9. Die laterale Retinaculumspaltung

Abstract

Zusammenfassung: Die vorliegende Arbeit beinhaltet eine Literaturrecherche über die laterale Retinaculumspaltung. Von den 30 beurteilten Arbeiten fand sich lediglich eine prospektive randomisierte Studie. Diese konnte keine signifikante Differenz zwischen arthroskopischer und offener lateraler Retinaculumspaltung dokumentieren. Generell wurden bei isolierter lateraler Retinaculumspaltung zur Therapie der Patellainstabilität schlechte Langzeitresultate gefunden. Dagegen stellt die laterale Retinaculumspaltung bei lateraler Hyperkompression mit stabiler Patella und fehlender Dysplasie eine Intervention mit guten Erfolgsaussichten dar. Die laterale Retinaculumspaltung mit gleichzeitiger Weichteilkorrektur vermindert die Häufigkeit der weiteren Patellaluxationen und ist der isolierten lateralen Retinaculumspaltung vorzuziehen. Bei der patellofemoralen Arthrose führt die isolierte laterale Retinaculumspaltung nur kurzfristig zu einer Verbesserung.

Abstract

Zusammenfassung: Die Gesetzgebung des europäischen Parlaments legt die Qualitäts- und Sicherheitserfordernisse für die Spende, Beschaffung, Testung, Konservierung und Verteilung von menschlichen Geweben für patientenbezogene Anwendung mit dem Ziel eines hohen Grades an Sicherheit für den Patienten fest. Mit dem Ziel der Vermeidung von übertragbaren Krankheiten in der Gewebetransplantation wurden spezifische Erfordernisse für alle individuellen Gewebegruppen humaner Natur definiert. Die Mitglieder der Europäischen Union müssen diese Richtlinien mit dem Ziel der Erreichung eines hohen Niveaus des Gesundheitsschutzes für menschliche Anwendungen umsetzen. Genaue Angaben für Gewebebanken sind erforderlich, um deren Organisation, Verarbeitung, Dokumentation und Qualitätskontrolle zu sichern. Unverwünschte Wirkungen dieser Transplantate müssen durch die jeweiligen Institutionen dokumentiert werden. Mit dem Ziel der einfachen Kommunizierbarkeit und der Kostenkontrolle wurden einheitliche administrative Vorgaben geschaffen. Eine einheitliche europäische Kodierung ist erforderlich, um humane Gewebe leichter zu verfolgen. Die EU-Direktive respektiert fundmentale Menschenrechte entsprechend der Charta der Europäischen Union.

Abstract

Background: Guidelines recommend against preoperative biliary drainage (PBD) in patients with pancreatic head cancer if bilirubin levels are = 250 and < 250.Methods: Patients were identified from databases of 3 centers. Outcomes were compared in patients with a bilirubin level >= 250 versus = 250. PBD technical success (83% vs. 81%, p = 0.80) and PBD related complications (33% vs. 29%, p = 0.60) did not differ between these groups. Analyzing bilirubin levels >= 250 versus = 250 and < 250. Our study does not support a different approach regarding PBD in patients with severe jaundice.

Abstract

BACKGROUND: No prognostic score is currently available for long-term survival in autoimmune hepatitis (AIH) patients. OBJECTIVE: The aim of this study was to develop and validate such a prognostic score for AIH patients at diagnosis. METHODS: The prognostic score was developed using uni- & multivariate Cox regression in a 4-center Dutch cohort and validated in an independent 6-center Belgian cohort.
