OBJECTIVE: Recent publications describing lingual movement strategies within the oral cavity of brass players have established the existence of efficient and predictable movement patterns in healthy performers for a variety of performance tasks. In this study we extend the research to include the playing of large interval slurs in horn players. METHODS: Real-time MRI films at 40-msec resolution were simultaneously obtained in the sagittal and coronal planes in 9 professional horn players as they performed 2 repetitions each of 3 slur sequences spanning 1 octave, 1 octave + 3rd, and 1 octave + 5th at a mezzo forte dynamic level. Nine profile lines were overlaid on the images allowing the measurement of dorsal tongue edge movement using a customized MATLAB toolkit. Movement along lines representing the anterior, middle, and posterior oral cavity in the sagittal plane, as well as the vertical height of an air channel observed in the coronal plane, are reported. RESULTS: Both sagittal and coronal views demonstrate patterned tongue movements that narrow and widen the air channel during ascending and descending slurs, respectively. The magnitude of these movements is greater during larger intervals, though not perfectly consistent within each slur sequence. Additionally, the tongue position during notes tends to drift in the direction of the subsequent note in each sequence. We suggest that the observed movements may help to modulate air speed through the lips, possible attenuating embouchure muscle tension changes by assisting changes in lip vibration frequency.

Velum movement signals generated from real-time magnetic resonance imaging videos of thirty-five German speakers were used to investigate the physiological conditions that might promote sound change involving the development of contrastive vowel nasality. The results suggest that, in comparison to when a nasal consonant precedes a voiced obstruent, the velum gesture associated with a nasal consonant preceding a voiceless obstruent undergoes gestural rescaling and temporal rephasing. This further suggests that the diachronic development of contrastive vowel nasality comprises two stages: the first stage involves gestural shortening and realignment, while the second stage involves a trading relationship between source and effect.

The purpose of this work was to develop an acquisition and reconstruction technique for two- and three-directional (2d and 3d) phase-contrast flow MRI in real time. A previous real-time MRI technique for one-directional (1d) through-plane flow was extended to 2d and 3d flow MRI by introducing in-plane flow sensitivity. The method employs highly undersampled radial FLASH sequences with sequential acquisitions of two or three flow-encoding datasets and one flow-compensated dataset. Echo times are minimized by merging the waveforms of flow-encoding and radial imaging gradients. For each velocity direction individually, model-based reconstructions by regularized nonlinear inversion jointly estimate an anatomical image, a set of coil sensitivities and a phase-contrast velocity map directly. The reconstructions take advantage of a dynamic phase reference obtained by interpolating consecutive flow-compensated acquisitions. Validations include pulsatile flow phantoms as well as in vivo studies of the human aorta at 3 T. The proposed method offers cross-sectional 2d and 3d flow MRI of the human aortic arch at 53 and 67 ms resolution, respectively, without ECG synchronization and during free breathing. The in-plane resolution was 1.5 × 1.5 mm2 and the slice thickness 6 mm. In conclusion, real-time multi-directional flow MRI offers new opportunities to study complex human blood flow without the risk of combining differential phase (i.e., velocity) information from multiple heartbeats as for ECG-gated data. The method would benefit from a further reduction of acquisition time and accelerated computing to allow for extended clinical trials.

PURPOSE: To achieve dynamic water/fat separation and B0 field inhomogeneity mapping via model-based reconstructions of undersampled triple-echo multi-spoke radial FLASH acquisitions. METHODS: This work introduces an undersampled triple-echo multi-spoke radial FLASH sequence, which uses (i) complementary radial spokes per echo train for faster spatial encoding, (ii) asymmetric echoes for flexible and nonuniform echo spacing, and (iii) a golden angle increment across frames for optimal k-space coverage. Joint estimation of water, fat, B0 inhomogeneity, and coil sensitivity maps from undersampled triple-echo data poses a nonlinear and non-convex inverse problem which is solved by a model-based reconstruction with suitable regularization. The developed methods are validated using phantom experiments with different degrees of undersampling. Real-time MRI studies of the knee, liver, and heart are conducted without prospective gating or retrospective data sorting at temporal resolutions of 70, 158, and 40 ms, respectively. RESULTS: Up to 18-fold undersampling is achieved in this work. Even in the presence of rapid physiological motion, large B0 field inhomogeneities, and phase wrapping, the model-based reconstruction yields reliably separated water/fat maps in conjunction with spatially smooth inhomogeneity maps. CONCLUSIONS: The combination of a triple-echo acquisition and joint reconstruction technique provides a practical solution to time-resolved and motion robust water/fat separation at high spatial and temporal resolution.

