Abstract: We study a flaring activity of the HSP Mrk421 that was characterized from radio to very-high-energy (VHE; E $>0.1$TeV) gamma rays with MAGIC, Fermi-LAT, Swift, XMM-Newton and several optical and radio telescopes. These observations included, for the first time for a gamma-ray flare of a blazar, simultaneous X-ray polarization measurements with IXPE. We find substantial variability in both X-rays and VHE gamma rays throughout the campaign, with the highest VHE flux above 0.2 TeV occurring during the IXPE observing window, and exceeding twice the flux of the Crab Nebula. However, the VHE and X-ray spectra are on average softer, and the correlation between these two bands weaker that those reported in previous flares of Mrk421. IXPE reveals an X-ray polarization degree significantly higher than that at radio and optical frequencies. The X-ray polarization angle varies by $\sim$100$^\circ$ on timescales of days, and the polarization degree changes by more than a factor 4. The highest X-ray polarization degree reaches 26%, around which a X-ray counter-clockwise hysteresis loop is measured with XMM-Newton. It suggests that the X-ray emission comes from particles close to the high-energy cutoff, hence possibly probing an extreme case of the Turbulent Extreme Multi-Zone model. We model the broadband emission with a simplified stratified jet model throughout the flare. The polarization measurements imply an electron distribution in the X-ray emitting region with a very high minimum Lorentz factor, which is expected in electron-ion plasma, as well as a variation of the emitting region size up to a factor of three during the flaring activity. We find no correlation between the fluxes and the evolution of the model parameters, which indicates a stochastic nature of the underlying physical mechanism. Abstract: OT 081 is a well-known, luminous blazar that is remarkably variable in many energy bands. We present the first broadband study of the source which includes very-high-energy (VHE, $E>$100\,GeV) $\gamma$-ray data taken by the MAGIC and H.E.S.S. imaging Cherenkov telescopes. The discovery of VHE $\gamma$-ray emission happened during a high state of $\gamma$-ray activity in July 2016, observed by many instruments from radio to VHE $\gamma$-rays. We identify four states of activity of the source, one of which includes VHE $\gamma$-ray emission. Variability in the VHE domain is found on daily timescales. The intrinsic VHE spectrum can be described by a power-law with index $3.27\pm0.44_{\rm stat}\pm0.15_{\rm sys}$ (MAGIC) and $3.39\pm0.58_{\rm stat}\pm0.64_{\rm sys}$ (H.E.S.S.) in the energy range of 55--300\,GeV and 120--500\,GeV, respectively. The broadband emission cannot be sucessfully reproduced by a simple one-zone synchrotron self-Compton model. Instead, an additional external Compton component is required. We test a lepto-hadronic model that reproduces the dataset well and a proton-synchrotron dominated model that requires an extreme proton luminosity. Emission models that are able to successfully represent the data place the emitting region well outside of the Broad Line Region (BLR) to a location at which the radiative environment is dominated by the infrared thermal radiation field of the dusty torus. In the scenario described by this flaring activity, the source appears to be an FSRQ, in contrast with past categorizations. Abstract: Imaging atmospheric Cherenkov telescopes (IACTs) are used to observe very high-energy photons from the ground. Gamma rays are indirectly detected through the Cherenkov light emitted by the air showers they induce. The new generation of experiments, in particular the Cherenkov Telescope Array Observatory (CTAO), sets ambitious goals for discoveries of new gamma-ray sources and precise measurements of the already discovered ones. To achieve these goals, both hardware and data analysis must employ cutting-edge techniques. This also applies to the LST-1, the first IACT built for the CTAO, which is currently taking data on the Canary island of La Palma. This paper introduces a new event reconstruction technique for IACT data, aiming to improve the image reconstruction quality and the discrimination between the signal and the background from misidentified hadrons and electrons. The technique models the development of the extensive air shower signal, recorded as a waveform per pixel, seen by CTAO telescopes' cameras. Model parameters are subsequently passed to random forest regressors and classifiers to extract information on the primary particle. The new reconstruction was applied to simulated data and to data from observations of the Crab Nebula performed by the LST-1. The event reconstruction method presented here shows promising performance improvements. The angular and energy resolution, and the sensitivity, are improved by 10 to 20% over most of the energy range. At low energy, improvements reach up to 22%, 47%, and 50%, respectively. 4. Conceptual and practical approaches for investigating irreversible processes

Abstract: Current research in statistical mechanics mostly concerns the investigation of out-of-equilibrium, irreversible processes, which are ubiquitous in nature and still far from being theoretically understood. Even the precise characterization of irreversibility is the object of an open debate: while in the context of Hamiltonian systems the one-century-old proposal by M. Smoluchowski looks still valid (a process appears irreversible when the initial state has a recurrence time that is long compared to the time of observation [1]), in dissipative systems, particularly in the case of stochastic processes, the problem is more involved, and quantifying the "degree of irreversibility" is a pragmatic need. The most employed strategies rely on the estimation of entropy production: this quantity, although mathematically well-defined, is often difficult to compute, especially when analyzing experimental data. Moreover, being a global observable, entropy production fails to capture specific aspects of irreversibility in extended systems, such as the role of different currents and their spatial development. This review aims to address various conceptual and technical challenges encountered in the analysis of irreversibility, including the role of the coarse-graining procedure and the treatment of data in the absence of complete information. The discussion will be mostly based on simple models, analytically treatable, and supplemented by examples of complex, more realistic non-equilibrium systems., Comment: 79 pages, 19 figures

