The evolution of mental disorders, notably the most severe forms such as schizophrenia, is a constant concern in terms of the necessary assessment of treatments and their efficiency, and to the human and economic cost of the 'chronicity' of the disorders. Many patients experience a positive evolution, evaluated in terms of subjective quality of life. However, the concept of recovery does not seem appropriate. The field of representations which the patient and the caregiver form with regard to the disease and its curability is an important element. The co-construction of the patient's future, in an approach centred on the patient, on their freedom and their autonomy, opens up the way towards recovery., (Copyright © 2017 Elsevier Masson SAS. Many long-term treatments appear to be an expression of therapeutic deadlock. The situation leads to a questioning of the concept of chronicity and the identification of the determining factors of situations which are apparently blocked, marked by the search for solutions taking a back seat to the taking of action. The original liaison between solitary confinement and behavioural disorder still appears to operate significantly on representations of this therapeutic modality and, ultimately, on its indications in daily practice. The continuing salience of this liaison seems a common thread to outline avenues for reflection.

Acute and transient psychotic disorder remains an important element of the clinical field. It was first observed by Philippe Pinel in the 19th century. Other figures, such as Philippe Chaslin and Valentin Magnan, have contributed to the identification of the disorders and a psychopathological classification. While the central clinical element of acute and transient psychotic disorder is delusion and hallucinations, its resolution can be quick or it may be a sign of the gradual onset of a psychiatric pathology.

The dismemberment of the concept of neurosis, particularly of hysteria, in the current classifications (DSM-III and following, ICD-10) has led the authors to consider the concept of somatisation and the various situations included in it. Somatic disorders as the manifestation of a mental disorder, often of a depressive or anxious nature, bring back to the forefront both hysteria and its semiological forms under the global term "somatoform disorders", and in parallel with new pathologies, somatic symptoms as specific behaviors associated with the representation of a morbid condition from the point of view of an inhabitual physical perception. The expression of the psychic apparatus into the body, and its various clinical manifestations, has no direct specificity in the elderly. However, it is both frequent, and frequently misunderstood, thus requiring particular vigilance from the physician when confronted with physical complaints among elderly patients in whom the link between the body and the psychic apparatus takes on a particular role with the approach of death and the bereavement process that it requires. A structural approach, and search for event-driven factors and psychodynamic elements should be the key points of the clinician's approach.

After recalling the need of explicit description of psychic disorders and the importance of the psychotherapeutic approach in the elderly subject, the author defines the function of support and the content of psychotherapeutic support. The authors consider the psychotherapeutic attitude of the clinician, which has an effect preceding that of psychotherapy using specific techniques. This attitude is not limited to support and empathy alone but brings into play complex interactive processes. The importance and the role of interactions between generations and the relationship of age in geriatric care leads to expanding psychotherapeutic support to the family circle as well as to volunteer and professional persons involved in care of the patient.

We studied the social status of psychiatric patients (N = 1085) in care for the first time in the public psychiatric department in an area surrounding Lyon. The survey shows some demographic and social factors characterizing this population. We examined also how clinicians take life-events into account during this first access. The findings are discussed by comparison with other works seen in the literature.

The authors report 2 cases of unilateral PURTSCHER'S retinopathia who became at the time of a car injury. They discuss about different possible mechanisms of this affection mainly a security belt wearing and tough slow down.
