Abstract

Our aim was to test if intra-thoracic impedance (ITI) measurements would improve short-term clinical outcome (i.e. all-cause mortality and heart failure hospitalizations) in patients with cardiac resynchronization therapy (CRT) who were followed in a telemonitoring program with structural biofeedback. Methods: CRT patients (n=200; age= 70 +/-14y) were included in a telemonitoring program at Ziekenhuis Oost-Limburg Genk, which comprised daily transmissions through a wireless system installed at their homes. All patients were followed for potential lead/device problems, episodes of anti-tachycardia pacing, device shocks, occurrence of arrhytmias, a decline in biventricular pacing percentage and/or a sudden drop in physical activity. Patients with missed transmissions were contacted by phone. Devices with ITI measurement ability were implanted in 108 patients. Those provided extra alerts when pre-specified ITI tresholds were crossed during at least 7 days. Heart failure nurses checked all telemonitoring alerts at daily time intervals. For clinically relevant alerts, as defined by protocol, direct biofeedback was provided to the patient by means of a structured questionnaire. Dependent on the outcome of the questionnaire, emphasis was placed on heart failure education with or without the advice of one of the following actions:(1) doubling of the dose of diuretics during 3 days after which the patient was contacted again;(2) contacting the general practitioner of the patient;(3) planning a visit at the multidisciplinary CRT clinic of Ziekenhuis Oost-Limburg. Heart-failure hospitalization and mortality data were collected for all patients by searching the electronic health records of the hospital. Results: During 250 patient-years of follow-up, there were 401 clinically relevant telemonitoring alerts. Forty-eight percent of these (n=194) were because of ITI treshold crossings. Those alerts led to an intervention in 43% of the cases. Mean follow-up was 13 +/-16 months, during which 22 patients (11%) died, 23 (12%) were admitted for heart failure, while 159 (79%) had an event-free survival. Time to death (P=0.211) and time to first heart failure admission (P=0.679) were similar between patients with and without ITI measurements. Annualizd mortality was 8% versus 10% respectively, which was also comparable among groups (P=0.206). Conclusion: In the setting of a structured heart failure follow-up clinic, ITI measurements in the context of an intensive telemonitoring program with structural biofeedback for CRT patients, did not result in an additional reduction of all-cause mortality and/or heart failure hospitalizations.

Abstract

Introduction: Among patients with unexplained dyspnea, left ventricular (LV) filling pressures (LVFP) is commonly estimated non-invasively by the E/e' ratio using Doppler echocardiography. However the accuracy of E/e' is controversial. We evaluated the correlation of E/e' ratio with invasively measured LVFP and of change in E/e' (ΔE/e') with change in LVFP. Methods: Supine and upright transthoracic echocardiography was performed in patients with unexplained dyspnea undergoing right heart catheterization. Patients with significant valvular disease and reduced LV ejection fraction (LVEF < 50%) were excluded. Pulmonary artery wedge pressure (PAWP) was used as the invasive indicator of LVFP. The mean of septal and lateral e' velocities was used for the calculation of E/e' ratio. Results: We studied 98 subjects with a mean age of 52 ± 20 years (69% of female gender). The supine E/e' and PAWP were 9.2 ± 3.2 and 12.1 ± 4.9 mmHg (range: 4-27 mmHg) respectively and were modestly correlated (r=0.38; p<0.001). With position change (supine to upright), ΔPAWP was -5.1 ± 4.3 mmHg and ΔE/e' was 0.17 ± 2.6, with no significant association between these two measures (r=0.003; p=0.98). Both E-wave (80 ± 22 to 65 ± 22 cm/s) and mean average e' (10.2 ± 3.6 to 7.3 ± 2.0 cm/s) decreased with the upright position. The ΔPAWP was correlated with ΔE-wave velocity (r=0.33; p=0.01), but not with Δe' (r=0.14; p=0.26). Conclusions: In patients with unexplained dyspnea and a preserved LVEF, E/e' is modestly, though significantly, correlated with PAWP. ΔE/e' is not correlated with ΔPAWP, partially related to the preload sensitivity of e'. Abstract

Purpose: With the advent of percutaneous transcatheter device closures in congenital heart defects and the emergence of percutaneous left atrial appendage closure, there is an increasingly important role for echocardiographic guidance and control of device position and function. Disc occluder devices frequently present as an unexplained ‘figure-of-8’ on echocardiography. The aim of this study was to clarify this ‘figure-of-8’ display and to relate its morphology to transducer position and device type. Methods: A mathematical model was developed to resemble disc occluder geometry and to allow a numerical simulation of the echocardiographic appearance. In addition, we developed an in vitro set-up for echocardiographic analysis of various disc occluders and various transducer positions. Results: In the mathematical model of an epitrochoid curve (closely resembling disc occluder geometry) a ‘figure-of-8’ display is obtained when emphasizing points with tangent vector perpendicular to the direction of ultrasound waves. Decreasing imaging depth results in a more asymmetric ‘figure-of-8’, with small upper part and wide lower part. Clinical and in vitro data are in close agreement with these results (Figure 1). Furthermore a ‘figure-of-8’ display is only obtained in a coronal imaging position, and is similar for different commercially available disc occluder types. Conclusions: The ‘figure-of-8’ display in the ultrasound image of a disc occluder is an imaging artifact due to the specific ‘epitrochoidal’ geometry of a deployed device and its interaction with ultrasound waves. Abstract

