Abstract Background The Alpe-DPD study (NCT02324452) demonstrated that prospective genotyping and dose-individualization using four alleles in DPYD (DPYD*2A/rs3918290, c.1236G > A/rs75017182, c.2846A > T/rs67376798 and c.1679 T > G/rs56038477) can mitigate the risk of severe fluoropyrimidine toxicity. However, this could not prevent all toxicities. The goal of this study was to identify additional genetic variants, both inside and outside DPYD, that may contribute to fluoropyrimidine toxicity. Methods Biospecimens and data from the Alpe-DPD study were used. Exon sequencing was performed to identify risk variants inside DPYD. In silico and in vitro analyses were used to classify DPYD variants. A genome-wide association study (GWAS) with severe fluoropyrimidine-related toxicity was performed to identify variants outside DPYD. Association with severe toxicity was assessed using matched-pair analyses for the exon sequencing and logistic, Cox, and ordinal regression analyses for GWAS. Results Twenty-four non-synonymous, frameshift, and splice site DPYD variants were detected in ten of 986 patients. Seven of these variants (c.1670C > T, c.1913 T > C, c.1925 T > C, c.506delC, c.731A > C, c.1740 + 1G > T, c.763 − 2A > G) were predicted to be deleterious. The carriers of either of these variants showed a trend towards a 2.14-fold (95% CI, 0.41–11.3, P = 0.388) increased risk of severe toxicity compared to matched controls (N = 30). Abstract Background The relationship between insufficient financial resources and psychological health has been extensively studied and established in various contexts. However, there remains uncertainty regarding the potential impact of the Nigerian naira currency redesign policy on the psychological well-being of Nigerians. This policy, which aimed to demonetize the economy and promote economic stability, involved changes to the physical appearance of some naira denominations (200, 500 and 1000). Understanding the effects of this policy on psychological health is essential for evaluating its overall societal impact and identifying potential areas for improvement in future currency redesign initiatives. Methods The study is a cross-sectional mixed-methods study involving 2237 respondents across the six geopolitical zones of Nigeria. Utilizing the simple random, snowball and convenience sampling technique, social media platforms (Facebook and WhatsApp) were used to recruit respondents. Variables were analyzed at descriptive and inferential levels. The qualitative component comprised seven (7) in-depth interviews with participants across the geo-political zones. Results The perceptions of respondents towards the policy were diverse across different demographic groups. It was widely perceived that the timing of the policy was inappropriate, considering the challenges faced in utilizing online payment platforms and the significant inaccessibility of cash. Furthermore, the analysis revealed that demographic variables played a role in explaining systematic variations in the experience of financial scarcity and its effect on psychological health during the cash crunch that ensued as a result of the Nigerian naira currency redesign policy. Conclusions This study identified a significant association between the psychological inventory of financial scarcity and psychological well-being among residents in Nigeria during the cash crunch resulting from the Naira redesign policy. The findings suggest that the financial scarcity experienced by Nigerians due to the policy had a substantial impact on individuals’ psychological well-being. We recommend that a holistic approach be undertaken by policymakers to ensure that policy actions not only address economic objectives but also safeguard the mental health and overall well-being of the population.

With the advent of modern technology, advancements in processing and storage techniques, and increasing medical knowledge, people are becoming aware of deterioration in the quality of medicinal products due to storage methods and time. In most cases, herbal products are not consumed immediately after production; as such, improper storage can result in physical, chemical, and microbiological changes. The study evaluated the effect of storage methods and time on the quality of oil extracted from Phyllanthus amarus Schumach and Annona muricata Linn and assessed their antidiabetic and antioxidative effects. Plants were air-dried, pulverized, and then subjected to Soxhlet extraction in petroleum ether. The oil was evaluated for phytochemical constituents and the effects of time and storage methods on its physicochemical properties. Characterization of the oil was done by spectroscopic techniques. Oils from both plants contained tannins, flavonoids, alkaloids, steroids, glycosides, terpenoids, phlobotannins, resins, reducing sugar, phenols, and saponins in different proportions. The oil from A. muricata had higher phenolic (3.11±0.31 mgGAE/g), flavonoid (11.82±0.08 mgQUE/g), alkaloid (16.37 ± ± 0.56 mgAPE/g), and tannin (7.13 ± 0.47 mgCE/g) contents than the oil from P. amarus , which had 0.54 ± ± 0.08 mgGAE/g, 7.83 ± 0.13 mgQUE/g, 9.87 ± 0.15 mgAPE, and 3.16 ± 0.12 mgCE/g for total phenolic, flavonoids, alkaloids, and tannins, respectively. Initial acid, iodine, peroxide, and saponification values recorded for P. amarus were 5.63 ± 0.82 mgKOH/g, 97.17 ±0.53 Wijis, 9.31 ± 0.15 mEq/kg, and 116.11 ± 0.74 mgKOH/g, respectively, significantly different from those of A. muricata, which had values of 1.17 ± 0.08 mgKOH, 76.23 ± ± 0.03 Wijis, 6.75 ± 0.47 mEq/kg, and 193.31 ± 0.52 mgKOH/g, respectively. FT-IR characterization of the oils revealed the presence of carboxylic acid, alkyl, alkene, alkane, haloalkane, aldehyde, aromatic amine, α-unsaturated and β-unsaturated esters, and phenol functional groups. P. amarus oil inhibited α-amylase (IC50 0.17 ± ± 0.03 mg/ml), α-glucosidase (IC50 0.64 ± 0.03 mg/ml), and xanthine oxidase (0.70 ± 0.01 mg/ml) to a greater extent than A. muricata oil, with IC50 values of 0.43 ± 0.05 mg/ml (α-amylase), 2.25 ± 0.31 mg/ml (α-glucosidase), and 0.78 ± 0.07 mg/ml (xanthine oxidase). This study showed that oils from the tested plants have low rancidity with a moderate shelf life. The extracts contained essential phytoconstituents that significantly inhibited α-glucosidase and xanthine oxidase. These effects of the oil indicate their potential to prevent diabetes, gout, and oxidative stress. Consequently, the supply of P. amarus and A. muricata in homemade diets is strongly encouraged for healthy living.

Fractionation of the stem bark of Enantia chlorantha Oliv yields three alkaloids, palmatine (1), jatrorrhizine (2), columbamine (3), and β-Sitosterol (4). In this investigation, density functional theory (DFT) calculations were carried out to evaluate the electronic structure and properties of 1–4 by DFT-B3LYP/6-31G level of theory using Gaussian 09 software. The highest occupied molecular orbital (HOMO), lowest unoccupied molecular orbital (LUMO), HOMO-LUMO energy difference (band gap), hardness (η), softness (S), dipole moment (μ), electronegativity (χ), hydrophobicity (logP), topological surface area (TPSA), and energy gap (Eg) were calculated. The in vitro cytotoxicity of the compounds was investigated against MCF-7 and HCT116 cancer cell lines using Wi-38 cells as a control. The compounds inhibited the proliferation of the MCF-7 and HCT116 cell lines and induced apoptosis via upregulation of caspase-3, Bax, PARP cleavage, and downregulation of Bcl-2. DFT analyses revealed that compounds 1 and 3 have smaller energy gaps, 0.072 and 0.071eV, respectively, with the highest dipole moments; hence, these compounds are more chemically reactive and exhibit better modulation of caspase-3 enzyme and inhibitory activities of the MCF-3 and HCT116 cell lines. The antimicrobial and antiparasitic evaluation of 1–4 showed moderate efficacy against the bacterial strains and moderate antiparasitic activity against Cichlidogyrus tilapia.

The practical utilization of TiO2 as a semiconductor in dye-sensitized solar cells (DSSCs) has been set back by poor visible light absorption, high charge carrier recombination, and low electrical conductivity, which reduce the power conversion efficiency (PCE) and sustainability of the device. In this respect, perovskites with excellent properties, such as large surface area, good optical properties, high electrical conductivity, and superior electrochemical stability, have recently emerged as promising alternatives capable of overcoming the drawbacks of TiO2. Herein, Sr0.7Sm0.3Fe0.4Co0.6O2.65 (SSFC) perovskites were prepared via the ball milling method at various milling times of 0, 5, and 10 h, and the obtained samples were denoted by SSFC-0, SSCF-5, and SSCF-10, respectively. Increasing the ball milling time led to a significant reduction in nanoparticle size and agglomeration, which, in turn, increased the surface area and electrical conductivity of the samples. As a consequence, the SSFC-10 perovskite exhibited the smallest average particle sizes (18.9 nm) with the largest surface area (61.8 m2 g−1) and minimum defects, which allowed for efficient electron transport, resulting in the best electrical conductivity of 49.8 S cm−1. Ultimately, DSSCs fabricated using SSFC-10 semiconductor layers achieved an optimum PCE of 6.01 %, which is an improvement of 8.67 %, 1.1 %, and 6.56 % for SSFC-0 (3.69 %), SSFC-5 (4.96 %), and TiO2 (5.64 %), respectively. Thus, varying the ball milling time can be used as an effective technique to tailor the physicochemical properties of SSFC to suit desired applications, particularly the fabrication of highly efficient and sustainable DSSC semiconductor layers.

