2. Children’s screen time and psychosocial symptoms at 5 years of age – the role of parental factors

Abstract Background and objectives Electronic media (e-media) has become a universal part of young children’s daily lives. Previous studies have found an association between increased screen time and children’s psychosocial symptoms. We investigated whether parents’ psychological distress and parenting style dimensions explain the association between children’s screen time and psychosocial symptoms. Moreover, we investigated whether parents’ mental well-being and parenting style dimensions moderate this association. Methods We used data from the Finnish CHILD-SLEEP birth cohort study. Parents and the child were assessed when the child was 5 years old (N = 671). The measure of screen time included program viewing from TV and other devices. Child’s psychosocial problems and parents’ depression, stress and parenting style dimensions were assessed by self-reports. Results A high level of screen time in children was associated with attention and concentration difficulties, hyperactivity and impulsivity symptoms as well as internalizing and externalizing symptoms among 5-year-olds. For the most part, the associations remained significant despite controlling for parents’ mental health, parenting style dimensions and multiple background factors, especially associations relating to attention and concentration difficulties and hyperactivity symptoms were robust. Maternal stress and depression moderated the association between children’s screen time and psychosocial symptoms, indicating a more pronounced association among stressed or depressed mothers. Conclusion There is an independent association between children’s screen time and psychosocial symptoms which is especially pronounced among those children whose mothers had poorer mental well-being. In clinical practice, the length of screen time should be inquired already at a young age and parents should be offered guidance to reduce the possible ill effects of excessive screen time, as well as help with their own mental health problems.

We present 10 years of R-band monitoring data of 31 northern blazars which were either detected at very high energy (VHE) gamma rays or listed as potential VHE gamma-ray emitters. The data comprise 11820 photometric data points in the R-band obtained in 2002-2012. We analyze the light curves by determining their power spectral density (PSD) slopes assuming a power-law dependence with a single slope $\beta$ and a Gaussian probability density function (PDF). We use the multiple fragments variance function (MFVF) combined with a forward-casting approach and likelihood analysis to determine the slopes and perform extensive simulations to estimate the uncertainties of the derived slopes. We also look for periodic variations via Fourier analysis and quantify the false alarm probability through a large number of simulations. Comparing the obtained PSD slopes to values in the literature, we find the slopes in the radio band to be steeper than those in the optical and gamma rays. Our periodicity search yielded one target, Mrk 421, with a significant (p<5%) period. Finding one significant period among 31 targets is consistent with the expected false alarm rate, but the period found in Mrk~421 is very strong and deserves further consideration}., Comment: 27 pages, 70 figures, accepted for publication in Astronomy & Astrophysics

Nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL) is a Hodgkin lymphoma expressing functional B-cell receptors (BCR). Recently, we described a dual stimulation model of IgD+ lymphocyte-predominant cells by Moraxella catarrhalis antigen RpoC and its superantigen MID/hag, associated with extralong CDR3 and HLA-DRB1*04 or HLADRB1* 07 haplotype. The aim of the present study was to extend the antigen screening to further bacteria and viruses. The fragment antibody-binding (Fab) regions of seven new and 15 previously reported cases were analyzed. The reactivity of non-Moraxella spp.-reactive Fab regions against lysates of Rothia mucilaginosa was observed in 5/22 (22.7%) cases. Galactofuranosyl transferase (Gltf) and 2,3-butanediol dehydrogenase (Bdh) of R. mucilaginosa were identified by comparative silver- and immuno-staining in two-dimensional gels, with subsequent mass spectrometry and validation by western blots and enzyme-linked immunosorbent assay. Both R. mucilaginosa Gltf and Bdh induced BCR pathway activation and proliferation in vitro. Apoptosis was induced by recombinant Gltf/ETA’-immunotoxin conjugates in DEV cells expressing recombinant R. mucilaginosa-reactive BCR. Reactivity against M. catarrhalis RpoC was confirmed in 3/7 newly expressed BCR (total 10/22 reactive to Moraxella spp.), resulting in 15/22 (68.2%) cases with BCR reactivity against defined bacterial antigens. These findings strengthen the hypothesis of bacterial trigger contributing to subsets of NLPHL.

