4. Management of a case of Castleman’s disease coexisting with or occurring after transplanted Hodgkin’s lymphoma

Abstract

Castleman’s disease is a benign lymphoproliferative disorder. The coexistence of Hodgkin’s lymphoma and multicentric Castle-man’s disease is a rare phenomenon. We discuss a case of a 48-year-old female patient who had been in the records of the Colentina Hematology Clinic since 2019, with the diagnosis of classic Hodgkin’s Lymphoma, nodular sclerosis type I BNLI, stage IIXB. For this, she underwent 3 courses of ABVD and 2 courses of BEACOPP, without showing complete remission on PET/CT evaluation at the end of treatment. After that, we initiated rescue therapy and performed 4 IGEV courses, followed by autologous stem cell transplantation. For maintenance treatment, we opted for Brentuximab, but it was discontinued after the first administration due to the appearance of adverse reactions. Subsequently, we decided to perform radiotherapy with 20 fractions cumulating a total dose of 36 Gy. Shortly after the radiotherapy, symptoms reappeared which were suspected to be in the context of a relapse of the disease. For confirmatory diagnosis, we performed a new PET-CT which highlighted metabolically active ganglion images. Further, were carried out lymph node biopsy for histopathological and immunohistochemical examinations were carried out. The underlying disease was diagnosed as plasmacytic subtype, HHV8 negative, multicentric Castleman’s disease. For treatment, we relied on administrations of Siltuximab treatment therapy that showed complete remission. Castleman’s disease presents a unique diagnostic challenge, but a confirmatory diagnosis can be based on a biopsy examination, advisable after each relapse.

Abstract

Background: In Italy, surgeons continue to drain the abdominal cavity in more than 50 per cent of patients after colorectal resection. The aim of this study was to evaluate the impact of abdominal drain placement on early adverse events in patients undergoing elective colorectal surgery. Methods: A database was retrospectively analysed through a 1:1 propensity score-matching model including 21 covariates. The primary endpoint was the postoperative duration of stay, and the secondary endpoints were surgical site infections, infectious morbidity rate defined as surgical site infections plus pulmonary infections plus urinary infections, anastomotic leakage, overall morbidity rate, major morbidity rate, reoperation and mortality rates. The results of multiple logistic regression analyses were presented as odds ratios (OR) and 95 per cent c.i. Results: A total of 6157 patients were analysed to produce two well-balanced groups of 1802 patients: group (A), no abdominal drain(s) and group (B), abdominal drain(s). Group A versus group B showed a significantly lower risk of postoperative duration of stay >6 days (OR 0.60; 95 per cent c.i. 0.51–0.70; P < 0.001). A mean postoperative duration of stay difference of 0.86 days was detected between groups. No difference was recorded between the two groups for all the other endpoints. Conclusion: This study confirms that placement of abdominal drain(s) after elective colorectal surgery is associated with a non-clinically significant longer (0.86 days) postoperative duration of stay but has no impact on any other secondary outcomes, confirming that abdominal drains should not be used routinely in colorectal surgery.

Abstract

Retrospective evaluation of the effects of mechanical bowel preparation (MBP) on data derived from two prospective open-label observational multicenter studies in Italy regarding elective colorectal surgery. MBP for elective colorectal surgery remains a controversial issue with contrasting recommendations in current guidelines. The Italian ColoRectal Anastomotic Leakage (iCral) study group, therefore, decided to estimate the effects of no MBP (treatment variable) versus MBP for elective colorectal surgery. A total of 8359 patients who underwent colorectal resection with anastomosis were enrolled in two consecutive prospective studies in 78 surgical centers in Italy from January 2019 to September 2021. A retrospective PSMA was performed on 5455 (65.3%) cases after the application of explicit exclusion criteria to eliminate confounders. The primary endpoints were anastomotic leakage (AL) and surgical site infections (SSI) rates; the secondary endpoints included SSI subgroups, overall and major morbidity, reoperation, and mortality rates. Overall length of postoperative hospital stay (LOS) was also considered. Two well-balanced groups of 1125 patients each were generated: group A (No MBP, true population of interest), and group B (MBP, control population), performing a PSMA considering 21 covariates. Group A vs. group B resulted significantly associated with a lower risk of AL [42 (3.5%) vs. 73 (6.0%) events; OR 0.57; 95% CI 0.38–0.84; p = 0.005]. No difference was recorded between the two groups for SSI [73 (6.0%) vs. 85 (7.0%) events; OR 0.88; 95% CI 0.63–1.22; p = 0.441]. Regarding the secondary endpoints, no MBP resulted significantly associated with a lower risk of reoperation and LOS > 6 days. This study confirms that no MBP before elective colorectal surgery is significantly associated with a lower risk of AL, reoperation rate, and LOS < 6 days when compared with MBP.

