2. Adult Patient's Opinions on Surgical Correction of Congenital Penile Curvature

Abstract

ABSTRACT Introduction Congenital penile curvature is common and presents with varying levels of severity. The curvature may present during infancy or post-pubertal. Historically, treatment has consisted of surgical correction, typically with penile plication or complex grafting procedures. Most pediatric urologists recommend surgical management for penile curvature (PC) based on curvature severity, with previous work identifying ≥ 30° as a common threshold to recommend correction. Objective To assess adult patients’ opinions on willingness to consider surgical correction for PC in order to determine if this aligns with pediatric urologists’ opinions. Methods In this cross-sectional study, a survey was administered to adult patients and their partners (> 18 years of age) in general adult urology clinics at 3 geographically separate institutions. The survey consisted of unlabeled images of penis models with varying degrees of PC (range 10-90°). Respondents were asked to select the images they would want surgically corrected for themselves or their partners. Univariable and multivariable analyses were performed to identify demographic variables associated with willingness to consider correction (p-values < .05 considered significant). Results Response rate was 77% (300/388). The majority (80%) of participants chose to surgically correct PC, however 20% chose to not surgically correct any degree of PC. Most participants were male (70%), married (62%), heterosexual (92%), and did not work in healthcare (80%) (Table 1). Among those who endorsed willingness to surgically correct PC, the average threshold for correction was 40.5° (SD 25.3). On univariable analysis, there was a significant difference in willingness to undergo PC correction based on gender, age groups, sexual orientation, and region (Table 1). Abstract

BACKGROUND: In the rush to develop health technologies for the COVID-19 pandemic, the unintended consequence of digital health inequity or the inability of priority communities to access, use, and receive equal benefits from digital health technologies was not well examined. OBJECTIVE: This scoping review will examine tools and approaches that can be used during digital technology innovation to improve equitable inclusion of priority communities in the development of digital health technologies. The results from this study will provide actionable insights for professionals in health care, health informatics, digital health, and technology development to proactively center equity during innovation. METHODS: Based on the Arksey and O'Malley framework, this scoping review will consider priority communities' equitable involvement in digital technology innovation. Bibliographic databases in health, medicine, computing, and information sciences will be searched. Retrieved citations will be double screened against the inclusion and exclusion criteria using Covidence (Veritas Health Innovation). Data will be charted using a tailored extraction tool and mapped to a digital health innovation pathway defined by the Centre for eHealth Research roadmap for eHealth technologies. An accompanying narrative synthesis will describe the outcomes in relation to the review's objectives. RESULTS: This scoping review is currently in progress. The search of databases and other sources returned a total of 4868 records. After the initial screening of titles and abstracts, 426 studies are undergoing dual full-text review. We are aiming to complete the full-text review stage by May 30, 2024, data extraction in October 2024, and subsequent synthesis in December 2024. Abstract

This paper proposes a methodology to calculate both the first and second derivatives of a vector function of one variable in a single computation step. The method is based on the nested application of the dual number approach for first order derivatives. It has been implemented in Fortran language, a module which contains the dual version of elementary functions as well as more complex functions, which are common in the field of rotational kinematics. Since we have three quantities of interest, namely the function itself and its first and second derivative, our basic numerical entity has three elements. Then, for a given vector function $f:\mathbb{R}\to \mathbb{R}^m$, its dual version will have the form $\tilde{f}:\mathbb{R}^3\to \mathbb{R}^{3m}$. Abstract

The problem of path generation for the spherical 4R mechanism is solved using the Differential Evolution algorithm (DE). Formulas for the spherical geodesics are employed in order to obtain the parametric equation for the generated trajectory. Direct optimization of the objective function gives the solution to the path generation task without prescribed timing. Therefore, there is no need to separate this task into two stages to make the optimization. Moreover, the order defect problem can be solved without difficulty by means of manipulations of the individuals in the DE algorithm. Abstract