Abstract Background Schistosomiasis control relies on praziquantel for preventive chemotherapy. Alternative drugs are needed for the treatment and control of schistosomiasis. Praziquantel is effective against adult schistosome worms but ineffective against larval stages of the parasite and cannot prevent re-infection or interrupt the transmission of infection. Continued reliance on praziquantel for wide-scale schistosomiasis control will likely accelerate the emergence of drug resistance. Artemisinin derivatives are effective against the juvenile stages but ineffective against adult worms. The SCHISTOACT study aimed to evaluate the efficacy and safety of praziquantel plus one of four artemisinin-based combinations in treating Schistosoma mansoni infection in Kenya. Methods The SCHISTOACT study is an open-label, head-to-head, five-arm, proof-of-concept, non-inferiority, individually randomized controlled trial with a follow-up of 12 weeks. A total of 540 primary school-aged children from the Mwea area, Kirinyaga County in central Kenya, diagnosed with S. mansoni infection (by Kato-Katz method) are randomly allocated (1:1:1:1:1) to a single dose of praziquantel plus a 3-day course of artesunate-sulfalene/pyrimethamine, or artesunate-amodiaquine, or artesunate plus mefloquine, or dihydroartemisinin-piperaquine, or praziquantel control arm. The primary endpoints are efficacy (cure rate, assessed by microscopy) and safety (adverse events) of each study arm 6 weeks after treatment. Secondary endpoints include cumulative cure rate, egg reduction rate, and re-infection 12 weeks after treatment. The non-inferiority margin is set at − 10 for the risk difference in cure rates between praziquantel and the combined treatment. Discussion This study assesses a strategy for repurposing artemisinin-based combination therapies (ACTs) for treating schistosomiasis. It adopts a head-to-head comparison of four different ACTs to test a non-inferiority hypothesis and to strengthen local capacity to conduct clinical trials for interventions against neglected tropical diseases. Cyperus conglomeratus has been utilized in traditional medicine as an emollient, diuretic, analgesic, anthelmintic, and for other diseases. Furthermore, several biological activities have been reported for the plant extract and the isolated metabolites. The chromatographic investigation of an ethyl acetate extract of the aerial parts led to the isolation of three aurone derivatives (1–3) from the plant for the first time. Their structures were identified as aureusidin (1), aureusidin-4-methyl ether (2), and 5-methyl aureusidin (3) using 1D and 2D NMR techniques, along with mass spectrometry. The compounds were tested for their inhibitory activities against enzymes vital in metabolic diseases, especially diabetes, such as α-amylase, α-glucosidase, and glycogen phosphorylase. The results were expressed as percentage inhibition. The inhibitory activity of aurones against the tested enzymes was also analyzed by computational docking studies to provide a rational explanation for the observed results. The tested compounds formed stable interactions in terms of hydrogen bonding and hydrophobic interaction with the active site residues of the tested enzymes, and the results are in agreement with those of the in vitro antidiabetic activity. The compounds were also evaluated for their ability to support the growth and viability of beneficial bacteria in terms of prebiotic activities using two species, Lacticaseibacillus paracasei and Lacticaseibacillus rhamnosus, through the determination of prebiotic activity scores (Pscore). The findings of this study showed that C. conglomeratus is a potential natural source of bioactive agents. There is, however, a need for in vivo testing to evaluate this plant’s efficacy for developing new drug entities in the future.

Chronic heart disease (CHD) is a widespread and persistent health challenge that demands immediate attention. Early detection and accurate diagnosis are essential for effective treatment and management of this condition. To overcome this difficulty, we created a state-of-the-art IoT-Based Ambulatory Blood Pressure Monitoring System that provides real-time blood pressure readings, systolic, diastolic, and pulse rates at predefined intervals. This unique technology comes with a module that forecasts CHD’s early warning score. Various machine learning algorithms employed comprise Naïve Bayes, K-Nearest Neighbors (K-NN), random forest, decision tree, and Support Vector Machine (SVM). Using Naïve Bayes, the proposed model has achieved an impressive 99.44% accuracy in predicting blood pressure, a vital aspect of real-time intensive care for CHD. This IoT-based ambulatory blood pressure monitoring (IABPM) system will provide some advancement in the field of healthcare. The system overcomes the limitations of earlier BP monitoring devices, significantly reduces healthcare costs, and efficiently detects irregularities in chronic heart diseases. By implementing this system, we can take a significant step forward in improving patient outcomes and reducing the global burden of CHD. The system’s advanced features provide an accurate and reliable diagnosis that is essential for treating and managing CHD. Overall, this IoT-based ambulatory blood pressure monitoring system is an important tool for the early identification and treatment of CHD in the field of healthcare.

Abstract AI and machine learning are increasingly often applied in the medical industry. The COVID‐19 epidemic will start to spread quickly over the planet around the start of 2020. At hospitals, there were more patients than there were beds. It was challenging for medical personnel to identify the patient who needed treatment right away. A machine learning approach is used to predict COVID‐19 pandemic patients at high risk. To provide input data and output results that execute the machine learning model on the backend, a straightforward Python Flask web application is employed. Here, the XGBoost algorithm, a supervised machine learning method, is applied. In order to predict high‐risk patients based on their current underlying health issues, the model uses patient characteristics as well as criteria like age, sex, health issues including diabetes, asthma, hypertension, and smoking, among others. The XGBoost model predicts the patient's severity with an accuracy of about 98% after data pre‐processing and training. The most important factors to the models are chosen to be age, diabetes, sex, and obesity. Patients and hospital personnel will benefit from this project's assistance in making timely choices and taking appropriate action. This will let medical personnel decide how much time and space to devote to the COVID‐19 high‐risk patients. providing a treatment that is both efficient and ideal. With this programme and the necessary patient data, hospitals may decide whether a patient need immediate care or not.

ABSTRACT Acinetobacter baumannii is a nosocomial multidrug-resistant pathogen that specifically colonizes and infects patients in intensive care units (ICUs). Genome plasticity of A. baumannii may contribute to the rapid development of A. baumannii resistance during patient treatment. There is little clarity on the dynamics of colonization of A. baumannii in humans. We studied 269 serial isolates of A. baumannii colonizing the rectum of 32 adult patients in an ICU; 5 to 16 isolates were obtained from each patient over a period of weeks to months. The isolates were sequenced using the Illumina platform. Multi-locus sequence typing revealed genetic diversity with 13 sequence types (STs); the dominant ST2 accounted for 202 of the 269 isolates and was isolated from each patient. A core genome comparison of isolates was used to elucidate a detailed understanding of the relationship among isolates within an ST and the genomic relationship among STs. One to three STs were observed in the isolates from each patient. Instances of unchanged colonization, sequential lineage colonization, and colonization with multiple lineages of A. baumannii were seen. The number of resistance genes carried by the isolates varied from 2 to 17. The predominant genes corresponded to those encoding resistance to β-lactam and aminoglycoside antibiotics. Generally, there was a strong correlation between ST and a particular antimicrobial resistance gene profile. This study makes an important baseline contribution to information that will help us understand the role of A. baumannii colonization in the development of opportunistic infections in ICU patients. IMPORTANCE Acinetobacter baumannii is a multidrug-resistant nosocomial pathogen that colonizes and infects debilitated patients in the ICU. There is very little information on the genomic characteristics of colonizing strains. This information is important to understand the evolution of lineages of A. baumannii that develop resistance while patients receive antibiotic treatment in the ICU. Our study demonstrated different patterns of colonization of the rectum of ICU patients with different STs of A. baumannii while one ST colonized all patients. Some STs carried more antibiotic resistance genes compared to others. However, there was a correlation between ST and a particular resistance gene profile. Our results further elucidate the dynamics of enteric colonization of this opportunistic pathogen.

Plasmodium falciparum infection activates adipokinetic hormone signaling and lipid mobilization in the Anopheles gambiae mosquito vector to promote sporogony.

An examination of the diversity of Zingiberaceae in Thailand led to the discovery of two undescribed taxa within the genus Kaempferia: K. sakolchaii P. Saensouk, Saensouk & Boonma, a newly described species, and K. phuphanensis var. viridans P. Saensouk, Saensouk & Boonma, a novel variety. Additionally, our investigation encompassed the chromosome numbers, karyotypes, and pollen morphological characteristics of these taxa. Both taxa share a chromosome number of 2n = 22, accompanied by distinctive karyotype patterns. Kaempferia sakolchaii displays a karyotype of 12m + 10sm chromosomes, while K. phuphanensis var. viridans presents a karyotype of 10m + 10sm + 2st chromosomes. The pollen grains of K. sakolchaii and K. phuphanensis var. viridans are monads, spheroidal in shape, radially symmetrical, apolar, inaperturate, and exine sculpturing irregularly arranged elongated ornamentation elements with verrucate features. This comprehensive analysis provides valuable insights into the taxonomic classification of these two taxa, enriching our understanding of Zingiberaceae diversity in Thailand.

