Abstract

Background: To study trends in the clinical presentation, electrocardiograms, and diagnostic imaging in patients with pulmonary embolism presenting as ST segment elevation.Methods: We performed a systematic literature search for all reported cases of pulmonary embolism mimicking ST-elevation myocardial infarction. Pre-specified data such as clinical presentation, electrocardiogram changes, transthoracic echocardiographic findings, cardiac biomarkers, diagnostic imaging, therapy, and outcomes were collected.Results: We identified a total of 34 case reports. There were 23 males. Mean age of the population was 56.5 ± 15.5 years. Patients presented with dyspnea (76.4%), chest pain (63.6%), and tachycardia (71.4%). All patients presented with ST-elevations, with the most common location being in the anterior-septal distribution, lead V3 (74%), V2 (71%), V1 (62%) and V4 (47%). ST-segment elevations in the inferior distribution were present in lead II (12%), III (18%), and aVF (21%). Presentation was least likely in the lateral distribution. Troponin was elevated in 78.9% of cases. Right ventricular strain was the most common echocardiographic finding. Over 80% of patients had findings consistent with elevated right ventricular pressure, with 50% reported RV dilatation and 20% RV hypokinesis. The most commonly used imaging modality was contrast-enhanced pulmonary angiography. There was a greater incidence of bilateral compared to unilateral pulmonary emboli (72.4% vs. 10%). About 65% patients received anticoagulation and 36.3% were treated with thrombolytics. Forty-six percent of patients required intensive care and 18.7% intubation. Overall mortality was 25.8%.Conclusions: A review of the literature reveals that in patients presenting with pulmonary embolism, electrocardiogram findings of ST-segment elevations will occur predominantly in the anterior-septal distribution. Abstract

Introduction:Peripheral venous pressure (PVP) monitoring is a minimally invasive method which can be performed to assess volume status. We investigated the correlation between PVP and central venous pressure (CVP) in heart failure (HF), heart transplant (HTx), and left ventricular assist device (LVAD) patients undergoing right heart catheterization (RHC).Methods:A prospective, cross-sectional study examining PVP from October 2018 to June 2019 was conducted. The analysis was limited to patients undergoing RHC admitted for HF, post-HTx monitoring, or LVAD hemodynamic testing. PVP was transduced from an antecubital or more distal peripheral intravenous (IV) catheter site, using a standard pressure transducer. Pressures were compared using Pearson?s correlation coefficient.Results:Seventy-five patients were enrolled with a mean age of 59 ? 13 years, and a mean ejection fraction of 44 ? 22%. Forty-nine percent of patients had HF, 40% were HTx patients, and 11% were LVAD patients. Median PVP was 7 (1 - 26) mmHg, median CVP was 6 (1 - 36) mmHg, and median PCWP was 14 (5 - 40) mmHg. The PVP and CVP were found to be highly correlated (r=0.92, p < 0.0001) (Figure). High correlation was also noted when broken down by HF (r=0.92, p < 0.0001), HTx (r=0.93, p < 0.0001), and LVAD groups (r=0.95, p < 0.0003). The mean difference between PVP and CVP was 0.12 mmHg. There was a modest correlation between PVP and PCWP (r = 0.66, p < 0.0001). High degree of correlation remained by peripheral IV insertion site: antecubital (r=0.96, p<0.00001), forearm (r=0.93, p<0.00001), and hand (r=0.94, p<0.00001), as well as by peripheral IV gauge: 20G (r=0.95, p<0.00001) and 22G (r=0.86, p<0.00001).Conclusion:There is a high degree of correlation between PVP and CVP in HF, HTx, and LVAD patients. PVP measurements can be used as a reliable estimate of volume status in these patient populations.

Abstract

Introduction:Gastrointestinal bleeding (GIB) in LVAD patients is most commonly caused by gastrointestinal angiodysplasia (GIAD). Non-pulsatile blood flow in LVAD patients can cause splanchnic mucosal hypoperfusion and hypoxia leading to activation of a Hypoxia Inducible Factor (HIF)1-? /Vascular Endothelial Growth factor (VEGF)/Angiopoietins (Ang) signaling cascade, that can trigger pathological angiogenesis. We have recently shown that the use of digoxin, an inhibitor of HIF1-?, is associated with a significant reduction in GIAD related GIB in LVAD patients.Hypothesis:We hypothesize that GIAD development driven by dysregulation of VEGF, Ang-1, and Ang-2 expression in patients with LVADs can be corrected by digoxin treatment.Methods:Blood samples were collected from 13 patients 1 week before and 6 weeks after LVAD implant (3 HeartMate (HM)2, 9 HM3), and from 12 patients supported with LVAD (5 HM2 and 7 HM3 devices) before and after 2 weeks of digoxin therapy (0.125 ?g daily). VEGF, Ang-1, and Ang-2 levels were measured in patients? serum by ELISA. Human microvascular endothelial cells (HMVEC) were incubated under hypoxic (1% O2) or normoxic conditions (21% O2), with or without pretreatment with digoxin (1nM, 10nM, and 100nM). Change in HIF1-? levels was assessed by Western blot and secreted VEGF, and Ang-2 levels were measured in cell culture media by ELISA.Results:In vivo studies: VEGF (114.8 to 389.1 ng/ml, p=0.044) and Ang-2 levels increased (6.8 to 10.4 ng/ml, p=0.016), and Ang-1 levels decreased (16.2 to 10.4 ng/ml, p=0.047) after pump implantation, indicating the development of angiogenic dysregulation. Digoxin treatment in LVAD patients decreased VEGF (405.0 to 272.8 ng/ml, p=0.036) and Ang-2 (14.2 to 8.5 ng/ml, p=0.007), and increased Ang-1 levels (10.6 to 26.4 ng/ml, p=0.003), reversing angiogenic switch. In vitrostudies: Digoxin prevented, in a dose-dependent fashion, the hypoxia-induced increase in HIF-1? levels, VEGF (169.7 vs 96 pg/ml, p=0.032) and Ang-2 (63.5 vs 29 ng/ml p=0.021) secretion in HMVECs.Conclusions:Digoxin by inhibiting hypoxia triggered HIF-1? signaling cascade may reverse angiogenic switch in patients supported by LVADs. The relevance of this finding for the prevention of GIB needs to be prospectively studied.
