5. A[Beta] Deposits in Older Non-Demented Individuals with Cognitive Decline Are Indicative of Preclinical Alzheimer's Disease

Abstract

Approximately 30% of healthy persons aged over 75 years show A[beta] deposition at autopsy. It is postulated that this represents preclinical Alzheimer's disease (AD). We evaluated the relationship between A[beta] burden as assessed by PiB PET and cognitive decline in a well-characterized, non-demented, elderly cohort. PiB PET studies and cognitive tests were performed on 34 elderly participants (age 73 [plus or minus] 6) from the longitudinal Melbourne Healthy Aging Study (MHAS). Subjects were classified as being cognitively "stable" or "declining" by an independent behavioural neurologist based on clinical assessment and serial word-list recall scores from the preceding 6-10 years. Decline was calculated from the slope of the word-list recall scores. A[beta] burden was quantified using Standardized Uptake Value normalized to cerebellar cortex. Ten subjects were clinically classified as declining. At the time of the PET scans, three of the declining subjects had mild cognitive impairment, one had AD, and six were declining but remained within the normal range for age on cognitive tests. Declining subjects were much more likely to show cortical PiB binding than stable subjects (70% vs. 17%, respectively). Neocortical A[beta] burden correlated with word-list recall slopes (r = -0.78) and memory function (r = -0.85) in the declining group. No correlations were observed in the stable group. A[beta] burden correlated with incident memory impairment and the rate of memory decline in the non-demented ageing population. These observations suggest that neither memory decline nor A[beta] deposition are part of normal ageing and likely represent preclinical AD. Further longitudinal observations are required to confirm this hypothesis. (Contains 4 tables and 4 figures.)

Abstract

Background and purpose Visualization of pathogenic protein aggregates is crucial to elucidate pathomechanisms and to make an accurate diagnosis in many neurodegenerative conditions. Aggregates of the microtubule-binding protein, tau, are one of the most important pathogenic molecules in neurodegenerative disorders. Progressive supranuclear palsy ( PSP) is characterized by the deposition of tau proteins in some specific area such as the basal ganglia and brainstem. We tried to detect tau lesions in the brains of living patients with PSP with a novel positron emission tomography (PET) tracer, [18F] THK-5351, which we have recently developed. Methods Paraffin-embedded brain sections of the patients with PSP were used for autoradiography with [3H] THK-5351 and immunohistochemistry. Nine healthy controls, 13 patients with Alzheimer's disease and three patients with PSP participated in this PET study with [18F] THK-5351. To detect amyloid-β deposition, PET imaging with Pittsburgh compound B was also performed. Results Autoradiography in the brain sections of patients with PSP demonstrated [3H] THK-5351 binding to tau deposits with a high selectivity. Although patients with PSP exhibited no remarkable [18F] THK-5351 retention in the temporal cortex, significantly higher tracer retention was observed in the globus pallidus and midbrain. In contrast, amyloid imaging with Pittsburgh compound B showed no remarkable accumulation in the cerebral cortex of PSP. Conclusions We conclude that [18F] THK-5351 PET can potentially be used to detect the regional brain distribution of tau lesions in PSP, thereby facilitating the differential diagnosis of neurodegenerative disorders associated with tau protein. Abstract

Pittsburgh Compound B (PiB) is a C11 PET tracer designed to bind to amyloid plaques, one of the hallmark of Alzheimer's disease. The potential of PiB as an early marker of Alzheimer's disease has lead to an increasing use of PiB and the development of several F18 equivalents. Quantitative analysis of PiB images requires an accurate normalisation, parcellation and estimation of retention in the brain's gray matter. Typically this relies on co-registered MRI to extract the cerebellum, compute the standardized uptake value ratio (SUVR) and provide parcellation and segmentation for quantification of neocortical SUVR. However, not all subjects undergo MRI. In this paper we propose a highly accurate MR-less parcellation, SUVR normalisation and quantification method for PiB images. This involves rigidly registering the raw PiB images to a PiB atlas, computing pair-wise normalised mutual information, and constructing a 2D manifold. Each new scan is mapped on the manifold and its k nearest neighbours are selected as atlases in a segmentation propagation scheme with their associated MRI segmentations and parcellation used as priors to estimate the SUVR normalisation and quantification. Comparison of our MRless approach to an MR-based approach showed a coefficient of correlation of neocortical PiB SUVR of R2=0.94 and an absolute mean error of 5.9%.
