Estimation of the Digital Surface Model (DSM) and building heights from single-view aerial imagery is a challenging inherently ill-posed problem that we address in this paper by resorting to machine learning. We propose an end-to-end trainable convolutional-deconvolutional deep neural network architecture that enables learning mapping from a single aerial imagery to a DSM for analysis of urban scenes. We perform multisensor fusion of aerial optical and aerial light detection and ranging (Lidar) data to prepare the training data for our pipeline. The dataset quality is key to successful estimation performance. Typically, a substantial amount of misregistration artifacts are present due to georeferencing/projection errors, sensor calibration inaccuracies, and scene changes between acquisitions. To overcome these issues, we propose a registration procedure to improve Lidar and optical data alignment that relies on Mutual Information, followed by Hough transform-based validation step to adjust misregistered image patches. We validate our building height estimation model on a high-resolution dataset captured over central Dublin, Ireland: Lidar point cloud of 2015 and optical aerial images from 2017. These data allow us to validate the proposed registration procedure and perform 3D model reconstruction from single-view aerial imagery. We also report state-of-the-art performance of our proposed architecture on several popular DSM estimation datasets.

Many applications, such as autonomous navigation, urban planning, and asset monitoring, rely on the availability of accurate information about objects and their geolocations. In this paper, we propose the automatic detection and computation of the coordinates of recurring stationary objects of interest using street view imagery. Our processing pipeline relies on two fully convolutional neural networks: the first segments objects in the images, while the second estimates their distance from the camera. To geolocate all the detected objects coherently we propose a novel custom Markov random field model to estimate the objects’ geolocation. The novelty of the resulting pipeline is the combined use of monocular depth estimation and triangulation to enable automatic mapping of complex scenes with the simultaneous presence of multiple, visually similar objects of interest. We validate experimentally the effectiveness of our approach on two object classes: traffic lights and telegraph poles. The experiments report high object recall rates and position precision of approximately 2 m, which is approaching the precision of single-frequency GPS receivers.

Introduction. In patients with drug-resistant temporal lobe epilepsy (TLE), surgical treatment is aimed to resect an epileptogenic zone (EZ) followed by seizure control. Despite complicated EZ location, surgical resection should be considered as a treatment of choice in bilateral TLE. Objective: to evaluate surgical outcomes and factors contributing to outcomes in patients with drug-resistant bilateral TLE confirmed via magnetic resonance imaging. Materials and methods. The study included patients with unilateral (n = 50) and bilateral (n = 50) temporal lobe involvement. The results of surgical treatment were evaluated according to the classification of J. Engel (1993). Results. Favorable outcomes of surgical treatment (Engel I and Engel II) in the group with unilateral temporal lobe involvement were found in 98% of patients after 12 months, in 88% after 24 months, and in 100% after 48 and 60 months after surgery. In the group with bilateral temporal lobe involvement outcomes of surgical treatment were favorable in 41% of patients after 12 months, in 50% after 24 months, in 39% after 48 months, and in 50% of patients after 60 months post-surgery. Conclusion. Early onset, burdened perinatal history, and MRI-confirmed left temporal lobe involvement contribute to the poor outcome (Engel III and Engel IV) in the bilateral TLE group. Engel I outcomes were more common in the patients with unilateral TLE while Engel IIIV outcomes were more common in the patients with bilateral TLE.

Background and Aim: The cause of most strokes is associated with the pathology of the carotid arteries. Many modern researchers suggest preventive surgery in the presence of arterial deformity to prevent strokes. The hemodynamic significance of carotid deformities is determined by morphological and functional disorders at the level of tortuosity, the reaction of cerebral blood flow is not often considered. Here, we assume that cerebral autoregulation in tortuosity can be different. Methods and Materials/Patients: A total of 64 patients (31-75 years old) with 110 carotid deformities were analyzed. Duplex color mapping, computed tomography angiography of carotid arteries, and computed tomography perfusion were performed by estimating the absolute and average values of cerebral blood flow (mL/100 g/min), cerebral blood volume (mL/100 g), Mean transit time (MTT) (s) in similar areas of the cortex. In 6 patients, the acetazolamide challenge test was used to evaluate the autoregulatory disturbances. Results: According to computed tomography angiography and duplex color mapping, 18(28.1%) patients had unilateral tortuosity, and 46(71.9%) patients had bilateral tortuo s ity. Hemodynamically significant deformities were detected in 33 cases (30% of tortuosity). In 54 cases (49% of tortuosity), the deformities were accompanied by carotid stenosis. Perfusion disorders were detected in 23 of 64 patients (35.9%). In the majority of cases (75% of all perfusion disorders), hypoperfusion was diagnosed on the side corresponding to the maximum degree of stenosis, regardless of the location of the tortuosity. Neurologically significant hypoperfusion, compensated by collateral blood flow revealed only in 7.8% of cases of hemodynamic significant internal carotid artery deformity without concomitant atherosclerosis. Conclusion: The decision on surgical correction of carotid artery tortuosity should be made while considering both local changes in hemodynamics and proven violations of autoregulation of cerebral blood flow, especially in patients with concomitant carotid stenosis.

