Abstract

Er heerst momenteel een overaanbod aan bio-geitenmelk, waardoor bio-geitenmelk soms gangbaar verwerkt wordt, of sommige bio-geitenbedrijven overschakelen op gangbare methoden. Bovendien blijft de productie van geitenmelk seizoensgebonden pieken in het voorjaar en zomer, wat vaak in deze periode voor overmelk zorgt. Deze mismatch tussen vraag en aanbod dreigt de melkprijs te doen inzakken. Het verpoederen van melkoverschotten zou, naast een oplossing voor de nieuwe wetgeving, ook een oplossing kunnen bieden voor de melkprijs. Enerzijds verhoogt het verpoederen de houdbaarheid van geitenmelk, waardoor de aanbodcurve veel flexibeler wordt en zich kan aanpassen aan de vraag. In deze context leek het ons opportuun om ook te onderzoeken in welke mate gedroogde biokoeienmelk uit de zomerpiek inzetbaar is voor de opfok van geiten. Hiervoor zou een nood kunnen ontstaan wanneer het bio-geitenmelk aanbod op een bepaald moment onvoldoende is, of om een overaanbod in bio-koeienmelk te ontlasten.

Abstract

Several anti-inflammatory, including nonsteroidal anti-inflammatory drugs (NSAIDs), such as aspirin and other drugs, have been implicated as a cause of upper gastrointestinal bleeding. Although there is extensive anecdotal evidence to support this link, only a very few studies involving a very small number of subjects have been performed with adequate control conditions to draw firm conclusions. To further investigate the relationship between anti-inflammatory drugs (aspirin as well as others) and gastrointestinal (GI) bleeding, a study was carried out with 161 patients (102 men and 59 women) who were being treated for either hematemesis (vomiting blood) or melena (blood in stool), and an age-matched control group of patients who were being treated for non-gastrointestinal maladies. There was significantly more use of both non-aspirin NSAIDs and aspirin in the gastrointestinal treatment group. There were no differences between the groups in the extent to which they used other drugs, including acetaminophen, corticosteroids, tobacco, and alcohol. When patient characteristics were calculated for users and non-users of the various drugs, NSAID users were found to be older, more likely to be female, and to have a higher mortality rate than non-users. Among those patients who were found to be suffering from bleeding peptic ulcer disease, NSAID users were equally likely to have reported previous symptoms of ulcer compared with non-users, and did not have a higher frequency of multiple gastric or duodenal ulcers. The increased risk of developing gastrointestinal bleeding attributable to non-aspirin NSAID use was higher than that attributable to the use of aspirin. 15. Aztreonam-flucloxacillin double beta-lactam treatment as empirical therapy of serious infections in very elderly patients

Abstract

Aztreonam, the first monomcyclic beta-lactam antibiotic with pure anti-Gram-negative activity, combined with flucloxacillin, a penicillinase resistant penicillin, was given as empirical treatment of 53 serious infections in very elderly people. Eighteen of the cases had positiove blood cultures and 11 had a clinical picture of sepsis without positive blood cultures: Of 49 evaluable infections, 45 (92%) were cured. In 40% of the infections, antibiotic treatment could be narrowed after 72 hours to one antibiotic. Diarrhoea, mostly transaction, was the only side-effects. Aztreonam-flucloxacilin combination is a safe and effective empirical treatment regimen for series. infection in very elderly patients.

Abstract

Since 1984, when the first fluoroquinolone, norfloxacin, was marketed in Europe, there has been a marked increase in the usage of this class of drugs. In order to evaluate the influence of this drug usage on the prevalence of resistance to fluoroquinolones in clinical isolates of the familyEnterobacteriaceae, Pseudomonas aeruginosa, Staphylococcus aureus, coagulase-negative staphylococci andEnterococcus faecalis we reviewed the susceptibility data from four collaborative surveys conducted between 1983 and 1990 by the Study Group ‘Bacterial Resistance’ of the Paul-Ehrlich-Society for Chemotherapy. All participating laboratories used the same standardized methods. Miminal inhibitory concentrations were determined by the broth microdilution method. More than 20,000 bacterial strains were tested. The results are presented for ciprofloxacin, which is regarded as the representative of the fluoroquinolones. Using ≥ 4 mg/l as a breakpoint for resistance to ciprofloxacin, the prevalence of resistant strains of the familyEnterobacteriaceae in Central Europe between 1983 and 1990 remained below 1%. In contrast, the resistance rates inP. aeruginosa were 0.7%, 1.0%, 3.8% and 7.0%, inS. aureus 0%, 0.5%, 6.6% and 6.8%, and inE. faecalis 2.2%, 0.7%, 4.9% and 7.7% in 1983, 1986, 1989 and 1990, respectively. The latest study carried out in cooperation with 78 laboratories from 12 European countries revealed great differences in the prevalence of resistance to fluoroquinolones from one species to another ranging from 0% withProteus vulgaris andSalmonella spp. to 26.7% withProvidencia stuartii. The highest rates of resistance were recorded for oxacillin-resistant strains ofS. aureus (70.6%) and oxacillin-resistant coagulase-negative staphylococci (51.2%). Resistance levels for individual species varied between countries, but they were consistently higher in Southern Europe than in Northwest and Central Europe. Resistance inS. aureus andE. faecalis was more prevalent in isolates from intensive care patients than in isolates from patients on normal wards. In addition,S. aureus isolates displayed a considerable difference in the resistance rates for blood (9.3%) and urine (34.4%).