9. Differential diagnosis and long-term outcomes of non-atrophic duodenal changes in children

Objectives and studyGastrointestinal endoscopy is often performed when investigating abdominal complaints in children. While atrophic changes of the duodenal mucosa are usually caused by celiac disease, the prevalence and clinical significance of non-atrophic duodenal changes are less clear. We studied these issues in a large pediatric endoscopic cohort.MethodsComprehensive data on clinical features, diagnostic findings and long-term outcomes of children who had undergone upper gastrointestinal endoscopy with systematic duodenal sampling were collected. Study variables were compared between children with non-atrophic changes and normal histology, and between those with non-atrophic changes who did and did not receive a diagnosis.ResultsThe study comprised 1,170 consecutive children, of whom 51 (4.4%) had non-atrophic and 315 (26.9%) atrophic duodenal changes and 804 (68.7%) normal histology. The most common non-atrophic findings were non-specific inflammation (n = 19) and intraepithelial lymphocytosis (n = 14). Patients with non-atrophic changes presented more often with blood in stools (23.5 vs. 11.3%; p = 0.009), anemia (43.2 vs. 36.5%; p = 0.028) and positive celiac serology (34.3 vs. 12.9%; p < 0.001) than those with a normal duodenum. Twenty-four (44%) of those with non-atrophic changes received an initial diagnosis, the most common of which were inflammatory bowel disease (IBD) (n = 8), Helicobacter pylori infection (n = 3) and food allergy (n = 3). The prevalence of the diagnoses did not differ from those with a normal duodenum. Those who received a diagnosis had more often blood in stools (37.5 vs. 11.1%; p = 0.027), anemia (70.6 vs. 20.0%; p = 0.002) and negative celiac serology (50.0 vs. 7.7%; p = 0.013) than those without diagnosis. During a follow-up of 6.1–13.3 years, five of the 12 initially undiagnosed seropositive patients developed celiac disease, and one patient also developed ulcerative colitis.ConclusionNon-atrophic duodenal changes are relatively common and associated with anemia, blood in stools, and positive celiac disease serology. Excluding potential celiac disease, those without an initial diagnosis have a favorable long-term prognosis.

Receiving polygenic risk estimates of future disease through health care or direct-to-consumer companies is expected to become more common in the coming decades. However, only a limited number of studies have examined if such estimates might evoke an adverse psychosocial reaction in receivers. The present study utilized data from a sub-section of a personalized medicine project (the P5 study) that combines genomic and traditional health data to evaluate participants’ risk for certain common diseases. We investigated how communication of future disease risk estimates related to type 2 diabetes and coronary heart disease influenced respondents’ risk perception, self-efficacy, disease-related worry, and other emotions. A randomized controlled trial was conducted, where the experimental group (n = 714) received risk estimates based on traditional and polygenic risk factors and the control group (n = 649) based solely on traditional risk factors. On average, higher disease risk was associated with higher perceived risk (ps, 0.06).

Nodular lymphocyte-predominant Hodgkin lymphoma with IgD+ lymphocyte-predominant (LP) cells is a rare clinical distinct lymphoma subset of B-cell origin. Here the authors show that antigens expressed by Moraxella catarrhalis are recognized by B cell receptors of IgD+ LP cells, suggesting the contribution of chronic antigen stimulation to lymphomagenesis.

14. Composition of Culturable Microorganisms in Dusts Collected from Sport Facilities in Finland during the COVID-19 Pandemic

Sport facilities represent extreme indoor environments due to intense cleaning and disinfection. The aim of this study was to describe the composition of the cultivated microbiota in dust samples collected in sport facilities during the COVID-19 pandemic. A dust sample is defined as the airborne dust sedimented on 0.02 m2 within 28 d. The results show that the microbial viable counts in samples of airborne dust (n = 9) collected from seven Finnish sport facilities during the pandemic contained a high proportion of pathogenic filamentous fungi and a low proportion of bacteria. The microbial viable counts were between 14 CFU and 189 CFU per dust sample. In seven samples from sport facilities, 20–85% of the microbial viable counts were fungi. Out of 123 fungal colonies, 47 colonies belonged to the potentially pathogenic sections of Aspergillus (Sections Fumigati, Nigri, and Flavi). Representatives of each section were identified as Aspergillus fumigatus, A. flavus, A. niger and A. tubingensis. Six colonies belonged to the genus Paecilomyces. In six samples of dust, a high proportion (50–100%) of the total fungal viable counts consisted of these potentially pathogenic fungi. A total of 70 isolates were considered less likely to be pathogenic, and were identified as Aspergillus section Nidulantes, Chaetomium cochliodes and Penicillium sp. In the rural (n = 2) and urban (n = 7) control dust samples, the microbial viable counts were >2000 CFU and between 44 CFU and 215 CFU, respectively, and consisted mainly of bacteria. The low proportion of bacteria and the high proportion of stress tolerant, potentially pathogenic fungi in the dust samples from sport facilities may reflect the influence of disinfection on microbial communities.

