter space. (iv) A nonlinear monotonic forward model gives rise to a skewed unimodal posterior PDF, implying a well-posed maximum likelihood inverse problem. (v) The presence of model error greatly increases the possibility of capturing multiple modes in the posterior PDF with the nonmonotonic nonlinear model. (vi) In the case of a nonlinear forward model, use of a Gaussian approximation for the prior update has a similar effect to an increase in model error, which indicates there is the potential to produce a biased mean central estimate even when observations and model are unbiased.

Abstract

This study addresses the problem of four-dimensional (4D) estimation of a cloudy atmosphere on cloud-resolving scales using satellite remote sensing measurements. The motivation is to develop a methodology for accurate estimation of cloud properties and the associated atmospheric environment on small spatial scales but over large regions to aid in better understanding of the clouds and their role in the atmospheric system. The problem is initially approached by the study of the assimilation of the Geostationary Operational Environmental Satellite (GOES) imager observations into a cloud-resolving model with explicit bulk cloud microphysical parameterization. A new 4D variational data assimilation (4DVAR) research system with the cloud-resolving capability is applied to a case of a multilayered cloud evolution without convection. In the experiments the information content of the IR window channels is addressed as well as the sensitivity of estimation to lateral boundary condition errors, model first guess, decorrelation length in the background statistical error model, and the use of a generic linear model error. The assimilation results are compared with independent observations from the Atmospheric Radiation Measurement (ARM) central facility archive. The modeled 3D spatial distribution and short-term evolution of the ice cloud mass is significantly improved by the assimilation of IR window channels when the model already contains conditions for the ice cloud formation. The assimilated ice cloud in this case is in good agreement with the independent cloud radar measurements. The simulation of liquid clouds below thick ice clouds is not influenced by the IR window observations. The assimilation results clearly demonstrate that increasing the observational constraint from individual to combined channel measurements and from less to more frequent observation times systematically improves the assimilation results. The experiments with the model error indicate that the current specification of this error in the form of a generic linear forcing, which was adopted from other data assimilation studies, is not suitable for the cloud-resolving data assimilation, instead, a parameter estimation approach may need to be explored in the future. The experiments with varying decorrelation lengths suggest the need to use the model horizontal grid spacing that is several times smaller than the GOES imager native resolution to achieve equivalent spatial variability in the assimilation.

Haywood et al. (2004) show that an aerosol layer above a cloud can cause a bias in the retrieved cloud optical thickness and effective radius. Monitoring for this potential bias is difficult because space ]based passive remote sensing cannot unambiguously detect or characterize aerosol above cloud. We show that cloud retrievals from aircraft measurements above cloud and below an overlying aerosol layer are a means to test this bias. The data were collected during the Intercontinental Chemical Transport Experiment (INTEX-A) study based out of Portsmouth, New Hampshire, United States, above extensive, marine stratus cloud banks affected by industrial outflow. Solar Spectral Flux Radiometer (SSFR) irradiance measurements taken along a lower level flight leg above cloud and below aerosol were unaffected by the overlying aerosol. Along upper level flight legs, the irradiance reflected from cloud top was transmitted through an aerosol layer. We compare SSFR cloud retrievals from below ]aerosol legs to satellite retrievals from the Moderate Resolution Imaging Spectroradiometer (MODIS) in order to detect an aerosol ]induced bias. In regions of small variation in cloud properties, we find that SSFR and MODIS-retrieved cloud optical thickness compares within the uncertainty range for each instrument while SSFR effective radius tend to be smaller than MODIS values (by 1-2 microns) and at the low end of MODIS uncertainty estimates. In regions of large variation in cloud properties, differences in SSFR and MODIS ]retrieved cloud optical thickness and effective radius can reach values of 10 and 10 microns, respectively. We include aerosols in forward modeling to test the sensitivity of SSFR cloud retrievals to overlying aerosol layers. We find an overlying absorbing aerosol layer biases SSFR cloud retrievals to smaller effective radii and optical thickness while nonabsorbing aerosols had no impact.

Abstract

In this paper, an interlaboratory comparison in the field of measurement of temperature is presented. Within the comparison, calibration of a standard platinum resistance thermometer (SPRT) by comparisons in the range from $$-80\,^{\circ }\hbox {C}$$ to $$300\,^{\circ }\hbox {C}$$ was performed. At the same time, in order to support the calibration and measurement capabilities (CMCs) entries of the participating laboratories, we have registered this as EURAMET Project 1251 (Comparison of the calibration of standard platinum resistance thermometers in the range from $$-80\,^{\circ }\hbox {C}$$ to $$300\,^{\circ }\hbox {C}$$ by comparison). It was recommended that the participants use their standard procedure for the calibration of the standard platinum resistance thermometers and follow instructions from the protocol of EURAMET Project 1251 during the temperature calibration and, if possible, avoid making extra time-consuming measurements. The interlaboratory comparison was organized by the University of Ljubljana, Faculty of Electrical Engineering, Laboratory of Metrology and Quality (MIRS/UL-FE/LMK) in the scope of the IPA 2011 project. The interlaboratory comparison included a maximum of eleven measurement points. However, certain laboratories did not perform measurements at all points in the range. They have performed only measurements in the range that they cover. Prior to the calibration by comparison in each laboratory, a test measurement at the triple point of water or ice point was done in order to assess the stability of the instruments. Results of the comparison show that all the measurements agree within declared uncertainties and thus supporting declared capabilities of the participating laboratories. [ABSTRACT FROM AUTHOR]

MIRS/UL-FE/LMK and MBM organized a bilateral comparison of a set of liquid-in-glass thermometers (LIGTs) in the range from $$-30\, ^{\circ }\hbox {C}$$ to $$150\, ^{\circ }\hbox {C}$$ . The circulating items were three total immersion LIGTs, manufactured by Thermo Schneider, resolution of $$0.1\, ^{\circ }\hbox {C}$$ , covering range from $$-30\, ^{\circ }\hbox {C}$$ to $$10\, ^{\circ }\hbox {C}$$ (Serial Number 991289), $$0\, ^{\circ }\hbox {C}$$ to $$50\, ^{\circ }\hbox {C}$$ (Serial Number 9710425), and $$100\, ^{\circ }\hbox {C}$$ to $$150\, ^{\circ }\hbox {C}$$ (Serial Number 991308). The range between $$50\, ^{\circ }\hbox {C}$$ and $$100\, ^{\circ }\hbox {C}$$ has been already compared in another interlaboratory comparison. This interlaboratory comparison was also registered as EURAMET 1249 comparison project (comparison of the calibration of liquid-in-glass thermometers in the range from $$-30\, ^{\circ }\hbox {C}$$ to $$150\, ^{\circ }\hbox {C}$$ ). The results of the comparison showed that all 18 pairs of measurements performed by the participating laboratories agree within declared uncertainties and thus supported declared calibration and measurement capabilities by MIRS/UL-FE/LMK and MBM.