It has been suggested that the development of contrastive vowel nasality in VN sequences may depend partly on the nature of the following consonant. In particular, there may be a preference for VN sequences preceding voiceless oral consonants to be phonologized due to aerodynamic constraints on velum height, resulting in temporal overlap of the vowel with a durationally constant velum gesture. We investigate the phonetic basis of this claim via direct imaging of velum kinematics in real-time MRI videos (50 fps) from 35 German speakers. The results show that, while the velum gesture does indeed begin and end earlier in /Vnt/ than in /Vnd/ sequences, the duration of the gesture itself is also shorter in this context. This suggests that increased temporal co-articulation in /Vnt/ sequences is not necessarily due to durational maintenance of the velum gesture, but to a temporally truncated velum gesture that is shifted in time.

PURPOSE: To develop a model-based reconstruction technique for single-shot T1 mapping with high spatial resolution, accuracy, and precision using an inversion-recovery (IR) fast low-angle shot (FLASH) acquisition with radial encoding. METHODS: The proposed model-based reconstruction jointly estimates all model parameters, that is, the equilibrium magnetization, steady-state magnetization, 1/ T1*, and all coil sensitivities from the data of a single-shot IR FLASH acquisition with a small golden-angle radial trajectory. Joint sparsity constraints on the parameter maps are exploited to improve the performance of the iteratively regularized Gauss-Newton method chosen for solving the nonlinear inverse problem. Validations include both a numerical and experimental T1 phantom, as well as in vivo studies of the human brain and liver at 3 T. RESULTS: In comparison to previous reconstruction methods for single-shot T1 mapping, which are based on real-time MRI with pixel-wise fitting and a model-based approach with a predetermination of coil sensitivities, the proposed method presents with improved robustness against phase errors and numerical precision in both phantom and in vivo studies. CONCLUSION: The comprehensive model-based reconstruction with L1 regularization offers rapid and robust T1 mapping with high accuracy and precision. PURPOSE: To develop a model-based reconstruction technique for real-time phase-contrast flow MRI with improved spatiotemporal accuracy in comparison to methods using phase differences of two separately reconstructed images with differential flow encodings. METHODS: The proposed method jointly computes a common image, a phase-contrast map, and a set of coil sensitivities from every pair of flow-compensated and flow-encoded datasets obtained by highly undersampled radial FLASH. Real-time acquisitions with five and seven radial spokes per image resulted in 25.6 and 35.7 ms measuring time per phase-contrast map, respectively. The signal model for phase-contrast flow MRI requires the solution of a nonlinear inverse problem, which is accomplished by an iteratively regularized Gauss-Newton method. Aspects of regularization and scaling are discussed. The model-based reconstruction was validated for a numerical and experimental flow phantom and applied to real-time phase-contrast MRI of the human aorta for 10 healthy subjects and 2 patients. RESULTS: Under all conditions, and compared with a previously developed real-time flow MRI method, the proposed method yields quantitatively accurate phase-contrast maps (i.e., flow velocities) with improved spatial acuity, reduced phase noise and reduced streaking artifacts. CONCLUSION: This novel model-based reconstruction technique may become a new tool for clinical flow MRI in real time.