15. Supply Chain Actors’ Perspectives Concerning the Cultivation of Specialty Wood-Decay Mushrooms in Finland

The objective of this paper was to understand the perspectives of multiple stakeholders on the adoption of specialty wood-decay mushroom cultivation on stumps in connection with timber harvesting in Finland. The perspectives of supply chain actors, i.e., forest owners, forest harvesting entrepreneurs, forest professionals, and natural product entrepreneurs, were assessed using web-based questionnaires. Forest professionals (34%) were most interested in specialty mushroom cultivation in co-operation with companies selling cultures or buying mushrooms, whereas about one-fourth of the forest owners (23%) indicated that interest. Forest owners were willing to do the cultivation by themselves and to lease their forests for mushroom cultivation. Compared to household users, forest owners who had commercialised the non-timber forest products (NTFPs) of their forests or were willing to sell licences for NTFP picking and gathering permits were more interested in mushroom cultivation. Additionally, background information on forest owners, such as part-time forestry entrepreneurship, younger age, male gender, and higher education, significantly increased the odds of indicating interest in participating in the supply chain. One-fourth of the harvesting entrepreneurs (25%) indicated an interest in marketing and providing specialty mushroom cultivation services to forest owners in timber harvesting. Specialty mushrooms were already used by 26% of natural product entrepreneurs, and 40% indicated that specialty mushrooms could be used in the future. The shortage of information on, for example, cultivation success, yield, and costs, as well as profitability analyses, was acknowledged by all supply chain actors as the most serious barrier inhibiting their partnership in the production and utilisation of specialty mushrooms.

We study the multifrequency emission and spectral properties of the quasar 3C 279. We observed 3C 279 in very high energy (VHE, E>100GeV) gamma rays, with the MAGIC telescopes during 2011, for the first time in stereoscopic mode. We combine these measurements with observations at other energy bands: in high energy (HE, E>100MeV) gamma rays from Fermi-LAT, in X-rays from RXTE, in the optical from the KVA telescope and in the radio at 43GHz, 37GHz and 15GHz from the VLBA, Mets\"ahovi and OVRO radio telescopes and optical polarisation measurements from the KVA and Liverpool telescopes. During the MAGIC observations (February to April 2011) 3C 279 was in a low state in optical, X-ray and gamma rays. The MAGIC observations did not yield a significant detection. These upper limits are in agreement with the extrapolation of the HE gamma-ray spectrum, corrected for extragalactic background light absorption, from Fermi-LAT. The second part of the MAGIC observations in 2011 was triggered by a high activity state in the optical and gamma-ray bands. During the optical outburst the optical electric vector position angle rotatated of about 180 degrees. There was no simultaneous rotation of the 43GHz radio polarisation angle. No VHE gamma rays were detected by MAGIC, and the derived upper limits suggest the presence of a spectral break or curvature between the Fermi-LAT and MAGIC bands. The combined upper limits are the strongest derived to date for the source at VHE and below the level of the previously detected flux by a factor 2. Radiation models that include synchrotron and inverse Compton emissions match the optical to gamma-ray data, assuming an emission component inside the broad line region (BLR) responsible for the high-energy emission and one outside the BLR and the infrared torus causing optical and low-energy emission. Following our recent discovery of a new magnetic DQ white dwarf (WD) with CH molecular features, we report the results for the rest of the DQ WDs from our survey. We use high signal-to-noise spectropolarimetric data to search for magnetic fields in a sample of 11 objects. One object in our sample, WD1235+422, shows the signs of continuum circular polarization that is similar to some peculiar DQs with unidentified molecular absorption bands, but the low S/N and spectral resolution of these data make more observations necessary to reveal the true nature of this object., Comment: 4 pages, 4 figures. We present a method for communicating personalized genetic risk information to citizens and their physicians using a secure web portal. We apply the method for 3,177 Finnish individuals in the P5 Study where estimates of genetic and absolute risk, based on genetic and clinical risk factors, of future disease are reported to study participants, allowing individuals to participate in managing their own health. Our method facilitates using polygenic risk score as a personalized tool to estimate a person’s future disease risk while offering a way for health care professionals to utilize the polygenic risk scores as a preventive tool in patient care.

