Abstract

Key Clinical Message Primary intraventricular hemorrhage (PIVH) is a rare condition in pediatric patients, presenting with headache, vomiting, and altered mental status. Surgical interventions, such as external ventricular drain placement, followed by ventriculoperitoneal shunting, show promising outcomes. Further research is needed to enhance understanding and optimize management strategies for pediatric PIVH. Abstract This case report describes a 10‐year‐old boy with isolated primary intraventricular hemorrhage (PIVH) in the fourth ventricle, shedding light on its clinical presentation and management challenges. The patient presented with headache, vomiting, and altered sensorium, and was subsequently diagnosed with obstructive hydrocephalus due to intraventricular bleeding. Emergency external ventricular drain (EVD) insertion was performed, followed by ventriculoperitoneal shunt placement, resulting in a favorable outcome. The etiology of PIVH in children differs from that in adults, with arteriovenous malformations, Moyamoya disease, and aneurysms being commonly implicated causes. Management strategies for pediatric PIVH are challenging due to limited research, but EVD placement and surgical interventions have shown promise.

Abstract

IntroductionChildhood cancer is a leading cause of death. It is unclear whether the COVID-19 pandemic has impacted childhood cancer mortality. In this study, we aimed to establish all-cause mortality rates for childhood cancers during the COVID-19 pandemic and determine the factors associated with mortality.MethodsProspective cohort study in 109 institutions in 41 countries. Inclusion criteria: children ResultsAll-cause mortality was 3.4% (n=71/2084) at 30-day follow-up, 5.7% (n=113/1969) at 90-day follow-up and 13.0% (n=206/1581) at 12-month follow-up. The median time from diagnosis to multidisciplinary team (MDT) plan was longest in low-income countries (7 days, IQR 3–11). Multivariable analysis revealed several factors associated with 12-month mortality, including low-income (OR 6.99 (95% CI 2.49 to 19.68); pConclusionsChildren with cancer are more likely to die within 30 days if infected with SARS-CoV-2. However, timely treatment reduced odds of death. This report provides crucial information to balance the benefits of providing anticancer therapy against the risks of SARS-CoV-2 infection in children with cancer.

16. Primary Immune Dysregulation in Subacute Sclerosing Panencephalitis: A Case-Control Study

Abstract

ObjectiveThe primary objective was to study the pattern of immune dysregulation in cases with subacute sclerosing panencephalitis (SSPE). The secondary objective was to assess the correlation between the measured immunological variables and disability/death at 6 months,BackgroundSSPE is a chronic progressive neurological condition caused by a defective measles virus. It is postulated that immune-dysregulation might result in persistent infection (immune evasion) as well as initiation of autoimmune phenomenon (via natural killer cells) leading to panencephalitis.Design/MethodsThis was a prospective observational study conducted at a tertiary-case referral-facility from January 2020 to September 2021. Thirty consecutive patients fulfilling the Dyken's criteria for SSPE and 30 age-and-sex-matched healthy controls were enrolled. Immunological profile constituted by lymphocyte subset analysis, immunoglobulin levels and complement levels were done in all cases and controls. Cases were staged as per Jabbour's system; disability was assessed using the modified Rankin Scale (mRS).ResultsPatients with SSPE had a mean age of 14.76 years (± 6.9 years). There were 25 males and 5 females; 6.7% cases belonged to Jabbour's first stage, 40% to second stage and 53.3% to third stage. Levels of absolute lymphocyte count, B-cells, T cells, helper T-cells and cytotoxic T-cells were significantly higher in cases. IgG, IgM and IgE levels were significantly higher while IgD levels were significantly lower in cases. At baseline, 13.3% of cases had a mRS score of 0-2 and 86.7% had a score of 3-6; at 6 months 10% had a mRS score 0-2 (favorable outcome) while 90% had a mRS score 3-6 (poor outcome). No correlation of immunological parameters with outcome was found.ConclusionsSignificant immune dysregulation in terms of lymphocyte subsets and immunoglobulin levels seem to exist in SSPE. These findings may pave way for targeted immunomodulator therapy that can be targeted in a larger cohort of patients.