5. Candidate neutrino-emitting blazars sharing physical properties with TXS 0506+056

Abstract: Context. The interest in blazars as candidate neutrino emitters grew after the 3{\sigma} evidence for a joint photon-neutrino emission from the flaring blazar TXS 0506+056 in 2017. Blazars are a class of extragalactic sources with relativistic jets pointing toward Earth, showing a broadband emission explained in terms of leptonic and hadronic process, with the latter relevant for proton acceleration and neutrino production. Several models have been developed to explain the multi-messenger emission from TXS 0506+056, but the details of the neutrino production and the nature of the source are not yet fully understood. Aims. In this work we investigate sources with observational features as TXS 0506+056, aiming at a better understanding of their nature as flat spectrum radio quasars (FSRQs) or BL Lac Objects (BL Lacs) and their potential neutrino production. Methods. We select candidate neutrino-emitting blazars from the Fermi 4LAC-DR2 catalog by constraining a number of key parameters in specific ranges centered on the values measured for TXS 0506+056. We analyze the efficiency of the accretion mechanism and model their broadband spectral energy distribution in terms of lepto-hadronic emission, gaining information on their potential neutrino flux and detectability prospects at TeV energies. Results. Our study shows the selected sources to have a high energy emission dominated by leptonic processes, and part of them also shows a high accretion rate, features typical of FSRQs. Moreover, the very high energy and neutrino fluxes appear to be not detectable by the current and future instruments if the source is in an average emission state. Our results thus suggest some of the candidates to have an efficient accretion and could potentially be masquerading BL Lacs. Lastly, TXS 0506+056 seems to have a more uncertain situation for which no strong conclusions can be drawn on its nature.

6. Response of Worm-like Chains to Traveling-Wave Active Forces

Abstract: We present a simplified theory for semiflexible flagella under the action of a traveling-wave perturbation that emulates the organized active forces generated by molecular motors, capable of inducing beating patterns to the filament. By modeling the flagellum as a worm-like chain (WLC), we explore the interplay (competition) between the externally applied perturbation and the intrinsic bending rigidity of the filament. Our analysis aims to understand how this interplay can lead to a selection of conformations with the spatiotemporal behavior resembling the beating dynamics of axonemes such as those in sperm tails, in Chlamydomonas cilia, or eukaryotic flagella in general. Through a systematic analysis of the WLC's response to traveling-wave perturbations, we try to identify the key parameters that mostly influence the flagellar motion and shape its beating-like profiles., Comment: 14 pages, 9 figures

Abstract: Instruments for gamma-ray astronomy at Very High Energies ($E>100\,{\rm GeV}$) have traditionally derived their scientific results through proprietary data and software. Data standardisation has become a prominent issue in this field both as a requirement for the dissemination of data from the next generation of gamma-ray observatories and as an effective solution to realise public data legacies of current-generation instruments. Specifications for a standardised gamma-ray data format have been proposed as a community effort and have already been successfully adopted by several instruments. We present the first production of standardised data from the Major Atmospheric Gamma-ray Imaging Cherenkov (MAGIC) telescopes. We converted $166\,{\rm h}$ of observations from different sources and validated their analysis with the open-source software Gammapy. We consider six data sets representing different scientific and technical analysis cases and compare the results obtained analysing the standardised data with open-source software against those produced with the MAGIC proprietary data and software. Aiming at a systematic production of MAGIC data in this standardised format, we also present the implementation of a database-driven pipeline automatically performing the MAGIC data reduction from the calibrated down to the standardised data level. In all the cases selected for the validation, we obtain results compatible with the MAGIC proprietary software, both for the manual and for the automatic data productions. Part of the validation data set is also made publicly available, thus representing the first large public release of MAGIC data. Abstract: Context: There are currently three pulsars firmly detected by imaging atmospheric Cherenkov telescopes (IACTs), two of them reaching TeV energies, challenging models of very-high-energy (VHE) emission in pulsars. More precise observations are needed to better characterize pulsar emission at these energies. The LST-1 is the prototype of the Large-Sized Telescope, that will be part of the Cherenkov Telescope Array Observatory (CTAO). Its improved performance over previous IACTs makes it well suited for studying pulsars. Aims: To study the Crab pulsar emission with the LST-1, improving and complementing the results from other telescopes. These observations can also be used to characterize the potential of the LST-1 to study other pulsars and detect new ones. Methods: We analyzed a total of $\sim$103 hours of gamma-ray observations of the Crab pulsar conducted with the LST-1 in the period from September 2020 to January 2023. The observations were carried out at zenith angles less than 50 degrees. A new analysis of the Fermi-LAT data was also performed, including $\sim$14 years of observations. Results: The Crab pulsar phaseogram, long-term light-curve, and phase-resolved spectra are reconstructed with the LST-1 from 20 GeV to 450 GeV for P1 and up to 700 GeV for P2. The pulsed emission is detected with a significance of 15.2$\sigma$. The two characteristic emission peaks of the Crab pulsar are clearly detected (>10$\sigma$), as well as the so-called bridge emission (5.7$\sigma$). We find that both peaks are well described by power laws, with spectral indices of $\sim$3.44 and $\sim$3.03 respectively. The joint analysis of Fermi-LAT and LST-1 data shows a good agreement between both instruments in the overlapping energy range. Abstract: The Lorentz Invariance Violation (LIV), a proposed consequence of certain quantum gravity (QG) scenarios, could instigate an energy-dependent group velocity for ultra-relativistic particles. This energy dependence, although suppressed by the massive QG energy scale $E_\mathrm{QG}$, expected to be on the level of the Planck energy $1.22 \times 10^{19}$ GeV, is potentially detectable in astrophysical observations. In this scenario, the cosmological distances traversed by photons act as an amplifier for this effect. By leveraging the observation of a remarkable flare from the blazar Mrk\,421, recorded at energies above 100 GeV by the MAGIC telescopes on the night of April 25 to 26, 2014, we look for time delays scaling linearly and quadratically with the photon energies. Using for the first time in LIV studies a binned-likelihood approach we set constraints on the QG energy scale. For the linear scenario, we set $95\%$ lower limits $E_\mathrm{QG}>2.7\times10^{17}$ GeV for the subluminal case and $E_\mathrm{QG}> 3.6 \times10^{17}$ GeV for the superluminal case. For the quadratic scenario, the $95\%$ lower limits for the subluminal and superluminal cases are $E_\mathrm{QG}>2.6 \times10^{10}$ GeV and $E_\mathrm{QG}>2.5\times10^{10}$ GeV, respectively.

