In this work, a conceptual framework is proposed for translating microbial research into usable design tools in an urban built environment. This study on Microbiomes of the Built Environment (MoBE), specifically the cultivable microbial count, incorporates building code and built environment design factors applied in the Philippine setting. The National Building Code of the Philippines with focus on minimum habitable room specifications was used to provide basis for the framework process. Human presence and ventilation were used as parameters to produce data that can be used as design tools. Using passive air sampling, the three experiments were conducted within Metro Manila according to the proposed framework. The first experiment using settle plates of culture media in the morning and afternoon seeks to understand how bacterial and fungal counts are affected in a naturally ventilated and in a non-ventilated room. The second and third experiments were done to test the effects of different window sizes and the number of humans on bacterial count, respectively. Results showed higher bacterial count in the room with natural ventilation compared to the room with no ventilation. The use of different window sizes did not show significant difference in counts with the number replicates used but as the number of humans in the area increased, the bacterial counts also increased. Data produced in the preliminary experiments were used to conceptually design a space in this paper. The authors suggest that this building-code inspired framework be used as a guide for MoBE studies as a starting point and be further developed to understand and eventually produce healthier built environments.

Abstract The aim of this cross-sectional study was to perform a three-dimensional (3D) assessment of the cranial base of patients with unilateral cleft lip and palate (UCLP). Cone-beam computed tomography (CBCT) scans of 52 UCLP patients (21 females and 31 males; mean age, 10.0 ± 2.12 years) were compared with the scans of 72 individuals (24 females and 48 males; mean age, 11.0 ± 2.11 years) without CLP, matched by gender and age (control group, CG). The 3D Euclidean distances of anterior cranial base (N-S), posterior cranial base (S-Ba), total cranial base (N-Ba) lengths, cranial base width (Po-Po), as well as the cranial base flexure (NSBa), were measured using open-source software ITK-SNAP and 3D Slicer. Statistical analyses were carried out with the Student’s t-test at a significance level of 5%. UCLP demonstrated shorter 3D distances than CG in the N-S, S-Ba, and N-Ba cranial base lengths (p < 0.001). In comparison with female CG, female UCLP had a smaller cranial base flexure (NSBa; p = 0.020). No statistically significant differences between UCLP and CG were found for the cranial base width (Po-Po). UCLP patients presented distinct morphological cranial base characteristics in comparison with CG. These results indicate that morphological and positioning changes in the maxillary bones are not solely attributable to the cleft and/or surgical procedures.

ABSTRACT Introduction: The early diagnosis of mandibular asymmetry (MA) in patients with unilateral cleft lip and palate (UCLP) can contribute to its treatment. Objective: The aim of this study was to evaluate the occurrence and the extent of MA in UCLP patients at different growth stages. Methods: Cone-beam computed tomography (CBCT) of 47 UCLP patients were included, and divided into two groups (prepubertal stage and pubertal stage). The mandibular ramus height (Co-Go), mandibular body length (Go-Me), total mandibular length (Co-Me), gonial angle (CoGoMe), the volume of the mandibular condyles, and lateral chin deviation were evaluated. The cleft side (CS) and noncleft side (NCS) were compared using the paired t-test for dependent samples. Chin deviation and its possible association with specific sides were evaluated using the Mann-Whitney and Fisher’s Exact tests, respectively. The significance level was set at 5%. Results: Comparison of CS and NCS revealed that in prepubertal stage, Co-Me was shorter (105.5 ± 5.7 mm; p = 0.036) in the CS, and that in pubertal stage, Co-Go was lower (46 ± 6 mm; p= 0.004) and Go-Me was greater (78.5 ± 5.8 mm; p = 0.026) in the CS. In both growth stages, a significant association (p< 0.05) was observed between the side to which the chin has deviated and the CS. Conclusions: Although patients with UCLP in the prepubertal and pubertal stages presented significant measurements revealing MA, only Co-Go in the pubertal stage showed a clinically relevant difference. This specific result indicates that MA must be monitored during patients’ growth.

