We study numerically and experimentally the second-harmonic generation (SHG) from rectangular metagratings of V-shaped gold nanoantennas. We show that by carefully engineering the array pitch to steer the diffraction orders toward the single antenna emission, the extracted signal is maximized. This enhancement is attributed to the angular overlap between the radiation pattern and array factor and is comparable to the improvement yielded by the coupling of surface lattice resonances (SLRs) with local modes. Moreover, we demonstrate a simple technique to experimentally reconstruct the emission diagram of an antenna from measurements of the collective grating response as a function of the excitation angle. Excellent agreement is obtained with simulations when the sample is immersed either in air or in water, which is crucial in view of future sensing application. Thanks to the high signal-to-noise ratio and low dependence on the statistical particle dispersity, this method constitutes an effective alternative to back-focal plane imaging when very weak signals such as SHG are involved.

The Arctic has warmed more rapidly than the global mean during the past few decades. The lapse rate feedback (LRF) has been identified as being a large contributor to the Arctic amplification (AA) of climate change. This particular feedback arises from the vertically non-uniform warming of the troposphere, which in the Arctic emerges as strong near-surface and muted free-tropospheric warming. Stable stratification and meridional energy transport are two characteristic processes that are evoked as causes for this vertical warming structure. Our aim is to constrain these governing processes by making use of detailed observations in combination with the large climate model ensemble of the sixth Coupled Model Intercomparison Project (CMIP6). We build on the result that CMIP6 models show a large spread in AA and Arctic LRF, which are positively correlated for the historical period of 1951–2014. Thus, we present process-oriented constraints by linking characteristics of the current climate to historical climate simulations. In particular, we compare a large consortium of present-day observations to co-located model data from subsets that show a weak and strong simulated AA and Arctic LRF in the past. Our analyses suggest that the vertical temperature structure of the Arctic boundary layer is more realistically depicted in climate models with weak (w) AA and Arctic LRF (CMIP6/w) in the past. In particular, CMIP6/w models show stronger inversions in the present climate for boreal autumn and winter and over sea ice, which is more consistent with the observations. These results are based on observations from the year-long Multidisciplinary Drifting Observatory for the Study of Arctic Climate (MOSAiC) expedition in the central Arctic, long-term measurements at the Utqiaġvik site in Alaska, USA, and dropsonde temperature profiling from aircraft campaigns in the Fram Strait. In addition, the atmospheric energy transport from lower latitudes that can further mediate the warming structure in the free troposphere is more realistically represented by CMIP6/w models. In particular, CMIP6/w models systemically simulate a weaker Arctic atmospheric energy transport convergence in the present climate for boreal autumn and winter, which is more consistent with fifth generation reanalysis of the European Centre for Medium-Range Weather Forecasts (ERA5). We further show a positive relationship between the magnitude of the present-day transport convergence and the strength of past AA. With respect to the Arctic LRF, we find links between the changes in transport pathways that drive vertical warming structures and local differences in the LRF. This highlights the mediating influence of advection on the Arctic LRF and motivates deeper studies to explicitly link spatial patterns of Arctic feedbacks to changes in the large-scale circulation.

Qualitative literature reports the sensory experiences of autistic individuals in the first person. Following a thematic synthesis procedure, this metasynthesis aimed to explore specifically the lived experience of these sensory features. Four databases were systematically searched for qualitative studies describing sensory issues reported by autistic individuals. Article quality was assessed with the Critical Appraisal Skills Programme. Thematic analysis was used to identify and synthesise key themes. The 32 articles included reported data from 430 participants. Data analysis produced two themes structuring the sensory experiences of autistic people: (1) the everyday experience through body, thoughts and emotions, and (2) the relational experience, relationships being experienced as both a problem and a solution. Four dimensions are found in our results: physical, emotional, relational and social. Whereas a scientific approach to the sensory experience of autistic people is based on a cause-effect model, our results suggest that autistic people experience these dimensions holistically, as inseparable, and not in terms of cause and effect. This original finding brings new research perspectives, beyond the dichotomy between sensory features and social communications issues, and concrete implications within the health care system to provide more effective and respectful care to autistic people.

