Objective: To determine the clinical and epidemiological characteristics of patients with noninfectious uveitis at a university hospital in Paraguay. Methodology: An observational, descriptive, cross-sectional, retrospective study was performed. Consecutive patients who attended the Ophthalmology Service of the Hospital de Clínicas from January 2020 to October 2021 and who were diagnosed with non-infectious uveitis were included. The variables studied were sex, age, origin, reason for consultation, anatomical classification, clinical course, systemic and ocular associations, and bilaterality. Results: A total of 78 medical records of patients meeting the inclusion and exclusion criteria were analyzed. Of the patients, 66.7% were female and 42 (53.8%) were from the Central Department. The anterior anatomical location of uveitis was the most frequent (53.9%), followed in decreasing order by panuveitis, posterior uveitis, and intermediate uveitis. Regarding anterior uveitis, 24 patients (57.1%) had unilateral uveitis, 40 (95.2%) were non-granulomatous, 19 (45.2%) presented an acute clinical course, the most frequent reason for consultation was "red and painful eye" with 15 patients (35.7%), the most frequent systemic association was HLA- B27+ in 16 patients (38.1%). As for panuveitis, 13 patients (50%) were associated with Vogt-Koyanagi-Harada syndrome, and of the latter, two patients were related to rheumatoid arthritis. Conclusion: The clinical and epidemiological patterns observed in this study were largely consistent with those reported in the literature. Notably, the most frequent associations differed from those reported in the literature: HLA-B27+ for anterior uveitis, rheumatoid arthritis for posterior uveitis, and a significant percentage of Vogt-Koyanagi-Harada syndrome among panuveitis cases.

Introduction: Scleral contact lenses (SCLs) are gas-permeable and are placed entirely on the sclera without contacting structures such as the cornea or limbus. These ECLs are designed to rehabilitate reduced vision of eyes with irregular corneas. Irregular astigmatism caused by corneal pathologies such as keratoconus, post-transplant, post-refractive surgery, or corneal degeneration produces poor visual acuity that cannot be corrected with air lenses, and the appearance of scleral contact lenses could be a therapeutic option in the treatment and optical correction of corneal pathologies. To determine the degree of improvement in visual acuity with adaptation of scleral lenses. Methodology: This was an observational, descriptive, retrospective, cross-sectional, retrospective study. Results: Forty cases of irregular astigmatism with different initial visual acuity effects were observed prior to the fitting of Scleral Lenses. The mean visual acuity (VA) prior to correction with scleral lenses observed in the 40 cases was 20/200 with correction on the Snellen scale with a minimum (worst VA) equal to "finger-count vision" (represented as cases with 20/ 400 vision for quantification purposes) and a maximum (best VA) of 20/70. Nineteen cases (47.5%) restored their VA to normal values (20/20), and 30 cases (75%) improved their VA by at least 10-fold from the baseline. Discussion: In this study, a remarkable and constant improvement in visual acuity was observed in all the patients. Some patients experienced an increase of up to 20 times their initial visual acuity, achieving, in many cases, a visual acuity of 20/20 on the Snellen scale. These results indicate a promising treatment strategy with fewer adverse effects., Introducción:Las lentes de contacto esclerales (LCE) son permeables al gas y se colocan totalmente en la esclera sin hacer contacto con estructuras como la córnea o el limbo. Estas LCE son diseñadas para rehabilitar la visión reducida de los ojos con córneas irregulares. Los astigmatismos irregulares producidos por patologías corneales como el queratocono, post trasplante, post cirugía refractiva o degeneraciones corneales producen una mala agudeza visual que no puede ser corregida con lentes aéreas, actualmente con laaparición de las lentes de contacto esclerales podrían ser una de las opciones terapéuticas en el tratamiento y corrección óptica de las patologías corneales.Determinar el grado de mejoría de la agudeza visual con la adaptación de lentes esclerales. Metodología:Observacional descriptivo de corte transversal, retrospectivo. Resultados:40 casos de astigmatismo irregular con distintas afectaciones iniciales en la agudeza visual previo a la colocación de las LCE. La media de agudeza visual (AV) previo a la corrección con las lentes esclerales observada en los 40 casos fue de 20/200 con corrección (CC) en la escala de Snellen con un mínimo (peor AV) igual a “visión cuenta dedos” (representado como casos con visión 20/400 con fines de cuantificación) y un máximo (mejor AV) de 20/70. 19 casos (47,5%) restauraron su AV a valores normales (20/20); 30 casos (el 75%) mejoraron su AV al menos 10 veces respecto al valor inicial.Discusión:En este estudio, se observó una notable y constante mejora en la agudeza visual de todos los pacientes estudiados. Algunos pacientes experimentaron aumentos de hasta 20 veces su agudeza visual inicial, logrando en muchos casos una agudeza visual de 20/20 en la escala de Snellen. Estos resultados indican un tratamiento prometedor con pocos efectos adversos evidentes.

