INTRODUÇÃO: A esclerodermia é uma doença autoimune rara, caracterizada por uma produção excessiva de colágeno e fibrose da pele e dos órgãos internos, levando a alterações vasculares e imunológicas. OBJETIVO: Esta revisão de literatura tem como objetivo analisar as dificuldades no diagnóstico e tratamento da esclerodermia no Brasil. MÉTODOS: A metodologia envolve a seleção do tema e dos objetos de estudo, a formulação da pergunta de pesquisa, a escolha de descritores e bases de dados, e a aplicação de critérios de inclusão e exclusão. Será utilizada a abordagem estruturada do método PRISMA, garantindo transparência e qualidade na análise dos resultados. RESULTADOS: Empregando uma cuidadosa análise dos títulos, resumos e artigos completos, seguindo critérios de inclusão e exclusão, foram escolhidos 24 artigos para compor a amostra final. Tais estudos mostram que as dificuldades no diagnóstico da esclerodermia no Brasil são multifatoriais e envolvem a falta de conhecimento dos profissionais de saúde, a limitada disponibilidade de exames específicos, as dificuldades dos métodos diagnósticos e a falta de padronização dos critérios diagnósticos. No Brasil, o tratamento da esclerodermia enfrenta desafios complexos, como a falta de acesso a medicamentos específicos, alto custo dos tratamentos, escassez de especialistas e falta de padronização dos protocolos terapêuticos. CONCLUSÃO: Para superar essas dificuldades, é necessário investir em educação continuada, ampliar o acesso a exames e estabelecer critérios diagnósticos consensuais. Além disso, pesquisas futuras devem focar em genética, novas abordagens terapêuticas e biomarcadores. Garantir acesso a medicamentos, capacitar especialistas e padronizar protocolos também são essenciais. Esforços conjuntos são necessários para melhorar os cuidados aos pacientes com esclerodermia no Brasil.

This paper presents an alternative way of making predictions on the effectiveness and efficacy of Renewable Energy (RE) policies using Decision Trees (DT). As a data-driven process for decision-making, the analysis uses the Renewable Energy (RE) target achievement, predicting whether or not a RE target will likely be achieved (efficacy) and to what degree (effectiveness), depending on the different criteria, including geographical context, characterizing concerns, and policy characteristics. The results suggest different criteria that could help policymakers in designing policies with a higher propensity to achieve the desired goal. Using this tool, the policy decision-makers can better test/predict whether the target will be achieved and to what degree. The novelty in the present paper is the application of Machine Learning methods (through the Decision Trees) for energy policy analysis. Machine learning methodologies present an alternative way to pilot RE policies before spending lots of time, money, and other resources. We also find that using Machine Learning techniques underscores the importance of data availability. A general summary for policymakers has been included.

Introduction Acute rhinosinusitis accounts for most of the cases of orbital infections and is the most common cause of periorbital oedema in children. Up to 10% of patients with orbital complications of acute rhinosinusitis may experience vision loss and other complications such as meningitis, intracranial abscess or even death. Therefore, these patients require prompt diagnosis and proper treatment. Objectives This study aims to report the clinical presentation and management of post-septal orbital complications of acute rhinosinusitis in the paediatric population. Materials and methods A retrospective medical chart review of all children aged under 18 years old who were diagnosed with post-septal orbital complications of acute rhinosinusitis at a tertiary academic hospital, between 01/2007 and 12/2020. Patients were grouped according to the Chandler Classification (groups 2–5). Results Fifty-five children (mean age of 6.91 ± 4.61 years) fulfilled the entry criteria for post-septal orbital complications of acute rhinosinusitis, based on clinical evaluation by an otorhinolaryngologist and CT-scan findings. Forty (72.72%) patients were also evaluated by an ophthalmologist. Most patients were male (76.36%). Twenty-four patients had post-septal cellulitis (43.63%), 21 patients had a subperiosteal abscess (38.18%) and 10 patients had an orbital abscess (18.18%). Eyelid swelling was the most frequent sign, followed by fever. Microbiology varied considerably and gram-positive agents were clearly predominant. Eighteen (32.73%) patients had been treated with oral antibiotics prior to hospital admission, exhibiting a significantly higher risk of recurrence of orbital infection (p = 0.020). Ethmoid and maxillary sinuses were the most involved paranasal sinuses (90.91%). Thirty-three patients (60%) were successfully treated medically, and 22 patients (40%) required surgical drainage. Seven patients (12.73%) developed further complications and six recovered without sequelae. The mean length of hospital stay was 8.0 ± 5.0 days and recurrence of orbital infection occurred in six patients (10.91%). The absolute neutrophil blood count was significantly different amongst Chandler groups (p = 0.021), with higher counts in patients with subperiosteal abscess. The duration of hospitalization was significantly higher in patients submitted to surgery (p Conclusion Post-septal orbital complications of acute rhinosinusitis are infrequent but dangerous events in the paediatric population. Close collaboration with Ophtalmology is paramount, as the child's vision is at risk. Eyelid swelling and proptosis are early signs. CT-scan imaging plays an invaluable role in the diagnosis and decision-making. Predictive indicators for surgery were not found. However, emergency endoscopic nasal surgery with abscess drainage should be considered whenever vision is at risk, if there is no improvement after aggressive medical treatment, and in cases of intracranial complications.