Viable airborne pathogenic fungi represent a potential health hazard when exposing vulnerable persons in quantities exceeding their resilience. In this study, 284 indoor fungal isolates from a strain collection of indoor fungi were screened for pathogenic potential through the ability to grow in neutral pH at 37 °C and 30 °C. The isolates were collected from 20 locations including 14 problematic and 6 non-problematic ordinary buildings. Out of the screened isolates, 170 isolates were unable to grow at 37 °C, whereas 67 isolates growing at pH 7.2 at 37 °C were considered as potential opportunistic pathogens. Forty-seven isolates growing at 30 °C but not at 37 °C were considered as less likely pathogens. Out of these categories, 33 and 33 strains, respectively, were identified to the species level. The problematic buildings included known opportunistic pathogens: Aspergillus calidoustus, Trichoderma longibrachiatum, Rhizopus arrhizus and Paecilomyces variotii, as well as less likely pathogens: Aspergillus versicolor, Chaetomium cochliodes, Chaetomium globosum and Chaetomium rectangulare. Opportunistic pathogens such as Aspergillus flavus, Aspergillus fumigatus, Aspergillus niger and Aspergillus tubingensis and less likely pathogens such as Aspergillus westerdijkiae, Chaetomium globosum and Dichotomopilus finlandicus were isolated both from ordinary and from problematic buildings. Aspergillus was the dominant, most diverse genus found during screening for potentially pathogenic isolates in the indoor strain collection. Studies on Aspergillus niger and Aspergillus calidodoustus revealed that tolerance to cleaning chemicals may contribute to the adaptation of Aspergillus species to indoor environments.

I will propose a new way of advancing white dwarf research. Open science is a method of doing research that lets everyone who has something to say about the subject take part in the problem solving process. Already now, the amount of information we gather from observations, theory and modelling is too vast for any one individual to comprehend and turn into knowledge. And the amount of information just keeps growing in the future. A platform that promotes sharing of thoughts and ideas allows us to pool our collective knowledge of white dwarfs and get a clear picture of our research field. It will also make it possible for researchers in fields closely related to ours (AGB stars, planetary nebulae etc.) to join the scientific discourse. In the first stage this project would allow us to summarize what we know and what we don't, and what we should search for next. Later, it could grow into a large collaboration that would have the impact to, for example, suggest instrument requirements for future telescopes to satisfy the needs of the white dwarf community, or propose large surveys. A simple implementation would be a wiki page for collecting knowledge combined with a forum for more extensive discussions. These would be simple and cheap to maintain. We present spectropolarimetric observations and modelling of 12 DQ white dwarfs. Modelling is based on the method presented in Berdyugina et al. (2005). We use the model to fit the C_2 absorption bands to get atmospheric parameters in different configurations, including stellar spots and stratified atmospheres, searching for the best possible fit. Aims. The intensity profiles of the C_2 Swan bands in cool DQ white dwarfs cannot be adequately fitted with models that otherwise succesfully reproduce spectral features of the molecule CH in these stars. Initial modelling showed that a two-component atmosphere in the style of a spot might be able to solve the problem. Methods. We photometrically observed the two cool DQ white dwarfs GJ1117 and EGGR78 to search for variability caused by stellar spots. Results. We have not found any such variability, but we estimate the effects of hypothetical spots on lightcurves. We also estimate detection probabilities for spots in different configurations. We report a discovery of the circularly polarized CH $A^2\Delta-X^2\Pi$ and $B^2\Sigma^- -X^2\Pi$ molecular bands in the spectrum of the DQ white dwarf GJ 841B. This is only the second such object since the discovery of G99-37 in the 1970s. GJ 841B is also the first WD to unambiguously show polarization in the C_2 Swan bands. By modeling the intensity and circular polarization in the CH bands we determine the longitudinal magnetic field strength of 1.3 +- 0.5 MG and the temperature of 6100 +- 200 K in the absorbing region. We also present new observations of G99-37 and obtain estimates of the magnetic field strength 7.3 +- 0.3 MG and temperature 6200 +- 200 K, in good agreement with previous results., Comment: 10 pages, 3 figures