Introduction: BK polyomavirus (PBK) infection is an emerging problem in kidney transplantation that contributes to the chronic loss of kidney grafts, and in which immunosuppression plays a decisive role in its appearance. Objective: To determine the prevalence of BK virus in transplant patients from the Hospital de Clínicas, FCM-UNA. Period August 2019 to May 2022. Materials and methods: Observational, descriptive, prospective, cross-sectional study. Twenty-seven patients who underwent kidney transplantation were included, and BKV dosage was requested. Results: 27 patients with VBK dosage were included, of which 11.11% (3) had positive isolation in plasma or urine. The average age of the patients with positive isolate was 39±3 years, with a maximum age of 52 and a minimum of 22 years. O them (66.6%) were female, 100% from the Central department. The average time between the transplant and the primary infection in blood or urine was 39+/- 13 months, with a range of 4-31 months, of which 66.6% occurred within 4 months. Regarding the type of donors, (66.6%) were cadaveric donors and (33.3%) were living. Conclusion: The prevalence of BKV in transplant patients from the Hospital de Clínicas was 11.11%, the average age of those infected was 39 years, predominantly female, from the Central Department, the average time elapsed between the trans- plant and the primary infection was 39 months. In those studied, the cadaveric donor predominated., Introducción: La infección por el poliomavirus BK (PBK) es un problema emergente en el trasplante renal que contribuye a la pérdida crónica de los injertos renales, y en el que la inmunosupresión desempeña un papel decisivo en su aparición. Objetivo: Determinar la prevalencia de virus BK en pacientes trasplantados del Hospital de Clínicas, FCM-UNA. Periodo agosto 2019 a mayo 2022. Materiales y métodos: Estudio observacional, descriptivo, prospectivo, de corte transversal. Se incluyó a 27 pacientes que fueron sometidos a trasplante renal, a los que se les solicito dosaje para VBK. Resultados: Se incluyó a 27 pacientes con dosaje para VBK, de los cuales 11,11% (3) tuvieron aislamiento positivo en plasma u orina. La edad promedio de los pa- cientes con aislamiento positivo fue de 39±3 años, con una edad máxima de 52 y mínima de 22 años. De ellos (66,6%) fueron de sexo femenino, 100% procedentes del departamento Central. El tiempo promedio trascurrido entre el trasplante y la primoinfección en sangre u orina fue de 39+/- 13 meses, con un rango de 4-31 meses, de los cuales en un 66,6% ocurrió entre los 4 meses. En cuanto al tipo de donantes, (66,6%) fueron donantes cadavéricos y (33,3%) vivos. Conclusión: La prevalencia de VBK en pacientes trasplantados del Hospital de Clí- nicas fue de 11,11 %, la edad promedio de los infectados fue de 39 años, predominantemente del sexo femenino, procedentes del departamento Central, el tiempo promedio transcurrido entre el trasplante y la primoinfección fue de 39 meses. Introduction: Toxoplasmosis is a parasitic disease, in Latin America the prevalence is relatively high, it is known that the disease is not only the most frequent cause of posterior uveitis, but also the main cause of uveitis itself. In Paraguay there are no published epidemiological data on it. Objective: To determine the characteristics of retinal lesions in patients diagnosed with ocular Toxoplasmosis. Methods: This study is observational, descriptive, cross-sectional, temporally retrospective, with non-probabilistic sampling of consecutive cases. It focused on patients diagnosed with Toxoplasmosis who attended the Ophthalmology service of the Hospital de Clínicas, during the period from June 2020 to February 2022. Results: 54.3% of women were observed, an average of 30.62 +/ - 12.96 years old and 62.8% of urban origin. 88.3% of the cases were unilateral, half were primary episodes, 93.6% acute cases, and 97.9% had an acquired transmission mechanism. Most presented as panuveitis, the most frequent locations of retinochoroidal lesions were the posterior pole (extramacular) and the periphery. The main therapeutic approach was treatment with trimethoprim sulfamethoxazole and systemic corticosteroids, the duration of treatment was 6.51 +/- 1.52 weeks, and the main complications were residual vitritis and ocular hypertension. Discussion: Ocular toxoplasmosis is an important cause of visual morbidity. It is essential to prevent infection by this parasite through hygienic-sanitary measures, as well as early diagnosis and treatment of infected patients, in order to reduce the complications associated with this pathology., Introducción: La toxoplasmosis es una enfermedad parasitaria, en América Latina la prevalencia es relativamente alta, se sabe que la enfermedad no solo es la causa más frecuente de uveítis posterior, sino también la causa principal de la uveítis en sí misma. En Paraguay no se dispone de datos epidemiológicos publicados sobre la misma. Objetivo: Determinar las características de las lesiones retinianas en pacientes con diagnóstico de Toxoplasmosis ocular. Metodología: El presente estudio es observacional, descriptivo, de corte transversal, temporalmente retrospectivo, con muestreo no probabilístico de casos consecutivos. Se enfocó en pacientes con diagnóstico de Toxoplasmosis que acudieron al servicio de Oftalmología del Hospital de Clínicas, durante el periodo de junio de 2020 a febrero 2022. Resultados: Se observó un 54,3 % de mujeres, una media de 30,62 +/- 12,96 años y un 62,8 % de procedencia urbana. El 88,3 % de los casos fueron unilaterales, la mitad de episodios primarios, el 93,6 % casos agudos, y el 97,9 % tuvo un mecanismo de trasmisión adquirido. La mayoría se presentó como Panuveítis, las localizaciones más frecuentes de las lesiones retinocoroideas fueron el Polo posterior (extramacular) y la periferia. El principal abordaje terapéutico fue el tratamiento con Trimetoprim Sulfametoxazol y corticoides sistémicos, la duración del tratamiento fue de 6,51 +/- 1,52 semanas, y las principales complicaciones fueron la Vitritis residual y la Hipertensión ocular. Discusión: La toxoplasmosis ocular es una causa importante de morbilidad visual. Se hace fundamental la prevención de la infección por este parásito mediante medidas higiénico-sanitarias, así como el diagnóstico y tratamiento precoz de los pacientes infectados, de tal manera a disminuir las complicaciones asociadas a esta patología.