Study aim. To analyse the surgical outcomes in patients with haemorrhagic stroke depending on the timing of surgery. Materials and methods. We performed a retrospective analysis of 500 patients (335 (67%) men and 165 (33%) women), who underwent surgical treatment of hypertensive intracranial hemorrhages at the N.V. Sklifosovsky Research Institute for Emergency Medicine between 1997 and 2020. The mean patient age was 53.1 12.2 years. The mean time until surgery was 3.3 2.6 days. Outcomes were assessed on day 30 from disease onset using the modified Rankin Scale (mRS). Results. In the total sample, outcomes as measured by the mRS were as follows: type 0 84 (16.8%) patients, type 1 37 (7.4%), type 2 46 (9.2%), type 3 38 (7.6%), type 4 43 (8.6%), type 5 142 (28.4%) and type 6 110 (22.0%). Treatment results were better when surgery was delayed (2 = 64.4; p 0.00001). Mortality was 36.4% after surgery conducted in the first day after haemorrhage, while mRS scores of 02 made up 18.6%. Mortality was 20.4% after surgery conducted on the second or third day, and mRS scores of 02 made up 29.6%. Mortality was 17.4% after surgery conducted on day 47, and mRS scores of 02 outcomes were present in 49.0% of subjects. Mortality was 8.8% when surgery was performed on day 8 or later, and favourable outcomes were present in 48.5% of patients. Conclusion. Intracerebral haematoma excision on day 23 leads to better outcomes in patients with reduced levels of alertness up to sopor, while surgery after day 3 leads to better results in alert patients and those with obtundation.

In this paper, a new approach to treating infected wounds is proposed. It is based on treating the wound surface with high-intensity pulsed optical radiation with a continuous spectrum, continuously covering the entire UV range (from 200 to 400 nm) and the visible and near-infrared regions of the spectrum. A pulsed xenon lamp is used as a radiation source. A description of the design and technical characteristics of the device, “Zarnitsa-A”, is presented, implementing the proposed medical technology for wound therapy. The results of our studies of the apparatus in vitro and in vivo are also presented. The article shows that exposure to the apparatus leads to pronounced bactericidal- and wound-healing properties. The apparatus’s action reliably provides higher rates of wound healing compared with only a typical antibacterial or wound healing agent, such as “levomekol” ointment.

Introduction.The frequency of adverse outcomes in patients with severe traumatic brain injury (TBI) exceeds 25%. Epileptic seizures and vasospasm, in the absence of pathogenetic treatment, cause irreversible brain damage and thus complicate the course of severe TBI. Bedside electroencephalography (EEG) is traditionally used to diagnose these conditions. However, its low spatial resolution when recording from the scalp and a large number of artefacts that make it challenging to analyse the data. Materials and methods.Electrocorticography (ECoG) monitoring was performed using subdural electrodes implanted in the traumatic brain lesion during TBI surgery in 11 patients during the acute period of severe TBI. All patients were concurrently monitored using scalp EEG with subdermal needle electrodes. Results.Analysis of scalp recordings showed frequency disturbances and oscillation asymmetry in all patients, while sporadic epileptiform activity and rhythmic and periodic patterns were detected in 18% and 64% of subjects, respectively. Analysis of invasive EEG showed sporadic epileptiform activity in 27% of patients, while rhythmic and periodic patterns were present in 91%. Moreover, epileptiform activity was registered only by the subdural leads in 3 patients. The total percentage of subjects in whom we registered clinical and electrographic signs of convulsive and non-convulsive status epilepticus using EEG and ECoG was 55%. We found indirect EEG signs of slow-spreading cortical depolarization in one patient whose level of consciousness was coma, followed by an electrographic pattern of status epilepticus on ECoG. Conclusion.ECoG recording, while patients with severe TBI are in the intensive care unit, increases the diagnostic capabilities of this method, allowing electrographic seizures to be recorded more often and more accurately, but also to detect indirect signs of slow-spreading cortical depolarization using standard EEG amplifiers. Electrode implantation during TBI surgery is safe and does not significantly change the surgical approach.