25. V405 Aurigae: A High Magnetic Field Intermediate Polar

Our simultaneous multicolor (UBVRI) circular polarimetry has revealed nearly sinusoidal variation over the WD spin cycle, and almost symmetric positive and negative polarization excursions. Maximum amplitudes are observed in the B and V bands (+-3 %). This is the first time that polarization peaking in the blue has been discovered in an IP, and suggests that V405 Aur is the highest magnetic field IP found so far. The polarized flux spectrum is similar to those found in polars with magnetic fields in the range B ~ 25-50 MG. Our low resolution circular spectropolarimetry has given evidence of transient features which can be fitted by cyclotron harmonics n = 6, 7, and 8, at a field of B = 31.5 +- 0.8 MG, consistent with the broad-band polarized flux spectrum. Timings of the circular polarization zero crossovers put strict upper limits on WD spin period changes and indicate that the WD in V405 Aur is currently accreting closely at the spin equilibrium rate, with very long synchronization timescales, T_s > 10^9 yr. For the observed spin to orbital period ratio, P_{spin}/P_{orb} = 0.0365, and P_{orb} ~ 4.15 hr, existing numerical accretion models predict spin equilibrium condition with B ~ 30 MG if the mass ratio of the binary components is q_1 ~ 0.4. The B-cell architecture of nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL) is complex since it is composed of malignant lymphocyte-predominant cells along with a rich B-cell bystander environment. To gain insight into molecular determinants of disease transformation, we studied B-cell evolutionary trajectories in lymphoma tissue from diagnosis to relapse or transformation to non- Hodgkin lymphoma by next-generation sequencing of immunoglobulin heavy chains. Patients with NLPHL that later transformed were older and showed IgD negativity, absence of the characteristic IGHV3/IGHD3/IGHJ6 lymphocyte-predominant rearrangement and high repertoire clonality. We constructed phylogenetic trees within the compartment of the malignant clone to investigate clonal evolution. In all relapsing cases, the lymphocyte-predominant rearrangement was identical at diagnosis and relapse. NLPHL cases with transformation showed more complex trajectories with strong intraclonal diversification. The dominant founder clone in transformations showed clonal evolution if derived from the same cell of origin, or arose from a different cell of origin. Together, our data point to a significant role of antigenic drive in the transformation of NLHPL and identify high B-cell repertoire clonality with dominant intraclonal lymphocyte-predominant cell diversification as a hallmark of transformation. Sequencing of initial paraffin-embedded tissue may therefore be applied diagnostically to identify NLPHL cases with high risk of transformation.

Objectives This study investigated the frequency of electronic media (e-media) usage by preschool children and the risks of high-dose e-media use on young children’s psychosocial well-being.Design Longitudinal associations between e-media use at 18 months and psychosocial symptoms at 5 years of age were studied, as well as cross-sectional associations between e-media use and psychosocial symptoms at 5 years.Setting Between 2011 and 2017 in Finland.Participants Children aged 5 years (n=699).Primary and secondary outcome measures Children’s psychosocial symptoms were determined at the age of 5 years using the parent-reported questionnaires Five-to-Fifteen (FTF) and the Strengths and Difficulties Questionnaire (SDQ).Results Based on our results, 95% of the preschool children exceeded the daily recommended use of e-media set by health professionals. Purpose: This study examined what support cancer patients and their close ones need and how this support should be organized when developing cancer care pathways. The study focused on the opinions of professionals of the Cancer Society of Finland (CSF), who play a central role in presenting the third sector’s perspective on care pathways. Method: Six semi-structured group interviews were carried out with counselling nurses (n = 12) and managers (n = 9) of the CSF during summer 2017. The results were analysed using content analysis. Results: Both patients and their close ones need more information, psychosocial support and financial counselling after diagnosis, during rehabilitation and follow-up, at relapse and during the palliative care phase; additionally, close ones require support after the patient’s death. Participants emphasized close collaboration between public healthcare and the CSF to meet the needs of patients and their close ones. Conclusion: Psychosocial support can—and should—be provided as part of the care pathway. This support can be provided by organizations in the third sector, such as the CSF, which have resources in this area.

Reported indoor air quality (IAQ) complaints are common even in relatively new or renovated school buildings in Finland. However, detecting the causes for complaints with commonly used indoor air measurements is difficult. This study presents data on perceived and measured IAQ in six comprehensive school buildings in Finland. The aim of this study was to discover the possible differences of perceived and measured IAQ between schools with reported IAQ complaints and schools without reported IAQ complaints. The initial categorisation of schools with (‘problematic schools’) and without (‘comparison schools’) complaints was ensured via a validated indoor climate survey and a recently developed online questionnaire, which were completed by 186 teachers and 1268 students from the six schools. IAQ measurements of physical parameters, gaseous pollutants, particulate matter and bioaerosols were conducted in four problematic school buildings (26 classrooms) and two comparison school buildings (12 classrooms). Using air sampling as well as exhaust air filters and classroom settled dust to detect the presence of elevated concentrations of airborne cultivable microbes and pathogenic, toxigenic and mycoparasitic Trichoderma strains were the most indicative methods in distinguishing problematic schools from comparison schools. Other IAQ-related measurements did not detect clear differences between problematic and comparison schools, as the concentration levels were very low. The results indicate that the complaints reported by occupants could have been related to excess moisture or mould problems that had not been found or repaired. Ventilation pressure condition investigations and simultaneous exhaust and supply air filter dust culture should be addressed precisely in future studies.