Purpose To design, evaluate, and apply a bow tie antenna transceiver radiofrequency (RF) coil array tailored for cardiac MRI at 7.0 Tesla (T). Methods The radiofrequency (RF) coil array comprises 16 building blocks each containing a bow tie shaped λ/2-dipole antenna. Numerical simulations were used for transmission field homogenization and RF safety validation. RF characteristics were examined in a phantom study. The array's suitability for high spatial resolution two-dimensional (2D) CINE imaging and for real time imaging of the heart was examined in a volunteer study. Results The arrays transmission fields and RF characteristics are suitable for cardiac MRI at 7.0T. The coil performance afforded a spatial resolution as good as (0.8 × 0.8 × 2.5) mm3 for segmented 2D CINE MRI at 7.0T which is by a factor of 12 superior versus standardized protocols used in clinical practice at 1.5T. The proposed transceiver array supports 1D acceleration factors of up to R = 6 without impairing image quality significantly. Conclusion The 16-channel bow tie antenna transceiver array supports accelerated and high spatial resolution cardiac MRI. The array is compatible with multichannel transmission and provides a technological basis for future clinical assessment of parallel transmission techniques at 7.0 Tesla.

Purpose: To provide multidimensional velocity compensation for real-time phase-contrast flow MRI. Methods: The proposed method introduces asymmetric gradi- ent echoes for highly undersampled radial FLASH MRI with phase-sensitive image reconstruction by regularized nonlinear inversion (NLINV). Using an adapted gradient delay correction the resulting image quality was analyzed by simulations and experimentally validated at 3 Tesla. For real-time flow MRI the reduced gradient-echo timing allowed for the incorporation of velocity-compensating waveforms for all imaging gradients at even shorter repetition times. Results: The results reveal a usable degree of 20% asymme- try. Real-time flow MRI with full velocity compensation elimi- nated signal void in a flow phantom, confirmed flow parameters in healthy subjects and demonstrated signal recovery and phase conservation in a patient with aortic valve insufficiency and stenosis. Exemplary protocols at 1.4–1.5 mm resolution and 6 mm slice thickness achieved total acquisition times of 33.3–35.7 ms for two images (7 spokes each) with and without flow-encoding gradient. Conclusion: Asymmetric gradient echoes were successfully implemented for highly undersampled radial trajectories. The resulting temporal gain offers full velocity compensation for real-time phase-contrast flow MRI which minimizes false- positive contributions from complex flow and further enhances the temporal resolution compared with acquisitions with symmetric echoes.

Purpose To develop a method for spoiling transverse magnetizations without additional gradients to minimize repetition times for radial fast low angle shot (FLASH) MRI. Methods Residual steady state transverse magnetizations and corresponding image artifacts were analyzed for radial gradient echo sequences with constant and randomized RF phases in comparison with a sequence with refocused frequency-encoding gradients, constant spoiler gradient, and conventional RF spoiling (gold standard). The spoiling performance was assessed for different radial trajectories using numerical simulations, phantom experiments, and in vivo MRI studies of the human brain. Results Simulations as well as phantom and in vivo measurements reveal a highly efficient spoiling capacity for randomized RF phases and radial FLASH sequences without the need for gradient rewinding and spoiler gradients. The data also demonstrate a strong dependence of the spoiling performance on the chosen radial trajectory (ie, the azimuthal angular increment between successive projections) with excellent results for an interleaved multiturn scheme. Conclusion Effective spoiling of transverse magnetizations in radial FLASH MRI may be achieved by randomized RF phases without additional spoiler gradients. The technique allows for short repetition times as required for high-speed real-time MRI.