Abstract

We aim to investigate the galaxy environment in GAMA Galaxy Groups Catalogue (G3C) using a volume-limited galaxy sample from the Kilo Degree Survey Data Release 3. The k-Nearest Neighbour technique is adapted to take into account the probability density functions (PDFs) of photometric redshifts in our calculations. This algorithm was tested on simulated KiDS tiles, showing its capability of recovering the relation between galaxy colour, luminosity and local environment. The characterization of the galaxy environment in G3C groups shows systematically steeper density contrasts for more massive groups. The red galaxy fraction gradients in these groups is evident for most of group mass bins. The density contrast of red galaxies is systematically higher at group centers when compared to blue galaxy ones. In addition, distinct group center definitions are used to show that our results are insensitive to center definitions. These results confirm the galaxy evolution scenario which environmental mechanisms are responsible for a slow quenching process as galaxies fall into groups and clusters, resulting in a smooth observed colour gradients in galaxy systems., Comment: 14 pages, Accepted to MNRAS

Abstract

We present a weak gravitational lensing analysis of 815 square degree of $i$-band imaging from the Kilo-Degree Survey (KiDS-$i$-800). In contrast to the deep $r$-band observations, which take priority during excellent seeing conditions and form the primary KiDS dataset (KiDS-$r$-450), the complementary yet shallower KiDS-$i$-800 spans a wide range of observing conditions. The overlapping KiDS-$i$-800 and KiDS-$r$-450 imaging therefore provides a unique opportunity to assess the robustness of weak lensing measurements. In our analysis, we introduce two new `null' tests. The `nulled' two-point shear correlation function uses a matched catalogue to show that the calibrated KiDS-$i$-800 and KiDS-$r$-450 shear measurements agree at the level of $1 \pm 4$\%. We use five galaxy lens samples to determine a `nulled' galaxy-galaxy lensing signal from the full KiDS-$i$-800 and KiDS-$r$-450 surveys and find that the measurements agree to $7 \pm 5$\% when the KiDS-$i$-800 source redshift distribution is calibrated using either spectroscopic redshifts, or the 30-band photometric redshifts from the COSMOS survey., Comment: 24 pages, 20 figures. Abstract

We present measurements of the weak gravitational lensing shear power spectrum based on $450$ sq. deg. of imaging data from the Kilo Degree Survey. We employ a quadratic estimator in two and three redshift bins and extract band powers of redshift auto-correlation and cross-correlation spectra in the multipole range $76 \leq \ell \leq 1310$. The cosmological interpretation of the measured shear power spectra is performed in a Bayesian framework assuming a $\Lambda$CDM model with spatially flat geometry, while accounting for small residual uncertainties in the shear calibration and redshift distributions as well as marginalising over intrinsic alignments, baryon feedback and an excess-noise power model. Moreover, massive neutrinos are included in the modelling. The cosmological main result is expressed in terms of the parameter combination $S_8 \equiv \sigma_8 \sqrt{\Omega_{\rm m}/0.3}$ yielding $S_8 = \ 0.651 \pm 0.058$ (3 z-bins), confirming the recently reported tension in this parameter with constraints from Planck at $3.2\sigma$ (3 z-bins). We cross-check the results of the 3 z-bin analysis with the weaker constraints from the 2 z-bin analysis and find them to be consistent. Abstract

