6. Association of serum iron studies in COVID associated mucormycosis with stage of the disease

Abstract

Background: Mucormycosis is a rare and lethal invasive fungal infection caused by opportunist and ubiquitous fungi. India has reported surge in cases of post COVID 19 mucormycosis due to the increasing frequency of risk factors like corticosteroid therapy, uncontrolled diabetes, DKA, neutropenia and iron overload and hence warrants a detailed study to identify potential contributing factors. Aim of this study was to study the clinical profile of patients with CAM and to correlate serum iron studies with severity and extent of disease in CAM patients. Methods: This prospective crosssectional study was conducted on 75 patients with CAM admitted to Bowring Hospital, Bangalore. Detailed history with clinical evaluation and appropriate investigations done and data was analysed. Results: The mean age of the subjects was 48.19 with 52 males and 23 females. Among 75 patients 90.7% were unvaccinated against COVID, 62.7% had oxygen and steroid use, with diabetes mellitus as most common comorbidity. Rhino orbital-cerebral mucormycosis was the most common clinical presentation. Patients with stage 4 CAM had elevated levels of inflammatory markers LDH (292), D-dimer (457), CRP (74.64), mean serum iron (50.37) and TIBC (255.37). Case fatality rate was 12%. Conclusions: The results of this study revealed significant correlation between the clinical severity of CAM and higher mortality, with increased serum iron levels and inflammatory markers. Therefore patients with elevated levels of available serum iron are uniquely susceptible to mucormycosis infection, suggesting dysregulated iron metabolism in its pathogenesis.

The forming limit diagrams for the Aluminium-2014 material at the stretching process was used to determine the room temperature in the experimental setting. From this stretching operation strength, tension, major strain, minor strain can be determined. Material properties of Al-2014 are estimated at different temperatures, and the effects of these richness, such as work hardening exponent, anisotropy and material strength, are correlated by FLD at different temperatures. 9. Association of Clinical Profile and Demographic Factors Associated with the Adverse Outcomes in Elderly with Covid-19 Infection in a Tertiary Care Hospital

Abstract

INTRODUCTION: Elderly people are at a higher risk of COVID-19 infection due to their decreased immunity and body reserves, as well as multiple associated comorbidities. Also, course of disease tends to be more severe in case of elderly resulting in higher mortality. MATERIALS AND METHODS: This retrospective study included 757 admitted covid-19 patients in a dedicated tertiary care hospital. The data was collected between December 2021 to March 2022 were analysed for the clinical profile, demographic factors with adverse outcomes in elderly patients. RESULTS: 308 were elderly patients, Mean age was 70.2,males-190(61.7%) females118(38.3%), 267(86.7%) were discharged,40 (13%) died,1(0.3%) went dama.6(1.9%) were asymptomatic,131(42.5%) had mild disease,128(41.6) had moderate disease , 43(14%) had severe disease. 52(16.9%) were vaccinated with covaxin,166(53.9%) were vaccinated with covishield, 90(29.2%) were not vaccinated. Diabetes(62%) and hypertension(53.9) were the major risk factors. Other risk factors included ischemic heart disease, chronic kidney disease, chronic obstructive pulmonary disease, seizures, pulmonary tuberculosis. CONCLUSION: The disease is more severe in patients with comorbidities and unvaccinated when compared to general population. Early diagnosis and individualized therapeutic management should be developed for elderly subjects based on personal medical history and comorbidities .

Abstract

Catalytic converter is an important after treatment device for emission control in automobiles. Conversion rate of harmful exhaust emission gas in catalytic converter and its efficiency is directly related to design factors of converters such as catalyst materials, wash coat material, substrate material, substrate design and other design factors. Amongst all the factors selection of material for catalyst is considered to be the most important as it deals with exhaust molecules to form nontoxic exhaust emission like nitrogen, carbon dioxide, hydrogen and also it can result in reduction of back pressure by design optimization. Various materials have been used as catalyst for catalytic converter which ranges from precious metals like platinum, palladium and rhodium etc. to non-noble metals such as cobalt, zirconium etc. The selection of materials has greater significance in catalytic converter design because of the vast activity in material science. Therefore, the range of material available to engineers are much larger than ever before. To accomplish better performance at lower price requires as more rational process for material selection. This paper deals with the recent material selection methodology by using weighted property index method (WPIM) to select material for catalytic converter. The emission performance of a converter is primarily function of properties of the catalyst. So, in case of selecting catalytic material, there are different parameters that one might consider for catalyst selection. However, in this method five different important catalytic properties are selected as required characteristic which includes valency, density, ionization energy, thermal conductivity, and thermal expansion. They were optimized using WPIM. The results of the present paper on WPIM gives best material selection for converter. Abstract

The present investigation was undertaken to find the impact of abiotic factors on seasonal incidence and sucking pest complex of brinjal under field conditions during kharif 2015-2016. The incidence of leaf hopper population (2.80 Lh/L i.e., Leaf hopper mean population/leaf) was noticed during 34th standard week and reached peak by 40th standard week (5.00 Lh/L) (October) whereas the aphid population was noticed during the 34th standard week (3.00 Lh/L) and peak population observed during the 40th standard week (4.60 Lh/L) (October). Correlation studies showed that among the various abiotic factors, maximum temperature showed highly significant positive correlation (r= 0.77) and sunshine hours (r = 0.61) showed significant positive correlation with the leaf hopper population. In case of aphid population, maximum temperature showed significant positive correlation (r = 0.70), rainfall showed highly significant negative correlation (r = -0.74) and relative humidity evening (r = -0.59) showed significant negative correlation with aphid population. The present investigations will give a brief idea about how the abiotic factors influencing the sucking pests of brinjal.

