Objectives: To evaluate the access to comprehensive diagnostics and novel antituberculosis medicines in European countries.Methods: We investigated the access to genotypic and phenotypic Mycobacterium tuberculosis drug susceptibility testing and the availability of antituberculosis drugs and calculated the cost of drugs and treatment regimens at major tuberculosis treatment centres in countries of the WHO European region where rates of drug-resistant tuberculosis are the highest among all WHO regions. Results were stratified by middle-income and high-income countries.Results: Overall, 43 treatment centres from 43 countries participated in the study. For WHO group A drugs, the frequency of countries with the availability of phenotypic drug susceptibility testing was as follows: (a) 75% (30/40) for levofloxacin, (b) 82% (33/40) for moxifloxacin, (c) 48% (19/40) for bedaquiline, and (d) 72% (29/40) for linezolid. Overall, of the 43 countries, 36 (84%) and 24 (56%) countries had access to bedaquiline and delamanid, respectively, whereas only 6 (14%) countries had access to rifapentine. The treatment of patients with extensively drug-resistant tuberculosis with a regimen including a carbapenem was available only in 17 (40%) of the 43 countries. The median cost of regimens for drug-susceptible tuberculosis, multidrug-resistant/rifampicin-resistant tuberculosis (shorter regimen, including bedaquiline for 6 months), and extensively drug-resistant tuberculosis (including bedaquiline, delamanid, and a carbapenem) were €44 (minimum–maximum, €15–152), €764 (minimum–maximum, €542–15152), and €8709 (minimum–maximum, €7965–11759) in middle-income countries (n = 12) and €280 (minimum–maximum, €78–1084), €29765 (minimum–maximum, €11116–40584), and €217591 (minimum–maximum, €82827–320146) in high-income countries (n = 29), respectively.Discussion: In countries of the WHO European region, there is a widespread lack of drug susceptibility testing capacity to new and repurposed antituberculosis drugs, lack of access to essential medications in several countries, and a high cost for the treatment of drug-resistant tuberculosis.