Abstract: This short paper represents a systematic literature review that sets the basis for the future development of a framework for digital twin-based decision support in the public sector, specifically for the smart city domain. The final aim of the research is to model context-specific digital twins for aiding the decision-making processes in smart cities and devise methods for defining the policy agenda. Overall, this short paper provides a foundation, based on the main concepts from existing literature, for further research in the role and applications of urban digital twins to assist decision- and policy-making in smart cities. The existing literature analyses common applications of digital twins in smart city development with a focus on supporting decision- and policy-making. Future work will centre on developing a digital-twin-based sustainable smart city and defining different scenarios concerning challenges of good governance, especially so-called wicked problems, in smaller-scale urban and non-urban contexts., Comment: 12 pages

Abstract: PG 1553+113 is one of the few blazars with a convincing quasi-periodic emission in the gamma-ray band. The source is also a very high-energy (VHE; >100 GeV) gamma-ray emitter. To better understand its properties and identify the underlying physical processes driving its variability, the MAGIC Collaboration initiated a multiyear, multiwavelength monitoring campaign in 2015 involving the OVRO 40-m and Medicina radio telescopes, REM, KVA, and the MAGIC telescopes, Swift and Fermi satellites, and the WEBT network. The analysis presented in this paper uses data until 2017 and focuses on the characterization of the variability. The gamma-ray data show a (hint of a) periodic signal compatible with literature, but the X-ray and VHE gamma-ray data do not show statistical evidence for a periodic signal. In other bands, the data are compatible with the gamma-ray period, but with a relatively high p-value. The complex connection between the low and high-energy emission and the non-monochromatic modulation and changes in flux suggests that a simple one-zone model is unable to explain all the variability. Abstract: In recent years, a new generation of optical intensity interferometers has emerged, leveraging the existing infrastructure of Imaging Atmospheric Cherenkov Telescopes (IACTs). The MAGIC telescopes host the MAGIC-SII system (Stellar Intensity Interferometer), implemented to investigate the feasibility and potential of this technique on IACTs. After the first successful measurements in 2019, the system was upgraded and now features a real-time, dead-time-free, 4-channel, GPU-based correlator. These hardware modifications allow seamless transitions between MAGIC's standard very-high-energy gamma-ray observations and optical interferometry measurements within seconds. We establish the feasibility and potential of employing IACTs as competitive optical Intensity Interferometers with minimal hardware adjustments. The measurement of a total of 22 stellar diameters are reported, 9 corresponding to reference stars with previous comparable measurements, and 13 with no prior measurements. A prospective implementation involving telescopes from the forthcoming Cherenkov Telescope Array Observatory's northern hemisphere array, such as the first prototype of its Large-Sized Telescopes, LST-1, is technically viable. This integration would significantly enhance the sensitivity of the current system and broaden the UV-plane coverage. Abstract: Axion-like particles (ALPs) are pseudo-Nambu-Goldstone bosons that emerge in various theories beyond the standard model. These particles can interact with high-energy photons in external magnetic fields, influencing the observed gamma-ray spectrum. This study analyzes 41.3 hrs of observational data from the Perseus Galaxy Cluster collected with the MAGIC telescopes. We focused on the spectra the radio galaxy in the center of the cluster: NGC 1275. By modeling the magnetic field surrounding this target, we searched for spectral indications of ALP presence. Despite finding no statistical evidence of ALP signatures, we were able to exclude ALP models in the sub-micro electronvolt range. Our analysis improved upon previous work by calculating the full likelihood and statistical coverage for all considered models across the parameter space. Consequently, we achieved the most stringent limits to date for ALP masses around 50 neV, with cross sections down to $g_{a\gamma} = 3 \times 10^{-12}$ GeV$^{-1}$., Comment: 25 pages, 10 figures, accepted for publication in Physics of the Dark Universe