Background: Inflammatory bowel disease (IBD) affects 5%–10% of ankylosing spondylitis (AS) patients. Prior data suggest AS patients with IBD may have more severe disease and lower HLA-B27 prevalence. Yet, little is known about potential distinctions in AS with IBD compared to those without IBD. Objective: To investigate the clinical characteristics and radiographic differences between patients with (AS) with and without concurrent IBD. Design: This multicenter, observational, cross-sectional study included patients meeting European Spondyloarthropathy Study Group criteria from the Registry of Spondyloarthritis of Spanish Rheumatology (REGISPONSER) and Ibero-American Registry of Spondyloarthropathies (RESPONDIA) registries. Methods: Characteristics and disease burden were compared between patients with and without IBD. Multivariate logistic regression identified factors independently associated with IBD presence in patients with AS. Results: We included a total of 2766 patients with AS (1254 from REGISPONSER and 1512 from RESPONDIA), among whom 142 patients (5.13%) presented with concomitant IBD. AS patients with concurrent IBD were less frequently male, had a lower prevalence of HLA-B27 positivity, experienced a lower prolonged diagnostic delay, had a lower frequency of enthesitis, and received more commonly intensified treatment compared to those without IBD. In terms of structural damage, the Bath Ankylosing Spondylitis Radiology Index (BASRI) score for the sacroiliac joints (SIJs), cervical spine, and lumbar spine was lower in patients with AS and IBD than in those without IBD. In the multivariable analysis, the presence of IBD was significantly associated with a lower prevalence of HLA-B27 and enthesitis, with odds ratios (OR) of 0.32 (95% confidence interval (CI): 0.20–0.52) and 0.58 (95% CI: 0.33–0.97), respectively. Furthermore, structural damage in SIJs (BASRI) was significantly decreased in patients with IBD, with an OR of 0.79 (95% CI: 0.64–0.99). Conclusion: The presence of IBD in AS is associated with lower HLA-B27 positivity, less enthesitis, and less radiographic damage in this large population study.

Since 2017, targeted therapies combined with conventional intensive chemotherapy have started to improve outcome of patients with acute myeloid leukemia (AML). However, even before these innovations outcomes with intensive chemotherapy have improved, which has not yet been extensively studied. Thus, we used a large pan-European multicenter dataset of the HARMONY Alliance to evaluate treatment-time dependent outcomes over two decades. In 5359 AML patients, we compared the impact of intensive induction therapy on outcome over four consecutive 5-year calendar periods from 1997 to 2016. During that time, the 5- year survival of AML patients improved significantly, also across different genetic risk groups. In particular, the 60-day mortality rate has dropped from 13.0% to 4.7% over time. The independent effect of calendar periods on outcome was confirmed in multivariate models. Glutaric aciduria type 1 (GA1) is a rare inherited metabolic disorder caused by a deficiency of glutaryl-coenzyme A dehydrogenase (GCDH), with accumulation of neurotoxic metabolites, resulting in a complex movement disorder, irreversible brain damage, and premature death in untreated individuals. While early diagnosis and a lysine restricted diet can extend survival, they do not prevent neurological damage in approximately one-third of treated patients, and more effective therapies are required. Here we report the efficacy of adeno-associated virus 9 (AAV9)-mediated systemic delivery of human GCDH at preventing a high lysine diet (HLD)-induced phenotype in Gcdh−/− mice. Neonatal treatment with AAV-GCDH restores GCDH expression and enzyme activity in liver and striatum. This treatment protects the mice from HLD-aggressive phenotype with all mice surviving this exposure; in stark contrast, a lack of treatment on an HLD triggers very high accumulation of glutaric acid, 3-hydroxyglutaric acid, and glutarylcarnitine in tissues, with about 60% death due to brain accumulation of toxic lysine metabolites. AAV-GCDH significantly ameliorates the striatal neuropathology, minimizing neuronal dysfunction, gliosis, and alterations in myelination. Magnetic resonance imaging findings show protection against striatal injury. Altogether, these results provide preclinical evidence to support AAV-GCDH gene therapy for GA1.

El artículo analiza la percepción de los estudiantes del Grado en Educación Infantil sobre el modelo de enseñanza-aprendizaje "Flipped Classroom". El estudio se llevó a cabo en el contexto Centro Universitario Sagrada Familia durante el curso 2023/2024. Los objetivos de la investigación incluyen analizar si el sistema de trabajo basado en el "Flipped Classroom" desemboca en niveles significativos de motivación en los estudiantes, así como evaluar la eficacia del modelo en la mejora del aprendizaje continuo a lo largo del cuatrimestre. Del mismo modo, también se evalúa la adquisición de competencias transversales establecidas en la guía docente de la asignatura. Para recopilar los datos, se aplicó un cuestionario ad hoc validado que constaba de 15 preguntas de valoración sobre aspectos clave como motivación, comprensión, autonomía, competencias y eficacia en el aprendizaje. Los resultados revelaron una evaluación favorable de los estudiantes con respecto al nivel alcanzado en competencias transversales fundamentales, destacando la contribución del "Flipped Classroom" al desarrollo de la creatividad. Aunque se observó una valoración positiva en general, se identificaron áreas de mejora en competencias específicas como el pensamiento crítico y la expresión oral y escrita. Estos hallazgos sugieren la efectividad del "Flipped Classroom" en la promoción de la motivación y el aprendizaje continuo, pero también señalan la necesidad de ajustes para fortalecer ciertas habilidades.