Abstract

Juvenile fishing of the migratory hilsa shad, Tenualosa ilisha (Hamilton, 1822), is rampant in the coastal waters of northern Bay of Bengal and associated Hooghly-Bhagirathi river system. The average annual economic loss due to this was estimated at `497.84 million (around US$ 7.8 million) necessitating urgent need for managing it to maximise benefits from the fishery as it forms a major fishery resource in the region. Factors leading to juvenile fishing and the possible solutions to control the resultant biological and economic loss are discussed.

Abstract

BACKGROUND Disorders affecting the ocular surface may present from mild ocular discomfort to blindness; therefore, prompt detection and apt management assume prime importance. MATERIALS AND METHODS A prospective, cross-sectional, observational study was done to identify and manage various disorders affecting the ocular surface in patients at a rural hospital. Purposive sampling was done, and proportion of various disorders was calculated. Twelve hundred patients with ocular surface disorders visiting the hospital were included in the study. After prior consent and comprehensive history, a thorough, ocular examination was done using slit lamp and accessories. Staining with fluorescein, Tear–film Break Up Time (TBUT), Schirmer’s tests and biopsies were done when necessary. General and systemic examination was performed to detect associated diseases. Specific management strategies were employed for each disorder. RESULTS The mean age was 42.27 ± 41.44 years. Conjunctivitis was the most common disorder present in 509 patients, followed by Dry Eye Disease (DED) in 292. Other disorders detected included Stevens-Johnson syndrome, clinically suspicious cicatricial pemphigoid, blepharitis, keratitis, chemical injuries, pterygium, ophthalmia nodosa, sebaceous gland carcinoma (SGC) and graft versus host disease. CONCLUSION Timely detection by thorough examinations and appropriate management of ocular surface diseases alleviates patient’s suffering, and in some cases prevents blindness.

Abstract

Health of the Hooghly estuary was estimated through Water Quality Index (WQI) developed using ten water quality parameters, viz., specific conductivity, turbidity, pH, total dissolved solids, dissolved oxygen, biochemical oxygen demand, chloride, total alkalinity, total hardness, and nitrate-N measured at three different stations along the salinity gradient. Weighted arithmetic index method was used for calculation of WQI. For classifying water quality in each of the sampling stations, five-point rating scale was used. Wide fluctuations in monthly WQI were observed in this estuarine zone with the highest values during pre-monsoon month of May and the lowest values during monsoon month of October. From WQI values (mostly >100) throughout the year, it was evident that Hooghly-Bhagirathi estuarine system is unsuitable for drinking and propagation of wildlife and freshwater aqua culture as per water quality standards. It was also observed that there is a severe anthropogenic influence on most of the parameters. Major causes of deterioration in water quality were discharge of untreated industrial and municipal wastes, besides high level of other anthropogenic activities. Abstract

In this paper, we investigate the behavior of 1200V Punch-Through, Trench gate, Field stop (FS) IGBT at various temperatures for different trench widths and depths. MOS-controlled devices with trench gates are the most desirable as their reduced VCE(sat) enables increased conduction current density in high power applications. The paper describes the simulation results of the trench gate IGBT. The effect caused in the I-V characteristics of IGBT by the trench design has been investigated for the first time. The results have been compared with different trench sizes. Technology-computer-aided-design (TCAD) simulations with Sentaurus and analytical solution reveal that the breakdown occurs under the gate. By enlarging the gate length, more freedom for conductivity modulation is provided decreasing the conduction losses. Trench gates provide higher channel density (small cell size) and reduce the JFET resistance in the IGBT structure. It is observed that the variation in trench width and depth affects the breakdown voltage, VCE(sat) and switching characteristics in turn varying the stability of the device. This device design not only reduces the chip size, but also improves current handling capability and latch-up current density.

Abstract

This paper presents a low cost innovative dual channel engineering to simulate the effects of arsenic and antimony implantation over breakdown voltage and on state resistance of an Ultra High Voltage (UHV) device. There are many devices in market with multiple conduction paths, but all these devices use high energy implants. But we have used a low cost and low energy implant over P-top to form an extra conduction path. Optimizations are done to obtain high breakdown voltage and lower Ron by varying the P-top and Ntop over P-top. We demonstrate interface charge analysis for both Antimony and Arsenic implantation over P-top and also we investigate the effect of N-top implantation before and after NDrift diffusion on breakdown voltage.
