4. Overcoming platinum-acquired resistance in ovarian cancer patient-derived xenografts

Abstract

Background: Epithelial ovarian cancer is the most lethal gynecological cancer and the high mortality is due to the frequent presentation at advanced stage, and to primary or acquired resistance to platinum-based therapy. Methods: We developed three new models of ovarian cancer patient-derived xenografts (ovarian PDXs) resistant to cisplatin (cDDP) after multiple in vivo drug treatments. By different and complementary approaches based on integrated metabolomics (both targeted and untargeted mass spectrometry-based techniques), gene expression, and functional assays (Seahorse technology) we analyzed and compared the tumor metabolic profile in each sensitive and their corresponding cDDP-resistant PDXs. Results: We found that cDDP-sensitive and -resistant PDXs have a different metabolic asset. In particular, we found, through metabolomic and gene expression approaches, that glycolysis, tricarboxylic acid cycle and urea cycle pathways were deregulated in resistant versus sensitive PDXs. In addition, we observed that oxygen consumption rate and mitochondrial respiration were higher in resistant PDXs than in sensitive PDXs under acute stress conditions. An increased oxidative phosphorylation in cDDP-resistant sublines led us to hypothesize that its interference could be of therapeutic value. Indeed, in vivo treatment of metformin and cDDP was able to partially reverse platinum resistance. Conclusions: Our data strongly reinforce the idea that the development of acquired cDDP resistance in ovarian cancer can bring about a rewiring of tumor metabolism, and that this might be exploited therapeutically.

Abstract

BACKGROUND: There is currently no predictive biomarker for anti-angiogenic drugs which could be used for patient selection purposes. Pre-clinical studies show that anti-angiogenic therapy exacerbates tumor hypoxia and activatesliver kinase B1 (LKB1)/AMP kinase (AMPK), a pathway involved in the regulation of tumor metabolism. Inspired by these findings, we investigated whether certain angiogenesis- and metabolism-related in situ biomarkers could predict response to regorafenib in the context of the phase 2 randomized REGOMA trial. MATERIAL AND METHODS: Immunohistochemistry was used to investigate expression in tumor sections of vascular endothelial growth factor receptor 2 (VEGFR2), platelet-derived growth factor β (PDGFRβ), fibroblast growth factor receptor (FGFR), the angiopoietin receptor (Tie2),monocarboxylate transporter 1 and 4 (MCT1, MCT4), phosphorylated AMPK (pAMPK) and phosphorylated acetyl-CoA carboxylase(pACC), which is a canonical target of AMPK activity.Positivity for the markers was independently assessed by two analists blinded to clinical data and results of the REGOMA trial. The status of each biomarker was associated with clinical endpoints, including overall survival (OS) and progression free survival (PFS) in patients (PTS) with relapsed GBM treated either with regorafenib or lomustine. Radiological assessment was performed by RANO criteria. RESULTS: Between November 2015 and February 2017, 119 PTS were enrolled ﴾n=59 REG; n=60 LOM﴿ and stratified for surgery at recurrence; baseline characteristics were balanced. Biomarker analysis was performed in 85 PTS (71%), including 43 PTS of the REG arm and 42 PTS of the LOM arm. Among all markers analyzed, only pACC showed predictive value. In fact, median OS was 8.0 months (m) (95% CI 5.6-) for REG and 5.3m (95% CI 4.2–6.6) for LOM for pACC positive patients. HR 0.44 (95% CI 0.22–0.87), log rank p-value = 0.012; Test for interaction = 0.0419. CONCLUSION: In this randomized, controlled study we report for the first time a predictive impact of pACC expression in patients with relapsed GBM treated with regorafenib. These results suggest that integrity of the AMPK pathway is associated with clinical benefit from treatment with regorafenib.

Abstract

OBJECTIVE— Stresshyperglycemiahasbeenassociatedwithincreasedmortalityinpatients with myocardial infarction (MI). We examined the association between plasma glucose levels, circulating inflammatory markers, T-cell activation, and functional cardiac outcome in patients with first MI. RESEARCH DESIGN AND METHODS — Echocardiographic parameters, circulating levels of interleukin-18 (IL-18), C-reactive protein (CPR), and the percent of CD16-CD56, CD4/CD8, CD152, and HLA-DR expression were investigated in 108 patients with acute MI on admission to the emergency ward. RESULTS — Our review found that 31 new hyperglycemic patients (glycemia 7 mmol/l) had higher infarct segment length (P 0.05) and myocardial performance index (P 0.02) and reduced transmitral Doppler flow (P 0.05), pulmonary flow analysis (P 0.02), and ejection fraction (P 0.05) compared with 36 hyperglycemic diabetic patients and 41 normoglycemic patients. Plasma IL-18 and CRP were higher in the hyperglycemic than in the normoglycemic patients (P 0.005), with the highest values in patients with new hyperglycemia (P 0.05). Hyperglycemic patients had a higher percent of CD16/CD56 cells and CD4/CD8 ratio (P 0.01), whereas they had lower CD152 expression (which has a negative regulatory function in T-cell activation) compared with normoglycemic patients (P 0.001). CONCLUSIONS— DuringMI,hyperglycemiaisassociatedwithincreasedlevelsofinflamma- tory markers, enhanced expression of cytotoxic T-cells, and reduced expression of T-cells, which are implicated in limiting the immune process. An increased inflammatory immune process seems a likely mechanism linking acute hyperglycemia to poor cardiac outcome in MI patients.

Abstract

The possible mechanisms inducing acute coronary obstruction starting from the atherosclerotic plaque are illustrated. The effects of the inhibitors of the platelet's receptor GPIIb/IIIa and the results of the main trials performed using these inhibitors are reported. Finally, the results obtained immediately before their employment in the coronary angioplasty with or without stent's implantation are also reported. The first applications in acute ischemic stroke appear to be favourable too.

Abstract

The aim of this review was to evaluate the possibility of correcting cerebral ischemia (including both extracranial varieties) using elective as well as emergency surgery in order to avoid the onset of acute cerebral ischemia. The study included patients suffering from cerebral ischemia with a thromboembolic etiology. The author describe the emergency surgical procedures currently used in embolic forms affecting the extracranial vessels and percutaneous transluminal angioplasty (PTA) which is confined solely to extracranial vessels at present. In the authors' opinion, this method (with or without the application of one or more stents) will soon represent the routine method of cerebral revascularisation in the near future. The authors review the results currently obtained using extracranial PTA in the main brain surgery centres worldwide, the most frequent complications and the problems still to be resolved. In conclusion, it is useful to use interventional procedures to treat both extracranial and intracranial lesions in order to significantly reduce mortality from acute cerebral ischemia and its functional sequelae.

Abstract

We have compared the cardiovascular and metabolic responses to five different ACE inhibitors in 86 patients matched for age, body mass index, blood pressure, fasting plasma glucose and insulin levels in a placebo-controlled, double-blind, crossover, randomised trial. In the active drug treatment phase the patients were randomly assigned to one of five ACE inhibitors: captopril (75 mg/day; n = 16); enalapril (20 mg/day; n = 14); quinapril (20 mg/day; n = 17); ramipril (5 mg/day; n = 21) and lisinopril (20 mg/day; n = 18). Placebo and ACE inhibition phases lasted two weeks and were separated by a one week wash-out period. At the end of each treatment period blood pressure and heart rate were recorded and a fasting sample intravenous glucose tolerance test was conducted. Our study demonstrated that ACE inhibition significantly reduces blood pressure and improves insulin sensitivity. All the ACE inhibitors studied had similar cardiovascular responses but lisinopril displayed the larger metabolic response.