Abstract: We perform the first broadband study of Mrk421 from radio to TeV gamma rays with simultaneous measurements of the X-ray polarization from IXPE. The data were collected within an extensive multiwavelength campaign organized between May and June 2022 using MAGIC, Fermi-LAT, NuSTAR, XMM-Newton, Swift, and several optical and radio telescopes to complement IXPE. During the IXPE exposures, the measured 0.2-1 TeV flux is close to the quiescent state and ranges from 25% to 50% of the Crab Nebula without intra-night variability. Throughout the campaign, the VHE and X-ray emission are positively correlated at a $4\sigma$ significance level. The IXPE measurements unveil a X-ray polarization degree that is a factor of 2-5 higher than in the optical/radio bands; that implies an energy-stratified jet in which the VHE photons are emitted co-spatially with the X-rays, in the vicinity of a shock front. The June 2022 observations exhibit a rotation of the X-ray polarization angle. Despite no simultaneous VHE coverage being available during a large fraction of the swing, the Swift-XRT monitoring unveils an X-ray flux increase with a clear spectral hardening. It suggests that flares in high synchrotron peaked blazars can be accompanied by a polarization angle rotation, as observed in some flat spectrum radio quasars. Finally, during the polarization angle rotation, NuSTAR data reveal two contiguous spectral hysteresis loops in opposite directions (clockwise and counter-clockwise), implying important changes in the particle acceleration efficiency on $\sim$hour timescales., Comment: Accepted for publication in Astronomy & Astrophysics. 29 pages, 22 figures. Abstract: Generative artificial intelligence has the potential to both exacerbate and ameliorate existing socioeconomic inequalities. In this article, we provide a state-of-the-art interdisciplinary overview of the potential impacts of generative AI on (mis)information and three information-intensive domains: work, education, and healthcare. Our goal is to highlight how generative AI could worsen existing inequalities while illuminating how AI may help mitigate pervasive social problems. In the information domain, generative AI can democratize content creation and access, but may dramatically expand the production and proliferation of misinformation. In the workplace, it can boost productivity and create new jobs, but the benefits will likely be distributed unevenly. In education, it offers personalized learning, but may widen the digital divide. In healthcare, it might improve diagnostics and accessibility, but could deepen pre-existing inequalities. In each section we cover a specific topic, evaluate existing research, identify critical gaps, and recommend research directions, including explicit trade-offs that complicate the derivation of a priori hypotheses. We conclude with a section highlighting the role of policymaking to maximize generative AI's potential to reduce inequalities while mitigating its harmful effects. We discuss strengths and weaknesses of existing policy frameworks in the European Union, the United States, and the United Kingdom, observing that each fails to fully confront the socioeconomic challenges we have identified. We propose several concrete policies that could promote shared prosperity through the advancement of generative AI. 21. Universality properties of forcing

Abstract: The purpose of this paper is to investigate forcing as a tool to construct universal models. In particular, we look at theories of initial segments of the universe and show that any model of a sufficiently rich fragment of those theories can be embedded into a model constructed by forcing. Our results rely on the model-theoretic properties of good ultrafilters, for which we provide a new existence proof on non-necessarily complete Boolean algebras.

Abstract: Gamma-ray bursts (GRBs) are explosive transient events occurring at cosmological distances, releasing a large amount of energy as electromagnetic radiation over several energy bands. We report the detection of the long GRB~201216C by the MAGIC telescopes. The source is located at $z=1.1$ and thus it is the farthest one detected at very high energies. The emission above \SI{70}{\GeV} of GRB~201216C is modelled together with multi-wavelength data within a synchrotron and synchrotron-self Compton (SSC) scenario. We find that SSC can explain the broadband data well from the optical to the very-high-energy band. For the late-time radio data, a different component is needed to account for the observed emission. Differently from previous GRBs detected in the very-high-energy range, the model for GRB~201216C strongly favors a wind-like medium. The model parameters have values similar to those found in past studies of the afterglows of GRBs detected up to GeV energies., Comment: 13 pages, 6 figures, 2 tables. Abstract: The BL Lac 1ES 2344+514 is known for temporary extreme properties (e.g., a shift of the synchrotron SED peak energy $\nu_{synch,p}$ above 1keV). While those extreme states were so far observed only during high flux levels, additional multi-year observing campaigns are required to achieve a coherent picture. Here, we report the longest investigation of the source from radio to VHE performed so far, focusing on a systematic characterisation of the intermittent extreme states. While our results confirm that 1ES 2344+514 typically exhibits $\nu_{synch,p}>$1keV during elevated flux periods, we also find periods where the extreme state coincides with low flux activity. A strong spectral variability thus happens in the quiescent state, and is likely caused by an increase of the electron acceleration efficiency without a change in the electron injection luminosity. We also report a strong X-ray flare (among the brightest for 1ES 2344+514) without a significant shift of $\nu_{synch,p}$. During this particular flare, the X-ray spectrum is among the softest of the campaign. It unveils complexity in the spectral evolution, where the common harder-when-brighter trend observed in BL Lacs is violated. During a low and hard X-ray state, we find an excess of the UV flux with respect to an extrapolation of the X-ray spectrum to lower energies. This UV excess implies that at least two regions contribute significantly to the infrared/optical/ultraviolet/X-ray emission. Using the simultaneous MAGIC, XMM-Newton, NuSTAR, and AstroSat observations, we argue that a region possibly associated with the 10 GHz radio core may explain such an excess. Finally, we investigate a VHE flare, showing an absence of simultaneous variability in the 0.3-2keV band. Using a time-dependent leptonic modelling, we show that this behaviour, in contradiction to single-zone scenarios, can instead be explained by a two-component model., Comment: Accepted for publication in Astronomy & Astrophysics