The Kilo-Degree Survey (KiDS) is an ongoing optical wide-field imaging survey with the OmegaCAM camera at the VLT Survey Telescope. It aims to image 1500 square degrees in four filters (ugri). The core science driver is mapping the large-scale matter distribution in the Universe, using weak lensing shear and photometric redshift measurements. Further science cases include galaxy evolution, Milky Way structure, detection of high-redshift clusters, and finding rare sources such as strong lenses and quasars. Here we present the third public data release (DR3) and several associated data products, adding further area, homogenized photometric calibration, photometric redshifts and weak lensing shear measurements to the first two releases. A dedicated pipeline embedded in the Astro-WISE information system is used for the production of the main release. Modifications with respect to earlier releases are described in detail. Photometric redshifts have been derived using both Bayesian template fitting, and machine-learning techniques. For the weak lensing measurements, optimized procedures based on the THELI data reduction and lensfit shear measurement packages are used. In DR3 stacked ugri images, weight maps, masks, and source lists for 292 new survey tiles (~300 sq.deg) are made available. The multi-band catalogue, including homogenized photometry and photometric redshifts, covers the combined DR1, DR2 and DR3 footprint of 440 survey tiles (447 sq.deg). Limiting magnitudes are typically 24.3, 25.1, 24.9, 23.8 (5 sigma in a 2 arcsec aperture) in ugri, respectively, and the typical r-band PSF size is less than 0.7 arcsec. The photometric homogenization scheme ensures accurate colors and an absolute calibration stable to ~2% for gri and ~3% in u. Abstract

We present cosmological parameter constraints from a tomographic weak gravitational lensing analysis of ~450deg$^2$ of imaging data from the Kilo Degree Survey (KiDS). For a flat $\Lambda$CDM cosmology with a prior on $H_0$ that encompasses the most recent direct measurements, we find $S_8\equiv\sigma_8\sqrt{\Omega_{\rm m}/0.3}=0.745\pm0.039$. This result is in good agreement with other low redshift probes of large scale structure, including recent cosmic shear results, along with pre-Planck cosmic microwave background constraints. A $2.3$-$\sigma$ tension in $S_8$ and `substantial discordance' in the full parameter space is found with respect to the Planck 2015 results. We use shear measurements for nearly 15 million galaxies, determined with a new improved `self-calibrating' version of $lens$fit validated using an extensive suite of image simulations. Four-band $ugri$ photometric redshifts are calibrated directly with deep spectroscopic surveys. The redshift calibration is confirmed using two independent techniques based on angular cross-correlations and the properties of the photometric redshift probability distributions. Our covariance matrix is determined using an analytical approach, verified numerically with large mock galaxy catalogues. We account for uncertainties in the modelling of intrinsic galaxy alignments and the impact of baryon feedback on the shape of the non-linear matter power spectrum, in addition to the small residual uncertainties in the shear and redshift calibration. The cosmology analysis was performed blind. Abstract

We present the Red-sequence Cluster Lensing Survey (RCSLenS), an application of the methods developed for the Canada France Hawaii Telescope Lensing Survey (CFHTLenS) to the ~785deg$^2$, multi-band imaging data of the Red-sequence Cluster Survey 2 (RCS2). This project represents the largest public, sub-arcsecond seeing, multi-band survey to date that is suited for weak gravitational lensing measurements. With a careful assessment of systematic errors in shape measurements and photometric redshifts we extend the use of this data set to allow cross-correlation analyses between weak lensing observables and other data sets. We describe the imaging data, the data reduction, masking, multi-colour photometry, photometric redshifts, shape measurements, tests for systematic errors, and a blinding scheme to allow for more objective measurements. In total we analyse 761 pointings with r-band coverage, which constitutes our lensing sample. Residual large-scale B-mode systematics prevent the use of this shear catalogue for cosmic shear science. The effective number density of lensing sources over an unmasked area of 571.7deg$^2$ and down to a magnitude limit of r~24.5 is 8.1 galaxies per arcmin$^2$ (weighted: 5.5 arcmin$^{-2}$) distributed over 14 patches on the sky. Photometric redshifts based on 4-band griz data are available for 513 pointings covering an unmasked area of 383.5 deg$^2$ We present weak lensing mass reconstructions of some example clusters as well as the full survey representing the largest areas that have been mapped in this way. Abstract

