Abstract

Background Mitochondrial functions are controlled by genes of both mitochondrial and nuclear DNA. Pathogenic variants affecting any of these are responsible for primary mitochondrial disorders (MIDs), which can be diagnosed during adulthood. Kidney functions are highly dependent on mitochondrial respiration. However, the prevalence of MID-associated nephropathies (MIDANs) is unknown in the adult population. We aimed to address this point and to provide a full characterization of MIDANs in this population. Methods We retrospectively included for observational study adults (≥16 years of age) with genetically diagnosed MID between 2000 and 2020 in our tertiary care academic centre when they had a chronic kidney disease (CKD) evaluation. MIDANs were ascertained by CKD occurring in MIDs. The phenotypic, biological, histopathological and genotypic characteristics were recorded from the medical charts. Results We included 80 MID-affected adults and ascertained MIDANs in 28/80 (35%). Kidney diseases under the care of a nephrologist occurred in only 14/28 (50%) of the adults with MIDAN. MIDANs were tubulointerstitial nephropathy in 14/28 patients (50%) and glomerular diseases in 9/28 (32.1%). In adults with MID, MIDAN was negatively associated with higher albumin levels {odds ratio [OR] 0.79 [95% confidence interval (CI) 0.67–0.95]} and vision abnormalities [OR 0.17 (95% CI 0.03–0.94)] and positively associated with hypertension [OR 4.23 (95% CI 1.04–17.17)]. Conclusion MIDANs are frequent among adult MIDs. They are mostly represented by tubulointerstitial nephropathy or glomerular disease. Vision abnormalities, hypertension and albumin levels were independently associated with MIDANs. Our results pave the way for prospective studies investigating the prevalence of MIDANs among undetermined kidney disease populations. [ABSTRACT FROM AUTHOR]

Abstract

BK virus (BKV) replication increases urinary chemokine C-X-C motif ligand 10 (uCXCL10) levels in kidney transplant recipients (KTRs). Here, we investigated uCXCL10 levels across different stages of BKV replication as a prognostic and predictive marker for functional decline in KTRs after BKV-DNAemia. uCXCL10 was assessed in a cross-sectional study (474 paired urine/blood/biopsy samples and a longitudinal study (1,184 samples from 60 KTRs with BKV-DNAemia). uCXCL10 levels gradually increased with urine (P-value < 0.0001) and blood BKV viral load (P < 0.05) but were similar in the viruria and no BKV groups (P > 0.99). In viremic patients, uCXCL10 at biopsy was associated with graft functional decline [HR = 1.65, 95% CI (1.08-2.51), P = 0.02], irrespective of baseline eGFR, blood viral load, or BKVN diagnosis. uCXL10/cr (threshold: 12.86 ng/mmol) discriminated patients with a low risk of graft function decline from high-risk patients (P = 0.01). In the longitudinal study, the uCXCL10 and BKV-DNAemia trajectories were superimposable. Stratification using the same uCXCL10/cr threshold at first viremia predicted the subsequent inflammatory response, assessed by time-adjusted uCXCL10/cr AUC (P < 0.001), and graft functional decline (P = 0.03). In KTRs, uCXCL10 increases in BKV-DNAemia but not in isolated viruria. uCXCL10/cr is a prognostic biomarker of eGFR decrease, and a 12.86 ng/ml threshold predicts higher inflammatory burdens and poor renal outcomes.

15. Expression de CMIP au cours du syndrome néphrotique idiopathique

Abstract

Idiopathic nephrotic syndrom (INS) pools two entities: minimal change disease (MCD) and focal segmental glomerulosclerosis (FSGS). MCD and FSGS are podocyte diseases. Podocyte foot process effacement is the first step of INS (MCD), the second one is the podocytes loss (FSGS). Our goal is to show if glomerular c-mip expression is a specific and prognostic biomarker of INS. Indeed, this protein plays a key role in the podocyte architecture and in apoptosis mechanisms. We included all the kidney biopsies, of Aquitaine centralized by two pathology laboratories of Bordeaux, with the final diagnosis was primitive MCD or FSGS. These biopsies were taken from January 2007 to December 2014 and were all native kidney biopsies. Biopsies of diabetic nephropathy (DiabN), IgA nephropathy (IgAN) or preimplantation tissue (preimp, before kidney allograft) were also included for comparison. We tested glomerular (podocytes) and tubular c-mip expression by immunohistochemistry. We tested 148 kidney biopsies (86 MCD, 39 FSGS, 10 diabN, 10 IgAN, 3 preimp). Interpretation of glomerular c-mip staining was performed for 120 biopsies: 68.3 % were positive. All the diabN and IgAN had a positive glomerular c-mip staining. The renal function estimated by MDRD at the time of biopsy (M0) is lower for glomerular positive c-mip patients than for negative c-mip patients (84.3 mL/min/1.73m2 vs 60.5, p = 0.0024). This difference was not significative at M12 (78.5 mL/min/1.73m2 vs 64.2, p = 0.10). The MDRD slope during the first year after the biopsy was better for glomerular positive c-mip patients (MDRD slope > 40 %: 33.3 % vs 8.7 %, p = 0.041). C-mip positive patients had more vascular chronic lesions in their biopsies than negative c-mip patients (p = 0.034). Glomerular c-mip staining was nonspecific of INS. But c-mip expression may allow us to distinguish MCD or HSF patients with a better renal course at M12., Le syndrome néphrotique idiopathique (SNI) regroupe le syndrome néphrotique à lésions glomérulaires minimes (SNLGM) et la hyalinose segmentaire et focale (HSF). Il existerait un continuum entre ces deux maladies du podocyte : effacement des pédicelles (SNLGM) et disparition des podocytes (HSF). L’expression de c-mip par le podocyte pourrait être un marqueur pronostic spécifique du SNI puisque cette protéine joue un rôle clef dans l’organisation du cytosquelette podocytaire. Toutes les biopsies de rein natif adultes pour lesquelles le diagnostic de SNLGM ou de HSF primitifs a été retenu entre janvier 2007 et décembre 2014 en Aquitaine ont été incluses. Des biopsies rénales de néphropathie diabétique (Ndiab), de néphropathie à dépôts mésangiaux d’IgA (NIgA) ainsi que des biopsies préimplantatoires (préimp, avant greffe rénale) ont été aussi analysées. Nous avons étudié par immunohistochimie l’expression de c-mip (glomérules et tubules) sur ces biopsies. Nous avons analysé 148 biopsies (86 SNLGM, 39 HSF, 10 Ndiab, 10 NIgA, 3 préimp). Le marquage glomérulaire de c-mip était interprétable pour 120 biopsies avec une positivité pour 68.3 % d’entre elles. Toutes les Ndiab et NIgA présentaient une expression glomérulaire de c-mip. La fonction rénale estimée par MDRD au moment de la biopsie (M0) était moins bonne chez les patients c-mip + par rapport aux patients c-mip - (84.3 mL/min/1.73m2 vs 60.5, p = 0.0024). La pente de DFG dans la première année suivant la biopsie était meilleure pour les patients avec c-mip + (augmentation de plus de 40 % du MDRD : 33.3 % vs 8.7 %, p = 0.041). Ces patients c-mip + présentaient plus de lésions vasculaires chroniques sur la biopsie que les patients c-mip - (p = 0.034), avec un marquage tubulaire de c-mip le plus souvent positif (p < 0.0001). Le marquage glomérulaire de c-mip n’est pas spécifique du SNI. Il permettrait d’identifier les patients atteints d’un SNI avec une meilleure évolution de la fonction rénale à un an.