Abstract: Aims. LST-1, the prototype of the Large-Sized Telescope for the upcoming Cherenkov Telescope Array Observatory, is concluding its commissioning in Observatorio del Roque de los Muchachos on the island of La Palma. The proximity of LST-1 (Large-Sized Telescope 1) to the two MAGIC (Major Atmospheric Gamma Imaging Cherenkov) telescopes permits observations of the same gamma-ray events with both systems. Methods. We describe the joint LST-1+MAGIC analysis pipeline and use simultaneous Crab Nebula observations and Monte Carlo simulations to assess the performance of the three-telescope system. The addition of the LST-1 telescope allows the recovery of events in which one of the MAGIC images is too dim to survive analysis quality cuts. Results. Thanks to the resulting increase in the collection area and stronger background rejection, we find a significant improvement in sensitivity, allowing the detection of 30% weaker fluxes in the energy range between 200 GeV and 3 TeV. The spectrum of the Crab Nebula, reconstructed in the energy range ~60 GeV to ~10 TeV, is in agreement with previous measurements., Comment: Accepted for publication in Astronomy & Astrophysics

25. Joint searches by FACT, H.E.S.S., MAGIC and VERITAS for VHE gamma-ray emission associated with neutrinos detected by IceCube

Abstract: The sources of the astrophysical flux of high-energy neutrinos detected by IceCube are still largely unknown, but searches for temporal and spatial correlation between neutrinos and electromagnetic radiation are a promising approach in this endeavor. All major imaging atmospheric Cherenkov telescopes (IACTs) - FACT, H.E.S.S., MAGIC, and VERITAS - operate an active follow-up program of target-of-opportunity observations of neutrino alerts issued by IceCube. These programs use several complementary neutrino alert streams. A publicly distributed alert stream is formed by individual high-energy neutrino candidate events of potentially astrophysical origin, such as IceCube-170922A (which could be linked to the flaring blazar TXS\,0506+056). A privately distributed alert stream is formed by clusters of neutrino events in time and space around either pre-selected gamma-ray sources or anywhere in the sky. Here, we present joint searches for multi-wavelength emission associated with a set of IceCube alerts, both private and public, received through mid-January 2021. We will give an overview of the programs of the participating IACTs. We will showcase the various follow-up and data analysis strategies employed in response to the different alert types and various possible counterpart scenarios. Abstract: In a 3-arm presurgical trial, four-six weeks exemestane 25 mg three times/week (TIW) was non-inferior to 25 mg/day (QD) in suppressing circulating estradiol in postmenopausal women with ER-positive breast cancer. Since obesity may decrease exemestane efficacy, we analyzed changes in sex steroids, adipokines, Ki-67, and drug levels in relation to obesity. Postmenopausal women with early-stage ER-positive breast cancer were randomized to either exemestane 25 mg QD (n = 57), 25 mg TIW (n = 57), or 25 mg/week (QW, n = 62) for 4-6 weeks before breast surgery. Serum and tissue pre- and post-treatment biomarkers were stratified by body mass index (BMI)< or ≥30 kg/m2. Post-treatment median exemestane and 17-OH exemestane levels were 5-6 times higher in the QD arm compared to the TIW arm. For obese women, TIW maintained comparable reductions to QD in systemic estradiol levels, although the reduction in estrone was less with the TIW regimen. There was less suppression of SHBG with the TIW versus the QD dose schedule in obese women which should result in less systemic bioavailable estrogens. Metabolically, the effect of the TIW regimen was similar to the QD regimen for obese women in terms of leptin suppression and increase in the adiponectin-leptin ratio. Reduction in tissue Ki-67 was less for obese women on the TIW regimen than QD, although changes were similar for non-obese women. Our findings suggest that TIW exemestane should be explored further for primary cancer prevention in both normal weight and obese cohorts.