Introduction: Glaucoma is the main cause of irreversible blindness worldwide. The global prevalence of glaucoma in people aged 40 to 80 years is estimated at 3.5%. With the increasing number and proportion of older people in the population, it is projected that 111.8 million people will have glaucoma by 2040. First-line treatment is usually a topical IOP-lowering medication or laser trabeculoplasty. The most widely used class of medication is prostaglandin analogues. Objective: To compare the intraocular pressure (IOP) lowering effect of 0.024% Latanoprostene bunod (LBN) with 0.005% Latanoprost in subjects with open-angle glaucoma (OAG) or ocular hypertension (OHT). Methodology: Prospective, randomized, parallel group, non-inferiority clinical trial. Results: It was performed in 28 patients (56 eyes) who were randomized into 2 parallel groups (28 eyes per group), the Latanoprost group and the Latanoprostene bunod (LBN) group. In the LBN group, the mean intraocular pressure before treatment was 25.3 ± 6.6 mmHg and the mean intraocular pressure after 1 month of treatment was 16.5 ± 4.9 mmHg (p=0.00). In the Latanoprost group, the mean intraocular pressure before treatment was 23.6 ± 3.6 mmHg and the mean intraocular pressure after 1 month of treatment with 0.005% Latanoprost was 15.3 ± 2.4 mmHg (p=0.00). However, when comparing the IOPs to the 1-month treatment with Latanoprostene bunod 0.024% and Latanoprost 0.005%, it is observed, through ANOVA, that the difference in intraocular pressure reduction between these two drugs is not significant (p= 0.238). Conclusion: Topical prostaglandins, with their potent ocular hypotensive effect (resulting from increased uveoscleral outflow), are an important treatment option for glaucoma. The IOP reduction is as expected with both drugs, however, there are no significant differences between the two. In the LBN group, more drug-related ocular adverse effects were found after 1 month of use., Introducción: El glaucoma es la principal causa de ceguera irreversible en el mundo. La prevalencia mundial de glaucoma en personas de 40 a 80 años se estima en un 3,5%. Una de las modalidades de tratamiento de primera línea es la utilización de medicamentos tópicos para reducir la presión intraocular (PIO). La clase de medicamento más utilizada son los análogos de prostaglandina. Objetivo: Comparar el efecto reductor de la PIO de Latanoprostene bunod (LBN) al 0,024% con Latanoprost al 0,005 % en sujetos con glaucoma de ángulo abierto (GAA) o hipertensión ocular (HTO). Metodología: Ensayo observacional de estudio de cohorte prospectivo. Resultados: Fue realizado en 28 pacientes (56 ojos) quienes fueron aleatorizados en 2 grupos paralelos (28 ojos por grupo), el grupo Latanoprost y el grupo LBN. En el grupo LBN la media de la PIO antes del tratamiento fue de 25,3 ± 6,6 mmHg y la media de la PIO luego de 1 mes de tratamiento fue de 16,5 ± 4,9 mmHg (p<0,05). En el grupo Latanoprost la media de la PIO antes del tratamiento fue de 23,6 ± 3,6 mmHg y la media de la PIO luego de 1 mes de tratamiento con Latanoprost al 0,005% fue de 15,3 ± 2,4 mmHg (p<0,05). Sin embargo, al comparar las PIOs luego de 1 mes de tratamiento con LBN 0,024% y Latanoprost 0,005% se objetiva que la diferencia en reducción de la presión intraocular entre estos dos fármacos no fue significativa (p= 0,238). Conclusión: Las prostaglandinas tópicas, con su potente efecto hipotensor ocular son una importante opción de tratamiento para el glaucoma. La reducción de la PIO es la esperada con ambos medicamentos, sin embargo, no existen diferencias significativas entre ambas luego de 1 mes de uso. Con respecto a los efectos secundarios, en el grupo LBN se encontró más efectos adversos oculares.