In this paper we present results of applying the shear-ratio method to the RCSLenS data. The method takes the ratio of the mean of the weak lensing tangential shear signal about galaxy clusters, averaged over all clusters of the same redshift, in multiple background redshift bins. In taking a ratio the mass-dependency of the shear signal is cancelled-out leaving a statistic that is dependent on the geometric part of the lensing kernel only. We apply this method to 535 clusters and measure a cosmology-independent distance-redshift relation to redshifts z~1. In combination with Planck data the method lifts the degeneracies in the CMB measurements, resulting in cosmological parameter constraints of OmegaM=0.31 +/- 0.10 and w0 = -1.02 +/- 0.37, for a flat wCDM cosmology., Comment: 9 pages, 5 figures. Abstract

We present measurements of the galaxy bias $b$ and the galaxy-matter cross-correlation coefficient $r$ for the BOSS LOWZ luminous red galaxy sample. Using a new statistical weak lensing analysis of the Red Sequence Cluster Lensing Survey (RCSLenS) we find the bias properties of this sample to be higher than previously reported with $b=2.45^{+0.05}_{-0.05}$ and $r=1.64^{+0.17}_{-0.16}$ on scales between $3'$ and $20'$. We repeat the measurement for angular scales of $20'\leq \vartheta \leq70'$, which yields $b=2.39^{+0.07}_{-0.07}$ and $r=1.24^{+0.26}_{-0.25}$. This is the first application of a data compression analysis using a complete set of discrete estimators for galaxy-galaxy lensing and galaxy clustering. As cosmological data sets grow, our new method of data compression will become increasingly important in order to interpret joint weak lensing and galaxy clustering measurements and to estimate the data covariance. In future studies this formalism can be used as a tool to study the large-scale structure of the Universe to yield a precise determination of cosmological parameters., Comment: 14 pages, 14 figures, accepted for publication in MNRAS

The PI(3,4)P2 4-phosphatase, INPP4B, functions as a tumour suppressor in triple negative breast cancer. Here, the authors show that INPP4B enhances proliferation and growth of PIK3CA-mutant ER+ breast cancers by promoting PI3Kα-dependent late endosome formation and trafficking that leads to the activation of Wnt/β-catenin signalling.

Abstract

The Kilo-Degree Survey (KiDS) is an optical wide-field survey designed to map the matter distribution in the Universe using weak gravitational lensing. In this paper, we use these data to measure the density profiles and masses of a sample of $\sim \mathrm{1400}$ spectroscopically identified galaxy groups and clusters from the Galaxy And Mass Assembly (GAMA) survey. We detect a highly significant signal (signal-to-noise-ratio $\sim$ 120), allowing us to study the properties of dark matter haloes over one and a half order of magnitude in mass, from $M \sim 10^{13}-10^{14.5} h^{-1}\mathrm{M_{\odot}}$. We interpret the results for various subsamples of groups using a halo model framework which accounts for the mis-centring of the Brightest Cluster Galaxy (used as the tracer of the group centre) with respect to the centre of the group's dark matter halo. We find that the density profiles of the haloes are well described by an NFW profile with concentrations that agree with predictions from numerical simulations. In addition, we constrain scaling relations between the mass and a number of observable group properties. We find that the mass scales with the total r-band luminosity as a power-law with slope $1.16 \pm 0.13$ (1-sigma) and with the group velocity dispersion as a power-law with slope $1.89 \pm 0.27$ (1-sigma). Finally, we demonstrate the potential of weak lensing studies of groups to discriminate between models of baryonic feedback at group scales by comparing our results with the predictions from the Cosmo-OverWhelmingly Large Simulations (Cosmo-OWLS) project, ruling out models without AGN feedback., Comment: 23 pages, 17 figures, MNRAS accepted, cross-references to other KiDS/GAMA papers fixed in v2; Weak lensing catalogues are available at http://kids.strw.leidenuniv.nl

Abstract

Euclid is the next ESA mission devoted to cosmology. It aims at observing most of the extragalactic sky, studying both gravitational lensing and clustering over $\sim$15,000 square degrees. The mission is expected to be launched in year 2020 and to last six years. The sheer amount of data of different kinds, the variety of (un)known systematic effects and the complexity of measures require efforts both in sophisticated simulations and techniques of data analysis. Abstract