Abstract

Chronic obstructive pulmonary disease (COPD) is a common and preventable disease, presently it is the fifth leading cause of death globally. The estimate in 2002 suggested worldwide prevalence of COPD was 11.6/1000 in men and 8.7/1000 in women. According, Global Burden of Disease Study, by the year 2020, COPD is likely to become the fifth leading cause of disability adjusted life years (DALYS), moving ahead from twelfth position it occupied in 1990. An institutional based prospective study was conducted during the period of Jan -2007 to July -2007 to know the outcome of patients with acute exacerbation of COPD and also to analyse the factors responsible for repeated admissions and study the prognostic factors in patients with COPD. Detailed clinical examination and investigations was carried out according to the proforma that was predesigned and pretested. In the present study we observed that 65% of patients were aged 65 years and above and 90% were male. Smoking was commonly associated with COPD and infection was one of the common cause for exacerbations. Presence of hospital admission in 1 year, number of pack years smoked, less use of inhaled short acting B2 agonists, hypoprotenemia and hypoalbuminemia were factors associated with hospital readmission. Abstract

Background: Antiasthmatic medications such as β2 agonists and corticosteroids have shown potential side effects such as increased caries risk and oral candidiasis. Studies evaluating microbial changes in adult asthmatics are very scanty in the literature. The present study aimed to evaluate the effects of asthma and its medication on cariogenic bacteria and Candida albicans in adult asthmatics. Aim and Objectives: Our aim was to evaluate and compare counts of Streptococcus mutans (SM) and lactobacilli in plaque and C. albicans in saliva samples of adult asthmatics with controls and during the course of medication longitudinally. Methodology: Samples were collected from twenty recently diagnosed asthmatic adults and twenty controls for estimation of microbial counts at baseline and at 3rd and 6th month after initiation of medication among cases. Results: Asthmatics at baseline had higher microbial counts than controls, but the difference was not statistically significant. Comparison between asthmatics at baseline and 3rd month after initiation of medication showed an increase in counts of SM, lactobacilli and decreased C. albicans counts though the difference was not significant. Comparison between asthmatics at baseline and 6th month and also between 3rd and 6th month showed significantly increased counts of SM. Although there was an increase in counts of lactobacilli and decreased C. albicans counts, significant results were not noted. Asthmatics showed increased microbial counts than controls overall. Conclusion: Asthmatics were found to have higher microbial counts than controls at baseline. Increase in SM and lactobacilli counts in asthmatics after medication emphasizes the need to monitor these patients regularly. [ABSTRACT FROM AUTHOR]

Abstract

Abstract Rice tungro disease is caused by the combined action of Rice tungro bacilliform virus (RTBV) and Rice tungro spherical virus (RTSV). The RTBV is involved in the development of symptoms while RTSV is essential for virus transmission. We attempted to study the mode of action of RTBV in the development of symptoms. The tungro disease symptoms were attributed to viral interference in chlorophyll and carotenoids biosynthesis, photosynthesis machinery, iron/zinc homeostasis, and the genes encoding the enzymes associated with these biological processes of rice. The adverse effects of virus infection in photosystem II (PSII) activity was demonstrated by analyzing the Fv/Fm ratio, expression of psbA and cab1R genes, and direct interaction of RTBV ORF I protein with the D1 protein of rice. Since ORF I function is not yet known in the RTBV life cycle, this is the first report showing its involvement in regulating host photosynthesis process and symptoms development. 40. Structure-Activity Relationship of Truncated 4'-Selenonucleosides: A 3 Adenosine Receptor Activity and Binding Selectivity.

Abstract

This study explored the impact of structural modifications on truncated 4'-selenonucleosides as ligands for the A 3 adenosine receptor (AR). We synthesized and evaluated a series of these compounds for their binding affinities, functional activities, and structural interactions by using computational modeling. The SAR study revealed that all compounds exhibited selective and notable hA 3 AR binding, among which 6l ( K i = 5.2 nM) and 6m ( K i = 5.7 nM) were found as the best binding compounds. The representative N 6 -cyclopropyl compound 6m was found to be a partial agonist, contrasting with the antagonist profiles of truncated 4'-oxo and 4'-thionucleosides. Computational docking highlighted 6m 's unique interaction with Thr94 at the A 3 AR binding site. Abstract

Bone tumours arising from the coracoid process of the scapula are very rare. We present a case of a 19-year-old man with left shoulder pain for 1 year, with associated infraclavicular swelling for 8 months. Examination revealed a tender, bony, hard swelling measuring 3×4 cm. Imaging revealed an expansile osteolytic lesion arising from the coracoid process with loss of corticomedullary differentiation. MRI showed altered signal in the adjacent soft tissues. The patient underwent en bloc resection of the lesion. Histology was suggestive of osteoblastoma. At the end of 2 years follow-up, the patient had no pain or recurrence of the lesion. We present this case for its rare location and have briefly discussed the difficulties in diagnosis of this condition., (2015 BMJ Publishing Group Ltd.)

Abstract

A case of bronchioloalveolar carcinoma, with widespread dissemination to both lungs and miliary mottling on chest X-ray is reported in a 40 year old male.