[eng] This study sets out to understand the experiences of young people as they undergo Sail Training aboard tall ships (i.e., traditional rigged sailing ships propelled exclusively by wind). It goes beyond current literature on the educational value of Sail Training by focusing on how youngsters actually experience sailing as a way of life. Many young people sense a tension between what they are expected to do, for themselves or their families, and what it feels like to live with everyday decisions. To find harmony in one’s path of life, just like in sailing a ship, one must deal with these contrary forces. On board a tall ship, a manifold of lines is continually generating harmony by way of the tension imparted from the ropes to the sails, from the sails to the whole boat, and from any part of the boat to its crew members. Dwelling on board arises from within this tug of contrary forces. It is a creative dwelling since movement is a constant to which every element in the oceanic medium adjusts itself. In the introduction, I establish the theoretical basis of the study in the maritime environment, in which sailing was practised long before the science of the West attempted to put the ocean under control as it had already done with the territorialisation of the land. To follow the history of the maritime environment and its inhabitants reveals a complex set of skills not only for building the boat as a craft, but also for staying afloat once at sea. Sailing on tall ships offers opportunities to become involved in this process in which, as in every society in past times, experts instruct novices in the course of working together. In the first chapter, tall ships are presented as large sailing vessels that, like earlier craft, need permanent maintenance to remain seaworthy. To keep them so, some friendship races and regattas are organised by Sail Training International, with host ports using the fleet to hold heritage events. These events help to raise the funds to enable young people to participate. Chapter two describes how youngsters without previous experience of sailing, and amateurs including myself, enter this environment as crew members. In the process of becoming familiar with this environment, they get their sea legs. Chapter three shows how the craft, as a shelter affording movement and perception, is both a home in which to stay afloat on the waves and a calming of the wind that, thanks to the sails, provides the required energy to glide the hull over the water. Chapter four describes the organisation on board, with its system of watches or work shifts. This seems simpler than life on land because one does not have to worry about anything other than the established routine. However, since one follows the same daily routines, an awareness of environmental changes emerges with the perception of non-human phenomena like wind, dolphins, waves, clouds and so on. Crew members become sensitised to the ocean environment, to the others and to themselves when responsibilities on board are shared day and night. Attention at sea is existential, it is a matter of life and death, whereas on land it remains contingent on particular interests. At sea, the watch system leads to the development of a meshwork of relationships, described here as a taskship, a creative and trustworthy bonding which lasts for the duration of the voyage. The mutuality on board, discussed in chapter five, allows crew members to develop a sense of who they are while in the company of others. Thus the taskship is constitutive of dwelling on board. Optimal sailing is evidence of the quality of the relationships on board and vice versa, the quality of the relationships on board is shown through the best set of sails to get underway. Finally, in chapter six, it is shown that the wisdom and skills obtained in this ocean-sky world make the difference between an experience that is intentionally transformative for the young person undergoing it and a sailing experience that opens up to an understanding of life., [cat] Aquesta tesi pretén entendre les formes d'habitar el mar a partir de les experiències de navegació d'altura en grans velers. A partir d’un extens treball de camp a bord de grans velers, s’hi detalla les particularitats d’aquest l'entorn, que són de flexibilitat, equitat i intimitat; com es gestiona l'organització social a bord en grups de guàrdies, a la vegada que a partir de les habilitats marineres, es construeix el vaixell com un lloc per viure i en permanent construcció. Aquest lloc habitat, es construeix com un refugi (‘shelter’) que al mar ofereix les condicions necessàries per sobreviure. El fet que ofereixi la protecció necessària sense ser una construcció físicament sòlida per a habitar el mar, fa que el mar es percebi de forma diferent a com percebem l’habitar la terra ferma. Al mar, una nova manera de percebre ‘hàptica’, requereix l'adaptació contínua dels membres de la tripulació al moviment i ritmes del mar. Gestionar aquest equilibri comporta uns tres dies de malestar i vòmits, i es diu que quan un ja té ‘cames de mar’, ja no es mareja més. Un cop obtingudes les cames de mar, el vaixell, viscut com una extensió del propi cos, ofereix a tots els participants sense previ coneixement d'aquest entorn, la possibilitat de conèixer i desenvolupar les habilitats marineres necessàries per a la navegació. A través d’aquestes habilitats s'estableix una relació amb els materials i les formes que es van conformant en l’embarcació (anomenat ‘taskship’ o línies de treball a bord), totes elles fruit de la participació i resposta de cadascun dels components humans i no humans d'aquest entorn. Per tant, la relació que s'estableix amb l'entorn és d'aprenentatge a través de processos concentració (i no pas de cognició) sobre l'experiència viscuda. A bord, el compromís moral preval al compromís per obligació, ja que, de no ser així, difícilment es podria forçar la participació de tots els tripulants en la construcció contínua del propi vaixell. Aquest compromís moral facilita l'equitat, amb la qual cada membre resulta rellevant en aquest entorn hostil, on d'un moment a l'altre tot pot canviar, i on el treball continu així ho exigeix. Una bona navegació reflecteix la qualitat de relacions a bord i de correspondències amb l'entorn, amb el vaixell i entre els participants. Finalment, aquest anàlisi reflecteix el concepte de lloc com un lloc amb profund moviment de relacions, més que un espai físicament localitzat. Aquest lloc és compartit i viscut amb gran atenció, i per aquest motiu roman en la memòria de tots els participants com un saber adaptar-se a allò que ha d’esdevenir, tot esperant sempre que es donin les millors correspondències per l’òptima qualitat de les relacions i de la navegació. És a dir, a través del flux en la navegació tradicional i la seva xarxa de correspondències, els participants en aquestes experiències entendran la vida principalment com un conjunt de relacions en moviment.