This prospective study of eight healthy volunteers evaluates peak flow velocities (PFV) in the ascending aorta using real-time phase-contrast magnetic resonance imaging (MRI) in comparison to cine phase-contrast MRI and echocardiography. Flow measurements by echocardiography and cine phase-contrast MRI with breath-holding were performed according to clinical standards. Real-time phase-contrast MRI at 40 ms temporal resolution and 1.3 mm in-plane resolution was based on highly undersampled radial fast low-angle shot (FLASH) sequences with image reconstruction by regularized nonlinear inversion (NLINV). Evaluations focused on the determination of PFV. Linear regressions and Bland-Altman plots were used for comparisons of methods. When averaged across subjects, real-time phase-contrast MRI resulted in PFV of 120±20 cm s(-1) (mean ± SD) in comparison to 122±16 cm s(-1) for cine MRI and 124±20 cm s(-1) for echocardiography. The maximum deviations between real-time phase-contrast MRI and echocardiography ranged from -20 to +14 cm s(-1) (cine MRI: -10 to +12 cm s(-1)). Thus, in general, real-time phase-contrast MRI of cardiac outflow revealed quantitative agreement with cine MRI and echocardiography. The advantages of real-time MRI are measurements during free breathing and access to individual cardiac cycles.

Purpose: To develop and evaluate a practical phase unwrapping method for real-time phase-contrast flow MRI using temporal and spatial continuity. Methods: Real-time phase-contrast MRI of through-plane flow was performed using highly undersampled radial FLASH with phase-sensitive reconstructions by regularized nonlinear inversion. Experiments involved flow in a phantom and the human aorta (10 healthy subjects) with and without phase wrapping for velocity encodings of 100 cm.s(-1) and 200 cm.s(-1). Phase unwrapping was performed for each individual cardiac cycle and restricted to a region of interest automatically propagated to all time frames. The algorithm exploited temporal continuity in forward and backward direction for all pixels with a "continuous" representation of blood throughout the entire cardiac cycle (inner vessel lumen). Phase inconsistencies were corrected by a comparison with values from direct spatial neighbors. The latter approach was also applied to pixels exhibiting a discontinuous signal intensity time course due to movement-induced spatial displacements (peripheral vessel zone). Results: Phantom and human flow MRI data were successfully unwrapped. When halving the velocity encoding, the velocity-to-noise ratio (VNR) increased by a factor of two. Conclusion: The proposed phase unwrapping method for real-time flow MRI allows for measurements with reduced velocity encoding and increased VNR.

This paper describes the use of high-speed real-time (RT) magnetic resonance imaging (MRI) in quantifying very rapid motor function within the oropharyngeal cavity of six elite horn players. Based on simultaneous sound recordings, the efficacy of RT-MRI films at 30 and 100 frames per second (fps) was assessed for tongue movements associated with double tonguing performance. Serial images with a nominal temporal resolution of 10.0 and 33.3 ms were obtained by highly undersampled radial fast low-angle shot (FLASH) sequences (5 and 17 spokes, respectively) using complementary sets of spokes for successive acquisitions (extending over 9 and 5 frames, respectively). Reconstructions of high-speed images were obtained by temporally regularized nonlinear inversion (NLINV) as previously described. A customized MATLAB toolkit was developed for the extraction of line profiles from MRI films to quantify temporal phenomena associated with task performance. The analyses reveal that for the present setting, which required the use of a temporal median filter to optimize image quality, acquisition rates of 30 fps are inadequate to accurately detect tongue movements during double tonguing, but that rates of 100 fps do allow for a precise quantification of movement. These data for the first time demonstrate the extreme performance of elite horn players. High-speed RT-MRI offers so far unavailable opportunities to study the oropharyngeal movements during brass playing with future potential for teaching and the treatment of patients suffering from dystonia.

Cardiac structure, function and flow are most commonly studied by ultrasound, X-ray and magnetic resonance imaging (MRI) techniques. However, cardiovascular MRI is hitherto limited to electrocardiogram (ECG)-synchronized acquisitions and therefore often results in compromised quality for patients with arrhythmias or inabilities to comply with requested protocols-especially with breath-holding. Recent advances in the development of novel real-time MRI techniques now offer dynamic imaging of the heart and major vessels with high spatial and temporal resolution, so that examinations may be performed without the need for ECG synchronization and during free breathing. This article provides an overview of technical achievements, physiological validations, preliminary patient studies and translational aspects for a future clinical scenario of cardiovascular MRI in real time.

