Protologue descriptions and original material of Agaricus cespitosus, Ag. oniscus, and Ag. sphagnicola were studied to determine the intended species concept for each. Agaricus cespitosus fits the second most common presentation of the current Lichenomphalia umbellifera and is placed in synonymy with it. Agaricus oniscus was created as a synonym of Ag. cespitosus, confirmed by its original material; therefore, it is also placed in synonymy with L. umbellifera, resolving problems noted with past attempts trying to apply the epithet to darker sphagnicolous species of Arrhenia. Careful analysis of the protologue for Ag. sphagnicola revealed a good fit with L. umbellifera but serious conflict if applied to species of sphagnicolous Arrhenia ; it was, therefore, also synonymized with L. umbellifera. Agaricus cespitosus and Ag. oniscus are lectotypified with illustrations from their original material, Ag. oniscus is epitypified with a modern sequenced collection from Sweden, and Ag. sphagnicola neotypified with a K collection made by Berkeley, both identified as L. umbellifera.

Molecular studies of sphagnicolous arrhenias in Newfoundland and Labrador (NL) revealed four clades in Arrhenia, three obligate (two scaly capped and one smooth-capped) and one facultative (smooth-capped) sphagnophiles. Critical nomenclatural review of 16 names used for omphalinoid sphagnicolous taxa in the past left five suitable to apply to this group. One scaly capped obligate sphagnophilic clade contained the type for Arr. gerardiana and the other, the type for Clitocybe gerardiana var. fusca; the latter we introduce as the novel species Arr. bigelowii. It differed from the first by longer spores and a darkening reaction in 10% of collections. The smooth-capped third obligate sphagnophilic clade contained the types of Agaricus telmatiaeus and Omphalina fusconigra; we recombined it as Arr. telmatiaea. This is the darkest species of the group, with a more northern distribution in NL. The facultative sphagnophile was identified as Arr. philonotis, a lighter smooth-capped species also with a more northern distribution in NL. Unexpectedly, we also collected an unidentified smooth-capped facultatively sphagnophilic species of Omphalina of the O. pyxidata complex. All five species are distributed in both Europe and North America. We describe each species of Arrhenia with a sequenced type, providing new type material where needed. Overall, this study adds new sequences from over 80 specimens of sphagnicolous arrhenias to the two existing in GenBank when we began in 2006, 11 new sequences of the unidentified species of Omphalina, and several other arrhenias.

This study describes four gray or brown species of Cuphophyllus (Hygrophoraceae, Agaricales), two of them new species, restricted to arctic-alpine and northern boreal zones of North America, and relates them morphologically and phylogenetically using multigene and nuc rDNA internal transcribed spacer ITS1-5.8S-ITS (ITS barcode) analyses to their similar, known counterparts. Cuphophyllus cinerellus, epitypified here, is shown to be a pan-palearctic species with sequence-confirmed collections from Fennoscandia and easternmost Asia. Occupying a similar habitat in the Nearctic is its sister species, the morphologically similar but novel C. esteriae, so far known only from eastern North America, including Greenland. Sister to the C. cinerellus–C. esteriae lineage, and known only from boreal raised Sphagnum bogs in Newfoundland, is a new medium-sized light cinereous brown species, C. lamarum. It has a yellow stipe but is phylogenetically distant from the yellow-stiped European C. flavipes and its North American sister species, Hygrophorus pseudopallidus. As cryptic speciation was discovered within C. flavipes, we lecto- and epitypify the name and transfer H. pseudopallidus to Cuphophyllus based on ITS analysis of the holotype. We also transfer the small European Hygrocybe comosa to Cuphophyllus based on morphology. Cuphophyllus hygrocyboides is reported from North America with the first sequence-confirmed collections from arctic-alpine British Columbia and Greenland. In addition, sequencing the holotype of C. subviolaceus identifies it as the sister species to the putative C. lacmus. Both species seem to have an intercontinental distribution. In total, we add new sequences to GenBank from 37 Cuphophyllus collections, including the holotypes of C. hygrocyboides and C. subviolaceus, the two new epitypes, and the two novel species.