Abstract In practice, the capacitance from the electrochemical double layer formation on porous carbon‐based electrodes is still below preferred values, limiting their use in electrochemical capacitors. The current drive is to innovate ways that generate additional capacitance in the electrochemical double layer capacitive nature of carbon nanomaterials towards both a high specific energy density (Es) and power density (Ps). Herein we report the use of metal‐organic chemical vapor deposition (MOCVD) to coat multiwalled carbon nanotubes (MWCNTs) with anatase titanium dioxide (TiO2) to induce pseudocapacitive charge storage characteristics on a carbon‐based electrode. The study shows that MWCNTs were coated in bundles, and targeted TiO2 loadings were successfully attained, though the TiO2 agglomerates also increased with TiO2 wt.%. The 10 wt.% TiO2 TiO2‐MWCNT material displayed the best capacitive behavior with associated specific discharge capacitance (Cd), Es, and Ps values of 907 F kg−1, 55.56 Wh kg−1, and 2.78 W kg−1 at 0.1 A g−1, respectively, due to the synergistic effect of the two components of the composite. Additionally, the integral capacitance (Cs) of the 20 wt.% TiO2 material was enhanced more than 5000‐fold relative to that of the 5 wt.% TiO2 TiO2‐MWCNT composite at higher scan speeds of 100 and 200 mV s−1. Electrochemical measurements further demonstrated the possible positive tuning of capacitive characteristics (charge/discharge rates, Cd and Cs) with TiO2 wt.% control. The MOCVD synthesis method imparted the TiO2‐MWCNT composites with suitable traits that showed high potential in improving physicochemical processes favorable in electrical energy storage.

Abstract The study uses numerical weather prediction models to predict the occurrence of heavy convective rainfall associated with the passage of the African Easterly Wave (AEW) during the period 23–26 August 2017 over Nigeria. Fraction skill score (FSS) and method for object‐based diagnostic evaluation (MODE) verification techniques were applied to verify how well the models predict the high‐impact event and to demonstrate how these tools can support operational forecasting. Ensemble model forecasts at a convective scale from UK Met Office Unified Model (MetUM) and a one‐way nested weather research and forecasting (WRF) model were compared with the integrated multisatellite retrievals for global precipitation measurement (IMERG GPM). The purpose is to examine skills of improved model resolution and ensemble in reproducing rainfall forecasts on useful scales and how the skill varies with spatial scale. WRF 2 and 6 km model forecasts show comparable skill at smaller grid scales. The skill of MetUM improves dramatically when the verification statistics are applied to the ensemble mean of the binary fields of the individual member forecast. The object‐based analysis reveals a similar structure as observed, although displaced eastwards. Most improvement occurred for heavier rainfall events associated with the passage of the AEW. WRF 6 km compares reasonably well with WRF 2 km in terms of shape and structure of rainfall underscoring the ability of the model to reasonably represent convection at 6 km horizontal resolution. The ensemble members in MetUM explicitly reproduce convection at 4 km resolution but are displaced at about 166 km behind observed rainfall.

Abstract Background The World Health Organization recommends quinine plus clindamycin as first-line treatment of malaria in the first trimester of pregnancy and as a second-line treatment for uncomplicated falciparum malaria when artemisinin-based drug combinations are not available. The efficacy of quinine plus clindamycin was compared with that of artemether-lumefantrine in the treatment of uncomplicated Plasmodium falciparum malaria in children below 5 years of age. Methods An open-label, phase 3, randomized trial was conducted in western Kenya. Children aged 6–59 months with uncomplicated falciparum malaria were randomly assigned (1:1) via a computer-generated randomization list to receive 3 days of twice a day treatment with either oral quinine (20 mg/kg/day) plus clindamycin (20 mg/kg/day) or artemether-lumefantrine (artemether 20 mg, lumefantrine 120 mg) as one (for those weighing 5–14 kg) or two (for those weighing 15–24 kg) tablets per dose. The primary outcome was a PCR-corrected rate of adequate clinical and parasitological response (ACPR) on day 28 in the per-protocol population. Results Of the 384 children enrolled, 182/192 (94.8%) receiving quinine plus clindamycin and 171/192 (89.1%) receiving artemether-lumefantrine completed the study. The PCR-corrected ACPR rate was 44.0% (80 children) in the quinine plus clindamycin group and 97.1% (166 children) in the artemether-lumefantrine group (treatment difference − 53.1%, 95% CI − 43.5% to − 62.7%). At 72 h after starting treatment, 50.3% (94 children) in the quinine plus clindamycin group were still parasitaemic compared with 0.5% (1 child) in the artemether-lumefantrine group. Three cases of severe malaria were recorded as serious adverse events in the quinine plus clindamycin group. Conclusions The study found no evidence to support the use of a 3-day low dose course of quinine plus clindamycin in the treatment of uncomplicated falciparum malaria in children under 5 years of age in Kenya, where artemether-lumefantrine is still effective. Abstract Background Plasmodium ovale is a neglected malarial parasite that can form latent hypnozoites in the human liver. Over the last decade, molecular surveillance studies of non-falciparum malaria in Africa have highlighted that P. ovale is circulating below the radar, including areas where Plasmodium falciparum is in decline. To eliminate malaria where P. ovale is endemic, a better understanding of its epidemiology, asymptomatic carriage, and transmission biology is needed. Methods We performed a pilot study on P. ovale transmission as part of an ongoing study of human-to-mosquito transmission of P. falciparum from asymptomatic carriers. To characterize the malaria asymptomatic reservoir, cross-sectional qPCR surveys were conducted in Bagamoyo, Tanzania, over three transmission seasons. Positive individuals were enrolled in transmission studies of P. falciparum using direct skin feeding assays (DFAs) with Anopheles gambiae s.s. (IFAKARA strain) mosquitoes. For a subset of participants who screened positive for P. ovale on the day of DFA, we incubated blood-fed mosquitoes for 14 days to assess sporozoite development. Results Molecular surveillance of asymptomatic individuals revealed a P. ovale prevalence of 11% (300/2718), compared to 29% (780/2718) for P. falciparum. Prevalence for P. ovale was highest at the beginning of the long rainy season (15.5%, 128/826) in contrast to P. falciparum, which peaked later in both the long and short rainy seasons. Considering that these early-season P. ovale infections were low-density mono-infections (127/128), we speculate many were due to hypnozoite-induced relapse. Six of eight P. ovale-infected asymptomatic individuals who underwent DFAs successfully transmitted P. ovale parasites to A. gambiae. Conclusions Plasmodium ovale is circulating at 4–15% prevalence among asymptomatic individuals in coastal Tanzania, largely invisible to field diagnostics. A different seasonal peak from co-endemic P. falciparum, the capacity to relapse, and efficient transmission to Anopheles vectors likely contribute to its persistence amid control efforts focused on P. falciparum. The rise of antimicrobial resistance is a global challenge that requires a coordinated effort to address. In this study, we examined the genetic similarity of carbapenem-resistant Klebsiella pneumoniae (CRKP) in countries belonging to the Gulf Cooperation Council (GCC) to gain a better understanding of how these bacteria are spreading and evolving in the region. We used in silico genomic tools to investigate the occurrence and prevalence of different types of carbapenemases and their relationship to specific sequence types (STs) of CRKP commonly found in the region. We analyzed 720 publicly available genomes of multi-drug resistant K. pneumoniae isolates collected from six GCC countries between 2011 and 2020. Our findings showed that ST-14 and ST-231 were the most common STs, and 51.7% of the isolates carried blaOXA-48-like genes. Additionally, we identified rare carbapenemase genes in a small number of isolates. We observed a clonal outbreak of ST-231 in Oman, and four Saudi isolates were found to have colistin resistance genes. Our study offers a comprehensive overview of the genetic diversity and resistance mechanisms of CRKP isolates in the GCC region that could aid in developing targeted interventions to combat this pressing global issue.

Transitions between the extreme angular-momentum states of alkali D lines hold the potential for enabling accurate high-field optical magnetometry because of their very simple magnetic field dependence described only by a linear and a quadratic term, characterized by the two coefficients γ_{1} and γ_{2}. Here, we present very accurate measurements of these coefficients, for the cesium D_{2} line, thereby overcoming a major obstacle for the realization of this future technology. By means of saturated absorption spectroscopy on a cesium gas, in 3 T and 7 T magnetic fields, we measure the linear magnetic frequency shift of the transition to be γ_{1}=13.994 301(11) GHz/T. This measurement corresponds to an optical magnetic field determination of better than 1 ppm accuracy. From this value, we can calculate the fine-structure Landé g-factor g_{J}(6^{2}P_{3/2})=1.334 087 49(52). This result is consistent with the previous best measurement, and it improves the accuracy by more than 2 orders of magnitude. We also measure, for the first time, the quadratic diamagnetic shift as γ_{2}=0.4644(35) MHz/T^{2}. Our work opens up the field of accurate high-field optical magnetometry using atomic cesium, with possible applications in, e.g., medical MRI, fusion reactors, and particle accelerators. These high-accuracy measurements also allow for testing of advanced atomic structure models, as our results are incompatible with the Russel-Saunders coupling value and the hydrogen-constant-core-model value by 31 and 7 standard deviations, respectively.