33. Role and Success of Finnish Early Childhood Education and Care in Supporting Child Welfare Clients: Perspectives from Parents and Professionals

Day care in Finland comprising care, education and teaching--called Educare or the ECEC model--can be used as an open care (community care) support measure for children whose development is at risk. The general aim of the study was to investigate whether the needs of child welfare client children and their parents are fulfilled in day care services from the perspective of parents and professionals. The questions were: (1) How are the individual needs of child protection client children met in day care? (2) In what way does day care support the parenting of child welfare client parents? (3) What kind of enabling and hindering factors, processes and mechanisms are there for good outcomes from day care for children and parents? The study uses both quantitative and qualitative approaches. The methods used were a questionnaire for parents (N = 42), group-level documentary materials from day care and social services officers about the problems faced by client children and parents, and interviews with ECEC and child welfare staff (N = 28). Parents experienced many problems, including exhaustion, poor mental health, substance abuse and parenting problems. Twenty percent of the children had individual support needs. The parents were very satisfied with the ECEC service their children were receiving and the parenting support provided. The ECEC professionals highlighted many positive changes in children in day care and they were also able to support parenting. The enabling and hindering factors for good outcomes were connected with the attitudes and problems of the clients, administrative processes within sectors, and cooperation between ECEC and child welfare staff. ECEC services can considerably increase the safety and well-being of child welfare client children and support their parents in their upbringing. The role and cooperation of ECEC and child welfare staff need clarification.

Child maltreatment is a major topic of concern in European countries. This article compares how Finnish and German school social workers (SSW) respond to child maltreatment (CM). For the purpose of this study, a case vignette was designed that implied multiple forms of CM, including child sexual abuse, the failure to provide a child with adequate nutrition and hygiene and exposure to violent environments. Semi-structured interviews were undertaken with eight SSW from Finland and Germany to examine their first reactions to the case, their professional approach and country-specific solutions. The transcribed interviews were subjected to content analysis. Concerning suspected child sexual abuse, the findings reveal that the Finnish SSW would inform the child protection services and the police directly after information regarding suspected maltreatment had been verified, whereas the German participants would seek professional advice and carry out a risk assessment. However, despite the fundamental differences between these approaches, both are in line with national legislation. Concerning the inadequate provision of nutrition, the findings reveal that German children are not provided with free school meals nationwide like Finnish children. Nonetheless, as this study shows, providing children with a free school lunch does not ensure that they are adequately provided for during the entire day. It is recommended children in both countries be provided with free school meals nationwide, to implement means-tested benefits for those who need greater support and to strengthen the education of SSW in the early identification of child neglect, in addition to fostering a holistic approach to treating clients.

T-cell/histiocyte-rich large B-cell lymphoma is a rare aggressive lymphoma showing histopathological overlap with nodular lymphocyte-predominant Hodgkin lymphoma. Despite differences in tumor microenvironment and clinical behavior, the tumor cells of both entities show remarkable similarities, suggesting that both lymphomas might represent a spectrum of the same disease. To address this issue, we investigated whether these entities share mutations. Ultra-deep targeted resequencing of six typical and 11 histopathological variants of nodular lymphocyte-predominant Hodgkin lymphoma, and nine cases of T-cell/histiocyte-rich large B-cell lymphoma revealed that genes recurrently mutated in nodular lymphocyte-predominant Hodgkin lymphoma are affected by mutations at similar frequencies in T-cell/histiocyte-rich large B-cell lymphoma. The most recurrently mutated genes were JUNB, DUSP2, SGK1, SOCS1 and CREBBP, which harbored mutations more frequently in T-cell/histiocyte-rich large B-cell lymphoma and the histopathological variants of nodular lymphocyte-predominant Hodgkin lymphoma than in its typical form. Mutations in JUNB, DUSP2, SGK1 and SOCS1 were highly enriched for somatic hypermutation hotspot sites, suggesting an important role of aberrant somatic hypermutation in the generation of these somatic mutations and thus in the pathogenesis of both lymphoma entities. Mutations in JUNB are generally rarely observed in malignant lymphomas and thus are relatively specific for nodular lymphocyte-predominant Hodgkin lymphoma and T-cell/histiocyte-rich large B-cell lymphoma at such high frequencies (5/17 and 5/9 cases with JUNB mutations, respectively). Taken together, the findings of the present study further support a close relationship between T-cell/histiocyte-rich large B-cell lymphoma and nodular lymphocyte-predominant Hodgkin lymphoma by showing that they share highly recurrent genetic lesions.

