Abstract

This study disentangles positive and negative reactions to home-to-work transitions (i.e. transitions from the home role to the work role during non-work hours; HWTs) and examines their consequences for employees' work engagement and psychological strain. Based on boundary theory and appraisal theories, we expected that positively appraised HWTs would relate to more engagement and less strain whereas negatively appraised HWTs would contribute to less engagement and more strain. We tested our hypotheses using two daily diary datasets from different Belgian companies, one collected before the COVID-19 pandemic during 13 workdays among 81 employees (678 observations; Study 1) and one collected during the pandemic during 9 workdays among 82 employees (516 observations; Study 2). Hypotheses were tested both on the within – and the between-person level using multilevel modelling to account for daily fluctuations in the appraisals of HWTs and between-person differences. As expected, positive appraisals were related to more engagement and less strain at the between-person level in both studies. We did not find this impact on the within-person level, nor did we find any within – or between-effects of negative appraisals. Our study highlights the relevance of positive appraisals for employees' between-level engagement and strain beyond the impact of HWTs themselves. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: The value of serum human epididymis protein 4 (HE4) in guiding referral decisions in patients with an ovarian mass remains unclear, because the majority of studies investigating HE4 were performed in oncology hospitals. However, the decision to refer is made at general hospitals with a low ovarian cancer prevalence. We assessed accuracies of HE4 in differentiating benign or borderline from malignant tumors in patients presenting with an ovarian mass at general hospitals. Method: Patients with an ovarian mass were prospectively included between 2017 and 2021 in nine general hospitals. HE4 and CA125 were preoperatively measured and the risk of malignancy index (RMI) was calculated. Histological diagnosis was the reference standard. Results: We included 316 patients, of whom 195 had a benign, 39 had a borderline and 82 had a malignant ovarian mass. HE4 had the highest AUC of 0.80 (95%CI 0.74–0.86), followed by RMI (0.71, 95%CI 0.64–0.78) and CA125 (0.69, 95%CI 0.62–0.75). Clinical setting significantly influenced biomarker performances. Applying age-dependent cut-off values for HE4 resulted in a better performance than one cut-off. Addition of HE4 to RMI resulted in a 32% decrease of unnecessary referred patients, while the number of correctly referred patients remained the same. Conclusion: HE4 is superior to RMI in predicting malignancy in patients with an ovarian mass from general hospitals. The addition of HE4 to the RMI improved HE4 alone. Although, there is still room for improvement, HE4 can guide referral decisions in patients with an ovarian mass to an oncology hospital.

Abstract

Careers exist in a societal context that offers both constraints and opportunities for career actors. Whereas most studies focus on proximal individual and/or organisational-level variables, we provide insights into how career goals and behaviours are understood and embedded in the more distal societal context. More specifically, we operationalise societal context using the career-related human potential composite and aim to understand if and why career goals and behaviours vary between countries. Drawing on a model of career structuration and using multilevel mediation modelling, we draw on a survey of 17,986 employees from 27 countries, covering nine of GLOBE's 10 cultural clusters, and national statistical data to examine the relationship between societal context (macrostructure building the career-opportunity structure) and actors' career goals (career mesostructure) and career behaviour (actions). We show that societal context in terms of societies' career-related human potential composite is negatively associated with the importance given to financial achievements as a specific career mesostructure in a society that is positively related to individuals' proactive career behaviour. Our career mesostructure fully mediates the relationship between societal context and individuals' proactive career behaviour. In this way, we expand career theory's scope beyond occupation- and organisation-related factors.

Abstract

Whereas many studies have shown that underemployed people experience lower objective career success and lower subjective career success while being underemployed, little research has been done on the lasting effects of underemployment. This study addresses the role of time in career success research by examining the impact of level underemployment, content underemployment and contingent employment on subsequent objective (i.e. salary) and subjective career success (i.e. job satisfaction). Our 10-year longitudinal dataset of 335 Dutch university graduates permits us to examine the impact of preceding underemployment as well as the specific timing of the underemployment in one's career. The multilevel analysis results illustrate that level and contingent underemployment have a negative impact on future pay, whereas content underemployment negatively affects job satisfaction five years later. In addition, for level underemployment also the timing turns out to matter, suggesting that the signal that it sends to employers may differ depending on when in one's career it happened. Taken together, these findings point to the importance of using a path-dependency perspective when trying to understand people's career success.

Abstract

Androgenen en oestrogenen spelen een rol in de pathofysiologie van acne. Androgenen, voornamelijk testosteron en dihydrotestosteron (DHT), verhogen via androgeenreceptoren de sebumproductie. Oestrogenen hebben een gunstig effect op acne door de aanmaak van androgenen en de hoeveelheid vrij testosteron te verminderen. Tevens werken zij als antagonist ter hoogte van de androgeenreceptor. Zo heeft anticonceptie onder de vorm van oestroprogestatieve combinatiepreparaten, voornamelijk als gevolg van de oestrogenencomponent, een gunstige invloed op acne. De synthetische progestagenen werken, naargelang van het type progestageen, als remmer of activator van de androgeenreceptoren, met een gunstig of een ongunstig effect op acne. Een meta-analyse bevestigt dat orale oestroprogestatieve combinatieanticonceptiva als behandeling van acne doeltreffender zijn dan een placebo. Een vergelijking tussen de verschillende preparaten is niet conclusief, waardoor er geen duidelijk superieur progestageen in de behandeling van acne naar voren komt. De vaginale ring en patch zijn dankzij hun oestrogeencomponent mogelijke alternatieven. De combinatie met de meeste antibiotica heeft geen ongunstig effect op de contraceptieve veiligheid.