The aim of this thesis is to carry out an in-depth study of various aspects of breast cancer epidemiology. Firstly, we have confirmed that DCIS incidence in Girona has increased over recent decades. Proportions of screen-detected cancers, interval cancers and non-screen-detected cancers during the start-up phase of the mammographic screening programme were found to be 42.2%, 5.8% and 52.2%, respectively. Secondly, we have found that luminal A-like was the most frequent subtype associated with the best survival rate, while triple-negative breast cancer was related to the lowest survival rate. Importantly, we have concluded that breast cancer molecular subtype defined by IHC biomarkers provides prognostic value, regardless of age, tumour size, histological grade, lymph node involvement and method of detection. Finally, we have demonstrated that method of detection also provides prognostic value regardless of age, tumour size, histological grade, lymph node involvement and breast cancer molecular subtype defined by IHC biomarkers., L’objectiu d’aquesta tesi és realitzar aprofundir en diversos aspectes de l'epidemiologia del càncer de mama. Hem confirmat que la incidència del DCIS a Girona ha augmentat en les últimes dècades. Les proporcions dels càncers detectats mitjançant el programa de cribratge, fora d’aquest i els càncers d'interval van ser del 42,2%, 52,2% i 5,8%, respectivament. Per altra banda, el subtipus amb la supervivència més elevada i més baixa van ser el luminal A-like i el triple negatiu, respectivament. És important destacar que el subtipus molecular de càncer de mama definit per biomarcadors determinats amb tècniques d’IHC proporciona valor pronòstic, independentment de l'edat, la mida, el grau histològic, l’afectació dels ganglis i el mètode de detecció. Finalment, hem demostrat que el mètode de detecció del càncer també proporciona valor pronòstic independentment de l'edat, la mida, el grau histològic, l'afectació dels ganglis i el subtipus molecular.

Introduction: Interval cancers are tumors arising after a negative screening episode and before the next screening invitation. They can be classified into true interval cancers, false-negatives, minimal-sign cancers, and occult tumors based on mammographic findings in screening and diagnostic mammograms. This study aimed to describe tumor-related characteristics and the association of breast density and tumor phenotype within four interval cancer categories. Methods: We included 2,245 invasive tumors (1,297 screening-detected and 948 interval cancers) diagnosed from 2000 to 2009 among 645,764 women aged 45 to 69 who underwent biennial screening in Spain. Interval cancers were classified by a semi-informed retrospective review into true interval cancers (n = 455), false-negatives (n = 224), minimal-sign (n = 166), and occult tumors (n = 103). Breast density was evaluated using Boyd’s scale and was conflated into: 75%. Tumor-related information was obtained from cancer registries and clinical records. Tumor phenotype was defined as follows: luminal A: ER+/HER2- or PR+/HER2-; luminal B: ER +/HER2+ or PR+/HER2+; HER2: ER-/PR-/HER2+; triple-negative: ER-/PR-/HER2-. The association of tumor phenotype and breast density was assessed using a multinomial logistic regression model. Adjusted odds ratios (OR) and 95% confidence intervals (95% CI) were calculated. All statistical tests were two-sided. Results: Forty-eight percent of interval cancers were true interval cancers and 23.6% false-negatives. True interval cancers were associated with HER2 and triple-negative phenotypes (OR = 1.91 (95% CI:1.22-2.96), OR = 2.07 (95% CI:1.42-3.01), respectively) and extremely dense breasts (>75%) (OR = 1.67 (95% CI:1.08-2.56)). The present study is focused on cross-cultural analysis of different Spanish companies operating in China. Mainly, the research examines the impact that cultural factors can have on the Spanish firms in China. And these cultural factors affect, first, to the external activity of the company, and secondly to human resources management, the formation and execution of strategy, form of organizational structure, and corporate governance within the enterprises. This thesis is a qualitative study. The study contains the particular experiences of four Spanish firms and their evolution in the Chinese market. The data of the research are collected through in-depths interviews with four Spanish companies that agreed to participate in this research. And it focuses on Spanish firms that have already settled their manufacturing plants in China. This research will provide the reader firstly with general information of Spanish firms in China, economic factors between the two countries and cultural orientation models. Later, the experiences of the case companies are added in the research. The study provides much detail information on the topics such as the process of negotiation in China, the marketing activities and advertisment done, and the daily operations inside the firm. And finding the most common difficulties, or otherwise discover new skills or knowledge of Chinese culture that firms have to deal with. Outgoing

A case of ruptured rectus abdominis muscle in a professional volleyball player is presented and discussed. The injury was located near the deep fascia of the upper stomach muscle on the non-dominant side. This injury is rarely specifically mentioned in the literature and there are few reported cases. The injury mechanism is usually indirect at the time of the flexion/extension transition when serving or attacking in the case of volleyball. The player remains active despite the injury and for this reason the treatment had to be modified.

