Background: The Alberta Stroke Program Early CT Score (ASPECTS) scale was developed for monitoring early ischemic changes on CT, being associated with clinical outcomes. The ASPECTS can also associate with peripheral biomarkers that reflect the pathophysiological response of the brain to the ischemic stroke. Objective: To investigate the association between peripheral biomarkers with the Alberta Stroke Program Early CT Score (ASPECTS) in individuals after ischemic stroke. Methods: Patients over 18 years old with acute ischemic stroke were enrolled in this study. No patient was eligible for thrombolysis. The patients were submitted to non-contrast CT in the first 24 hours of admission, being the Alberta Stroke Program Early CT Score and clinical and molecular evaluations applied on the same day. The National Institutes of Health Stroke Scale (NIHSS), modified Rankin scale and the Mini-Mental State Examination for clinical evaluation were also applied to all subjects. Plasma levels of BDNF, VCAM-1, VEGF, IL-1β, sTNFRs and adiponectin were determined by ELISA. Results: Worse neurological impairment (NIHSS), cognitive (MEEM) and functional (Rankin) performance was observed in the group with changes in the NCTT. Patients with NCTT changes also exhibited higher levels of IL-1β and adiponectin. In the linear multivariate regression, an adjusted R coefficient of 0.515 was found, indicating adiponectin and NIHSS as independent predictors of ASPECTS. Conclusion: Plasma levels of adiponectin are associated with the ASPECTS scores. RESUMO Introdução: A Alberta Stroke Early Score (ASPECTS) foi desenvolvida para monitorização de alterações isquêmicas precoces na tomografia computadorizada de crânio, estando associada a desfechos clínicos. A ASPECTS também pode se associar aos biomarcadores periféricos que refletem a resposta fisiopatológica do cérebro ao AVC isquêmico. Objetivo: Investigar à associação entre os parâmetros periféricos com a Alberta Stroke Early Score (ASPECTS) em indivíduos após acidente vascular cerebral isquêmico. Métodos: Pacientes acima de 18 anos com AVC isquêmico agudo foram incluídos neste estudo. Nenhum paciente foi elegível para trombólise. Os pacientes foram submetidos à tomografia computadorizada sem contraste nas primeiras 24 horas da admissão, a ASPECTS e as avaliações clínicas e moleculares aplicadas no mesmo dia. O National Institutes of Health Stroke Scale (NIHSS), a escala de Rankin modificada e o Mini Exame do Estado Mental para avaliação clínica também foram aplicados a todos os indivíduos. Os níveis plasmáticos de BDNF, VCAM-1, VEGF, IL-1β, sTNFRs e adiponectina foram determinados por ELISA. Resultados: Pior desempenho neurológico (NIHSS), cognitivo (MEEM) e funcional (Rankin) foram observados no grupo com alterações na ASPECTS. Pacientes com alterações na ASPECTS também exibiram níveis mais altos de IL-1β e adiponectina. Na regressão multivariada linear, foi encontrado um coeficiente R ajustado de 0,515, indicando adiponectina e NIHSS como preditores independentes para a ASPECTS. Conclusão: Os níveis plasmáticos de adiponectina estão associados aos escores da ASPECTS.