ABSTRACT: Objectives: Carbapenem-resistant Enterobacteriaceae (CRE) have become one of the most challenging problems in infectious diseases worldwide. Unrecognised personnel such as food handlers (FHs) colonised with CRE serve as a reservoir for transmission. This study assessed the prevalence and susceptibility patterns of CRE isolates from FHs working in commercial eateries in the community (CFHs) and healthcare settings (HCFHs) in Kuwait over the period 2016–2018. Methods: Representative colonies from faecal samples were identified by API 20E and a VITEK®2 ID System. Susceptibility testing against 21 antibiotics was performed by Etest and agar dilution. Results: A total of 681 isolates of the family Enterobacteriaceae were isolated from 405 FHs, of which 425 (62.4%) were Escherichia coli and 126 (18.5%) were Klebsiella pneumoniae. The prevalence of CRE among FHs was 7.7% (31/405), comprising 32% CFHs (10/31) and 68% HCFHs (21/31). Ampicillin, tetracycline and cefalotin showed very poor activities against most isolates with resistance rates of 63.3%, 41.7% and 40.8%, respectively. The prevalence of multidrug-resistant (MDR) isolates was 30.5%, including 130 E. coli (30.6%) and 22 K. pneumoniae (17.5%). An alarming level of colistin resistance (11.3%) was noted. A significant proportion of FH isolates (13.2%) exhibited extended-spectrum β-lactamases (ESBL) phenotypes, including 80 E. coli (18.8%) and 5 K. pneumoniae (4.0%). Conclusion: This study revealed that asymptomatic intestinal carriage of CRE, including MDR and ESBL isolates, was relatively common in our community. It is conceivable that FHs may pose a significant risk to consumers for the acquisition and spread of resistant strains.

Abstract The frequency of flash floods resulting from heavy rainfall over West Africa has increased in recent years with serious socio‐economic consequences. Therefore, the need to utilize numerical weather prediction models to forecast heavy rainfall events reliably is also rising at many operational meteorological centres in West Africa. This paper evaluates the performance of the Consortium for Small‐scale Modelling (COSMO) model of the German Meteorological Services (DWD) in predicting rainfall over West Africa for high‐impact rainfall events that occurred between 19 and 26 August 2017. The paper aims to investigate the synoptic forcings modulating daily rainfall variability during that period. Results show that COSMO simulates adequately the spatio‐temporal variability of rainfall distribution over West Africa, though with inherent biases. COSMO displays a decreasing skill in producing spatial rainfall distribution as rainfall amounts tend to 30 mm and above. Additionally, areas of heavy rainfall, mostly about 100–300 km southwest of the core of the Africa Easterly Jet (AEJ), often coincide with areas of decreasing mean sea level pressure of at least 0.6 hPa and areas of increasing convective available potential energy of at least 500 J/kg. Although not in all cases, the trough of the Africa Easterly Wave (AEW) is always located to the east of these areas. We show that not every storm, especially east of the prime meridian, is associated with an AEW trough. COSMO is able to reproduce the atmospheric dynamics modulating the daily rainfall variability, in addition to capturing the daily propagation of the AEW trough, and the core of the AEJ. However, the reproducibility skill of the model in predicting atmospheric dynamics may not transform into the predictive skill of the model in producing rainfall. Nevertheless, operational forecasters may be able to determine likely areas of heavy rainfall by estimating the position of the AEJ core based on the position of areas of the least falling pressure from COSMO. Finally, the incorporation of the fractions skill score metric based on the neighbourhood approach could also assist operational forecasters to decide at which scale a severe weather alert can be issued.

BackgroundAntipsychotic drugs are primarily efficacious in treating positive symptoms by blocking the dopamine D2 receptor, but they fail to substantially improve negative symptoms and cognitive deficits. The limited efficacy may be attributed to the fact that the pathophysiology of psychosis involves multiple neurotransmitter systems. In patients with chronic schizophrenia, memantine, a non-competitive glutamatergic NMDA receptor antagonist, shows promise for ameliorating negative symptoms and improving cognition. Yet, it is unknown how memantine modulates glutamate levels, and memantine has not been investigated in patients with first-episode psychosis.AimsThis investigator-initiated double-blinded randomized controlled trial is designed to (1) test the clinical effects on negative symptoms of add-on memantine to antipsychotic medication, and (2) neurobiologically characterize the responders to add-on memantine.Materials and EquipmentAntipsychotic-naïve patients with first-episode psychosis will be randomized to 12 weeks treatment with [amisulpride + memantine] or [amisulpride + placebo]. We aim for a minimum of 18 patients in each treatment arm to complete the trial. Brain mapping will be performed before and after 12 weeks focusing on glutamate and neuromelanin in predefined regions. Regional glutamate levels will be probed with proton magnetic resonance spectroscopy (MRS), while neuromelanin signal will be mapped with neuromelanin-sensitive magnetic resonance imaging (MRI). We will also perform structural and diffusion weighted, whole-brain MRI. MRS and MRI will be performed at an ultra-high field strength (7 Tesla). Alongside, participants undergo clinical and neuropsychological assessments. Twenty matched healthy controls will undergo similar baseline- and 12-week examinations, but without receiving treatment.Outcome MeasuresThe primary endpoint is negative symptom severity. Secondary outcomes comprise: (i) clinical endpoints related to cognition, psychotic symptoms, side effects, and (ii) neurobiological endpoints related to regional glutamate- and neuromelanin levels, and structural brain changes.Anticipated ResultsWe hypothesize that add-on memantine to amisulpride will be superior to amisulpride monotherapy in reducing negative symptoms, and that this effect will correlate with thalamic glutamate levels. Moreover, we anticipate that add-on memantine will restore regional white matter integrity and improve cognitive functioning.PerspectivesBy combining two licensed, off-patent drugs, AMEND aims to optimize treatment of psychosis while investigating the memantine response. Alongside, AMEND will provide neurobiological insights to effects of dual receptor modulation, which may enable future stratification of patients with first-episode psychosis before initial antipsychotic treatment.Clinical Trial Registration[ClinicalTrials.gov], identifier [NCT04789915].

Human African Trypanosomiasis (HAT) is a disease of major economic importance in Sub-Saharan Africa. The HAT is caused by Trypanosoma brucei rhodesiense (Tbr) parasite in eastern and southern Africa, with suramin as drug of choice for treatment of early stage of the disease. Suramin treatment failures has been observed among HAT patients in Tbr foci in Uganda. In this study, we assessed Tbr parasite strains isolated from HAT patients responsive (Tbr EATRO-232) and non-responsive (Tbr EATRO-734) to suramin treatment in Busoga, Uganda for 1) putative role of suramin resistance in the treatment failure 2) correlation of suramin resistance with Tbr pathogenicity and 3) proteomic pathways underpinning the potential suramin resistance phenotype in vivo. We first assessed suramin response in each isolate by infecting male Swiss white mice followed by treatment using a series of suramin doses. We then assessed relative pathogenicity of the two Tbr isolates by assessing changes pathogenicity indices (prepatent period, survival and mortality). We finally isolated proteins from mice infected by the isolates, and assessed their proteomic profiles using mass spectrometry. We established putative resistance to 2.5 mg/kg suramin in the parasite Tbr EATRO-734. We established that Tbr EATRO-734 proliferated slower and has significantly enriched pathways associated with detoxification and metabolism of energy and drugs relative to Tbr EATRO-232. The Tbr EATRO-734 also has more abundantly expressed mitochondrion proteins and enzymes than Tbr EATRO-232. The suramin treatment failure may be linked to the relatively higher resistance to suramin in Tbr EATRO-734 than Tbr EATRO-232, among other host and parasite specific factors. However, the Tbr EATRO-734 appears to be less pathogenic than Tbr EATRO-232, as evidenced by its lower rate of parasitaemia. The Tbr EATRO-734 putatively surmount suramin challenges through induction of energy metabolism pathways. These cellular and molecular processes may be involved in suramin resistance in Tbr.

Abstract Malaria transmission-blocking vaccines (TBVs) prevent the completion of the developmental lifecycle of malarial parasites within the mosquito vector, effectively blocking subsequent infections. The mosquito midgut protein Anopheline alanyl aminopeptidase N (AnAPN1) is the leading, mosquito-based TBV antigen. Structure-function studies identified two Class II epitopes that can induce potent transmission-blocking (T-B) antibodies, informing the design of the next-generation AnAPN1. Here, we functionally screened new immunogens and down-selected to the UF6b construct that has two glycine-linked copies of the T-B epitopes. We then established a process for manufacturing UF6b and evaluated in outbred female CD1 mice the immunogenicity of the preclinical product with the human-safe adjuvant Glucopyranosyl Lipid Adjuvant in a liposomal formulation with saponin QS21 (GLA-LSQ). UF6b:GLA-LSQ effectively immunofocused the humoral response to one of the key T-B epitopes resulting in potent T-B activity, underscoring UF6b as a prime TBV candidate to aid in malaria elimination and eradication efforts.