Background: While cardiovascular magnetic resonance (CMR) commonly employs ECG-synchronized cine acquisitions with balanced steady-state free precession (SSFP) contrast at 1.5 T, recent developments at 3 T demonstrate significant potential for T1-weighted real-time imaging at high spatiotemporal resolution using undersampled radial FLASH. The purpose of this work was to combine both ideas and to evaluate a corresponding real-time CMR method at 1.5 T with SSFP contrast. Methods: Radial gradient-echo sequences with fully balanced gradients and at least 15-fold undersampling were implemented on two CMR systems with different gradient performance. Image reconstruction by regularized nonlinear inversion (NLINV) was performed offline and resulted in real-time SSFP CMR images at a nominal resolution of 1.8 mm and with acquisition times of 40 ms. Results: Studies of healthy subjects demonstrated technical feasibility in terms of robustness and general image quality. Clinical applicability with access to quantitative evaluations (e.g., ejection fraction) was confirmed by preliminary applications to 27 patients with typical indications for CMR including arrhythmias and abnormal wall motion. Real-time image quality was slightly lower than for cine SSFP recordings, but considered diagnostic in all cases. Conclusions: Extending conventional cine approaches, real-time radial SSFP CMR with NLINV reconstruction provides access to individual cardiac cycles and allows for studies of patients with irregular heartbeat.

Velocity-encoded phase-contrast MRI of cardiovascular blood flow commonly relies on electrocardiogram-synchronized cine acquisitions of multiple heartbeats to quantitatively determine the flow of an averaged cardiac cycle. Here, we present a new method for real-time phase-contrast MRI that combines flow-encoding gradients with highly undersampled radial fast low-angle shot acquisitions and phase-sensitive image reconstructions by regularized nonlinear inversion. Apart from calibration studies using steady and pulsatile flow, preliminary in vivo applications focused on through-plane flow in the ascending aorta of healthy subjects. With bipolar velocity-encoding gradients of alternating polarity that overlap the slice-refocusing gradient, the method yields flow-encoded images with an in-plane resolution of 1.8 mm, section thickness of 6 mm and measuring time at 3 T of 24 ms (TR/TE = 3.44/2.76 ms; flip angle, 10º; seven radial spokes per image). Accordingly, phase-contrast maps and corresponding velocity profiles achieve a temporal resolution of 48 ms. The evaluated peak velocities, stroke volumes, flow rates and respective variances over at least 20 consecutive heartbeats are in general agreement with literature data.

The utilization of dendrimers in medicine holds great potential in emerging applications of diagnostic imaging, as well as the promise of new capabilities for delivering therapies tailored and targeted for specific diseases. In this perspective, radiolabeled dendritic frameworks are gaining in importance, particularly for the use in tumor imaging and therapy.[1] Pegylation as well as carbohydration of radiolabeled compounds is of considerable interest to improve the pharmacokinetics, biocompatibility, and tumor accumulation.[2,3] In this context, DOTA-modified glycodendrimers with dense maltose shell (Figure: structure of 4th generation dendrimer) were selected from the point of view to use the highly homogenous dendritic structure for improved tumor imaging and therapy and to enhance the biocompatibility of dendrimers by the decoration with carbohydrates. The outcome of the synthetic effort was the synthesis of 4th and 5th generation glycodendrimers with the variation of chemically attached DOTA chelators (1, 3, 9 DOTA units/4th generation dendrimer; 9 and 18 DOTA units/5th generation dendrimer), which form stable complexes with a large number of radiometals. The multivalent decoration with DOTA on dendrimer surface was also stimulated by the facts that radiolabeling kinetics can be accelerated and to achieve enhanced specific activity. The labeling conditions of the glycodendrimers synthesized with 64Cu were optimized, and the influence of reaction time, temperature, buffer conditions and the dendrimer amount on the radiochemical yield were studied using Radio-TLC and Radio-SEC. The radiocopper(II) complexes of the DOTA-functionalized glycodendrimers show a high in vitro stability. Preliminary biodistribution studies of 64Cu-labeled 4th generation dendrimers with maltose shell in healthy Wistar rats indicate the preferred accumulation in the liver.