Objetivos: en un momento en el que miles de códigos éticos empiezan a aprobarse en las Administraciones públicas españolas, este artículo trata de aportar luz sobre sus posibilidades y limitaciones, considerando, especialmente, los retos cognitivos que representan y su inserción en entornos organizativos que, en ocasiones, socializan en malas prácticas a sus empleados. Metodología: se trata de un texto teórico y de reflexión, basado en teoría moral aplicada, centrado en la denominada ética administrativa, al que se aporta investigación reciente sobre ciencia cognitiva. Resultados: el texto resalta el destacado papel de la motivación moral y la construcción del carácter para el éxito de los códigos, pero también nos permite comprender los problemas que se derivan de los conflictos de valores en el servicio público y cómo afrontarlos. Se demuestra el papel de las emociones, sentimientos e intuiciones en la conformación de las decisiones éticas, así como el riesgo de los sesgos cognitivos en la toma de decisiones frente a problemas morales. Finalmente, se trata el problema de los riesgos de socialización y normalización de prácticas corruptas en las organizaciones. Conclusiones: derivadas de los resultados, se aportan recomendaciones para las personas y organizaciones públicas a la hora de desarrollar y utilizar los códigos.

In 2015, Catalonia introduced sickle cell disease (SCD) screening in its newborn screening (NBS) program along with standard-of-care treatments like penicillin, hydroxyurea, and anti-pneumococcal vaccination. Few studies have assessed the clinical impact of introducing NBS programs on SCD patients. We analyzed the incidence of SCD and related hemoglobinopathies in Catalonia and the change in clinical events occurring after introducing NBS. Screening 506,996 newborns from 2015 to 2022, we conducted a retrospective multicenter study including 100 screened (SG) and 95 unscreened (UG) SCD patients and analyzed SCD-related clinical events over the first six years of life. We diagnosed 160 cases of SCD, with an incidence of 1 in 3169 newborns. The SG had a significantly lower median age at diagnosis (0.1 y vs. 1.68 y, p < 0.0001), and initiated penicillin prophylaxis (0.12 y vs. 1.86 y, p < 0.0001) and hydroxyurea treatment earlier (1.42 y vs. 4.5 y, p < 0.0001). The SG experienced fewer median SCD-related clinical events (vaso-occlusive crisis, acute chest syndrome, infections of probable bacterial origin, acute anemia requiring transfusion, acute splenic sequestration, and pathological transcranial Doppler echography) per year of follow-up (0.19 vs. 0.77, p < 0.0001), a reduced number of annual emergency department visits (0.37 vs. 0.76, p < 0.0001), and fewer hospitalizations (0.33 vs. 0.72, p < 0.0001). SCD screening in Catalonia’s NBS program has effectively reduced morbidity and improved affected children’s quality of life.

The main objective of the present study was to conduct a systematic review and meta-analysis of eye-tracking studies in adults with ASD. A total of 22 studies were included for meta-analysis. Eyes and Non-Social regions proved better for discriminating between ASD and non-ASD adults, while fixation duration seems to be the outcome to choose. Active engaged tasks seem to reduce differences between ASD and non-ASD adults, regardless of the emotional content of the stimuli/task. Proportional fixation duration on eyes and non-social areas in non-active tasks (e.g. free viewing) seems to be the best eye-tracking design for increasing the sensitivity and specificity in ASD adults.

Abstract The key issues in any fire emergency are recognising fire hotspots, locating the emergency intervention team (EI), following the evolution of the fire, and selecting the evacuation path. This leads to the study and development of HelpResponder, a solution capable of detecting the focus of interest in hostile spaces derived from fire due to high temperatures without visibility. A study is conducted to determine which model best predicts measured $$\text {CO}_2$$ CO 2 levels. The variables used are temperature, humidity, and air quality, obtained from sensors installed in a fire tower. The statistical methods applied, namely ARIMAX, KNN, SVM, and TBATS, allow the adjustment and modelling of the variables. Explanatory variables with temporal structure are incorporated into SVM, a new improvement proposal. Moreover, combining different models showed the best efficiency in forecasting. In fact, another contribution of our work lies in offering a small-scale prediction system that is specifically designed to save batteries. The system has been tested and validated in a hostile environment (building), simulating real emergency situations. This reduces the risks associated with the lack of information and improves the time for tactical operations, which could save lives.