Introdução: A anestesia opiod-free (OFA) é a prática anestésica sem o uso de opioides intraoperatórios, feita através de uma abordagem multimodal que resulta em anestesia satisfatória e uma melhor recuperação sem os danos amplamente conhecidos relacionados aos opióides. Contudo, essa técnica ainda precisa ser avaliada em diversos contextos cirúrgicos. Objetivo: Esta revisão sistemática teve como objetivo comparar abordagens de anestesia geral livre de opioides e anestesia com opióides e sua eficácia sobre variáveis de recuperação pós-operatória de colecistectomias laparoscópicas eletivas. Método: Utilizando os unitermos “opioid free anesthesia”, “nonopioid” e “laparoscopic cholecystectomy”, com o operador booleano ?AND?, foram encontrados 752 artigos nas bases de dados PubMed, Scielo, Cochrane, NBCI e Google Scholar. Os critérios de inclusão eram ensaios clínicos que avaliaram parâmetros de recuperação no pós-operatório de colecistectomias laparoscópicas eletivas como náuseas e vômitos, dor pós-operatória e necessidade de opioides de resgate, dentro do intervalo de 2015 a 2020. Ao final, 6 estudos atenderam aos critérios de inclusão e exclusão. Revisão de literatura: Bakan (2015), utilizando anestesia venosa total, concluiu que o grupo OFA apresentou menor consumo de fentanil no pós-operatório, e nenhum paciente precisou de ondansetrona para Náusea e Vômito no Pós-operatório (NVPO). Shalaby (2018), Elsayea (2019), Toleska (2019), Abdelmoniem (2020) e Ahmed (2020), utilizando abordagens balanceadas, encontraram resultados concordantes entre si. Quatro estudos mostraram menor necessidade de antieméticos para NVPO e todos concluíram que a anestesia sem opióides fornece melhor controle da dor e menor necessidade de opióides no pós-operatório, sem efeitos adversos significativos em comparação com os grupos baseados em opióides. Conclusão: A anestesia sem opióides foi superior à baseada em opioides em relação aos parâmetros analisados na recuperação de pacientes submetidos a colecistectomias laparoscópicas eletivas, promovendo melhor analgesia, menor necessidade de opioides, menor incidência de NVPO e, de acordo com os autores, sempre com estabilidade hemodinâmica.

Introdução: A obesidade é um grave problema de saúde e uma alternativa para seu tratamento é a cirurgia bariátrica, que demanda anestesia geral, durante a qual são frequentes complicações pulmonares como a atelectasia. As manobras de recrutamento alveolar (MRA) são processos dinâmicos de aumento transitório da pressão pulmonar, utilizadas para recuperar alvéolos atelectasiados, aumentando a área pulmonar disponível para a troca gasosa e melhorando a oxigenação arterial. Objetivo: Realizar revisão sistemática para analisar as MRA na melhora da mecânica ventilatória em pacientes submetidos a cirurgia bariátrica. Método: Utilizando os unitermos “obesity”, “PEEP”, “recruitment maneuvers” e “bariatric surgery”, com operador booleano ‘’AND’’, encontraram-se 15 artigos, sendo incluídos os quatro que preencheram os critérios de inclusão: ensaios clínicos randomizados, entre 2009-2020, nas línguas portuguesa, inglesa ou francesa das plataformas PUBMED e BVS. Revisão de literatura: Ao total, os artigos analisaram 2142 pacientes, entre 18-65 anos e IMC > 30kg/m², submetidos à cirurgia bariátrica laparoscópica e aberta. Wei utilizou MRA com incrementos graduais de PEEP em um grupo e MRA com PEEP de 8 cmH2O em outro, observando otimização da oxigenação arterial, tempo para extubação e redução da driving pressure nestes grupos comparado ao controle. Souza, utilizando estratégia semelhante, descreveu melhor PaO2/FiO2 no grupo com PEEP de 30 cmH2O com dois minutos de manobra. Valenza utilizou a posição cadeira de praia e PEEP de 10 cmH2O com as MRA, observando em ambas neutralização semelhante das principais perturbações da anestesia. Por fim, Tafer utilizou MRA e PEEP de 10 cmH2O em um grupo e no outro apenas PEEP, observando melhora da mecânica ventilatória no primeiro, porém como efeito transitório. Conclusão: As abordagens utilizando MRA com níveis mais altos de PEEP, assim como a posição cadeira de praia, foram eficazes em prevenir atelectasia, melhorar oxigenação e mecânica ventilatória em pacientes obesos submetidos à cirurgia bariátrica.