Teaching of Psychology includes a great variety of topics, course formats, and assessment approaches. A central concept that incorporated the interface between teaching goals, instructional methods, and examination modalities is referred to as Constructive Alignment (CA). This model addresses possible designs of teaching to improve students' learning outcomes as well as enhance their learning experiences, and claims to be applicable independent of disciplinary culture or content. Despite the importance of this approach from an instructional point of view, there is hardly any research, so far, that has been concerned with capturing the three dimensions of CA. As a consequence, the aim of our study was to create an instrument to assess the quality of CA within psychology classes. A questionnaire was designed and was additionally analyzed with regard to students' judgements about overall course evaluation. The questionnaire was employed in two lectures within the field of educational psychology for teacher training students. Results reveal that overall course evaluation can be predicted by the match between course objectives and instructional methods whereas other course evaluation factors failed as predictors. With a high internal consistency, the instrument provides an alternative or a supplement for traditional course evaluation instruments.

Abstract

Background: Since most anastomoses after left-sided colorectal resections are performed with a circular stapler, any technological change in stapling devices may influence the incidence of anastomotic adverse events. The aim of the present study was to analyze the effect of a three-row circular stapler on anastomotic leakage and related morbidity after left-sided colorectal resections. Materials and methods: A circular stapled anastomosis was performed in 4255 (50.9%) out of 8359 patients enrolled in two prospective multicenter studies in Italy, and, after exclusion criteria to reduce heterogeneity, 2799 (65.8%) cases were retrospectively analyzed through a 1:1 propensity score-matching model including 20 covariates relative to patient characteristics, to surgery and to perioperative management. Two well-balanced groups of 425 patients each were obtained: group (A) – true population of interest, anastomosis performed with a three-row circular stapler; group (B) – control population, anastomosis performed with a two-row circular stapler. The target of inferences was the average treatment effect in the treated (ATT). The primary endpoints were overall and major anastomotic leakage and overall anastomotic bleeding; the secondary endpoints were overall and major morbidity and mortality rates. The results of multiple logistic regression analyses for the outcomes, including the 20 covariates selected for matching, were presented as odds ratios (OR) and 95% confidence intervals (95% CI). Abstract

Severe shortages of skilled health workforce remain a major barrier to universal health coverage in low income countries including Liberia where nurses and midwives form more than 50% of the health workforce. According to the 2018 Service Availability and Readiness Assessment (SARA) report, Liberia has 10.7 core healthcare workers per 10,000 people, far below the WHO benchmark of 23/10,000 people. High quality training for nurses and midwives is one of the most important strategies to addressing these health workforce shortages. Since 2015, William V.S Tubman University (TU) faculty and Partners in Health (PIH) have partnered in nursing and midwifery education to address nursing and midwifery workforce shortages in Southeast Liberia. In our collaboration we have sought to not only increase the quantity of graduate nurses and midwives but also improve the quality of the training to ensure they are equipped to serve the population. TU strives to produce highly competent generic nurses who will excel in their clinical practice and future specialized training. By applying the theory of deliberate practice, learners are allowed to practice and self-evaluate repeatedly until they attain proficiency. Simulation training was adopted early in the training of nurses and midwives at TU to ensure students are well-prepared for real-life patient care. TU also established a preceptorship program to ensure that students receive skilled mentorship during clinical rotations. Internship for graduating senior Nursing/Midwifery students, where they focus on enhancing psychomotor and assessment skills, professional communication, safety and organization, medication administration and documentation, ensures successful integration into clinical practice after graduation. This progression of the student nurse or midwife from the exposure in the skills lab during pre-clinical modules, to individual preceptorship during clinical rotations to a structured internship experience with an intensive pre-internship “boot camp” have been the major innovations that have helped our partnership flourish. The foundation of these interventions is strong and sustained investment in nursing and midwifery faculty both at the university and the health facilities.

