Lower incisor and its position in mandibular arch and in facial pattern are considered to be particularly important when we establish orthodontic diagnostic and treatment plan. In our study, we aim to determine which dental and skeletal pattern are recurrent in orthodontic patients from urban area (Cluj Napoca, Romania) and correlate the data with lower incisor position and standard cephalometric values. An observational analytic longitudinal prospective study was conducted, based on lateral cephalograms. Cephalometric parameters analysed were: Interincisive Angle, FMIA, IMPA, ANB. Aim of the study .The open bite is a part of dentomaxillary anomalies and is classified by anatomic open bite and functional open bite. In this study, we demonstrated the frequency of the open bite , the dysfunctional factors involved as well as the etiopathogenesis of the anomaly. Materials and methods. We did a retrospective study for a group of 290 patients who were treated at the clinic Orthodontics “Iuliu Haţieganu” Cluj Napoca (2015-2017). The study was based on data collected from completed forms for each patient. Results. The data were systematized in tables and were subjected to statistical analysis. Our study focuses on a group of 290 patients of which 26 (9%) have an open bite, and 264 patients (91%) have other dentomaxillary anomalies. Conclusions . Prophylaxis means and interception will eliminate dysfunctional factors and ensure the normal development of the dentomaxillary apparatus.

4. The Effects of Low-Dose Irradiation on Human Saliva: A Surface-Enhanced Raman Spectroscopy Study

Biological effects of low-dose ionizing radiation (IR) have been unclear until now. Saliva, because of the ease of collection, could be valuable in studying low-dose IR effects by means of surface-enhanced Raman spectroscopy (SERS). The objective of our study was to compare the salivary SER spectra recorded before and after low-dose IR exposure in the case of pediatric patients (PP). Unstimulated saliva was collected from ten PP before and after irradiation with a cone beam computed tomography (CBCT) machine used for diagnostic purposes. The SERS measurements have been recorded on dried saliva samples using a solid nanosilver plasmonic substrate synthesized using an original method developed in our laboratory. The experimental results showed that salivary SER spectra are dominated by three vibrational bands (441,735 and 2107 cm−1) that can be assigned to bending and stretching vibrations of salivary thiocyanate (SCN-). After exposure, an immediate increase of vibrational bands assigned to SCN- has been recorded in the case of all samples, probably as a result of IR interaction with oral cavity. This finding suggests that SCN- could be used as a valuable biomarker for the detection and identification of low-dose radiation effects.

Facial palsy and paralysis causes facial asymmetry, with negative effects on the functions of the stomatognathic system and on the quality of life of the affected patients. These patients have multiple risks in the dental office because of the extremely diverse etiological factors that can be found in facial paralysis. This paper addresses clinical aspects of central and peripheral facial paralysis, with examples of clinical cases and differential diagnosis of these disorders. The risks that the dentist should evaluate in the patient with facial paralysis and the way in which these can interfere with dental treatment are also considered.

The term molar-incisor hypomineralization describes an entity characterized by the hypomineralization of permanent first molars and commonly affecting permanent incisors. The aim of this paper was to describe a case of a female patient attended with dental sensibility and phonetic difficulties. We emphasis on the complex approach of this case and the importance of regular follow up, in order to achiev aesthetic and functional rehabilitation. Unfortunatelly, patient and familly support was unconstant and treatment results were above expectation. In conclusion, MIH treatment impeel short and long term stategy for accurate resultas.

7. Impacted canines: Clinical and therapeutical aspects

Impacted canines represent an increasingly frequent anomaly, of particular importance for the functionality of the dentomaxillary system and aesthetic appearance. Aim. To present the incidence and the degree of impacted canine in a group of children aged who need an orthodontic treatment. Material and method. We conducted a study in 18 patients, who requested a treatment plan in the period 2014-2017. The materials used in the study were: patients’ medical records, the orthopantomographic radiographs, study models and photographs of each case. Results. The frequency of canine impaction following the study was 8%, more frequent involvement of the male sex, with a percentage of 56% compared to the female sex, with 44%. The most frequent degree of impaction found in this study was 2nd degree, followed by 3rd degree, and 1st degree. Conclusion. In the great majority of the cases, impaction was located in the maxillary arch. The most frequent location was palatal, 78%.

Objectives. To determine the correlations between carioactivity, quantified by the cariogenic index, and total plasma proteins, protein fractions and salivary immunoglobulins. Material and method. We conducted the study on a group of 23 patients with chronic liver disease of alcoholic etiology, who underwent clinical and paraclinical examinations. Total plasma proteins, albumin, alpha-1-globulins, alpha-2-globulins, beta-globulins, gamma-globulins, plasmatic IgG, IgA, IgM, plasma IgA/IgG and salivary immunoglobulins values were determined. Results. We determined the following mean values: cariogenic index 1.78±1.00, total plasmatic protein concentration 7.45±0.51 g/l, albumin 59.90±6.63%, α1-globulin 3.79±1.58%, α2-globulin 9.08±0.95%, β-globulin 11.44±2.80%, γ-globulin 15.79±3.42%, plasmatic IgG 14.76±3.64 g/l, plasmatic IgA 3.58±1.94 g/l, plasmatic IgA/IgG ratio 0.24±0.13, plasmatic IgM 1.43±0.65 g/l, total salivary proteins 1.25±0.85 g/l, salivary IgG 3.05±0.63 mg/dl, salivary IgA 22.18±7.33 mg/dl, salivary IgA/IgG ratio 7.31±2.60. Conclusions. We have established statistically significant correlations between the cariogenic index and the total plasma proteins, plasmatic alpha-2 fraction and plasmatic IgG.