Recently, a highly purified human menopausal gonadotrophin preparation (HMO) was launched. The composition and purity of this HMG (Menopur®; Ferring Pharmaceuticals) with a claimed 1:1 ratio of FSH and LH was determined. Three gonadotrophins were observed: FSH, LH and human chorionic gonadotrophin (HCG). The immunoactivity for HCG was three-fold higher than the immunoactivity for LH. Because of the longer half-life of HCG as compared with LH, about 95% of the in-vivo LH-receptor-mediated bioactivity is attributable to the presence of HCG. This is substantiated by biochemical analyses. To the best of the authors' knowledge, this relatively high amount of HCG can only be explained by assuming the addition of HCG from external sources, which is a well established practice for standardization purposes. In addition to gonadotrophins, a number of other proteins were detected. The amount of these impurities, as determined by reversed-phase high-performance liquid chromatography on a peak-area basis, is at least 30%. Therefore, it is concluded that this HMG preparation contains at most 70% gonadotrophins. Via a proteomics approach three major impurities were identified: leukocyte elastase inhibitor, protein C inhibitor, and zinc-α[sub2]-glycoprotein. 48. A tailored substrate-based approach using focal pulsed field catheter ablation in patients with atrial fibrillation and advanced atrial substrate: Procedural data and 6-month success rates.

Abstract

Background: Focal pulsed field ablation (F-PFA) integrated in electroanatomic mapping systems allows tailored lesion sets in patients with atrial fibrillation (AF)., Objective: We aimed to determine feasibility, safety, and 6-month outcome of F-PFA for a tailored substrate-based catheter ablation approach in patients with AF and advanced atrial substrate., Methods: Consecutive patients with AF and advanced atrial substrate treated by an F-PFA system (CardioFocus) through contact force-sensing catheters integrated in electroanatomic mapping systems were prospectively enrolled. The tailored substrate-based catheter ablation approach included isolation of all pulmonary veins with wide area circumferential ablation, posterior wall isolation, mitral anterior line, and cavotricuspid isthmus ablation, according to substrate. At 6 months, feasibility, arrhythmia recurrence, and safety were evaluated., Results: In 83 patients (33% female; indexed left atrial volume, 44 ± 15 mL/m 2 ; 80% persistent AF or atrial flutter [AFl]; 57% redo procedures), successful de novo pulmonary vein isolation was performed in 36 patients and pulmonary vein reisolation in 30 patients. Mitral anterior line was performed in 19 patients with atypical AFl or anterior low-voltage areas; posterior wall isolation was performed in 38 patients with low-voltage areas or evoked delayed electrograms during premature atrial extrastimuli; and cavotricuspid isthmus ablation was performed in 24 patients with typical AFl. Median procedural and fluoroscopy times were 115 and 7 minutes, respectively. No complications occurred. At 6 months, arrhythmia recurred in 30 of 83 patients (21 AF; 9 atypical AFl)., Conclusion: Tailored substrate-based F-PFA in patients with AF and advanced atrial substrate is safe and effective. Abstract

Aims: Reduction of atrial fibrillation (AF) burden is the preferred outcome measure over categorical AF rhythm recurrence after AF ablation. In this sub-analysis of the TeleCheck-AF project, we tested the feasibility of smartphone app-based approximation of time spent with AF and/or symptoms., Methods and Results: Patients scheduled for at least one teleconsultation during the 12-month follow-up after AF ablation were instructed to use a smartphone photoplethysmography-based application for simultaneous symptom and rhythm monitoring three times daily for 1 week. Proxies of time spent with AF and/or symptoms (% recordings, load, and % days), temporal aggregation of AF and/or symptoms (density), and symptom-rhythm correlation (SRC) were assessed. In total, 484 patients (60% male, 62 ± 9.9 years) were included. Adherence, motivation, and patient satisfaction were high. %AF recordings, AF load, and %AF days (rs = 0.88-0.95) and %symptom recordings, symptom load, and %symptom days (rs = 0.95-0.98) showed positive correlations. The SRC correlated negatively with time spent with symptoms (rs = -0.65-0.90) and with time spent with AF (rs = -0.31-0.34). In patients with paroxysmal AF before ablation and AF during the monitoring period, 87% (n = 39/44) had a low-density score <50% ('paroxysmal AF pattern') while 5% (n = 2/44) had a high-density score >90% ('persistent AF pattern'). Abstract

Aims: Early evaluation of the natriuretic response is recommended to guide diuretic therapy in acute decompensated heart failure (ADHF). However, its implementation in daily practice is hampered by implementation barriers and increased time constraints. The Readily Available Urinary Sodium Analysis in Patients with Acute Decompensated Heart Failure (EASY-HF) study assessed the feasibility, efficacy and safety of a nurse-led urinary sodium-based diuretic titration protocol with the use of a point-of-care urinary sodium sensor., Methods and Results: The EASY-HF study was a single-centre, randomized, open-label study comparing diuretic management at the treating physician's discretion as standard of care (SOC) with a nurse-led natriuresis-guided protocol in patients with ADHF. The LAQUAtwin Sodium Meter (HORIBA) was used as point-of-care sensor to measure urine sodium concentration. The primary endpoint was natriuresis after 48 h. Secondary endpoints included safety profile and user-friendliness of both the protocol and the point-of-care sensor. Sixty patients were randomized towards SOC (n = 30) versus protocolized care (n = 30). The mean age was 80 ± 8 years, 25% were women and median N-terminal pro-B-type natriuretic peptide was 4667 (2667-7709) ng/L. Natriuresis after 48 h was significantly higher in the protocolized versus SOC group (820 ± 279 vs. 657 ± 273 mmol; p = 0.027). Pre-defined safety endpoints were similar among both groups. The sensor-based protocol was evaluated as easy to use by the nursing staff, and preferred over urinary collections., Conclusion: A nurse-led diuretic titration protocol via a point-of-care urinary sodium sensor was feasible, safe and resulted in an increased natriuresis in ADHF compared to SOC., (© 2024 European Society of Cardiology.)