Abstract: Abstract Small, hormone receptor-positive (HR+), HER2-negative (HER2-), lymph node-negative breast cancers are associated with relatively low rates of disease recurrence and have therefore been underrepresented in clinical trials assessing the effects of systemic therapy. Consequently, it remains uncertain if this patient population derives benefit from these treatments. For this exploratory analysis, we selected MINDACT (NCT00433589) patients with a HR+, HER2-, T1ab (≤1 cm) tumor and negative lymph nodes. Patients with discordant clinical risk and MammaPrint genomic risk classification were randmomized to receive chemotherapy based on either the clinical or the genomic risk assessment. Endocrine therapy treatment was based on local guidelines. 715/6693 (10.7%) MINDACT patients had HR+, HER2-, T1abN0 breast cancer and were included in this analysis. All were clinically low-risk, 124/715 (17.3%) were genomic high-risk. For genomic high-risk tumors, 8-year distant metastasis-free survival (DMFS) was 92.9% (95% CI 86.2–96.4%) compared to 95.0% (95% CI 92.8–96.6%) for genomic low-risk tumors. For genomic high-risk tumors treated with or without chemotherapy, 8-year DMFS was 89.2% (95% CI 73.6–95.8%) and 94.1% (95% CI 82.9–98.1%), respectively. For genomic low-risk tumors, the 8-year DMFS and disease-free survival (DFS) were 96.1% (95% CI 93.4–97.6%) and 89.3% (95% CI 85.5–92.2%) when treated with endocrine therapy and 92.9% (95% CI 87.9–95.9%) and 79.4% (95% CI 72.5–84.8%) without. In conclusion, although the number of randomized patients is small, patients with small, genomic high-risk breast cancer did not seem to derive benefit from chemotherapy. Endocrine therapy was associated with improved outcomes even in genomic low-risk breast cancers.

Abstract: Abstract The prevalence of malignant cells in clinical specimens, or tumour purity, is affected by both intrinsic biological factors and extrinsic sampling bias. Molecular characterization of large clinical cohorts is typically performed on bulk samples; data analysis and interpretation can be biased by tumour purity variability. Transcription-based strategies to estimate tumour purity have been proposed, but no breast cancer specific method is available yet. We interrogated over 6000 expression profiles from 10 breast cancer datasets to develop and validate a 9-gene Breast Cancer Purity Score (BCPS). BCPS outperformed existing methods for estimating tumour content. Adjusting transcriptomic profiles using the BCPS reduces sampling bias and aids data interpretation. BCPS-estimated tumour purity improved prognostication in luminal breast cancer, correlated with pathologic complete response in on-treatment biopsies from triple-negative breast cancer patients undergoing neoadjuvant treatment and effectively stratified the risk of relapse in HER2+ residual disease post-neoadjuvant treatment.

Abstract: Nowadays many tools, e.g. fluctuation relations, are available to characterize the statistical properties of non-equilibrium systems. However, most of these tools rely on the assumption that the driving noise is normally distributed. Here we consider a class of Markov processes described by Langevin equations driven by a mixture of Gaussian and Poissonian noises, focusing on their non-equilibrium properties. In particular, we prove that detailed balance does not hold even when correlation functions are symmetric under time reversal. In such cases, a breakdown of the time reversal symmetry can be highlighted by considering higher order correlation functions. Furthermore, the entropy production may be different from zero even for vanishing currents. We provide analytical expressions for the average entropy production rate in several cases. We also introduce a scale dependent estimate for entropy production, suitable for inference from experimental signals. The empirical entropy production allows us to discuss the role of spatial and temporal resolutions in characterizing non-equilibrium features. Finally, we revisit the Brownian gyrator introducing an additional Poissonian noise showing that it behaves as a two dimensional linear ratchet. It has also the property that when Onsager relations are satisfied its entropy production is positive although it is minimal. Abstract: CTA (Cherenkov Telescope Array) is the next generation ground-based observatory for gamma-ray astronomy at very-high energies. The Large-Sized Telescope prototype (LST-1) is located at the Northern site of CTA, on the Canary Island of La Palma. LSTs are designed to provide optimal performance in the lowest part of the energy range covered by CTA, down to $\simeq 20$ GeV. LST-1 started performing astronomical observations in November 2019, during its commissioning phase, and it has been taking data since then. We present the first LST-1 observations of the Crab Nebula, the standard candle of very-high energy gamma-ray astronomy, and use them, together with simulations, to assess the basic performance parameters of the telescope. The data sample consists of around 36 hours of observations at low zenith angles collected between November 2020 and March 2022. LST-1 has reached the expected performance during its commissioning period - only a minor adjustment of the preexisting simulations was needed to match the telescope behavior. The energy threshold at trigger level is estimated to be around 20 GeV, rising to $\simeq 30$ GeV after data analysis. Performance parameters depend strongly on energy, and on the strength of the gamma-ray selection cuts in the analysis: angular resolution ranges from 0.12 to 0.40 degrees, and energy resolution from 15 to 50%. Flux sensitivity is around 1.1% of the Crab Nebula flux above 250 GeV for a 50-h observation (12% for 30 minutes). The spectral energy distribution (in the 0.03 - 30 TeV range) and the light curve obtained for the Crab Nebula agree with previous measurements, considering statistical and systematic uncertainties. Abstract: Background. The large number of lesions detected via high-definition (HD) imaging during colonoscopy calls for the reliable real-time histological characterization of polyps, especially diminutive and small ones, to permit tailored management based on the neoplastic risk, such as a “resect-and-discard” or a “diagnose-and-leave” strategy for low-risk adenomas and hyperplastic polyps (HPs). The Kudo classification of glandular pit pattern is currently used for predicting polyp histology. Aim. The aim in this study was to assess whether Kudo’s glandular pit pattern, assessed via HD digital chromoendoscopy (i-Scan) without magnification and optical enhancement, reliably predicts polyp histology and differentiates neoplastic lesions (NLs) from non-neoplastic lesions (non-NLs) during routine colonoscopy. Methods. Consecutive colorectal lesions recorded in a database over 12 months, with Kudo’s glandular pit pattern classification, were retrospectively compared with histology. The diagnostic accuracy and negative predictive value (NPV) for adenomatous histology of Kudo’s pit patterns were assessed separately for diminutive (≤5 mm) and small (6–9 mm) polyps, accordingly to the American Society for Gastrointestinal Endoscopy (ASGE) Preservation and Incorporation of Valuable Endoscopic Innovations (PIVI), and in large (≥10 mm) lesions. Results. A total of 2230 lesions were recorded: 898 diminutive, 704 small, and 628 large. Kudo’s type II pit pattern was prevalent in diminutive polyps and recognized mostly in HPs (83.27%); it was also found in 38.8% of adenomas. In the right colon, Kudo’s type II pit pattern was prevalent in adenomas (70.04% vs. 20.74% in HPs); among the serrated lesions, it was evenly distributed between HPs and adenomas. Kudo’s type IIIL/IIIs/IV pit pattern was prevalent in NLs (61% vs. 8.37% of non-NLs) in diminutive polyps, evenly distributed between non-NLs and NLs in small polyps, and found only in NLs in large polyps. Kudo’s type Vi/Vn pit pattern correctly identified all but one adenocarcinoma. The NPV for adenomatous histology did not reach the recommended 90% PIVI threshold for differentiation between NLs and non-NLs in diminutive polyps showing Kudo’s type II pit pattern and in small polyps showing type IIIL/IIIs/IV pit pattern. Conclusions. Kudo’s pit pattern classification carried out with digital chromoendoscopy (i-Scan) during routine colonoscopy does not allow the reliable differentiation between non-NLs and NLs in diminutive and small polyps, so a “diagnose-and-leave” strategy for diminutive polyps may leave undetected adenomas, while a “resect-and-discard” strategy could miss lesions requiring closer follow-up.