Pseudoomphalina kalchbrenneri is a Eurasian species. In North America the Pso. kalchbrenneri complex is represented by Pseudoomphalina anticostica sp. nov. and Pso. compressipes in the east and Pso. intermedia in the west. Pseudoomphalina farinacea and Pso. felleoides are later synonyms for Pso. compressipes. The somewhat similar Pseudolaccaria pachyphylla occupies a temperate band through Eurasia, confirmed by sequencing in both eastern and western North America. It differs from species of the Pso. kalchbrenneri complex by smaller size, finely granular pileus, non-decurrent gills, and lack of hymenial cystidia. All species of the Pso. kalchbrenneri complex are uncommon, and macroscopically similar. Pseudoomphalina kalchbrenneri and Pso. intermedia can be identified by their distribution (Europe and western North America, respectively) and lack of cystidia. The cystidiate eastern North American Pso. anticostica and Pso. compressipes can be differentiated by the smaller spores of the latter. Pseudoomphalina cokeri is the most ancestral species on the Pseudoomphalina lineage, while Clitocybe thujana and Agaricus apertus (Clitocybe/Clitocybula aperta) fall outside Pseudoomphalina and Pseudolaccaria. We add 21 new sequences to GenBank, including six types (Agaricus compressipes, Clitocybe felleoides, C. farinacea, C. intermedia, C. thujana, Agaricus apertus), and one new species (Pso. anticostica).

A molecular genetic study of the Arrhenia acerosa complex using the ITS fungal barcoding marker revealed unexpected diversity along a cascading group supporting over 20 lineages. Among these, we identified five previously described species: A. acerosa s.str., A. glauca, A. latispora, A. subglobisemen, and Rhodocybe tillii (recombined as A. tillii). We described four new species: A. fenicola from Canadian prairie grasslands, A. juncorum and A. leucotricha, both on live and dead herbaceous material in European wetlands, and A. svalbardensis from the high Arctic. All nine taxa treated here were fixed with sequenced types. In addition, we identified seven other lineages, some only represented by a single collection, requiring further study before description, and four groups of two species or more, also requiring further dissection before circumscription of their constituents. The diversity of the complex with respect to size, colour, habitat, range, distribution, and substrate preference is made more intriguing by the presence of several lineages of brown omphalinoid species, differing from the typically pleurotoid forms in this complex. We generated 97 of the 131 ITS sequences studied, adding 65 new sequences from the acerosa complex.

Molecular studies have shown the type collection of Omphalina oreades to be conspecific with a small brown basidiolichen from the Appalachian range in Newfoundland, both with 4-spored basidia. Two sequences deposited in GenBank, originally identified as O. grisella, fell in the same clade. Sequences of the type collection of Omphalia grisella, with 2-spored basidia, formed a sister clade together with two GenBank deposits, one identified as O. grisella and the other as Omphalina velutina. Omphalina oreades is recombined here as Lichenomphalia oreades comb. nov., and the species redescribed and illustrated. Sequences of the internal transcribed spacer regions of nuclear ribosomal DNA (ITS rDNA) from the algae associated with two collections of L. oreades fell within a highly supported clade with members of an undetermined species of Coccomyxa. The most abundant algal ribosomal large subunit sequence from granules at the base of a different collection matched GenBank sequences identified as Chloroidium ellipsoideum, which is probably either a free-living algal species or a partner to a species of Trapeliopsis. The second most abundant sequence matched Coccomyxa subellipsoidea and is the most likely photobiont of L. oreades. Further studies are required to elucidate the relationship of L. velutina to these taxa.

Inappropriate implantable cardioverter-defibrillator (ICD) shocks are associated with greater healthcare resource utilization, poorer quality-of-life, and higher mortality. We aimed to investigate the performance of enhanced supraventricular tachycardia (SVT) discrimination algorithms (morphology discrimination, rate stability, and sudden or chamber onset) for reducing inappropriate ICD therapies in patients with ICD/cardiac resynchronization therapy devices.This prospective, non-randomized, multicenter study (ReduceIT) study took place at 56 sites across Germany and Estonia. Adults at risk of sudden cardiac death undergoing St. Jude Medical™ ICD or CRT-D implantation were included. The primary endpoint was freedom from inappropriate ICD shock at 12 months and was analyzed in the intention to treat (ITT) and per-protocol population.Overall, 733 patients (65.9 ± 11.4 years) were included, of which 40.9% and 59.1% received a single- and dual-chamber detection device, respectively. During follow-up (median 11.9 [0-21.6] months), 96.3% of patients experienced no inappropriate therapy (ITT). The sensitivity, specificity, and accuracy for VT/VF were 91.9%, 95.5%, and 94.7%, respectively. In the per-protocol population (n = 620), the proportion of patients free from inappropriate shock at 12 months was 98.4% (n = 610; 95% CI 97.1-99.2%) and exceeded the expected value of 93% (p 0.0001) which was derived from the rates in the SPICE, ATPonFastVT, and DECREASE studies. A total of 44 patients (6.0%) died during follow-up, 19 deaths were cardiac-related which is consistent with a meta-analysis of EMPIRIC, MADIT-RIT, ADVANCE III, and PROVIDE. Serious device and procedure-related adverse effects occurred in 9.8% of patients.In ICD/CRT-D devices with advanced SVT discriminators, device programming according to clinical setting and detection chamber significantly reduces the rate of inappropriate ICD shocks without compromising patient safety. The algorithms and settings described herein have particular clinical importance and their employment may be of benefit to ICD recipients.