Abstract Introduction The aggressive search for renewable energy resources and essential pyrosynthetic compounds has marked an exponential rise in the thermal degradation of biomass materials. Consequently, clean and sustainable transport fuels are increasingly desirable in a highly industrialized economy, for energy security and environmental protection. For this reason, biomass materials have been identified as promising alternatives to fossil fuels despite the challenges resulting from the possible formation of toxic nitrogen-based molecules during biomass degradation. In order to understand the free radical characteristic challenges facing the use of bio-oil, a brief review of the effects of free radicals in bio-oil is presented. Methodology Pyrolysis was conducted in a tubular flow quartz reactor at a residence time of 2 s at 1 atm. pressure, for a total pyrolysis time of 5 min. The thermal degradation of biomass components was investigated over the temperature range of 200 to 700 °C typically in 50 °C increments under two reaction conditions; pyrolysis in N2 and oxidative pyrolysis in 5% O2 in N2. The pyrolysate effluent was analysed using a Gas chromatograph hyphenated to a mass selective detector (MSD). Results The yield of levoglucosan in the pyrolysis of cellulose in the entire pyrolysis temperature range was 68.2 wt % under inert conditions and 28.8 wt % under oxidative conditions. On the other hand, formaldehyde from pyrolysis of cellulose yielded 4 wt % while that from oxidative pyrolysis was 7 wt % translating to ⁓ 1.8 times higher than the yield from pyrolysis. Accordingly, we present for the first time dioxin-like and dibenzofuran-like nitrogenated analogues from an equimassic pyrolysis of cellulose and tyrosine. Levoglucosan and formaldehyde were completely inhibited during the equimassic pyrolysis of cellulose and tyrosine. Conclusion Clearly, any small amounts of N-biomass components such as amino acids in cellulosic biomass materials can inhibit the formation of levoglucosan–a major constituent of bio-oil. Overall, a judicious balance between the production of bio-oil and side products resulting from amino acids present in plant matter should be taken into account to minimize economic losses and mitigate against negative public health concerns.

Dryland ecosystems have always been prone to relatively high vegetation and general environmental degradation; translating to changes in soil physical and chemical properties and massive carbon losses. Despite their vast surface area, Carbon sequestration therein still remains low. However, this low carbon means they are less saturated and therefore a tremendous potential therein to sequester more Carbon. Conservation agriculture with trees (CAWT) presents an opportunity to reduce the degradation and enhance the carbon stocks. This study was set to compare the biomass productivity and carbon sequestration potential of agroforestry between conventional and conservation agriculture practice. The study was carried out as part of ongoing experimentation established in short rain (SR) season of 2012 by the World Agroforestry Centre in a trial site at the Agricultural Training Centre (ATC) in Machakos county, Kenya. The trials adopted a split plot arranged in a randomized complete block design with two farming systems (conventional and conservation agriculture) as the main blocks, 7 treatments and three replicates, summing to a total of 42 plots. In the fields, two shrub species (Calliandra calothyrsus Meissn. and Gliricidia sepium Jacq.) were planted in three different spacing (1.5x1 m, 3x1 m, 4.5x1 m) for maize-legume intercrops. Trees were harvested by coppicing, weighed and leaf/twig samples taken for determination of biomass, which was then converted to Carbon using a conversion factor 0.5. Diverse P,N-phosphine ligands reported to date have performed exceptionally well as auxiliary ligands in organometallic catalysis. Phosphines bearing 2-pyridyl moieties prominently feature in literature as compared to phosphines with five-membered N-heterocycles. This discussion seeks to paint a broad picture and consolidate different synthetic protocols and techniques for N-heterocyclic phosphine motifs. The introduction provides an account of P,N-phosphine ligands, and their structural and coordination benefits from combining heteroatoms with different basicity in one ligand. The body discusses the synthetic protocols which focus on P–C, P–N-bond formation, substrate and nucleophile types and different N-heterocycle construction strategies. Selected references are given in relation to the applications of the ligands.

Abstract Background Some strains of Bacteroides fragilis species are associated with diarrhea as a result of enterotoxin production (bft or fragilysin). Fragilysin is activated by C11 protease (fpn) and together with C10 protease (bfp) play a significant role in its invasiveness. The objectives of this study were to investigate the proportion of clinical isolates from extra-intestinal sources that are toxin producers and characterize the genes mediating toxin production. Clinical isolates submitted to our reference laboratory over the last 13 years were screened for toxin production using PCR technique. All stool isolates were excluded. The isolates were tested for their susceptibility to 8 antimicrobial agents by E test. Carbapenem resistance gene cfiA was detected by PCR. Results A total of 421 B. fragilis isolates were viable. Out of these, bft was detected in 210 (49.9%) isolates. Of the 210 bft-positive isolates, 171 (81.4%), 33 (15.7%) and 6 (2.8%) harbored bft-1, bft-2, and bft-3 genes, respectively. Twenty (9.5%) of the bft-positive strains originated from bloodstream infections. Twenty-five, 20 and 9 strains harbored bfp-1, bfp-2 and bfp-3 gene, respectively. Two, 3, 4 bfp isotypes were detected simultaneously in some of strains. The resistance rates against amoxicillin-clavulanic acid was 32%, clindamycin 62%, cefoxitin 26%, imipenem 11%, meropenem 17%, metronidazole 4%, piperacillin 61% and tigecycline 14%. A chromosomally located cfiA gene that encode metallo-β-lactamase was identified in only 34 isolates (16.2%). Conclusions The prevalence of enterotoxin-producing B. fragilis was high among the extra-intestinal isolates. Metronidazole was the most active agent against all isolates. There was no statistically significance difference between resistance rates among bft-positive and bft-negative isolates except for clindamycin.

In recent years, carbon-based materials, particularly carbon nanotubes (CNTs), have gained intensive research attention in the fabrication of organic solar cells (OSCs) due to their outstanding physicochemical properties, low-cost, environmental friendliness and the natural abundance of carbon. In this regard, the low sheet resistance and high optical transmittance of CNTs enables their application as alternative anodes to the widely used indium tin oxide (ITO), which is toxic, expensive and scarce. Also, the synergy between the large specific surface area and high electrical conductivity of CNTs provides both large donor-acceptor interfaces and conductive interpenetrating networks for exciton dissociation and charge carrier transport. Furthermore, the facile tunability of the energy levels of CNTs provides proper energy level alignment between the active layer and electrodes for effective extraction and transportation of charge carriers. In addition, the hydrophobic nature and high thermal conductivity of CNTs enables them to form protective layers that improve the moisture and thermal stability of OSCs, thereby prolonging the devices’ lifetime. Recently, the introduction of CNTs into OSCs produced a substantial increase in efficiency from ∼0.68 to above 14.00%. Thus, further optimization of the optoelectronic properties of CNTs can conceivably help OSCs to compete with silicon solar cells that have been commercialized. Therefore, this study presents the recent breakthroughs in efficiency and stability of OSCs, achieved mainly over 2018–2021 by incorporating CNTs into electrodes, active layers and charge transport layers. The challenges, advantages and recommendations for the fabrication of low-cost, highly efficient and sustainable next-generation OSCs are also discussed, to open up avenues for commercialization.

Background Physical activity (PA) is associated with numerous health benefits among children and youth. However, few studies have examined how active transportation (AT) and device-based measures of PA vary within and between countries in sub-Saharan Africa. Purpose This cross-sectional study sought to investigate the prevalence and correlates of AT and device-measured PA among children living in urban, peri-urban and rural areas in three African countries representing Eastern, Western and Southern regions of Africa. Methods 3,205 participants (53.3% girls; 46.7% boys) aged 10–12 years were recruited in Kenya, Nigeria and Mozambique. Data were collected using a child questionnaire, a parent/guardian questionnaire and PiezoRx® pedometers. ANCOVA and binary logistic regression analyses were used to examine the correlates of AT and PA while controlling for gender, age, parent education and vehicle ownership. Results Participants accumulated an average of 45.6±23.5 min/day of moderate-to-vigorous physical activity (MVPA) and 11,215±4,273 steps/day. Kenyan and Mozambican children were significantly more active than their Nigerian counterparts (pConclusions Majority of children engaged in AT, but still failed to meet MVPA recommendations. Most correlates of AT and PA were country-specific, suggesting that strategies to encourage both behaviours should be informed by local evidence.