RESUMO Objetivo: analisar a evolução da deglutição de pacientes após acidente vascular cerebral. Métodos: trata-se de estudo exploratório descritivo com amostra não probabilística em que foram acompanhados 100 pacientes admitidos com o diagnóstico de acidente vascular cerebral no Hospital Público Regional de Betim. Os pacientes foram submetidos à avaliação fonoaudiológica estruturada em dois momentos: nas primeiras 48 horas após acidente vascular cerebral e no momento da alta hospitalar. Utilizou-se a escala Gugging Swallowing Screen que é um instrumento padronizado e validado para ser utilizado na beira do leito. Resultados: na avaliação fonoaudiológica inicial, a frequência da disfagia foi de 52%, sendo que 28% dos pacientes foram classificados como disfagia grave com alto risco de aspiração. A média de tempo entre a avaliação inicial da deglutição e a do momento da alta hospitalar foi de 22,1 dias. Na alta, apenas 2,1% dos pacientes ainda apresentavam disfagia grave. Observou-se mudança do perfil de deglutição do paciente de acordo com a gravidade da disfagia e da consistência da dieta oral. Conclusão: a frequência de disfagia após acidente vascular cerebral é alta, mas há progressiva mudança no perfil de deglutição do paciente durante o período de internação. ABSTRACT Purpose: to analyze the evolution of swallowing after acute stroke. Methods: this is a descriptive exploratory study with a non-probabilistic sample where 100 stroke patients were followed in the Regional Public Hospital of Betim. The patients were subjected to a structured speech evaluation in two different times: in the first 48 hours after stroke and at the time of hospital discharge. The Gugging Swallowing Screen scale was used. It is a standardized and valid tool to be used at the bedside. Results: in the initial speech evaluation, the frequency of dysphagia among patients was of 52%, being that 28% of them were diagnosed with severe dysphagia with high risk of aspiration. The average time between the initial swallowing assessment and the assessment at the hospital discharge was 22.1 days. At the hospital discharge, only 2.1% of patients still presented severe dysphagia. A change of the swallowing profile with the severity of dysphagia and texture of the oral feeding was observed. Conclusion: the frequency of post-stroke dysphagia is high, but there are progressive changes in the swallowing profile of the patients during their hospital stay.

OBJETIVOS: avaliar de forma perceptivo-auditiva as características vocais de idosos institucionalizados, identificar se essas características interferem no processo de comunicação e correlacioná-las com a avaliação das estruturas do sistema estomatognático e do padrão de fala. MÉTODOS: estudo clínico do tipo transversal, no qual foram realizadas anamneses e avaliações fonoaudiológicas em uma amostra aleatória de 48 indivíduos idosos, residentes na Casa do Ancião Francisco Azevedo - Belo Horizonte/MG, que não apresentavam nenhum tipo de alteração neurológica, uma vez que, buscou-se traçar as manifestações fonoaudiológicas de idosos em processo de envelhecimento sadio. Utilizou-se protocolos específicos, desenvolvidos pelas autoras, de acordo com os aspectos pertinentes aos objetivos do presente estudo. RESULTADOS: na avaliação perceptivo-auditiva da qualidade vocal, constatou-se predominantemente qualidade vocal rouca (70,8%), em grau moderado (33,3%), loudness reduzida (56,2%), pitch grave (62,5%) e tempos máximos de fonação reduzidos (81,2%). Dos 48 participantes, 85,4% relataram que a voz não interfere no processo de comunicação. Em relação aos padrões de fala, predominaram inteligibilidade preservada (83,3%), articulação preservada (72,9%) e precisão articulatória preservada (83,3%). CONCLUSÕES: existem alterações nos parâmetros referentes à voz decorrentes da idade, sendo que elas não interferem na comunicação e mantêm relação diversa com outras mudanças nas estruturas do sistema estomatognático. Este estudo veio complementar as pesquisas na área de voz envolvendo indivíduos da terceira idade, sob processo de envelhecimento sadio e residentes em instituições de longa permanência. PURPOSES: to investigate vocal aspects related to healthy aging in the institutionalized elderly people, and to identify if these aspects interfer with communication and correlate vocal changes with motor oral system evaluation and speech patterns. METHODS: transversal clinic research in which the methods used were interview and phonoaudiological examination of a randomized sample of 48 aged patients, resident in Casa do Ancião Francisco Azevedo - Belo Horizonte/MG, who did not show neurological disabilities. Specific protocols were used, developed by the authors, according to aspects related to the purposes of this study RESULTS: vocal parameters were analyzed and the following conditions were observed: hoarse voice (70,8%), in moderate degree (33,3%), reduced loudness (56,2%), bass pitch (62,5%) and short maximum phonation periods (81,2%). About 41 (85,4%) patients reported that vocal problems do not interfer with their communication process. Normal aging did not result directly in alterations in speech patterns, but changes in motor oral system due to health aging showed different associations with those patterns. CONCLUSIONS: this study showed the presence of alterations in the aspects related to voice but they do not interfer with the communication process. There were not alterations in speech patterns due to health aging but changes in motor oral system showed different association with those patterns. This study has contributed to improve knowledge about voice and elderly that suffer changes due to health aging and live in old people's homes.