Abstract

In this paper we present results from the weak lensing shape measurement GRavitational lEnsing Accuracy Testing 2010 (GREAT10) Galaxy Challenge. This marks an order of magnitude step change in the level of scrutiny employed in weak lensing shape measurement analysis. We provide descriptions of each method tested and include 10 evaluation metrics over 24 simulation branches. GREAT10 was the first shape measurement challenge to include variable fields; both the shear field and the Point Spread Function (PSF) vary across the images in a realistic manner. The variable fields enable a variety of metrics that are inaccessible to constant shear simulations including a direct measure of the impact of shape measurement inaccuracies, and the impact of PSF size and ellipticity, on the shear power spectrum. To assess the impact of shape measurement bias for cosmic shear we present a general pseudo-Cl formalism, that propagates spatially varying systematics in cosmic shear through to power spectrum estimates. We also show how one-point estimators of bias can be extracted from variable shear simulations. The GREAT10 Galaxy Challenge received 95 submissions and saw a factor of 3 improvement in the accuracy achieved by shape measurement methods. The best methods achieve sub-percent average biases. We find a strong dependence in accuracy as a function of signal-to-noise, and indications of a weak dependence on galaxy type and size. Some requirements for the most ambitious cosmic shear experiments are met above a signal-to-noise ratio of 20. These results have the caveat that the simulated PSF was a ground-based PSF. Our results are a snapshot of the accuracy of current shape measurement methods and are a benchmark upon which improvement can continue. Abstract

We analyse the KSB method to estimate gravitational shear from surface-brightness moments of small and noisy galaxy images. We identify three potentially problematic assumptions. These are: (1) While gravitational shear must be estimated from averaged galaxy images, KSB derives a shear estimate from each individual image and then takes the average. Since the two operations do not commute, KSB gives biased results. (2) KSB implicitly assumes that galaxy ellipticities are small, while weak gravitational lensing assures only that the change in ellipticity due to the shear is small. (3) KSB does not invert the convolution with the point-spread function, but gives an approximate PSF correction which - even for a circular PSF - holds only in the limit of circular sources. The effects of assumptions (2) and (3) partially counter-act in a way dependent on the width of the weight function and of the PSF. We quantitatively demonstrate the biases due to all assumptions, extend the KSB approach consistently to third order in the shear and ellipticity and show that this extension lowers the biases substantially. Abstract

We construct two linear filtering techniques based on weak gravitational lensing to constrain the inner slope $\alpha$ of the density profile of dark-matter halos. Both methods combine all available information into an estimate of this single number. Under idealised assumptions, $\alpha$ is constrained to ~15% if the halo concentration c is known, and to < 30% if not. We argue that the inevitable degeneracy between density-profile slope and halo concentration cannot be lifted under realistic conditions, and show by means of Fisher-matrix methods which linear combination of $\alpha$ and c is best constrained by our filtering of lensing data. This defines a new parameter, called P1, which is then constrained to ~15% for a single massive halo. In order to study the process of cooling in dark-matter (DM) halos and assess how well simple models can represent it, we run a set of radiative SPH hydrodynamical simulations of isolated halos, with gas sitting initially in hydrostatic equilibrium within Navarro-Frenk-White (NFW) potential wells. [...] After having assessed the numerical stability of the simulations, we compare the resulting evolution of the cooled mass with the predictions of the classical cooling model of White & Frenk and of the cooling model proposed in the MORGANA code of galaxy formation. We find that the classical model predicts fractions of cooled mass which, after about two central cooling times, are about one order of magnitude smaller than those found in simulations. Although this difference decreases with time, after 8 central cooling times, when simulations are stopped, the difference still amounts to a factor of 2-3. We ascribe this difference to the lack of validity of the assumption that a mass shell takes one cooling time, as computed on the initial conditions, to cool to very low temperature. [...] The MORGANA model [...] better agrees with the cooled mass fraction found in the simulations, especially at early times, when the density profile of the cooling gas is shallow. With the addition of the simple assumption that the increase of the radius of the cooling region is counteracted by a shrinking at the sound speed, the MORGANA model is also able to reproduce for all simulations the evolution of the cooled mass fraction to within 20-50 per cent, thereby providing a substantial improvement with respect to the classical model. A project was undertaken at Cuyahoga Community College (CCC), in Ohio, to develop guidelines to maximize the utilization of advisory committees in technical programs. Surveys were distributed to 200 advisory committee members under the auspices of Business and Technology at CCC's Western Campus asking them to rate 26 essential characteristics of advisory committees and members presented in alphabetical order. Based on responses received from 94 members, the following 12 categories were identified as measurable guidelines for the role of advisory committee members: (1) active participation; (2) curriculum development; (3) donations; (4) employment opportunities; (5) field trips to businesses; (6) guest speakers; (7) internships and externships; (8) knowledge and expertise; (9) quality control and standards; (10) student recruitment; (11) building the part-time and substitute teaching pool; and (12) the promotion of programs. The 14 additional areas also received support from respondents and were related to the affective domain or personal traits and characteristics of members. These include communication skills, computational skills, and employability skills. 36. Management of a case of Castleman’s disease coexisting with or occurring after transplanted Hodgkin’s lymphoma