Abstract: Abstract Intra-abdominal infections (IAIs) are common surgical emergencies and are an important cause of morbidity and mortality in hospital settings, particularly if poorly managed. The cornerstones of effective IAIs management include early diagnosis, adequate source control, appropriate antimicrobial therapy, and early physiologic stabilization using intravenous fluids and vasopressor agents in critically ill patients. Adequate empiric antimicrobial therapy in patients with IAIs is of paramount importance because inappropriate antimicrobial therapy is associated with poor outcomes. Optimizing antimicrobial prescriptions improves treatment effectiveness, increases patients’ safety, and minimizes the risk of opportunistic infections (such as Clostridioides difficile) and antimicrobial resistance selection. The growing emergence of multi-drug resistant organisms has caused an impending crisis with alarming implications, especially regarding Gram-negative bacteria. The Multidisciplinary and Intersociety Italian Council for the Optimization of Antimicrobial Use promoted a consensus conference on the antimicrobial management of IAIs, including emergency medicine specialists, radiologists, surgeons, intensivists, infectious disease specialists, clinical pharmacologists, hospital pharmacists, microbiologists and public health specialists. Relevant clinical questions were constructed by the Organizational Committee in order to investigate the topic. The expert panel produced recommendation statements based on the best scientific evidence from PubMed and EMBASE Library and experts’ opinions. The statements were planned and graded according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE) hierarchy of evidence. On November 10, 2023, the experts met in Mestre (Italy) to debate the statements. After the approval of the statements, the expert panel met via email and virtual meetings to prepare and revise the definitive document. This document represents the executive summary of the consensus conference and comprises three sections. The first section focuses on the general principles of diagnosis and treatment of IAIs. The second section provides twenty-three evidence-based recommendations for the antimicrobial therapy of IAIs. The third section presents eight clinical diagnostic-therapeutic pathways for the most common IAIs. The document has been endorsed by the Italian Society of Surgery.

Abstract: Appropriate antibiotic treatment for critically ill patients with serious Gram-negative infections in the intensive care unit is crucial to minimize morbidity and mortality. Several new antibiotics have shown in vitro activity against carbapenem-resistant Enterobacterales (CRE) and difficult-to-treat resistant Pseudomonas aeruginosa. Cefiderocol is the first approved siderophore beta-lactam antibiotic with potent activity against multidrug-resistant, carbapenem-resistant, difficult-to-treat or extensively drug-resistant Gram-negative pathogens, which have limited treatment options. The spectrum of activity of cefiderocol includes drug-resistant strains of Acinetobacter baumannii, P. aeruginosa, Stenotrophomonas maltophilia, Achromobacter spp. and Burkholderia spp. and CRE that produce serine- and/or metallo-carbapenemases. Phase 1 studies established that cefiderocol achieves adequate concentration in the epithelial lining fluid in the lung and requires dosing adjustment for renal function, including patients with augmented renal clearance and continuous renal-replacement therapy (CRRT); no clinically significant drug-drug interactions are expected. The non-inferiority of cefiderocol versus high-dose, extended-infusion meropenem in all-cause mortality (ACM) rates at day 14 was demonstrated in the randomized, double-blind APEKS-NP Phase 3 clinical study in patients with nosocomial pneumonia caused by suspected or confirmed Gram-negative bacteria. Furthermore, the efficacy of cefiderocol was investigated in the randomized, open-label, pathogen-focused, descriptive CREDIBLE-CR Phase 3 clinical study in its target patient population with serious carbapenem-resistant Gram-negative infections, including hospitalized patients with nosocomial pneumonia, bloodstream infection/sepsis, or complicated urinary tract infections. However, a numerically greater ACM rate with cefiderocol compared with BAT led to the inclusion of a warning in US and European prescribing information. Cefiderocol susceptibility results obtained with commercial tests should be carefully evaluated due to current issues regarding their accuracy and reliability. Since its approval, real-world evidence in patients with multidrug-resistant and carbapenem-resistant Gram-negative bacterial infections suggests that cefiderocol can be efficacious in certain critically ill patient groups, such as those requiring mechanical ventilation for COVID-19 pneumonia with subsequently acquired Gram-negative bacterial superinfection, and patients with CRRT and/or extracorporeal membrane oxygenation. In this article, we review the microbiological spectrum, pharmacokinetics/pharmacodynamics, efficacy and safety profiles and real-world evidence for cefiderocol, and look at future considerations for its role in the treatment of critically ill patients with challenging Gram-negative bacterial infections.