With the rapid increase of diabetes mellitus cases in the world, management and control of the disease has become a complex and highly dynamic process. This challenge requires a multifaceted approach to manage and control the complications associated with the hyperglycaemia or hypoglycaemia conditions. This paper presents a mathematical model for determining the influence of combined intervention strategies in the management and control for the plasma glucose of the type II diabetes. System dynamics (SD) techniques were used in modelling the sub-compartments of biological systems of an Identifiable Patient (IP). The system dynamic model developed gave an illustration on how typical glucose-insulin dynamics occur at different intervention strategies involving varying amounts of carbohydrates taken, intensity of physical exercises, stress levels and the amount of exogenous insulin administered. The model was conceptualized within a semi-closed loop system representing the patient ecosystem by extending the Bergman Minimal Model. Stochastic differential equations (SDE) were used to capture the non-linear, continuous time varying interactions of the measurements associated with plasma glucose-insulin dynamics. The estimated results from the model showed combined intervention strategies of reduced amounts of carbohydrates intake, reduced stress levels and varying moderately high-to-low exercise intensity at a constant unit of exogenous insulin produced good plasma glucose levels control.

Objective: The Child Behavior Checklist (CBCL) is a commonly used measure of child and adolescent functioning, which includes seven items that can be aggregated to provide a purportedly valid measure of sleep functioning. The objective of this study was to examine the convergent validity of the CBCL in a paediatric ADHD population and to evaluate the sensitivity of the instrument when benchmarked against the Sleep Disorders Scale for Children (SDSC). Methods: The parents of 215 individuals (ages 6–17 years, 86% male) completed the CBCL and SDSC as part of a battery of measured administered as part of a specialised ADHD service located in Perth, Western Australia. All participants had a diagnosis of ADHD confirmed by a paediatrician or psychiatrist prior to attending the service. Results: The CBCL Sleep Composite Scale was strongly correlated with the SDSC, but reported below adequate internal reliability. Receiver Operating Characteristic (ROC) suggests that a cut-off score of 4 may have good diagnostic accuracy compared to SDSC. Conclusions: The CBCL Sleep Composite Scale may be reasonable to use if no purpose-developed sleep screening tool is available. The CBCL sleep items demonstrated good convergent validity, however, did not otherwise demonstrate acceptable psychometric properties that would endorse its use in an ADHD sample. The development of a specific measure of sleep in children with ADHD children is recommended.

Carbapenem-resistant Enterobacterales (CRE) are pathogens that have been found in several countries, with a significant public health concern. Characterizing the mode of resistance and determining the prevailing clones are vital to the epidemiology of CRE in our community. This study was conducted to characterize the molecular mode of resistance and to determine the clonality of the CRE fecal isolates among community food handlers (FHs) vs. infected control patients (ICPs) in Kuwait. Fecal CRE isolates obtained from FHs and ICPs from September 2016 to September 2018 were analyzed for their resistance genes. Gene characterization was carried out by polymerase chain reaction (PCR) assays and sequencing. Clonality of isolates was established by multilocus sequence typing (MLST). Of the 681 and 95 isolates of the family Enterobacterales isolated from FHs and ICPs, 425 (62.4%) and 16 (16.8%) were Escherichia coli, and 18 (2.6%) and 69 (72.6%) were Klebsiella pneumoniae, respectively. A total of 36 isolates were CRE with a prevalence of 5.3% among FH isolates and 87 (91.6%) among the ICPs. Of these, carbapenemase genes were detected in 22 (61.1%) and 65 (74.7%) isolates, respectively (p < 0.05). The detected specific genes among FHs and ICPs were positive for blaKPC 19 (86.4%) and 35 (40.2%), and blaOXA 10 (45.5%) and 59 (67.8%), in addition to blaNDM 2 (9.1%) and 32 (36.8%), respectively. MLST assays of the E. coli and K. pneumoniae isolates revealed considerable genetic diversity and polyclonality as well as demonstrated multiple known ST types and eight novel sequence types. The study revealed a relatively high number of CRE harboring predominantly blaKPC-mediated CRE among the community FH isolates vs. predominant blaOXA genes among the ICPs. Those heterogeneous CRE isolates raise concerns and mandate more efforts toward molecular surveillance. A multinational study is recommended to monitor the spread of genes mediating CRE in the community of Arabian Peninsula countries.

The low fabrication cost, solution processability, and easy scalability of perovskite solar cells (PSCs), coupled with the rapid increase in their power conversion efficiency (PCE) from an initial value of 3.8% to a recently certified value of ≈25.5%, have enabled PSCs to compete with silicon‐based solar cells that currently exhibit PCEs of above 26.0%. However, unlike silicon‐based devices that are commercialized, the commercialization of PSCs is being hindered by factors, such as their poor long‐term operational stability, the high toxicity of lead (Pb), and the use of expensive materials, e.g., gold (Au) or silver (Ag). Hence, to address the aforementioned issues, significant research effort is exerted on novel graphene‐based materials, with their merits, which include low cost, excellent stability, nontoxicity, and remarkable optoelectronic properties. Nevertheless, graphene‐based PSCs are relatively less studied and are still in their infancy. This study presents recent developments in applying graphene‐based materials in electrodes, perovskite active layers, charge transport layers, and encapsulation layers of PSCs, focusing particularly on breakthroughs achieved over the last three years (2018–2020). The merits, shortcomings, and outlook of this field are discussed to propose future research directions for the low‐cost fabrication and commercialization of highly efficient and sustainable PSCs.

Abstract Adsorptive removal of organic pollutant (pyrene) was carried out using coordination polymer [Cu(Pic)2(H2O)2]·H2O (CP-1) (where “Pic” represents picolinic acid), which was prepared from copper acetate monohydrate and picolinic acid. The compound was characterized by spectroscopic techniques, SEM, elemental and thermal analyses and X-ray crystallographic analysis. The crystal structure of CP-1 shows a centrosymmetric triclinic space group P-1 in which a = 5.0924 (4) Å, b = 7.5172 (6) Å, c = 9.0965 (8) Å. The Cu2+ ion is seen to have an octahedral geometry, which is bonded to oxygen atoms from the picolinic acid and nitrogen atoms from the pyridine rings. Pyrene adsorption from aqueous media was studied with the prepared adsorbent (CP-1). The kinetic model was observed to be second order, and the sorption data fitted best into the Langmuir model. The absorbent, CP-1, was shown to have a 90.91 mg/g adsorption capacity (maximum) for pyrene in this study at 25 °C. Compound CP-1 can thus be presented as a potential adsorbent in the treatment of pyrene-polluted water.

Background: Infected diabetic foot ulcer (IDFU) is a public health issue and the leading cause of non-traumatic limb amputation. Very few published data on IDFU exist in most West African countries. Objective: The study investigated the aetiology and antibacterial drug resistance burden of IDFU in tertiary hospitals in Osun state, Nigeria, between July 2016 and April 2017. Methods: Isolates were cultured from tissue biopsies or aspirates collected from patients with IDFU. Bacterial identification, antibiotic susceptibility testing and phenotypic detection of extended-spectrum beta-lactamase and carbapenemase production were done by established protocols. Specific resistance genes were detected by polymerase chain reaction. Results: There were 218 microorganisms isolated from 93 IDFUs, comprising 129 (59.2%) Gram-negative bacilli (GNB), 59 (27.1%) Gram-positive cocci and 29 (13.3%) anaerobic bacteria. The top five facultative anaerobic bacteria isolated were: Staphylococcus aureus (34; 15.6%), Escherichia coli (23; 10.6%), Pseudomonas aeruginosa (20; 9.2%), Klebsiella pneumoniae (19; 8.7%) and Citrobacter spp. (19; 8.7%). The most common anaerobes were Bacteroides spp. (7; 3.2%) and Peptostreptococcus anaerobius (6; 2.8%). Seventy-four IDFUs (80%) were infected by multidrug-resistant bacteria, predominantly methicillin-resistant S. aureus and GNB producing extended-spectrum β-lactamases, mainly of the CTX-M variety. Only 4 (3.1%) GNB produced carbapenemases encoded predominantly by blaVIM. Factors associated with presence of multidrug-resistant bacteria were peripheral neuropathy (adjusted odds ratio [AOR] = 4.05, p = 0.04) and duration of foot infection of more than 1 month (AOR = 7.63, p = 0.02). Conclusion: Multidrug-resistant facultative anaerobic bacteria are overrepresented as agents of IDFU. A relatively low proportion of the aetiological agents were anaerobic bacteria.