The treatment of glial brain tumors is an unresolved problem in neurooncology, and all existing methods (tumor resection, chemotherapy, radiotherapy, radiosurgery, fluorescence diagnostics, photodynamic therapy, etc.) are directed toward increasing progression-free survival for patients. Fluorescence diagnostics and photodynamic therapy are promising methods for achieving gross total resection and additional treatment of residual parts of the tumor. However, sometimes the use of one photosensitizer for photodynamic therapy does not help, and the time until tumor relapse barely increases. This translational case report describes the preliminary results of the first combined use of 5-ALA and chlorin e6 photosensitizers for fluorescence-guided resection and photodynamic therapy of glioblastoma, which allowed us to perform total resection of tumor tissue according to magnetic resonance and computed tomography images, remove additional tissue with increased fluorescence intensity without neurophysiological consequences, and perform additional therapy. Two months after surgery, no recurrent tumor and no contrast uptake in the tumor bed were detected. Additionally, the patient had ischemic changes in the access zone and along the periphery and cystic-glial changes in the left parietal lobe.

The implementation of measures aimed to improve medical care for patients with acute cerebrovascular accidents in the Russian Federation has made it possible to establish a unified healthcare system for patients with stroke, to provide regional vascular centers with modern equipment, to train medical personnel, to introduce modern effective methods of treatment of acute cerebrovascular accidents-thrombolytic therapy and thrombus extraction. The extent of operative treatment of brain aneurysms and hemorrhagic stroke increased more than 7 times. A positive experience has been gained in the arrangement of medical care for a patient with acute cerebrovascular accident at the pre-hospital stage. Measures for reduction of mortality from the main causes are being monitored, and a geo-information map embracing the main statistical parameters of each region is uploaded into the healthcare unified state information system. In the period of 20052016, the death rate from cerebrovascular diseases decreased by more than 40%, including 41% reduction of the death rate from stroke.

Background: In humans, early pathological activity on invasive electrocorticograms (ECoGs) and its putative association with pathomorphology in the early period of traumatic brain injury (TBI) remains obscure. Methods: We assessed pathological activity on scalp electroencephalograms (EEGs) and ECoGs in patients with acute TBI, early electrophysiological changes after lateral fluid percussion brain injury (FPI), and electrophysiological correlates of hippocampal damage (microgliosis and neuronal loss), a week after TBI in rats. Results: Epileptiform activity on ECoGs was evident in 86% of patients during the acute period of TBI, ECoGs being more sensitive to epileptiform and periodic discharges. A “brush-like” ECoG pattern superimposed over rhythmic delta activity and periodic discharge was described for the first time in acute TBI. In rats, FPI increased high-amplitude spike incidence in the neocortex and, most expressed, in the ipsilateral hippocampus, induced hippocampal microgliosis and neuronal loss, ipsilateral dentate gyrus being most vulnerable, a week after TBI. Epileptiform spike incidence correlated with microglial cell density and neuronal loss in the ipsilateral hippocampus. Conclusion: Epileptiform activity is frequent in the acute period of TBI period and is associated with distant hippocampal damage on a microscopic level. This damage is probably involved in late consequences of TBI. The FPI model is suitable for exploring pathogenetic mechanisms of post-traumatic disorders.

Effective production assets of region or country are the assets that really commit in production development. According to the neoclassical rationality principle of economic agents, the effective assets have to be considered in the production functions representing the mathematical models of large production facilities (production associations, sectors, economies of regions and countries). Existence of adequate production function allows making the deep economic analysis of facility and solving planning and forecasting problems. However, the researchers have only information on the balance funds, which are often poorly reflecting the real state of economy. The indistinct effective fund concept can be formalized and assessed quantitatively within the method of the production functions (PF) on the basis of V. K. Gorbunov and A. G. Lvov’s model (2012) of the standard macroeconomic PF development, one of which factors — the capital according to the statistical information on production investments. Effective funds are estimated together with the PF parameters and the main indicators of fund dynamics: amortization and initial value. In the article, the coefficient of marketability on investment, which is a share of real use of the allocated production investments, is additionally introduced into the dynamic equation. The comprehensive assessment results of the PF parameters and effective funds for economies of the Volga and Ural Federal Districts with use of several classes of two-factorial PF are given. As the second factor, the work and energy consumption are alternatively considered. As the result of development of region PF, the macroeconomic indicators: average and marginal returns of factors, factorial elasticity, marginal rate of factor substitution, elasticity of substitution, are calculated. It increases the quality of the quantitative analysis of regional economies and objectivity of their comparison.