Abstract: The characterization of the distance from equilibrium is a debated problem in particular in the treatment of experimental signals. If the signal is a 1-dimensional time-series, such a goal becomes challenging. A paradigmatic example is the angular diffusion of a rotator immersed in a vibro-fluidized granular gas. Here, we experimentally observe that the rotator's angular velocity exhibits significative differences with respect to an equilibrium process. Exploiting the presence of two relevant time-scales and non-Gaussian velocity increments, we quantify the breakdown of time-reversal asymmetry, which would vanish in the case of a 1D Gaussian process. We deduce a new model for the massive probe, with two linearly coupled variables, incorporating both Gaussian and Poissonian noise, the latter motivated by the rarefied collisions with the granular bath particles. Our model reproduces the experiment in a range of densities, from dilute to moderately dense, with a meaningful dependence of the parameters on the density. Abstract: Line-like features in TeV $\gamma$-rays constitute a ''smoking gun'' for TeV-scale particle dark matter and new physics. Probing the Galactic Center region with ground-based Cherenkov telescopes enables the search for TeV spectral features in immediate association with a dense dark matter reservoir at a sensitivity out of reach for satellite $\gamma$-ray detectors, and direct detection and collider experiments. We report on 223 hours of observations of the Galactic Center region with the MAGIC stereoscopic telescope system reaching $\gamma$-ray energies up to 100 TeV. We improved the sensitivity to spectral lines at high energies using large-zenith-angle observations and a novel background modeling method within a maximum-likelihood analysis in the energy domain. No line-like spectral feature is found in our analysis. Therefore, we constrain the cross section for dark matter annihilation into two photons to $\langle \sigma v \rangle \lesssim 5 \times 10^{-28}\,\mathrm{cm^3\,s^{-1}}$ at 1 TeV and $\langle \sigma v \rangle \lesssim 1 \times 10^{-25}\,\mathrm{cm^3\,s^{-1}}$ at 100 TeV, achieving the best limits to date for a dark matter mass above 20 TeV and a cuspy dark matter profile at the Galactic Center. 49. Is CMV DNAemia an early marker of CMV colitis in patients with active ulcerative colitis?

Abstract: ABSTRACT Cytomegalovirus (CMV) colitis is a serious concern worsening the prognosis of patients with ulcerative colitis (UC). We aimed to assess risk factors and prognostic impact of CMV colitis in patients with moderate-to-severe UC flare. We conducted a retrospective, observational, single-center study. Consecutive adult patients hospitalized for moderate-to-severe UC from January 2020 to June 2023 were included. The primary endpoint was a diagnosis of CMV-colitis according to immunohistochemistry on tissue biopsies. The secondary endpoint was the need for colectomy within 30 days. Overall, 135 patients were included. CMV colitis was diagnosed in n = 37 (27.4%): n = 19 (51.4%) endoscopically, the remaining on surgical specimens. Of them, n = 23 (62.2%) had positive CMV-DNAemia with a median value of 1,008 cp/mL (interquartile range 318–2,980). Differences between the two groups (CMV colitis vs non-CMV) included age (60 vs 41 years, P = 0.004), Charlson Comorbidity Index (1 vs 0, P = 0.003), steroid refractoriness (86.5% vs 62.2%, P = 0.007), and positive CMV-DNAemia (62.2% vs 10.1%, P < 0.001). At multivariable analysis, steroid-refractory disease, Charlson Comorbidity Index, and CMV-DNAemia were associated with CMV colitis. Overall, n = 54 (39.7%) patients underwent colectomy, and this was significantly more common in patients with CMV colitis vs non-CMV group (54.1% vs 34.4%, P = 0.049). Kaplan-Meier showed that antiviral therapy seems to have a relevant impact on colectomy (P < 0.001). CMV-DNA blood detection is independently associated with CMV-positive refractory UC. Since CMV colitis may increase the risk of colectomy and antiviral treatment seems to reduce such risk, prospective studies are needed to confirm the role of CMV-DNA blood detection to early diagnose CMV colitis.IMPORTANCECytomegalovirus (CMV) colonic reactivation worsens the prognosis of patients with active ulcerative colitis. Blood CMV-DNA reactivation is strongly associated with CMV colitis. Prompt diagnosis and treatment of CMV colitis can avoid surgery in most cases.