The benefits of physical activity (PA) on children's health and well-being are well established. However, many children do not meet the PA recommendations, increasing their risk of being overweight, obese, and non-communicable diseases. Environmental characteristics of homes and neighborhoods may constrain a child's ability to engage in PA, but evidence is needed to inform country-specific interventions in understudied low-income countries. This study assessed the associations between parental-perceived home and neighbourhood, built environment characteristics, and moderate-to-vigorous physical activity (MVPA) among children in Kampala city, Uganda. In this cross-sectional study, data were obtained from 256 children (55.5% girls) aged between 10 and 12 years and their parents. Children's MVPA was measured using waist-worn ActiGraph accelerometers. The environments were assessed using a valid self-reported parent survey. Linear regression models with standard errors (clusters) were used to analyze the relationship between environmental variables and children's MVPA. Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterised by a triad of behavioural impairments and includes disruption in social behaviour. ASD has a clear genetic underpinning and hundreds of genes are implicated in its aetiology. However, how single penetrant genes disrupt activity of neural circuits which lead to affected behaviours is only beginning to be understood and less is known about how low penetrant genes interact to disrupt emergent behaviours. Investigations are well served by experimental approaches that allow tractable investigation of the underpinning genetic basis of circuits that control behaviours that operate in the biological domains that are neuro-atypical in autism. The model organism C. elegans provides an experimental platform to investigate the effect of genetic mutations on behavioural outputs including those that impact social biology. Here we use progeny-derived social cues that modulate C. elegans food leaving to assay genetic determinants of social behaviour. We used the SAFRI Gene database to identify C. elegans orthologues of human ASD associated genes. We identified a number of mutants that displayed selective deficits in response to progeny. The genetic determinants of this complex social behaviour highlight the important contribution of synaptopathy and implicates genes within cell signalling, epigenetics and phospholipid metabolism functional domains. The approach overlaps with a growing number of studies that investigate potential molecular determinants of autism in C. elegans. However, our use of a complex, sensory integrative, emergent behaviour provides routes to enrich new or underexplored biology with the identification of novel candidate genes with a definable role in social behaviour.

Chromatographic fractionation of the methanolic extract of Asteriscus hierochunticus whole plant led to the identification of a new humulene glucoside (1). The chemical structure of the isolated compound was elucidated by IR, 1D, 2D NMR, and HRESIMS data analysis to be (-)-(2Z,6E,9E)8α-hydroxy-2,6,9-humulatrien-1(12)-olide. In this study, we report the in silico binding affinities of 1 against four different SARS-CoV-2 proteins (COVID-19 main protease (PDB ID: 6lu7), nonstructural protein (PDB ID: 6W4H), RNA-dependent RNA polymerase (PDB ID: 7BV2), and SARS-CoV-2 helicase (PDB ID: 5RMM)). The isolated compound showed excellent binding affinity values (ΔG) of −21.65, −20.05, −28.93, and −21.73 kcal/mol, respectively, against the target proteins compared to the cocrystallized ligands that exhibited ΔG values of −23.75, −17.65, −23.57, and −15.30 kcal/mol, respectively. Further in silico investigations of the isolated compound (1) for its ADMET and toxicity profiles revealed excellent drug likeliness. On the other hand, the results obtained from in vitro antitrypanosomal, antileishmanial, and antimalarial activities of (1) were not promising.

Glutamate detection in pons and thalamus using proton magnetic resonance spectroscopy (1H-MRS) after an intervention is of interest for studying various brain disorders. However, 1H-MRS in these brain regions is challenging and time-consuming, especially in longitudinal study designs. 1H-MRS of more cortical structures at the ultrahigh magnetic field strength of 7T yields an improved spectral output, including separation of the glutamate signal from the glutamine signal, in a shorter and more feasible scan time, as compared to conventional clinical field strengths. For this purpose, we compared the feasibility of 1H-MRS at 3T and 7T in pons and thalamus by applying a longitudinal study design of repeated measures on same day and three separate days at both field strength in five healthy participants. Total 1H-MRS acquisition time was reduced by a factor 3.75 for pons and by a factor 3 for thalamus at 7T as compared to 3T. We found higher spectral signal-to-noise ratio (SNR) (p < 0.001), lower linewidth (p = 0.001) and lower Cramér–Rao lower bounds (CRLB) (p < 0.001) for the combined glutamate and glutamine signal (Glx) in thalamus at 7T as compared to 3T. In pons, CRLB of Glx and SNR were lower at 7T (p = 0.002 and p = 0.006), with no differences in linewidth compared to 3T. Mean within-subject variability of Glx concentration estimates was lower at 7T compared to 3T for both pons and thalamus. At 7T, it was possible to assess glutamate and γ-aminobutyric acid (GABA) simultaneously in pons and thalamus. In conclusion, 1H-MRS at 7T resulted in improved spectral quality while allowing shorter scan times than at 3T as well as estimation of the pure glutamate signal in pons and thalamus. This opens up the opportunity for multimodal study designs and multiregional subcortical 1H-MRS research. Glutamate and GABA measurement at 7T in pons and thalamus is advantageous for future investigations of excitatory–inhibitory mechanisms in brain disorders.

Tsetse fly exhibit species-specific olfactory uniqueness potentially underpinned by differences in their chemosensory protein repertoire. We assessed 1) expansions of chemosensory protein orthologs in Glossina morsitans morsitans, Glossina pallidipes, Glossina austeni, Glossina palpalis gambiensis, Glossina fuscipes fuscipes and Glossina brevipalpis tsetse fly species using Café analysis (to identify species-specific expansions) and 2) differential expressions of the orthologs and associated proteins in male G. m. morsitans antennae and head tissues using RNA-Seq approaches (to establish associated functional molecular pathways). We established accelerated and significant (P90% of chemosensory transcripts) included cilium-associated mechanoreceptors, chemo-sensation, neuronal controlled growth/differentiation and regeneration/responses to stress. The expanded and tsetse fly species specific orthologs includes those associated with known tsetse fly responsive ligands (4-methyl phenol, 4-propyl phenol, acetic acid, butanol and carbon dioxide) and potential tsetse fly species-specific responsive ligands (2-oxopentanoic acid, phenylacetaldehyde, hydroxycinnamic acid, 2-heptanone, caffeine, geosmin, DEET and (cVA) pheromone). Some of the orthologs can potentially modulate several tsetse fly species-specific behavioral (male-male courtship, hunger/host seeking, cool avoidance, hygrosensory and feeding) phenotypes. The putative tsetse fly specific chemosensory gene orthologs and their respective ligands provide candidate gene targets and kairomones for respective downstream functional genomic and field evaluations that can effectively expand toolbox of species-specific tsetse fly attractants, repellents and other tsetse fly behavioral modulators.

Acinetobacter baumannii is an opportunistic pathogen of intensive care unit (ICU) patients. A. baumannii colonizes many parts of the body including the gastrointestinal tract. Endemic and epidemic strains are polyclonal. There is no clarity on the origin of polyclonality of A. baumannii. The objective of the study was to define the genetic relatedness of serial isolates and the origin of polyclonality. Serial rectal isolates from ICU patients whose rectum was colonized on ≥5 sampling occasions were selected. From a total of 32 eligible colonized patients, isolates from a subgroup of 13 patients (a total of 108 isolates) showing different patterns of colonization as revealed by pulsed-field gel electrophoresis (PFGE) were studied. The isolates were analyzed by PFGE pulsotypes, sequence types (STs) by multi-locus sequence typing (MLST) and clonal complex (CC) by eBURST analysis. Serial isolates constituted a mixture of identical, related and unrelated pulsotypes. Analysis by STs and CCs were less discriminatory. The data suggest a combination of an initial colonizing isolate undergoing mutation as well as colonization by independent isolates. Further clarity on the origin of diversity should be better obtained by whole-genome sequencing.

BackgroundThe current international physical activity guidelines for health recommend children to engage in at least 60 minutes of moderate-to-vigorous physical activity (MVPA) daily. Yet, accurate prevalence estimates of physical activity levels of children are unavailable in many African countries due to the dearth of accelerometer-measured physical activity data. The aim of this study was to describe the prevalence and examine the socio-demographic correlates of accelerometer-measured physical activity among school-going children in Kampala city, Uganda.MethodsA cross-sectional study design was used to recruit a sample of 10-12 years old school-going children (n = 256) from 7 primary schools (3 public schools and 4 private schools) in Kampala city, Uganda. Sedentary time, light-intensity physical activity (LPA), moderate-intensity physical activity (MPA) and vigorous-intensity physical activity (VPA) were measured by accelerometers (ActiGraph GT3X+ [Pensacola, Florida, USA]) over a seven-day period. Socio-demographic factors were assessed by a parent/guardian questionnaire. Weight status was generated from objectively measured height and weight and computed as body mass index (BMI). Multi-level logistic regressions identified socio-demographic factors that were associated with meeting physical activity guidelines.ResultsChildren's sedentary time was 9.8±2.1 hours/day and MVPA was 56±25.7 minutes/day. Only 36.3% of the children (38.9% boys, 34.3% girls) met the physical activity guidelines. Boys, thin/normal weight and public school children had significantly higher mean daily MVPA levels. Socio-demographic factors associated with odds of meeting physical activity guidelines were younger age (OR = 0.68; 95% CI = 0.55-0.84), thin/normal weight status (OR = 4.08; 95% CI = 1.42-11.76), and socioeconomic status (SES) indicators such as lower maternal level of education (OR = 2.43; 95% CI = 1.84-3.21) and no family car (OR = 0.31; 95% CI = 0.17-0.55).ConclusionChildren spent a substantial amount of time sedentary and in LPA and less time in MVPA. Few children met the physical activity guidelines. Lower weight status, lower maternal education level and no family car were associated with meeting physical activity guidelines. Effective interventions and policies to increase physical activity among school-going children in Kampala, are urgently needed.