Adolescents who need both child welfare services and mental health interventions have usually experienced various traumatic events in their early lifespan. The purpose of this study is to identify the elements of communication that can either empower traumatized adolescents in their rehabilitation process or become an obstacle in their recovery. The data consist of interviews of 19 young adults with a substitute care background. Their experiences of communication with welfare professionals in child welfare and mental health services during adolescence are analysed using the method of content analysis in a framework of communication competence that is outlined with four elements: cognitive, affective, behavioural and ethical. The role of communication is crucial while helping the adolescents. The trauma-informed capabilities of welfare professionals should be strengthened. Future research is needed on the processes, enabling the voice of this vulnerable clientele to be heard, here and in previous stages of services. [ABSTRACT FROM AUTHOR]

Objective. Insulinomas are rare pancreatic tumours. Population-based data on their incidence, clinical picture, diagnosis, and treatment are almost nonexistent. The aim of this study was to clarify these aspects in a nationwide cohort of insulinoma patients diagnosed during three decades. Design and Methods. Retrospective analysis on all adult patients diagnosed with insulinoma in Finland during 1980–2010. Results. Seventy-nine patients were diagnosed with insulinoma over the research period. The median follow-up from diagnosis to last control visit was one (min 0, max 31) year. The incidence increased from 0.5/million/year in the 1980s to 0.9/million/year in the 2000s (p=0.002). The degree of dysplasia is the most important prognostic factor for patients with resected intraductal papillary mucinous neoplasms. Intraductal papillary mucinous neoplasms are predominantly premalignant conditions; in most cases, surveillance is an adequate treatment. If worrisome features are present, surgery should be considered. However, there is limited data on the long-term prognosis of resected intraductal papillary mucinous neoplasms. We aimed to ascertain the nationwide survival of patients with resected intraductal papillary mucinous neoplasms and identify factors associated with survival. This is a retrospective nationwide cohort study. All intraductal papillary mucinous neoplasms operated on in Finland between 2000 and 2008 were identified. Patient records were evaluated, and original radiologic data and histologic samples were re-evaluated. Survival data were collected after a 10-year follow-up period. Out of 2,024 pancreatic resections, 88 were performed for intraductal papillary mucinous neoplasm. The median age of the patients was 65 years. Histologic diagnoses were main duct intraductal papillary mucinous neoplasm 47/88 (53,4%), mixed-type intraductal papillary mucinous neoplasm 27/88 (30.7%), and branchduct intraductal papillary mucinous neoplasm 14/88 (15.9%). Of the tumors, 40/88 (45.5%) were low-grade dysplasia, 9/88 (10.2%) high-grade, and 39/88 (44.3%) were invasive cancer. The median survival was 121 (range 0–252) months. Ten-year survival was 72.5%, 66.7%, and 23.1% in the low-grade dysplasia, high-grade dysplasia, invasive cancer groups, respectively. Ten-year mortality for pancreatic cancer was 5%, 9.1%, and 71.8% in the low-grade dysplasia, high-grade dysplasia, invasive cancer groups, respectively. Overall, 44.3% of the patients had a malignant tumor, and three-quarters (74.5%) of the main duct intraductal papillary mucinous neoplasms were malignant or high-grade dysplasia at the time of surgery. Ten-year survival was significantly better in patients operated on at the stage of a premalignant tumor (low-grade dysplasia + high-grade dysplasia) than in patients operated on at the stage of a malignant tumor. 46. Children's Participation in Child-Protection Processes as Experienced by Foster Children and Social Workers

- Abstract
Children in foster care often have no means of influencing matters that concern them, and can easily become outsiders in their own lives. The United Nations Convention on the Rights of the Child enshrines the rights of capable children to express their views freely in matters affecting them and to be heard in any judicial or administrative proceedings concerning them. The aim of this study is to analyse foster children's participation in child welfare processes in different time periods and contexts from the perspective of children and social workers. The data comprise semi-structured interviews of eight children and young people aged seven to 17 in family foster care, as well as interviews of four child welfare social workers. Ethical questions were taken carefully into account. The results suggest that participation in matters concerning them is very significant to children, although they do not always want be active participants (e.g. in meetings). The children hoped that social workers would take a genuine interest in them, listen to them and take their opinions and wishes into consideration. Children sought true and essential information about the reasons for placements and the plans for their future. They felt they were better heard and more able to influence their own affairs after being placed in a foster home than during earlier phases of services. The study identifies many obstacles in children's participation at different systemic levels. Children's loyalty to their parents may prevent them from expressing their opinions. According to social workers, the most serious obstacles in participatory work with children are related to a lack of human and time resources. Social workers need time, work practices, skills and practical wisdom through which children's personal experiences, opinions and wishes can be better heard. They also seek support in handling the emotional aspects of child-protection work and suggest some other measures to develop their work. (Contains 1 table and 3 figures.)