Abstract

Werkloosheid is een gebeurtenis die nog steeds heel wat mensen treft. Bovendien blijft werkloosheid niet zonder gevolgen. Verscheidene studies hebben reeds aangetoond dat werkloosheid gepaard gaat met een aantal negatieve gevolgen op vlak van inkomen, gezondheid, sociaal netwerk, zelfwaarde en toekomstige loopbaankansen (bijvoorbeeld Price, Friedland, & Vinokur, 1998; Price, Choi, & Vinokur 2002). Het hoeft dan ook niet te verbazen dat heel wat onderzoek tracht te begrijpen hoe werklozen hun zoektocht naar een job kunnen aanpakken om de kans op een nieuwe job - en vooral op een kwaliteitsvolle nieuwe job - te verhogen. Onderzoek tot nu toe heeft veelal gefocust op de impact van het kwantitatief zoekgedrag van een werkloze; oftewel hoe vaak en intensief iemand zoekacties onderneemt. We weten echter heel wat minder over de impact van de kwaliteit van iemands zoekgedrag. In deze studie focussen we daarom op een kwalitatieve zoekgedragindicator, meer specifiek op zoekstrategieën oftewel de manier waarop mensen hun zoektocht naar een nieuwe job aanpakken, bijvoorbeeld gefocust of eerder willekeurig. Meer specifiek bestuderen we hoe en wanneer zoekstrategieën de kans op en kwaliteit van hertewerkstelling beïnvloeden.

Abstract

BACKGROUND: As farmers do not often keep a record of the expenditures for rearing, an economic tool that provides insight into the cost of rearing is useful. In the Netherlands, an economic tool (Jonkos) has been developed that can be used by farmers to obtain insight into the cost of rearing on their farm. The first objective of this study is to calculate the total cost of rearing young stock in Dutch dairy herds using Jonkos. The second objective is to compare the calculated total cost of rearing with the farmers' own estimation of the cost of rearing (the perceived cost). FINDINGS: Information was available for 75 herds that reared their own young stock and who had used the Jonkos tool. The perceived cost of rearing young stock was only available for 36 herds. In the 75 herds, the average herd size was 100 dairy cows. The average calculated total cost of rearing a heifer was €1,790. The average perceived total cost of rearing a heifer (including labour and housing costs) was €1,030. CONCLUSION: Most Dutch farmers in the study underestimated the total cost of rearing. The Jonkos economic tool has the advantage that herd-specific information can be entered as input values. The output of the tool can improve the awareness of farmers about the total costs of rearing. This awareness can lead to a higher priority of young stock rearing and consequently to an improved quality of young stock rearing.

Abstract

Research into human nutrition has led to an awareness of the health benefits that dietary modification can offer. However, with regard to osteoporosis, our understanding of the influence of nutrition on bone health is limited because most studies to date concentrated primarily on the role of calcium. There is now an increasing rationale to focus on other micronutrients, the major class of interest being phytoestrogens. The aim of the present study was to investigate the potential preventive effect of long-term isoflavone consumption on ovariectomy-induced osteopenia in the adult rat, a model for postmenopausal osteoporosis. The study was carried out on 7-mo-old Wistar rats housed in individual cage. Fifty rats were fed a soy-protein-free powdered semipurified diet (mean amount consumed the previous day). Ten rats were sham operated (SH) at day 1 of the experiment; 40 were ovariectomized (OVX) and then allocated to four equal groups that received soy isoflavones orally for 90 d. Isoflavones (Soylife[TM] 100, batch n [degrees] 32K/154/H, containing 35 mg/g isoflavones [9.26 mg genistin, 25.6 mg daidzin], Soylife, The Netherlands BV) were added to the diet at 0 (OVX), 4 (IF4), 8 (IF8), and 16 (IF16) mg isoflavones/g body weight daily. Body weight increased throughout the experimental period, with no significant difference observed among the OVX animals (OVX, IF4, IF8, and IF16). Body composition (assessed by dual energy x-ray absorptiometry) was not modified by isoflavone consumption (SH: 23% [+ or -] 2 vs. OVX: 27% [+ or -] 2% fat). At necropsy, the success of ovariectomy was checked by atrophy of uterine horns (0.18 [+ or -] 0.01 g vs. 0.74 [+ or -] 0.07 g in SH). Regardless of dose, soy isoflavones did not elicit any uterotropic activity. With regard to bone metabolism, the highest dose of isoflavones (but not the two lowest doses) prevented OVX-induced bone loss. Indeed, total bone mineral density (IF16:0.2348 [+ or -] 0.008 vs. OVX: 0.2092 [+ or -] 0.0042 g/[cm.sup.2]), diaphyseal bone mineral density (IF16:0.2156 [+ or -] 0.0065 vs. OVX: 0.2161 [+ or -] 0.0047 g/[cm.sup.2]), and metaphyseal femoral bone mineral density (IF16:0.2423 [+ or -] 0.0088 vs. OVX: 0.1886 [+ or -] 0.004 g/ [cm.sup.2]) were higher than in the OVX controls. However, bone strength (N) was not improved: femoral failure load was lower in OVX than in SH regardless of diet. Urinary deoxypyridinoline excretion (SH: 76 [+ or -] 6 vs. OVX: 122.5 [+ or -] 3 nmol/mmol creatinine), a marker of bone resorption, and plasma osteocalcin level (SH: 18 [+ or -] 1 vs. OVX: 29 [+ or -] 2 [micro]/L), a marker of osteoblast activity, stayed higher in OVX animals than in SH animals, even when they were given soy isoflavones. In conclusion, these results show that daily consumption of 16 [micro]g isoflavones/g body weight (i.e., 4.23 [micro]g genistin and 11.70 [micro]g daidzin) for 90 d prevents osteopenia in the ovariectomized rat, a model for postmenopausal osteoporosis, as shown by bone mineral density assessment. This diet was devoid of any uterotropic effect.