BackgroundFecal colonization by carbapenem-resistant Enterobacteriaceae (CRE) can be the main reservoir for transmission of these resistant organisms especially in the Intensive Care Units (ICUs).AimThis study was conducted to evaluate the rate of rectal carriage and molecular characterization of CRE in patients hospitalized in the ICUs of 2 major hospitals (Adan and Mubarak Al Kabeer Hospitals) in Kuwait.Materials and methodsRectal swabs were collected from all patients at admission, 48 h after admission and once weekly from April 2017- March 2018. Initial CRE screening was carried out on MacConkey agar on which meropenem disc 10μg was placed. Identification of isolates was by API 20E. Susceptibility testing was performed using the E-test method. Polymerase chain reaction (PCR) was used to detect the carbapenemase-encoding genes. Clonal relationship was investigated by pulsed-field electrophoresis (PFGE). Genes of blaOXA-181 and blaNDM-5-carrying plasmids were detected in some strains.ResultsA total of 590 patients were recruited into the study. Of these, 58 were positive for CRE, giving a prevalence of 9.8%; 25/320 (7.8%) in Adan and 33/270 (12.2%) in Mubarak Al Kabeer Hospitals. All isolates were resistant to multiple antibiotics. Resistance rates to colistin and tigecycline were 17% and 83%, respectively. Single genes of blaOXA-181 were detected in isolates from 38 (65.5%) out of 58 patients and in 5 patients colonized by blaOXA-48-positive CRE. A combination of 2 genes was detected in 12 isolates; 5 blaKPC-2 and blaOXA-181, 4 blaVIM-1 and blaOXA-181, and 3 blaNDM-5 and blaOXA-181. PFGE showed an overall level of similarity of 38%. Southern hybridization studies localized the blaOXA-181 and blaNDM-5 genes to a large plasmid of 200kb in 3 K. pneumoniae isolates and a small plasmid of 80kb in 2 E. coli isolates, respectively.ConclusionThe prevalence of CRE colonization in the 2 hospital ICUs was relatively high and the emergence of blaOXA-181-mediated CRE is a cause for concern as there is the possibility of rapid horizontal spread among hospital patients in Kuwait. Active surveillance of CRE in the ICUs is highly recommended to stem its spread.

Consumers’ consumption culture of the twentieth century has culminated into environmental degra-dation and its adverse consequences on health and the ecosystem. Despite these concerns, consumers still fail to purchase environmentally-friendly or green products due to the lack of awareness and belief about the benefits of such products. This study seeks to understand consumers’ attitude towards green products by investigating the effect of consumers’ green product knowledge, green product belief, environment affect and environmental concern on green product purchase intention drawing on the cognitive-affect-behaviour model. Multiple regression analysis was used to analyse the data from 348 Health workers from a South-eastern Health Institution in Nigeria and examined the proposed hypotheses. The result shows that green product purchase intention was significantly influenced by green product knowledge and environmental affect but no significant effect was found for green product belief and environmental concern. Environmental effects had the most im-pact on green product purchase intention. Therefore, it is recommended that marketers focus their marketing campaigns towards educating consumers on the availability and benefits of green prod-ucts by using eco-labels and emphasizing the health and environment benefits of green products; appealing to consumers’ morals and emotions in marketing campaigns for green products to spur consumers’ interest in green products and adopting a green lifestyle.

Objective: To examine the factor structure of the Sleep Disorder Scale for Children (SDSC) in children and adolescents with attention deficit and hyperactivity disorder (ADHD). Method: The caregivers of 307 children with ADHD completed the SDSC. Standard and bifactor confirmatory factor analysis (CFA) evaluated the goodness-of-fit of competing factor structures. Results: The original and unidimensional factor structure produced sub-optimal fit. Bifactor exploratory factor analysis (EFA) was performed to examine the underlying structure of the SDSC. A revised bifactor solution comprising six-specific factors and a general factor was identified. A nested version of this model was deemed to be the preferred model, which also demonstrated good psychometric properties. Conclusion: There is evidence of a ‘general sleep difficulties’ factor in children with ADHD. Four of the six original factors were replicated in this study. However, the revised factor structure suggests that clinicians should be cautious of the utility of subscale scores pending further validation in ADHD samples.

Abstract Background The atmosphere has become a major transport corridor for free radicals and particulate matter from combustion events. The motivation behind this study was to determine the nature of particulate emissions and surface bound radicals formed during the thermal degradation of diesel blends in order to assess the health and environmental hazards of binary transport fuels. Methodology Accordingly, this contribution explored the interactions that occur when Croton megalocarpus biodiesel and fossil diesel in the ratio of 1:1 by weight were co-pyrolyzed in a quartz reactor at a residence time of 0.5 s under an inert flow of nitrogen at 600 °C. The surface morphology of the thermal char formed were imaged using a Feld emission gun scanning electron microscope (FEG SEM) while Electron paramagnetic resonance spectrometer (EPR) was used to explore the presence of free radicals on the surface of thermal char. Molecular functional groups adsorbed on the surface of thermal char were explored using Fourier transform infrared spectroscopy (FTIR). Results FTIR spectrum showed that the major functional groups on the surface of the char were basically aromatic and some methylene groups. The particulate emissions detected in this work were ultrafine (~ 32 nm). The particulates are consistent with the SEM images observed in this study. Electron paramagnetic resonance results gave a g-value of 2.0027 characteristic of carbon-based radicals of aromatic nature. Spectral peak-to-peak width (∆Hp-p) obtained was narrow (4.42 G). Conclusions The free radicals identified as carbon-based are medically notorious and may be transported by various sizes of particulate matter on to the surface of the human lung which may trigger cancer and pulmonary diseases. The nanoparticulates determined in this work can precipitate severe biological health problems among humans and other natural ecosystems.

Abstract Background There is currently a rapid physical activity transition taking place in developing countries that includes a decrease in active transportation. Building on findings from an earlier systematic review, this paper describes the development and convergent validity of self-administered child and parent questionnaires assessing active transportation of children in three African countries: Kenya, Mozambique and Nigeria. Methods A pilot study was conducted to examine the convergent validity of the developed questionnaires by comparing responses between children and their parents (N = 121; n = 43 for Mozambique, n = 24 for Kenya and n = 54 for Nigeria). After modification, the questionnaires were then administered to a larger convenient sample of both children and parents from Kenya (n = 1123), Mozambique (n = 1097) and Nigeria (n = 831) which defined the main study. The questionnaires assessed active transportation to/from 8 categories of destinations including school, friends’ and relatives’ home/houses, parks and playgrounds among others. Twenty items were used to assess child - and parent-perceived barriers to active transportation, and the parent questionnaire inquired about parent education and availability of cars, motorcycles, and bicycles. Spearman’s rho was used to compare children’s mode of travel in the pilot study while the prevalence-adjusted bias-adjusted kappa (PABAK) coefficient was used to compare convergent validity between children’s and parents responses on active transportation in the main study. Results Findings of the main study show that convergent validity for active transportation to and from each destination in the combined sample ranged from 0.472 (from school) to 0.998 (to other places). Convergent validity for challenges/barriers to active transportation to school ranged from fair (0.30 - The route does not have good lighting) to substantial (0.77 - My child has a disability). It varied between countries from fair (n = 11-items) to moderate (n = 9-items) agreement in Kenya and from poor (n = 2-items) to fair (n = 16-items) agreement in Nigeria. Data from Mozambique was however missing and therefore could be included. Conclusions The questionnaires provided valid information on the number of trips to/from various destinations and show acceptable and modest convergent validity for measuring barriers to active transport in a sample of children from three African countries. These questionnaires may be suitable for future research on active transport among school children in Sub-Saharan African countries.

We present a system-dynamics model to simulate the interplay between water management, hydrological extremes (droughts and floods), and society. We illustrate the potential and limitations of the model with an example application to the Brisbane river basin (Australia). In particular, we test its capability to explain various phenomena that have been empirically observed, including the levee paradox, (mal)adaptation, and supply-demand cycles. To illustrate, we consider four water-management strategies: no actions, in which no measures are adopted to mitigate droughts and floods; fighting floods, in which a levee system is built and raised to cope with flooding; water conservation, in which demand management is implemented to cope with drought; and water exploitation, in which the water supply is increased to cope with drought. Our findings show that changes in flood and drought awareness can help contribute to the emergence of multiple phenomena. Moreover, the outcomes from the proposed coupled-modeling framework indicate that water-management strategies aimed at specific hydrological extremes can in turn shape the severity of opposite natural hazards. Given its explanatory value, the model can contribute to a better interpretation of changes in drought and flood risk and the role of alternative water-management strategies.