An uncommon, small, orange-capped Lactarius Pers. with white latex was confirmed, using molecular studies of the type specimens, to be Lactarius splendens Hesler & A.H. Sm. Phylogenetic analysis revealed that the species falls within Lactarius section Deliciosi (Fr.) Redeuilh, Verbeken & Walleyn., very closely related to L. rubrilacteus Hesler & A.H. Sm. and L. porninsis Rolland. Hitherto L. porninsis was the only white-latex species known in the otherwise orange-latex section Deliciosi; both are associated with Larix Mill.

Three species of golden chanterelles were found in Newfoundland and Labrador and were compared with other Cantharellus species by macromorphology, microscopy, and multilocus phylogenetic studies. The commonest species is a member of the C. cibarius group, usually found with Picea, and is differentiated from European C. cibarius by its pinkish-orange rather than yellow hymenium, and from both C. cibarius and C. roseocanus of the Pacific Northwest by its ITS and TEF1 sequences. We describe it as a new species, Cantharellus enelensis; published sequences extend its range to Michigan and Illinois. An uncommon species with reduced, merulioid hymenophore, found growing only with Betula, has rDNA and TEF1 sequences nearly identical to C. amethysteus, but only occasionally shows the amethyst scales on its cap characterizing that species in Europe. Ours is the first report of this species from North America. A third species was recognized by its sequences as C. camphoratus, but our collections, found with Abies balsamea, lack the odour of camphor for which this species was named and have longer and more slender spores than in the original description. This species has not been reported since its description from a single collection in Nova Scotia. All three species are edible.

Aims Non-vitamin K antagonist oral anticoagulants represent a new option for prevention of embolic events in patients with atrial fibrillation (AF). However, little is known about the impact of non-cardiac comorbidities on the efficacy and safety profile of these drugs. Methods and results In a post hoc analysis of the ENGAGE AF-TIMI 48 trial, we analysed 21 105 patients with AF followed for an average of 2.8 years and randomized to either a higher-dose edoxaban regimen (HDER), a lower-dose edoxaban regimen, or warfarin. We used the updated Charlson Comorbidity Index (CCI) to stratify the patients according to the burden of concomitant disease (CCI = 0, 1, 2, 3, and ≥4). The treatment groups were then compared for safety, efficacy, and net clinical outcomes across CCI categories. There were 32.0%, 7.3%, 42.1%, 12.7%, and 6.0% of patients with CCI scores of 0, 1, 2, 3, and ≥4, respectively. A CCI score ≥4 was associated with significantly higher rates of thromboembolic events, bleeding, and death compared to CCI = 0 (P 0.10 for each). Conclusion Although increasing CCI scores are associated with worse outcomes, the efficacy, safety, and net clinical outcomes of edoxaban vs. warfarin were independent of the degree of comorbidity present.

Morphological and molecular phylogenetic studies of true morels (Morchella) in the Canadian province of Newfoundland and Labrador resulted in the discovery of two undescribed species in the M. elata clade that we initially distinguished by the informal designations Mel-19 and Mel-36. The latter species, also collected in New Brunswick, Canada, is hitherto known only from the St Lawrence River Basin. Mel-36 is described here as a novel, phylogenetically distinct species, M. laurentiana. Before the discovery of Mel-19 in Newfoundland and Labrador, New Brunswick and Washington state it was only known from central China and central and northern Europe. Mel-19 is described here as a novel species, M. eohespera.
