Study Design Prospective randomized controlled study. Purpose This study aimed to assess the effects of a different combination of anesthetic techniques in lumbar and thoracolumbar spinal surgeries in terms of muscle relaxation and surgical field in addition to hemodynamic parameters. Overview of Literature Adequate relaxation of the erector spinae muscle and good surgical fields are preferred in lumbar spine surgeries. The effects of anesthetic techniques on these parameters have not been evaluated from a surgeon’s perspective to date. We propose a novel combined anesthetic technique that improves the working conditions from a surgeon’s perspective as we hypothesize that combining general anesthesia (GA) with regional anesthesia (RA) might provide benefits for both techniques. Methods A prospective randomized study of 76 patients who underwent lumbar and thoracolumbar surgeries was conducted by randomly allocating patients into three anesthetic groups: GA alone, GA with spinal anesthesia (SA), and GA with erector spinae plane block (ESPB) by the allocation concealment method to avoid selection bias. The working conditions were assessed by the same operating surgeon who was blinded by the type of anesthesia to eliminate the assessment bias. Muscle relaxation and surgical field were compared among the three groups along with other hemodynamic parameters to identify any significant differences. Results Significantly better muscle relaxation, surgeon satisfaction, postoperative analgesia, and blood pressure (BP) were observed in the GA+RA when compared to GA alone (p0.05). Complications were only observed in the GA+SA group (19%). Conclusions The study results suggest that the addition of RA to GA may provide better working conditions and surgeon’s satisfaction by improving relaxation of the erector spinae in addition to decreasing the BP and postoperative pain in contrast to the use of GA alone. The combined GA and ESPB techniques may be a viable anesthetic alternative to provide better working conditions for surgeons.

Aim: The purpose of this study was to determine the closest matching shade for primary maxillary anterior teeth from the most widely used Vitapan classical shade guide available for permanent teeth.Materials and Methods: A total of 313 children aged between 3 and 5 years were evaluated. Vitapan classical shade guide was used to determine the shade of six primary maxillary anterior teeth. Scores obtained were noted down in a scoring sheet and values were tabulated. Data collected were tabulated and statistically analyzed using Pearson Chi-square test.Results: A1 was found to be the closest matching shade for primary maxillary anterior teeth. D3 was found to be the least prevalent shade for primary maxillary anterior teeth. There was no statistical significant difference in the shades among teeth of same quadrant (p > 0.05) and also between teeth of right and left quadrants (p > 0.05), respectively.Conclusion: A1 shade of Vitapan classical shade guide is the most prevalent shade for primary maxillary anterior teeth and can be satisfactorily reproduced to all primary maxillary anterior teeth in general. Objectives: To estimate the concentration of nitrous oxide (N 2 O) required for effective sedation to render dental treatment for an anxious child and to assess the child's behavior, patient acceptance, parental satisfaction, postoperative complications, and ease of manipulation of Porter Silhouette mask by the dentist during N 2 O-O 2 sedation., Study Design: A total of 40 children, 6-10-year-old requiring dental treatment, were treated using N 2 O sedation. The level of N 2 O required for optimal sedation, patient behavior, level of acceptance of N 2 O by the patient, clinical recovery score, and postoperative complications were observed throughout the study. At the end of the treatment, a questionnaire was given to the parents to evaluate parental satisfaction., Results: The sedation was very effective, with 25-50% of N 2 O concentration. About 92.5% of children were fully cooperative, the dentist was able to place the mask with comfort in 92.5% of children, significant improvement was seen in the patient's behavior with minimal complications, and 100% of the parents were satisfied with the treatment under sedation., Conclusion: Inhalational sedation with N 2 O using Porter Silhouette mask results in effective sedation, with increased patient comfort, and parental acceptance in providing dental treatment., How to Cite This Article: AKR SP, Mungara J, Vijayakumar P, et al.