Frequentemente empenhados em analisar as situações de desigualdade e exclusão social enfrentadas por minorias étnicas, pesquisadores da Comunicação lidam com questões metodológicas de peso: como adequar o ímpeto racionalizante do estudo ao modo de vida do outro? Como preservar e valorizar esse modo de vida do outro – sua cultura, seus costumes e crenças, sua alteridade mesma – sem abdicar do olhar crítico e do rigor teórico-conceitual postulados pelo saber acadêmico? Como tornar o saber acadêmico permeável a outros saberes?

O objetivo deste artigo é apresentar o contexto a partir do qual foi instituída, por meio de um novo arcabouço legal, mecanismos para simplificar a execução de uma política pública, no caso a implementação de tecnologias sociais de acesso à água para a população rural de baixa renda, materializada no Programa Cisternas. A partir da apresentação da lógica de implementação até então utilizada, a perspectiva é mostrar de que forma o aprendizado sobre o desenho institucional da política contribuiu para a formatação de um novo arcabouço legal, tendo como horizonte conferir maior celeridade, eliminando gargalos burocráticos que resultavam em atrasos significativos. O desafio era criar mecanismos adaptados à lógica de execução de cada parceiro e da própria política. Os primeiros resultados mostram que a sistemática criada com esse novo arcabouço legal tem conseguido dar as respostas esperadas, considerando que a primeira etapa de implementação tem sido realizada de forma sensivelmente mais célere do que a partir dos instrumentos utilizados anteriormente.

Generative design systems coupled with objective functions can be efficiently explored through the use of stochastic optimization algorithms, such as genetic algorithms. The first step in implementing genetic algorithms is to define a representation, that is, the data structure representative of the genotype space and its mathematical relation to the data of the phenotype space — the variables of the real problem. This can be a hard task, particularly if the design system contains dependency between variables. This paper presents a general representation, which enables the use of standard variation operators, allows defining both continuous and discrete variables from a single type of gene and is easily adaptable to different problems, with a larger or smaller number of variables. This representation was created to solve the representation problem in the design system for Frank Lloyd Wright's prairie houses, a shape grammar that was converted into a parametric design system.

The envelope shape, the most salient design characteristic in a building, has significant influence on its energy performance. However, in the early design stages, when the envelope shape is defined, energy performance information is normally nonexistent, due to modeling for energy simulation being a time-consuming task, frequently overlooked at this phase. This paper presents a methodology to assist design decisions regarding the building envelope shape considering its implications on energy performance. Basically, this methodology involves a flexible design system, to generate alternative envelope shape designs, with integrated energy simulation, to calculate the energy demand of each design. Shape grammars are particularly suitable to encode architectural design systems, given their ability to encode compositional design principles. Their downside is the complexity in developing computer implementations. This methodology converts a grammar into a parametric design system and is illustrated with an application to the grammar for Frank Lloyd Wright's prairie houses.

The architectural design variables which most influence the energy performance of a building are the envelope materials, shape and window areas. As these start to be defined in the early design stages, designers require simple tools to obtain information about the energy performance of the building for the design variations being considered at this phase. The shape factor is one of those tools, but it fails to correlate with energy demand in the presence of important solar gains. This paper presents a new design indicator of energy performance for residential buildings, the Envelope-Related Energy Demand (ERED), which aims to overcome the shortcomings of the shape factor while maintaining a reasonable simplicity of use. The inputs to ERED are areas of envelope elements (floor, walls, roofs and windows), U-values of envelope materials, solar heat gain coefficients (SHGC) of windows and site related parameters, concerning temperature and solar irradiation. ERED was validated against detailed simulation results of 8000 hypothetical residential buildings, varying in envelope shape, window areas and materials. Results show that there is a strong correlation between ERED and simulated energy demand. These results confirm the adequacy of ERED to assist design decisions in early stages of the design process.

Este trabalho propõe uma análise do filme “Salve o cinema” (“Salaam cinema”, 1995), do diretor iraniano Mohsen Makhmalbaf, a partir de uma leitura do conceito de dispositivo. Busca-se, a partir da observação dos pontos de contato entre partes de construção e de imprevisibilidade do filme, compreender a dinâmica entre o registro e a produção dos acontecimentos. A partir das características particulares do dispositivo construído em “Salve o cinema”, indaga-se sobre a relação que ele estabelece com sua exterioridade, ultrapassando a pura contingência do acontecimento filmado e introduzindo tensões entre os regimes documental e ficcional