Abstract

Castleman’s disease is a benign lymphoproliferative disorder. The coexistence of Hodgkin’s lymphoma and multicentric Castle-man’s disease is a rare phenomenon. We discuss a case of a 48-year-old female patient who had been in the records of the Colentina Hematology Clinic since 2019, with the diagnosis of classic Hodgkin’s Lymphoma, nodular sclerosis type I BNLI, stage IIXB. For this, she underwent 3 courses of ABVD and 2 courses of BEACOPP, without showing complete remission on PET/CT evaluation at the end of treatment. After that, we initiated rescue therapy and performed 4 IGEV courses, followed by autologous stem cell transplantation. For maintenance treatment, we opted for Brentuximab, but it was discontinued after the first administration due to the appearance of adverse reactions. Subsequently, we decided to perform radiotherapy with 20 fractions cumulating a total dose of 36 Gy. Shortly after the radiotherapy, symptoms reappeared which were suspected to be in the context of a relapse of the disease. For confirmatory diagnosis, we performed a new PET-CT which highlighted metabolically active ganglion images. Further, were carried out lymph node biopsy for histopathological and immunohistochemical examinations were carried out. The underlying disease was diagnosed as plasmacytic subtype, HHV8 negative, multicentric Castleman’s disease. For treatment, we relied on administrations of Siltuximab treatment therapy that showed complete remission. Castleman’s disease presents a unique diagnostic challenge, but a confirmatory diagnosis can be based on a biopsy examination, advisable after each relapse.

In the literature, various papers have developed more parsimonious Markov model structures concerning full Markov chain models, such as Variable Length Markov Chains (VLMC) and Partition Markov Models (PMM), see [1] and [2]. In the case of a multivariate Markov process, a family of models reduces the number of parameters considering a structure of conditional dependence for the marginal processes (structure of interaction of the process), see [3]. In this work, we infer the interaction structure for three correlates of rhythm for each one of three languages classified as having different rhythmic properties. Abstract

Two-region image segmentation is the process of dividing an image into two regions of interest, i.e., the foreground and the background. To this aim, Chan et al. (SIAM J Appl Math 66(5):1632–1648, 2006) designed a model well suited for smooth images. One drawback of this model is that it may produce a bad segmentation when the image contains oscillatory components. Based on a cartoon-texture decomposition of the image to be segmented, we propose a new model that is able to produce an accurate segmentation of images also containing noise or oscillatory information like texture. The novel model leads to a non-smooth constrained optimization problem which we solve by means of the ADMM method. The convergence of the numerical scheme is also proved. Several experiments on smooth, noisy, and textural images show the effectiveness of the proposed model.

