Abstract

Background Factors Influencing Depression Endpoints Research (FINDER) is a 6-month, prospective, observational study carried out in 12 European countries aimed at investigating health-related quality of life (HRQoL) in outpatients receiving pharmacological treatment for a first or new depressive episode. Baseline characteristics of patients enrolled in Italy are presented. Methods All treatment decisions were at the discretion of the investigator. Data were collected at baseline and after 3 and 6 months of treatment. Baseline evaluations included demographics, medical and psychiatric history, and medications used in the last 24 months and prescribed at enrolment. The Hospital Anxiety and Depression Scale (HADS), was adopted to evaluate depressive symptoms, while somatic and painful physical symptoms were assessed by using the Somatic Symptom Inventory (SSI) and a 0 to 100 mm visual analogue scale (VAS), HRQoL via 36-item Short Form Health Survey (SF-36), and the European Quality of Life 5-Dimensions (EQ-5D) instrument. Results A total of 513 patients were recruited across 38 sites. The mean ± standard deviation (SD) age at first depressive episode was 38.7 ± 15.9 years, the mean duration of depression 10.6 ± 12.3 years. The most common psychiatric comorbidities in the previous 24 months were anxiety/panic (72.6%) and obsessive/compulsive disorders (13.4%), while 35.9% had functional somatic syndromes. Most patients (65.1%) reported pain from any cause. Monotherapy with selective serotonin reuptake inhibitors (SSRIs) and tricyclic antidepressants (TCAs) was prescribed at enrolment in 64.5% and 6.4% of the cases, respectively. The most commonly prescribed agents were sertraline (17.3%), escitalopram (16.2%), venlaflaxine (15.6%) and paroxetine (14.8%). The mean HADS subscores for depression and anxiety were 13.3 ± 4.2 and 12.2 ± 3.9, respectively; 76.4% of patients could be defined as being 'probable cases' for depression and 66.2% for anxiety. The mean total score of VAS-pain in the last week was 42.9 ± 27.1, with highest scores reported in the 'interference of pain with daily activities' and in 'amount of time patient was awake and had pain'. From SF-36, the worst health status was found for role limitations due to emotional problem, mental health and social functioning. A mean score < 50 (that is, below the standardised population norm) was also found in all remaining domains. The SF-36 summary scores and EQ-5D (health status and VAS) were lower in patients with moderate/severe pain than in those with no or mild pain. Conclusion The baseline results of patients enrolled in the FINDER study in Italy show clinical and functional impairments, and poor HRQoL. The results obtained after 6 months of therapy will permit better understanding the effects of different variables on clinical outcomes and HRQoL.

Abstract

This study focuses on the environmental impact assessment of three different high efficiency residential pellet boilers manufactured by an Austrian company. A Life Cycle Analysis (LCA) and an Environmental Life Cycle Costing (ELCC) analysis were performed, along with air quality modelling simulations for two EU case studies to evaluate CO, PM10, SO2, NO2 concentration reductions due to the substitution of the innovative boilers in place of obsolete and non environmentally compliant biomass boilers. The SimaPro software (v. 8.05.13, Recipe Midpoint Method) was used for the LCA and ELCC analysis while CALPUFF (v. 6.42) was used for air quality simulations. Boilers emission and efficiency factors operating in real life conditions were used as input data for all environmental impact assessment analysis. Results showed an interesting reduction of both the environmental impact and pellet consumption cost (linked to the highest monetary value in the ELCC analysis) associated to the most innovative boiler model among the three envisaged. Furthermore, it emerged that air quality could be improved in the short term (2020) with the substitution of obsolete and end of life biomass boilers with high efficiency pellet boilers in EU areas where biomass is the main source of residential heating. A potential reduction of local CO, SO2 and PM10 concentrations from 9% to 23% depending on the pollutant and on the specific case study considered is found for the short term scenario (2020) with respect to the baseline scenario (2010). No significant differences in air quality were evidenced for NO2 concentrations.