Abstract Many studies have demonstrated the short-term efficacy and tolerability of methylphenidate treatment adolescents with attention deficit hyperactivity disorder (ADHD). Qualitative literature on this matter focused on school outcomes, long-term side effects, family conflicts, personality changes and stigmatization. Yet, no qualitative study has crossed the perspectives of child and adolescent psychiatrists (CAPs) prescribing methylphenidate and adolescents with ADHD. This French qualitative study followed the five stages IPSE—Inductive Process to analyze the Structure of lived Experience-approach. Fifteen adolescents with ADHD and 11 CAPs were interviewed. Data collection by purposive sampling continued until data saturation was reached. Data analysis, based on a descriptive and structuring procedure to determine the structure of lived experience characterized by the central axes of experience, produced two axes: (1) The process of methylphenidate prescription, highlighting how this prescription was motivated from the exterior, experienced as passive by the adolescents and required commitment from the CAPs; and (2) the perceived effects of methylphenidate treatment, in three domains: at school, in relationships and in the sense of self. Findings raised both the issues of the epistemic position and social representation of the adolescents about ADHD and methylphenidate within this specific French context, and the self-awareness and perception of the adolescents with ADHD. We conclude that these two issues need to be regularly addressed by the CAPs prescribing methylphenidate to avoid epistemic injustice and prevent the harmful effects of stigmatization.

BACKGROUND. The SARS-CoV-2 pandemic has a significant impact on the global health care system, so effective treatments for coronavirus disease (COVID-19) are urgently needed. Nowadays, drug repurposing is widely considered for COVID-19 therapy; significant attention is paid to inhibitors of transmembrane serine proteases (TMPRSS2), which ensure the penetration of SARS-CoV-2 into the human cells and contribute to their infection. ε-aminocaproic acid (ACA), which has been used worldwide for many years to correct blood loss as a fibrinolysis inhibitor, is also known for its ability to block TMPRRS2. It is approved by the Ministry of Health of Ukraine for the treatment of influenza and acute respiratory viral infections. OBJECTIVE. The aim of our study was to evaluate the antiviral effect of ACA in vitro by staining of SARS-CoV-2 viral antigen (spike protein) and by visual scoring of cytopathogenic effect (CPE). RESULTS AND DISCUSSION. Using immunohistochemistry assay it was found that the mean value of EC50 for ACA on Caco-2 cells was 2.5 mg/ml and on Calu-3 cells – 17.3 mg/ml. Using CPE assay it was identified that the mean value of EC50 for ACA on Caco-2 cells was 6.4 mg/ml and on Calu-3 cells – 8.7 mg/ml. Additional analysis was shown that ACA has low cytotoxicity with CC50 values of >50 mg/ml on Caco-2 cells after 24h and 48h incubation and 37,57 and 41,29 mg/ml on Calu-3 cells after 24h and 48h incubation, respectively. Antiviral activity of ACA was detected when using non-toxic concentrations of the drug and did not depend on the time of introduction of ACA (before the introduction of the virus simultaneously with the pathogen after 1-hour incubation). ACA can be recommended for further in vivo studies on laboratory animals.

Abstract Background The COVID-19 pandemic has directly impacted the field of child and adolescent psychiatry, affecting all aspects of the lives of children and their families and increasing their risk of distress and mental health issues, especially among children with preexisting psychiatric disorders. Child and adolescent psychiatrists (CAPs) across the world have had to adapt their practice, due to lockdown and social distancing measures. This study aimed to explore how CAPs experienced their clinical practice in these singular conditions. Methods This exploratory international qualitative study used the Inductive Process to analyse the Structure of lived Experience (IPSE) approach, which is a five-stage inductive process used to explore the lived experience of participants in depth and to analyze their structure of lived experience. This study took place from March through July 2020 through individual in-depth video interviews. The sample size was determined according to the principles of theoretical sufficiency. Results 39 CAPs from 26 countries participated (age range 32–70 years; 23 women). Data analysis produced a structure of lived experience comprising three central axes of experience: (1) lost in space, lost in time, describing CAPs’ experience of disorganization of their clinical practice in the dimensions of lived time and lived space, (2) the body—of CAPs and patients—underlining their disconcerting experience of both sensory aspects and the non-embodied encounter during clinical practice, and (3) unpleasant emotions, with angst and loneliness the two main feelings coloring their clinical practice experience. Conclusions This analysis of the structure of lived experience of CAPs went beyond the sole context of the pandemic and revealed key aspects of what usually organizes CAP clinical practice. It identified two blind spots or conceptual voids within the child and adolescent psychiatry field: first, the intrinsic therapeutic function of a CAP clinical practice and, second, the important diagnostic and therapeutic function of the embodied encounter during CAP consultations. Beyond the context of COVID-19, further research should investigate these aspects to better define what a CAP does in practice and to increase both attractiveness and recruitment in this specialty.