Es presenta i es discuteix un cas de trencament del múscul recte abdominal en un jugador professional de voleibol de platja. La lesió es localitza prop de l’aponeurosi profunda del ventre muscular superior al costat no dominant. En la literatura epidemiològica aquesta lesió poques vegades es contempla de manera específica, i se n’han descrit pocs casos. El mecanisme de lesió acostuma a ser indirecte en el moment de la transició extensió- flexió durant el gest del servei o de l’atac en el cas del voleibol. En aquest cas l’ecografia és l’eina de confirmació diagnòstica. Tot i patir la lesió, el jugador es va mantenir actiu, per la qual cosa es va haver de modificar la pauta de tractament.

Introducció: El dolor lumbar té una alta prevalença entre els esportistes, s’ha relacionat amb dèficits en la força extensora lumbar, i el fet de patir-ne representa un obstacle important per a la pràctica d’esports d’alta intensitat. Mètode: S'ha mesurat la força lumbar en dos grups de practicants d'hoquei sobre herba mitjançant màquina MedX® i un test de resistència isomètric lumbar. Resultats: Entre ambdós grups els resultats han estat molt homogenis. Els dos tests no presenten relació entre si, ni amb les característiques biomèdiques dels jugadors (edat, índex de massa corporal o VO2màx). Els nivells de força màxima i resistència isomètrica obtinguts han estat superiors als de referència entre sedentaris. D’una manera característica, entre els jugadors d'hoquei la relació entre la força extensora del tronc en flexió (M) respecte la força extensora del tronc en extensió (m) és més gran que en altres esportistes (ràtio M/m > 1,6, mentre que en la població normal és 1,4) a causa probablement de la posició en semiflexió pròpia de l'hoquei. Conclusió: Els resultats dels test de força extensora lumbar tenen unes característiques pròpies entre els jugadors d’hoquei.

Impulsado por la problemática actual en el campo de la automoción, concretamente en el mal uso de los sistemas de retención de pasajeros, el presente proyecto consiste en el desarrollo de una idea, la investigación de su novedad, el análisis y estrategia de sus alternativas, el diseño de la solución, el cumplimiento de la normativa correspondiente y la redacción de una patente de la invención. La evolución de la idea nace con el concepto de un bloqueo electrónico de la hebilla del cinturón de seguridad para impedir el desabrochado por parte de niños o personas dependientes (enfermos psíquicos, personas mayores con Alzheimer, etc.) durante la conducción. A lo largo del estudio, la invención adquiere más volumen, al añadir la detección y aseguramiento del abrochado correcto del cinturón al bloqueo de la hebilla, para asegurar su novedad y carga inventiva y hacer posible su protección en el ámbito de la propiedad industrial. De esta manera se consigue definir y analizar detalladamente las características de la solución, con el fin de redactar la memoria de la patente y sus dibujos descriptivos. Como metodología de análisis y desarrollo del concepto, se emplean herramientas de ingeniería y diseño de producto, tales como el método FAST, la planificación por GANTT o herramientas informáticas de dibujo como el programa CATIA. Además, los resultados obtenidos se deben a la recogida de documentación tanto de publicaciones como conocimientos de expertos en los distintos campos que abarca la invención y a datos experimentales facilitados por usuarios de los sistemas de retención infantil. Adicionalmente, se contrata a una ingeniería para la fabricación de un prototipo del mecanismo de bloqueo, el cual muestra la funcionalidad y viabilidad técnica de la invención, de cara a tomar decisiones internas en cuanto a un posible montaje del dispositivo en los vehículos de la empresa. El trabajo concluye con un estudio económico, temporal y medioambiental del proyecto y la obtención de la solicitud de una Patente Nacional Española titulada Sistema y método de detección del correcto abrochado del cinturón de seguridad, la cual incluye también el bloqueo electrónico de la hebilla. El departamento de Estrategia de Innovación y Patentes del Centro Técnico de SEAT ha sido el entorno físico, que ha hecho posible la realización de este trabajo de investigación, gracias a su carácter joven y creativo.