The understanding of how pressure changes affect elastic properties of reservoir rocks is valuable information in time-lapse seismic studies. Nevertheless, it is not an easy task to accurately predict the elastic behavior of heterogeneous reservoirs rocks. In this paper we present an approach based on porosity which can be a powerful tool for improving velocity and elastic moduli predictions on carbonate reservoirs undergoing production. Moreover, besides reducing uncertainties, the proposed method allows to incorporate them in order to create different scenarios. A large set of dry samples ultrasonic velocity measurements of Brazilian pre-salt carbonates was analyzed. Among other interesting conclusions we observed that high porosity samples tend to present greater changes in elastic properties due to pressure variation. On the other hand, low porosity samples show larger uncertainties ranges. Because of that, considering optimists and pessimists scenarios, which mean large and small elastic properties variation, respectively, we can found different behaviors. So for an optimist scenario a carbonate reservoir low porosity regions can be more detectable in terms of 4D signal – considering only pressure effect – than average scenarios of higher porosity regions.

Planning the transition period for the adoption of alternative fuel-technology powertrains is a challenging task that requires sophisticated analysis tools. In this study, a system dynamic approach was applied to analyze the bi-directional interaction between the development of the refueling station network and vehicle sales. Besides, the developed model was used to estimate the transition cost to reach a predefined target (share of alternative fuel vehicles) in different scenarios. Several scenarios have been analyzed to investigate the effectiveness and cost of incentives on the initial price of vehicles, and on the evolution of fuel and refueling stations. Obtained results show that a combined set of incentives will be more effective than just a single specific type of incentives.

O objetivo central deste trabalho é analisar os determinantes políticos da execução das emendas orçamentárias individuais no Brasil, consideradas como recursos tipicamente clientelistas – conhecidos como pork barrel. A dissertação parte do pressuposto que as emendas são elementos importantes para o sistema político brasileiro, pois além de serem utilizadas como mecanismo para a construção da governabilidade – com todos os custos e benefícios inerentes – seriam fundamentais para reforçar o vínculo entre representantes e representados. A literatura que discute a distribuição desses recursos pode ser dividida basicamente sob o ponto de vista de um mercado competitivo: que envolve tanto a oferta de parlamentares como a demanda dos eleitores para obras e programas decorrentes do pork. Porém, pouco ou nada tem sido produzido sob a capacidade política e a necessidade técnico-social dos municípios em receber os recursos de emendas orçamentárias individuais. Não tem havido uma discussão fundamentada sobre os fatores que atraem o pork para os municípios, se são meramente políticos – como prega a sabedoria convencional – ou se há evidências de que fatores técnicos também afetam essa distribuição. Utilizando estatísticas descritivas e métodos econométricos, investigamos que características políticas e mesmo socioeconômicas “atraem” o pork no Brasil. A idéia é ir além da relação entre parlamentares e o Executivo federal – que é responsável pela execução, com discricionariedade, dos recursos dessas emendas – e analisar de que forma o quadro político local, estadual e federal, além do cenário econômico e social dos municípios são importantes para determinar as chances de uma emenda aportar em uma localidade e não em outra, ou porque algumas dessas localidades atraem um maior número e maiores valores em termos de pork em detrimento de outras. _________________________________________________________________________________ ABSTRACT The main objective of this work is analyzing the technical e political determinants of execution of the budget amendment in Brazil, measured as pork barrel. The paper assumes that the amendments are important for the Brazilian political system, as well as being used as a mechanism for the construction of governance – with all the costs and benefits – would be crucial to reinforce the link between representatives and represented. The literature on the distribution of these resources can be basically divided from the point of view of a competitive market, which involves the provision of parliamentary voters as demand for works and programs under the pork. However, little or nothing has been produced under the political capacity and technical and social needs of the municipalities receive the resources of individual budget amendments. There hasn‟t been a discussion based on the factors that attract the pork to the municipalities, whether they are purely political – how to fold the conventional wisdom – or if there is evidence that technical factors also affect this distribution. Using descriptive statistics and econometric methods, we investigate which features political and socioeconomic "attract" the pork in Brazil. The idea is to go beyond the relationship between lawmakers and the Federal Executive – which is responsible for implementation, with discretion, the resources of these amendments – and to examine how the political local, state and federal, as well as social and economic scene of the municipalities are important in determining the chances of a amendment goes to a locality and not to another, or because some of these places attract a greater number and higher resources in terms of pork at the expense of others.

The European Commission has recently launched the Smart Cities Initiative to demonstrate and disseminate how to foster a quick transition towards local sustainable energy systems. Within this initiative, the three main challenges faced by pioneering cities, are to reduce or modify the demand for energy services, to improve the uptake of energy efficient technologies and to improve the uptake of renewables in the urban environment. We find that enough resources will need to be provided to a significant number of pioneering cities, and propose that the initiative would allocate these resources through project competition, rewarding innovation, ambition and performance, which have been ingredients of success at Member State level.