OBJECTIVE: Burnout among physicians in public hospital has become a major public health issue in most Western countries. Qualitative literature has underlined the importance of interpersonal and group aspects in this context. Yet, no qualitative study has ever explicitly explored workplace direct environment’s association with physicians’ burnout. This study aimed to fill this gap. METHODS: This qualitative study used the five-stage inductive process to analyse the structure of lived experience (IPSE) approach and was conducted in French hospitals. We interviewed 45 participants – 16 with a lived experience of burnout and 29 of their colleagues – 19 women/26 men, (13 radiologists, 12 gastroenterologists, 10 gastrointestinal surgeons and 10 residents) from February 2018 to April 2019. Data analysis followed the IPSE analytic procedure and was conducted in two stages: three individual researchers carried out independent work and the group collectively pooled data. RESULTS: Three axes of experience were identified: (i) the loss of meaning, that is being a doctor, no longer has any meaning in the actual context of public hospitals; (ii) “the tower of Babel”, the impossibility of dialogue with both management and colleagues; and (iii) physicians’ daily interactions: too many conflicts, too much pressure and not enough recognition. CONCLUSION: Physicians in this study described being exposed to a deleterious atmosphere, experiencing both emotional abuse and structural violence within the workplace. They considered that such an environment could contribute to the development of burnout. Further research is necessary to assess this hypothesis.

Abstract Primary Care Providers (PCPs) often deal with patients on daily clinical practice without knowing anything about their smoking status and willingness to quit. The aim of this metasynthesis is to explore the PCPs and patients who are smokers perspectives regarding the issue of smoking cessation within primary care settings. It relies on the model of meta-ethnography and follows thematic synthesis procedures. Twenty-two studies are included, reporting on the view of 580 participants. Three main themes emerge: (i) What lacks, (ii) Some expectations but no request, and (iii) How to address the issue and induce patients’ motivation. Our results reveal a global feeling of a lack of legitimacy among PCPs when it comes to addressing the issue of tobacco and smoking cessation with their patients, even though they have developed creative strategies based on what is at the core of their practice, that is proximity, continuity, long-term and trustworthy relationship.

ObjectiveSexual abuse is a major public health problem. Its disclosure to a health professional could help to reduce its impact on survivors’ lives. The objective of this metasynthesis, combining a systematic review and an analysis of the qualitative studies, was to explore the qualitative literature concerning the experience of a survivor disclosing sexual violence experienced in childhood to a health professional, from the perspective of both.Methods and Data SourcesWe used four databases and two journals (Medline, PsycINFO, EMBASE, and SSCI, and the Journal of Sexual Abuse and Child Abuse and Neglect) to identify studies concerning this disclosure of sexual abuse to healthcare professionals from the point of view of the survivors and the health professionals. After assessing the methodological quality of the articles with the “Critical Appraisal Skills Program (CASP),” we conducted a thematic analysis of the data extracted during the review.ResultsThis review includes 20 articles, covering the data of 612 participants: 291 who were adults at the time of the study but abused in childhood, 152 minors, 14 parents of adolescents, and 155 healthcare professionals. Two themes emerged from the analysis: (1) the disclosure as experienced by the professionals, and (2) the disclosure as experienced by the survivors.ConclusionOur results show that survivors had a diachronic approach to the experience of disclosure. They suggest a change over time in how survivors experience disclosure: relief and release were seen only among the adult participants, at a distance from — long after — the disclosure. This study made it possible to identify new perspectives for research in the field of child psychiatry and to formulate concrete clinical proposals, in particular, by applying the principle of patient experts to involve now-adult survivors in training and increase the awareness of the healthcare professionals concerned.

Schizophrenia is often a severe and debilitating mental illness, frequently associated with impairments in social cognition that hinder individuals' abilities to relate to others and integrate effectively in society. Oxytocin has emerged as a putative therapeutic agent for treating social deficits in schizophrenia, but the mode of action remains unclear. This placebo-controlled crossover study aimed to elucidate the neural underpinnings of oxytocin administration in patients with schizophrenia. 20 patients with schizophrenia were examined using functional magnetic resonance imaging under oxytocin (40 IU) or placebo nasal spray. Participants performed a stochastically rewarded decision-making task that incorporated elements of social valence provided by different facial expressions, i.e. happy, angry and neutral. Oxytocin attenuated the normal bias in selecting the happy face accompanied by reduced activation in a network of brain regions that support mentalising, processing of facial emotion, salience, aversion, uncertainty and ambiguity in social stimuli, including amygdala, temporo-parietal junction, posterior cingulate cortex, precuneus and insula. These pro-social effects may contribute to the facilitation of social engagement and social interactions in patients with schizophrenia and warrant further investigation in future clinical trials for social cognitive impairments in schizophrenia.
