15. Percutaneous renal artery embolisation of non-functioning renal allografts with clinical intolerance

Abstract

The aim of the study was to evaluate the efficacy and safety of percutaneous renal artery embolisation of non-functioning renal allografts in patients with graft intolerance syndrome (GIS). Transcatheter artery embolisation was performed in 30 kidney transplant recipients with GIS. The duration of graft function had been 60+/-45 months. Infectious disease was ruled out in all patients. Embolisation consisted of the injection of polyvinyl alcohol microspheres followed by the insertion of a stainless steel coil in the renal artery branches. Symptoms of GIS included: fever-graft pain (44%, n=13), fever-hematuria-pain (20%, n=6), fever-hematuria (13%, n=4) and fever alone (23%, n=7). Latency time between graft failure and embolisation was 184+/-227 (17-1181) days. Embolisation was clinically successful with the prolonged disappearance of GIS in 24 patients (80%). Six patients showed initial clinical improvement, but GIS reappeared at 40+/-18 days, and graft nephrectomy was required. There were no major complications associated with embolisation and no deaths. Perirenal collateral supply was a risk factor for the reappearance of GIS. Renal vascular embolisation is a simple, safe and effective technique for treating renal allograft intolerance syndrome and could be a feasible alternative for the first-line treatment.

Abstract

Simultaneous kidney-pancreas (SKP) transplantation is associated with increased risk of kidney rejection (KR) due to an unknown mechanism. The aim of this study is to analyze the characteristics of KR in 44 SKP transplantations under quadruple immunosuppressive therapy and to evaluate the response to treatment and its effect on renal allograft survival and renal function. The mean follow-up was 25 +/- 14 months. Seventy-seven percent of the patients (34 of 44) presented an acute renal allograft rejection. Sixty-six percent (29 of 44) had one rejection episode and 11% (5 of 4) 2 episodes. KR was early (85% in the first month after transplantation), intense (3.7-fold increase in creatinine) and had great clinical features. Twenty-eight percent of the patients had an early relapse during the first month after treatment. KR did not affect the survival of the renal allograft in the short-term (1 and 2 years). Overall, 62% were corticosensitive (CS) and the remaining 38% were corticoresistant (CR). The group with an isolated rejection without relapse was CS in 69% of the cases, achieved complete remission in 73% and renal function was not affected at 1 and 2 years [115 +/- 26 mumol/l (1.3 +/- 0.3 mg/dl) and 150 +/- 53 mumol/l (1.7 +/- 0.6 mg/dl)] in comparison with the group without rejection [97 +/- 18 mumol/l (1.1 +/- 0.2 mg/dl) and 115 +/- 35 mumol/l (1.3 +/- 0.4 mg/dl); p = NS]. On the other hand, the group with an early relapse of the first rejection and the group with two rejections were principally CR (62 and 60%, respectively), had partial remission with treatment (50 and 60%) and had worse renal function at 1 and 2 years [212 +/- 71 mumol/l (2.4 +/- 0.8 mg/dl) and 221 +/- 53 mumol/l (2.5 +/- 0.6 mg/dl)] than in the group with isolated KR (p0.05 and p0.001). In conclusion, despite intense immunosuppressive treatment, the frequency of rejection of a renal allograft in SKP is high. The response to treatment is satisfactory and does not affect the survival of the allograft in the short-term. However, multiple episodes or early relapse of rejection are associated with higher creatinine levels.

Abstract

PubMed ID: 12829583, Several studies have compared bone marrow (BM) and peripheral blood (PB) as stem cell sources in patients receiving allografts, but the cell doses infused have not been considered, especially for BM. Using the ALWP/EBMT registry, we retrospectively studied 881 adult patients with acute myelocytic leukemia (AML), who received a non-T-depleted allogeneic BM (n = 515) or mobilized PB (n = 366) standard transplant, in first remission (CR1), from an HLA-identical sibling, over a 5-year period from January 1994. The BM cell dose ranged from 0.17 to 29 × 10 8 /kg with a median of 2.7 × 10 8 /kg. The PB cell dose ranged from 0.02 to 77 × 10 8 /kg with a median of 9.3 × 10 8 /kg. The median dose for patients receiving BM (2.7 × 10 8 /kg) gave the greatest discrimination. In multivariate analyses, high-dose BM compared to PB was associated with lower transplant-related mortality (RR = 0.61; 95% CI, 0.39-0.98; P = .04), better leukemia-free survival (RR = 0.65; 95% CI, 0.46-0.91; P = .013), and better overall survival (RR = 0.64; 95% CI, 0.44-0. 92; P = .016). The present study in patients with AML receiving allografts in first remission indicates a better outcome with BM as compared to PB, when the dose of BM infused is rich. © 2003 by The American Society of Hematology.

