Abstract

National audience; Increased susceptibility of experimental animals with immune suppression to alveolar echinococcosis (AE), due to Echinococcus multilocularis infection, has been known for more than 30 years, as well as the fast progression of residual AE lesions in patients with liver transplantation. However, occurrence of AE in patients with acquired immune suppression has been published as isolated cases only from the beginning of the 21st century. Based on the French Registry of Alveolar Echinococcosis cases (FrancEchino), a systematic study of AE cases in immunosuppressed patients, patients with AIDS, with cancer and malignant hematological disorders, with chronic inflammatory diseases, and with organ transplantations, has been performed. This study has confirmed the significant increase of AE cases in patients with immune suppression since 2000, compared to previous years, and of the percentage of such cases among all French AE patients, thus giving AE the status of 'opportunistic infection'. AE is most often an incidental finding in patients with a follow-up for the underlying disease. AE diagnosis is generally delayed and the treatment often erroneous because of misleading diagnostic findings, especially in patients with cancer. When the patients are symptomatic, unusual acute symptoms that simulate a liver abscess may be seen; these presenting symptoms, as well as more frequent negative serology and unusual imaging findings than in patients without immune suppression contribute to delayed diagnosis. Usual therapeutic strategy nevertheless applies to AE in immunosuppressed patients, with complete surgical resection of AE lesions whenever possible, anatomically and in the context of the underlying disease, and long-term administration of albendazole in non-operable patients. In such cases, albendazole efficacy is often excellent and fast; however, adverse effects of albendazole seem more frequent than in other patients. Complementary studies are necessary to understand whether AE occurrence in immunosuppressed patients is due to newly acquired infection or to the reactivation of dormant microscopic lesions.

Abstract

International audience; The earlier recommendations of the WHO-Informal Working Group on Echinococcosis (WHO-IWGE) for the treatment of human echinococcosis have had considerable impact in different settings worldwide, but the last major revision was published more than 10 years ago. Advances in classification and treatment of echinococcosis prompted experts from different continents to review the current literature, discuss recent achievements and provide a consensus on diagnosis, treatment and follow-up. Among the recognized species, two are of medical importance -Echinococcus granulosus and Echinococcus multilocularis - causing cystic echinococcosis (CE) and alveolar echinococcosis (AE), respectively. For CE, consensus has been obtained on an image-based, stage-specific approach, which is helpful for choosing one of the following options: (1) percutaneous treatment, (2) surgery, (3) anti-infective drug treatment or (4) watch and wait. Clinical decision-making depends also on setting-specific aspects. The usage of an imaging-based classification system is highly recommended. For AE, early diagnosis and radical (tumour-like) surgery followed by anti-infective prophylaxis with albendazole remains one of the key elements. However, most patients with AE are diagnosed at a later stage, when radical surgery (distance of larval to liver tissue of >2cm) cannot be achieved. The backbone of AE treatment remains the continuous medical treatment with albendazole, and if necessary, individualized interventional measures. With this approach, the prognosis can be improved for the majority of patients with AE. The consensus of experts under the aegis of the WHO-IWGE will help promote studies that provide missing evidence to be included in the next update.

Abstract

International audience; OBJECTIVE: To identify mixed infection of Echinococcus granulosus and E. multilocularis in a dog from Xinjiang. METHODS: Thirty dogs from the pasture area were dissected and over 10,000 Echinococcus adult worms were found from one dog. Morphological observation revealed possible mixed infection of the two Echinococcus species. Further identifcation was made by amplification of the target gene DNA fragment (mitochondrial 12S rRNA gene). RESULTS: The adult worms of E. granulosus showed a relatively longer and larger gravid proglottid, its genital pore situated near or below the middle-side of the segment. The uterus was in a sacculate shape with irregular branches and approximately over 200 - 800 eggs in it. Morphology of the adult worms of E. multilocularis was similar to E. granulosus, slightly smaller, consisting of 4 to 5 proglottids. The uterus was not sacculate and with no branch. Its lateral genital pore often situated in the anterior part of the segment. Sequence analysis of mitochondrial 12S rRNA gene showed that amplification with the Eg1f/r primers shared complete identity with E. granulosus G1 genotype (GentBank accession no. AY462129), while that witht the EmH15/17 primers shared complete identity with E. multilocularis (GenBank accession no. AB031351). The presence of both E. granulosus and E. multicularis was confirmed by microscopy and gene identification. CONCLUSION: Mixed infection of the two species of Echinococcus has been confirmed in the dog by morphological observation and PCR technique.

Abstract

Cystic echinococcosis (CE)\cystic hydatid disease is one of the most widespread and important global helminth zoonosis. The parasite Echinococcus granulosus, continues to be a cause and consequence of poverty through its impact on public health and animal production. Human behaviour is largely responsible for the perpetuation of the parasite which flourishes in low socio-economic regions of the world where there are poor veterinary and medical facilities and where home slaughter of livestock is performed and dogs go untreated. The parasite is maintained in a wide spectrum of intermediate hosts, including sheep, goats, camels, cattle, pigs and equines. A number of wild intermediate hosts occur, including cervids in the northern part of the North American continent and Eurasia, marsupials in Australia and wild herbivores in East and southern Africa. The application of a range of molecular techniques to the characterization of the parasite has confirmed the existence of mostly host-adapted strains and genotypes of the parasite. Several new species have been proposed which include: the sheep strain (E. granulosus, sensu stricto, G1, G2, G3), the horse strain (E. equinus, G4), the cattle strain (E. ortleppi, G5), the camel (G6) and pig (G7) strain (E.intermedius, G6, G7, G9), the cervid strain (E. canadensis, G8, G10) and the lion strain (E. felidis). All strains except the horse and lion strain are known to be infective to humans. The ubiquitous domestic dog serves as the most important definitive host for the transmission of the parasite throughout its wide geographical range. Lions, hyenas and jackals also serve as definitive hosts in the wildlife cycles in Africa, wolves, foxes and jackals in the northern hemisphere and dingoes in Australia. A wide range of diagnostic techniques, including necropsy, arecoline purgation, coproantigen ELISA and DNA based tests are available for detecting E. granulosus infection in the definitive host. Abstract

The effect of genetic variability as indicated by allozyme heterozygosity on developmental homeostasis as expressed by fluctuating morphological asymmetry (FA) is under current debate. Recent studies of brown hares Lepus europaeus Pallas, 1778 in Austria (Hartl et al. 1995) revealed a negative relationship between FA and allozyme heterozygosity in non-metric but not in metric skull characters. Authors aimed to find out whether non-metric traits of brown hares are generally more prone to FA than metric, or whether metric traits also show increased FA in populations with drastically reduced genetic variability. Authors studied variation in 34 enzyme systems and the relationship between overall individual heterozygosity based on polymorphic loci and FA in 27 non-metric and six metric bilateral skull characters of 96 brown hares from three populations in Britain and two in New Zealand.