The optimal dimensional synthesis for planar mechanisms using differential evolution (DE) is demonstrated. Four examples are included: in the first case, the synthesis of a mechanism for hybrid-tasks, considering path generation, function generation, and motion generation, is carried out. The second and third cases pertain to path generation, with and without prescribed timing. Finally, the synthesis of an Ackerman mechanism is reported. Order defect problem is solved by manipulating individuals instead of penalizing or discretizing the search space for the parameters. A technique that consists in applying a transformation in order to satisfy the Grashof and crank conditions to generate an initial elitist population is introduced. Abstract

Open-angle glaucoma (OAG), the most prevalent clinical type of glaucoma, is still the main cause of irreversible blindness worldwide. OAG is a neurodegenerative illness for which the most important risk factor is elevated intraocular pressure (IOP). Many questions remain unanswered about OAG, such as whether nutritional or toxic habits, other personal characteristics, and/or systemic diseases influence the course of glaucoma. As such, in this study, we performed a multicenter analytical, observational, case-control study of 412 participants of both sexes, aged 40-80 years, that were classified as having ocular hypertension (OHT) or OAG. Our primary endpoint was to investigate the relationship between specific lifestyle habits; anthropometric and endocrine-metabolic, cardiovascular, and respiratory events; and commonly used psychochemicals, with the presence of OHT or OAG in an ophthalmologic population from Spain and Portugal. Demographic, epidemiological, and ocular/systemic clinical data were recorded from all participants. Data were analyzed using the R Statistics v4.1.2 and RStudio v2021.09.1 programs. The mean age was 62 +/- 15 years, with 67-80 years old comprising the largest subgroup sample of participants in both study groups. The central corneal thickness (ultrasound pachymetry)-adjusted IOP (Goldman tonometry) in each eye was 20.46 +/- 2.35 and 20.1 +/- 2.73 mmHg for the OHT individuals, and 15.8 +/- 3.83 and 16.94 +/- 3.86 mmHg for the OAG patients, with significant differences between groups (both p = 0.001). The highest prevalence of the surveyed characteristics in both groups was for overweight/obesity and daily coffee consumption, followed by psychochemical drug intake, migraine, and peripheral vasospasm. Our data show that overweight/obesity, migraine, asthma, and smoking are major risk factors for conversion from OHT to OAG in this Spanish and Portuguese population.

Abstract

One of the most pronounced effects of climate change on the world’s oceans is the (generally) poleward movement of species and fishery stocks in response to increasing water temperatures. In some regions, such redistributions are already causing dramatic shifts in marine socioecological systems, profoundly altering ecosystem structure and function, challenging domestic and international fisheries, and impacting on human communities. Such effects are expected to become increasingly widespread as waters continue to warm and species ranges continue to shift. Actions taken over the coming decade (2021–2030) can help us adapt to species redistributions and minimise negative impacts on ecosystems and human communities, achieving a more sustainable future in the face of ecosystem change. We describe key drivers related to climate-driven species redistributions that are likely to have a high impact and influence on whether a sustainable future is achievable by 2030. We posit two different futures—a ‘business as usual’ future and a technically achievable and more sustainable future, aligned with the Sustainable Development Goals. We then identify concrete actions that provide a pathway towards the more sustainable 2030 and that acknowledge and include Indigenous perspectives. Achieving this sustainable future will depend on improved monitoring and detection, and on adaptive, cooperative management to proactively respond to the challenge of species redistribution. We synthesise examples of such actions as the basis of a strategic approach to tackle this global-scale challenge for the benefit of humanity and ecosystems.

Abstract

The International Bottom Trawl Survey Working Group (IBTSWG) coordinates fishery-independent bottom trawl surveys in the ICES area in the Northeast Atlantic and the North Sea. These long-term monitoring surveys provide data for stock assessments and facilitate examina-tion of changes in fish distribution and relative abundance. The group also promotes the stand-ardization of fishing gears and methods as well as survey coordination. This report summarizes the national contributions in 2021–2022 and plans for the 2022–2023 surveys coordinated by IBTSWG.