The Acute Leukemia Working Party (ALWP) of the European Cooperative Group for Blood Marrow Transplantation (EBMT)

Abstract

Several studies have compared bone marrow (BM) and peripheral blood (PB) as stem cell sources in patients receiving allografts, but the cell doses infused have not been considered, especially for BM. Using the ALWP/EBMT registry, we retrospectively studied 881 adult patients with acute myelocytic leukemia (AML), who received a non-T-depleted allogeneic BM (n = 515) or mobilized PB (n = 366) standard transplant, in first remission (CR1), from an HLA-identical sibling, over a 5-year period from January 1994. The BM cell dose ranged from 0.17 to 29 × 108/kg with a median of 2.7 × 108/kg. The PB cell dose ranged from 0.02 to 77 × 10 8/kg with a median of 9.3 × 108/kg. The median dose for patients receiving BM (2.7 × 108/kg) gave the greatest discrimination. In multivariate analyses, high-dose BM compared to PB was associated with lower transplant-related mortality (RR = 0.61; 95% CI, 0.39-0.98; P = .04), better leukemia-free survival (RR = 0.65; 95% CI, 0.46-0.91; P = .013), and better overall survival (RR = 0.64; 95% CI, 0.44-0. 92; P = .016). The present study in patients with AML receiving allografts in first remission indicates a better outcome with BM as compared to PB, when the dose of BM infused is rich. © 2003 by The American Society of Hematology.

Abstract

La determininación del módulo de elasticidad del hormigón de presas es difiícil tanto en medidas directas en la presa como mediante probetas, ya que se requieren ensayos con probetas de gran dimensión y, consecuentemente, grandes prensas de ensayo, asi como medios auxiliares de manipulación. En la presente comunicación se muestra un método altemativo de obtención del valor del módulo de deformación longitudinal del hormigón de presa. El mismo parte de ensayos convencionales con probetas cilíndricas (15x30 cm) y con probetas de mortero, a cuyos resultados se ajusta, modelos bifásicos. Los resultados obtenidos con l.os modelos se contrastan con probetas prismáticas grandes de 45x45x90 cm, fabricadas con el hormigón de presa (tamaño máximo del árido de 120 mm). Este contraste se ha hecho para edades desde 7 a 180 días, con el hormigón utilizado en la presa de la Llosa del Cavall. El conjunto de resultados ha sido bueno, permitiendo un metodo sencillo y fiable de obtención del modulo del hormigón (como material), a partir de ensayos estandar.

Abstract

La misión principal del pretensado circunferencial de la pared de un depósito cilíndrico de hormigón es la de compensar las atracciones anulares originadas por la carga de agua y, en menor medida, contrarrestar las debidas a otras solicitaciones (gradiente térmico, retracción, carga sin simetría de revolución, etc.). Cuando la unión de la pared y la solera es continua, el movimiento radial del borde inferior de la pared está impedido, y el pretensado próximo a la base no sólo no comprime apenas la pared, sino que introduce esfuerzos de flexión general importantes. Con la intención de reducir en lo posible este comportamiento de flexión, varios autores han recurrido a formular leyes óptimas de pretensado que redistribuyen los tendones de pretensado en la parte inferior del depósito para conseguir su máxima eficacia en la compresión radial de la pared. En este trabajo se presenta un estudio comparativo entre cinco funciones de pretensado, algunas de ellas propuestas en la literatura técnica y otras propuestas por los autores del presente artículo. Dicho estudio se ha llevado a cabo con un modelo de análisis analítico lineal, que considera el comportamiento conjunto pared-solera-cimentación, y se ha basado en unos requisitos comunes de diseño del pretensado, previamente establecidos. Los resultados obtenidos en el estudio han permitido determinar una función óptima de pretensado, tanto en lo que concierte a su ley de distribución como en lo que se refiere a la fuerza necesaria de pretensado. También han mostrado algunas diferencias significativas entre los resultados obtenidos en este estudio y los presentados por otros autores en la literatura técnica, debidas, principalmente, al hecho de haber estudiado el depósito con unión continua en la base con un modelo que contempla la capacidad real de movimiento de la unión.