Objectives. Our research team aimed to evaluate the effects smoking has on several salivary parameters and to assess if there are any correlations between smoking and the cariogenic activity. Material and method. The present research included a total of 35 participants. Saliva was collected from every participant to the study. Salivary chloride, calcium and potassium levels, as well as salivary flux and pH were determined for all subjects. Results. Our results showed significantly lower salivary flux and pH levels in smokers compared to nonsmokers (p < 0.01). Chloride, calcium and potassium we found to have higher concentrations in smokers compared to the control group. However, a statistical significance could be found only for calcium (p = 0.02). Moreover, we were able to find in the smokers group a positive correlation in smokers between salivary chloride levels and cariogenic activity as well as a negative correlation could be found in smokers between salivary calcium levels and the cariogenic activity Conclusions. The results of the present study show that smoking alters salivary parameters and that these modifications can favour the development of dental caries.

The treatment of the stomatognat system dysfunction is a complex one and depends on the diagnosis and symptomatology to each case.The aim of this study is to identify the clinical signs of muscle in the dysfunctional syndrome and also the type of treatment applied successfully or failure.Materials and methods. This study includes 952 patients treated, 462 men and 490 women between 18 and 80 years old. The results obtained after the clinical and paraclinical examinations were processed with the special statistical programs.* Microsoft Excel and Statistic 2000.Results. Discutions. One of the dysfunctional syndrome factor is the stress. The results of this study showed that only 11% of patients recognize the presence of the sress in their lives. The muscle relaxation is another stage of treatment by applying occlusal interceptors. Our study releaved that the application of relaxation splints for 15% of cases had good results. Other patients have benefited symptomatic therapy : 18% of cases hydrotermotherapy,9% of cases fototherapy, 14% of cases ultrasonotherapy and 31% of cases medicinal therapy with miorelaxants.Conclusions. The treatment of neuromuscular component is associated in most of the cases with specify therapy of the other component of the stomatognat system.

Objectives. The establishment of the frequency of class II/1 in the requests of treatment in Clinica de Ortodontie Cluj between the years 2014 and 2017, the clinical and telegraphic evaluation of the cases with class II/1 and the therapeutically approach of class II/1 cases. Material and method. The study was made on a lot of 1,830 patients, which came to two of the clinics’ offices. Results and discussions. The incidence of class II/1 at two of the Clinics offices between the years 2014 and 2017 was 47% (861 cases) of the total of studied cases. The repartition regarding the sexes showed a predominance of 69% at girls (592 cases). Conclusions. The frequency of class II/1 anomalies was high in treatment requests, 47%. Treatment in temporary and mixed dentition was done mainly with the functional appliances. The evolution of cases was favorable generally, demonstrated by clinical and cephalometric modifications.There were some limitations in the treatment with functional and removable appliances, the finishing stages will be done with fixed appliances.

Numeric tooth anomalies involving a fewer number of teeth (hypodontia) are considered dental dystrophies determined by disturbances during the odontogenesis stage. The aim of this study was to investigate the incidence of the anomaly and its clinical manifestations in two representative groups of children: an unselective group (2017 children in Cluj-Napoca aged between 7 and 15) and a group of orthodontic patients (2,000 patients of the same ages, with various dental-maxillary anomalies), using the statistical validation test ANOVA. Our findings included a high incidence of hypodontia, with varied clinical aspects, a low addressability and “accidental” diagnosis during investigation for other anomalies.

Objectives: The purpose of this study was to track the frequency of agenesis in a group of children and adolescents who underwent treatment at the Orthodontic Clinic in Cluj-Napoca between 2014 and 2017. Material and Method: We studied a group of 700 children aged 7-18 years. Classification was based on gender, clinical abnormalities, teeth, associated anomalies. Results: In our study, 4% of patients had at least one dental agenesis, the upper lateral incisors being the most frequently affected (47%). In terms of gender, the female was affected with a higher percentage (69%). Conclusions: Treatment of hypodontia is often an individualized interdisciplinary treatment, depending on the severity of the anomaly and associated general diseases

14. ORAL REHABILITATION OF A PATIENT WITH ECTODERMIC DYSPLASIA SYNDROME

Oral clinical manifestations of ectodermal dysplasia are in a wide variety, ranging from the presence of 2 or 4 to 6 teeth (oligodontia) usually with abnormal shape and size, with short, curved and thin roots to subtotal anodontia or total anodontia. There is no rule regarding missing teeth, nor the position or form under which they are found. Often the position of the teeth on the arcade is unfavorable, requiring a complex interdisciplinary treatment, involving the orthodontic repositioning first, followed by the prosthetic restoration through the infant prosthesis during childhood, and ultimately over-implant prosthetic restoration after the end of the growth process. For oral rehabilitation it is essential to know the age of the patient, the number and condition of the teeth present, and the level of the growth process.A 4-year-old patient who visited our clinic was treated only prosthetically, because the clinical situation allowed us to address this treatment option. Infant prostheses were applied at this age, the final treatment plan having as objective the rehabilitation of all the functions affected by supraimplantation prostheses after the end of the growth processes.