Abstract

Background: Enhanced recovery after surgery (ERAS) programs influence morbidity rates and length of stay after colorectal surgery (CRS), and may also impact major complications and anastomotic leakage rates. A prospective multicenter observational study to investigate the interactions between ERAS program adherence and early outcomes after elective CRS was carried out. Methods: Prospective enrolment of patients submitted to elective CRS with anastomosis in 18 months. Adherence to 21 items of ERAS program was measured upon explicit criteria in every case. After univariate analysis, independent predictors of primary endpoints [major morbidity (MM) and anastomotic leakage (AL) rates] were identified through logistic regression analyses including all significant variables, presenting odds ratios (OR). Results: Institutional ERAS protocol was declared by 27 out of 38 (71.0%) participating centers. Median overall adherence to ERAS program items was 71.4%. Among 3830 patients included in the study, MM and AL rates were 4.7% and 4.2%, respectively. MM rates were independently influenced by intra- and/or postoperative blood transfusions (OR 7.79, 95% CI 5.46–11.10; p < 0.0001) and standard anesthesia protocol (OR 0.68, 95% CI 0.48–0.96; p = 0.028). AL rates were independently influenced by male gender (OR 1.48, 95% CI 1.06–2.07; p = 0.021), intra- and/or postoperative blood transfusions (OR 4.29, 95% CI 2.93–6.50; p < 0.0001) and non-standard resections (OR 1.49, 95% CI 1.01–2.22; p = 0.049). Conclusions: This study disclosed wide room for improvement in compliance to several ERAS program items. It failed to detect any significant association between institutionalization and/or adherence rates to ERAS program with primary endpoints. These outcomes were independently influenced by gender, intra- and postoperative blood transfusions, non-standard resections, and standard anesthesia protocol.

Abstract

Background: In 2020, ACIE Appy study showed that COVID-19 pandemic heavily affected the management of patients with acute appendicitis (AA) worldwide, with an increased rate of non-operative management (NOM) strategies and a trend toward open surgery due to concern of virus transmission by laparoscopy and controversial recommendations on this issue. The aim of this study was to survey again the same group of surgeons to assess if any difference in management attitudes of AA had occurred in the later stages of the outbreak. Methods: From August 15 to September 30, 2021, an online questionnaire was sent to all 709 participants of the ACIE Appy study. The questionnaire included questions on personal protective equipment (PPE), local policies and screening for SARS-CoV-2 infection, NOM, surgical approach and disease presentations in 2021. The results were compared with the results from the previous study. Results: A total of 476 answers were collected (response rate 67.1%). Screening policies were significatively improved with most patients screened regardless of symptoms (89.5% vs. 37.4%) with PCR and antigenic test as the preferred test (74.1% vs. 26.3%). More patients tested positive before surgery and commercial systems were the preferred ones to filter smoke plumes during laparoscopy. Laparoscopic appendicectomy was the first option in the treatment of AA, with a declined use of NOM. Conclusion: Management of AA has improved in the last waves of pandemic. Increased evidence regarding SARS-COV-2 infection along with a timely healthcare systems response has been translated into tailored attitudes and a better care for patients with AA worldwide.

Abstract

This retrospective analysis of the prospective IGOMIPS registry reports on 1191 minimally invasive pancreatic resections (MIPR) performed in Italy between 2019 and 2022, including 668 distal pancreatectomies (DP) (55.7%), 435 pancreatoduodenectomies (PD) (36.3%), 44 total pancreatectomies (3.7%), 36 tumor enucleations (3.0%), and 8 central pancreatectomies (0.7%). Spleen-preserving DP was performed in 109 patients (16.3%). Overall incidence of severe complications (Clavien-Dindo & GE; 3) was 17.6% with a 90-day mortality of 1.9%. This registry analysis provided some important information. First, robotic assistance was preferred for all MIPR but DP with splenectomy. Second, robotic assistance reduced conversion to open surgery and blood loss in comparison to laparoscopy. Robotic PD was also associated with lower incidence of severe postoperative complications and a trend toward lower mortality. Fourth, the annual cut-off of & GE; 20 MIPR and & GE; 20 MIPD improved selected outcome measures. Fifth, most MIPR were performed by a single surgeon. Sixth, only two-thirds of the centers performed spleen-preserving DP. Seventh, DP with splenectomy was associated with higher conversion rate when compared to spleen-preserving DP. Eighth, the use of pancreatojejunostomy was the prevalent reconstruction in PD. Ninth, final histology was similar for MIPR performed at high- and low-volume centers, but neoadjuvant chemotherapy was used more frequently at high-volume centers. Finally, this registry analysis raises important concerns about the reliability of R1 assessment underscoring the importance of standardized pathology of pancreatic specimens. In conclusion, MIPR can be safely implemented on a national scale. Further analyses are required to understand nuances of implementation of MIPR in Italy.