Abstract

Fifteen patients with severe acute vascular graft rejection (twelve of them steroid-resistant) treated with OKT3 monoclonal antibody (8) or antilymphocyte serum (7) were sequentially studied with duplex-Doppler ultrasonography (DDUS). Measurements of intrarenal resistance were obtained by means of the resistive index (RI). During rejection episodes, the intrarenal resistance was increased in all patients (RI range: 0.89–1), with absent or reversed diastolic blood flow. Normalization of diastolic blood flow was documented prior to the decrease of serum creatinine level in all patients who recovered from a rejection episode. We conclude that DDUS is a noninvasive technique useful in the diagnosis and monitoring of acute vascular graft rejection. Prediction of graft outcome after rejection treatment could probably be made based on DDUS findings.

Abstract

In a consecutive series of 639 Renal Transplants (RT) performed between January 1st 1978 and December 31st 1988, 75 Renal Transplanctectomies (RTX) were carried out. Of those, fifty-seven (76%) were due to immunological causes, 10 (13.3%) to vascular complications, 7 (9.3%) to urological problems and 1 (1.3%) to deep infection. The time elapsed between RTX and RT was as follows: 25 were performed earlier than 6 weeks, 17 between 6 weeks to 6 months, and 33 after 6 months. The higher number of complications did not happened, as it is frequently described, in the first group; on the contrary, it fell in the group that had been transplanted longer. There were 13 immunologically caused renal rhexis, 5 of which were RTX, another 5 were operated on and diagnosed at perioperation, and immediately received the appropriate treatment so they were able to keep their kidney, and finally, 3 patients were diagnosed a rejection with renal rhexis, ecographically confirmed. These patients were given only medical treatment, which was enough to allow jugulation of the picture. RTX techniques used were: extracapsular in 28 cases and intracapsular in 47. Surgical approach, except for 2 cases of simultaneous kidney and pancreas transplantation, was extraperitoneal.

Abstract

The modulus of elasticity of dam concrete is difficult to determine directly from tests due to the necessity for large specimens and testing machines. In order to study the applicability of simple elastic models for predicting the modulus from standard size specimens, tests were conducted on prisms of 45×45×90 cm fabricated with dam concrete (maximum aggregate of 120 mm). The tests on standard 15×30 cm cylinders were made with the mortar and wet-screened components of this concrete. It is seen that the use of the data from these components together with estimated values of the modulus of the aggregates gives reasonable predictions of the moduli of the dam concrete. Abstract

The abscisic acid-responsive gene rab17 is induced during maize embryo maturation and in vegetative tissues under water stress conditions. To investigate how ABA is involved in the induction of the rab17 gene, we present here a genetic approach to analyse the transcriptional regulation of the 1.3 kb rab17 promoter fragment in transgenic wild-type Arabidopsis and mutants which are deficient (aba) and insensitive (abi1, abi2 and abi3) to ABA. During seed development the rab17 promoter fragment confers similar temporal and spatial regulation on the reporter gene GUS, both in transgenic wild-type and ABA-deficient and ABA-insensitive mutants. The rab17 promoter was only active in embryo and endosperm during late seed development, although the ABA-deficient embryo mutant showed a reduction in the level of GUS activity. During germination rab17 promoter activity decreases, and GUS activity is not enhanced by water stress in transgenic wild-type and mutant plants. In contrast, transcription of the Arabidopsis endogenous rab gene is stimulated by water stress, both in wild-type and ABA-insensitive mutants. Our data suggest that different molecular mechanisms mediate seed-specific expression and ABA water stress induction of the rab17 gene and indicate strong conservation of the seed-specific regulatory mechanism for rab genes in monocot and dicot plants. © 1994 Kluwer Academic Publishers.