Synchrony and asynchrony are essential aspects of the functioning of interconnected neuronal cells and networks. New information on neuronal synchronization can be expected to aid in understanding these systems. Synchronization provides insight in the functional connectivity and the spatial distribution of the information processing in the networks. Synchronization is generally studied with time domain analysis of neuronal events, or using direct frequency spectrum analysis, e.g. in specific frequency bands. However, these methods have their pitfalls. Thus, we have previously proposed a method to analyze temporal changes in the complexity of the frequency of signals originating from different network regions. The method is based on the correlation of time varying spectral entropies (SEs). SE assesses the regularity, or complexity, of a time series by quantifying the uniformity of the frequency spectrum distribution. It has been previously employed, e.g., in electroencephalogram analysis. Here, we revisit our correlated spectral entropy method (CorSE), providing evidence of its justification, usability, and benefits.Here, CorSE is assessed with simulations and in vitro microelectrode array (MEA) data. CorSE is first demonstrated with a specifically tailored toy simulation to illustrate how it can identify synchronized populations. To provide a form of validation, the method was tested with simulated data from integrate-and-fire model based computational neuronal networks. To demonstrate the analysis of real data, CorSE was applied on in vitro MEA data measured from rat cortical cell cultures, and the results were compared with three known event based synchronization measures. Finally, we show the usability by tracking the development of networks in dissociated mouse cortical cell cultures.The results show that temporal correlations in frequency spectrum distributions reflect the network relations of neuronal populations. In the simulated data, CorSE unraveled the synchronizations. With the real in vitro MEA data, CorSE produced biologically plausible results. Since CorSE analyses continuous data, it is not affected by possibly poor spike or other event detection quality. We conclude that CorSE can reveal neuronal network synchronization based on in vitro MEA field potential measurements. CorSE is expected to be equally applicable also in the analysis of corresponding in vivo and ex vivo data analysis.

Evolutionary origin and physiological significance of the tetrodotoxin (TTX) resistance of the vertebrate cardiac Na+ current (INa) is still unresolved. To this end, TTX sensitivity of the cardiac INa was examined in cardiac myocytes of a cyclostome (lamprey), three teleost fishes (crucian carp, burbot and rainbow trout), a clawed frog, a snake (viper) and a bird (quail). In lamprey, teleost fishes, frog and bird the cardiac INa was highly TTX-sensitive with EC50-values between 1.4 and 6.6 nmol·L−1. In the snake heart, about 80% of the INa was TTX-resistant with EC50 value of 0.65 μmol·L−1, the rest being TTX-sensitive (EC50 = 0.5 nmol·L−1). Although TTX-resistance of the cardiac INa appears to be limited to mammals and reptiles, the presence of TTX-resistant isoform of Na+ channel in the lamprey heart suggest an early evolutionary origin of the TTX-resistance, perhaps in the common ancestor of all vertebrates.

50. Online Questionnaire as a Tool to Assess Symptoms and Perceived Indoor Air Quality in a School Environment

School environments are a complex entirety where various different exposure factors are related that contribute to the indoor air quality (IAQ) and may affect occupants’ health and well-being. Indoor air questionnaires are useful for collecting information about the occupants’ experiences and perceptions of the indoor air and for evaluating the results of the measures taken. A common way to implement health questionnaires is to ask the respondents to describe symptoms at certain time points, such as weeks or months. The aim of our study was to develop a short and easy online questionnaire to assess symptoms and perceived IAQ. We also aimed to test the usability of the questionnaire in school buildings and assess the differences between the online measurement data (CO2, T, and RH) and the IAQ complaints and symptoms reported by the pupils. A total of 105 teachers and 1268 pupils in 36 classrooms at six schools answered the questionnaires over a two-week period. The participants completed the questionnaire always after the lesson in the studied classroom. We received 719 answers from the teachers and 6322 answers from the pupils. The results demonstrated that the teachers reported more IAQ problems and symptoms than the pupils did. Differences between classrooms were observed in both the IAQ problem and reference schools. The most common significant differences (p-value > 0.05) between the classrooms were among humidity, too cold air, and stuffy air, and among symptoms, dry/sore throat, tiredness, headache, and skin symptoms. Maximum values of CO2 measurements and the highest prevalence of stuffy air were relatively consistent. The testing process demonstrated that such a questionnaire was suitable for adults and children aged at least 12 years. The results of our study suggest that a quick and easy online questionnaire that is completed within a short period may be useful for gathering valuable knowledge about perceived IAQ. It could be used in combination with other indoor environment investigations to produce detailed results and restorative measures.