Abstract

Il saggio delinea una geniale impostura che occupò la scena palermitana di fine Settecento seguendo la ricostruzione di uno storico ottocentesco, Domenico Scinà, e la sua riscrittura in un romanzo di Leonardo Sciascia. Un solo protagonista, l’abate Vella, nel testo dello storico; due personaggi, il Vella e il Di Blasi, nel "Consiglio d’Egitto" (1963). Scinà, in uno scritto del 1824, presenta l’abate Vella come un avventuriero e ripercorre l'impostura grazie alla quale egli aveva costruito dal nulla una falsa storia della Sicilia, tutta orientata a rafforzare e ampliare prerogative e diritti della monarchia contro le appropriazioni, ritenute indebite, dei grandi feudatari. Nel romanzo storicamente fedele è la ricostruzione dell’impostura, rigorosamente storici e reali sono i personaggi, tutta sciasciana (nel senso che ci si ritrova l’ideologia, a volte l’ironia, spesso lo sconforto dell’autore) è l’invenzione narrativa che presiede alla scrittura dei monologhi, dei dialoghi, alla ricostruzione delle situazioni, degli ambienti che fanno da sfondo alle vicende, elementi questi ultimi necessari per costruire «il vero romanzesco». Alla fine, prendendo spunto dall’inganno velliano, altre falsificazioni, altre imposture si materializzeranno nel romanzo: supreme quelle della storia, della scrittura, dei libri e, quindi, della letteratura; «tragica» quella della congiura giacobina palermitana del 1795 e del suo fallimento.

Abstract

Antonio Veneziano (1543-1593), famoso per avere scritto “Celia”, un canzoniere sul modello di quello petrarchesco, compose i capitoli, “Puttanismu”, “Curnaria”, “Narratione di un poviru carceratu” e “Per le signore monache claustrale”. Per queste composizioni, databili con buona probabilità agli anni della maturità, non solo viene utilizzato il dialetto siciliano, più icastico, violento, espressionistico del volgare, ma anche il capitolo in terza rima, la forma metrica per eccellenza delle satire e delle rime burlesche. Veneziano dà sfogo, senza remore e falsi pudori, alla sua insofferenza nei confronti di donne puttane, di uomini cornuti, di suore e frati corrotti, trasferendo «in opere d’inchiostro» la mordacità, l’irruenza e la prepotenza che, secondo i suoi biografi, sarebbero state le note caratterizzanti un’esistenza vissuta sempre sopra le righe. L’opzione per il genere satirico e la scelta dei temi trattati sono però supportate da solidi studi filosofici e teologici, giuridici e letterari; e quando decide di parlare di ‘puttanesimo’ ha come modelli inimitabili, e sia pure irraggiungibili, l’Aretino, il Berni di alcuni sonetti e capitoli.

Abstract

I mostri che popolano le pagine di due voluminosi trattati tardo cinquecenteschi, "Il serraglio de gli stupori del mondo" di Tomaso Garzoni (1613) e la "Monstorum Historia cum paralipomenis historiae ommium animalium" di Ulisse Aldrovandi (1642), sono tanti e vari, arrivano dal mondo animale, vegetale e cosmico, sono descritti con grande attenzione, la loro origine ed esistenza sono oggetto di causidiche discussioni, la loro veridicità storica è indagata e registrata in modo pseudorigoroso, ma hanno irrimediabilmente perduto il fascino del fantastico e dell’inverosimile che li aveva accompagnati per secoli. Bisognerà sconfinare nell’ambito della produzione fiabesca e rileggere una famosa raccolta napoletana di fiabe, "Lo cunto de li cunti" di Giambattista Basile, che cronologicamente si situa tra la pubblicazione dei trattati (1632-1636), per ritrovare negli orchi, le streghe e i draghi che la popolano, l’immutato incanto dello straordinario, dello strano, dello spaventoso.

33. Resilience and Frailty in People Living With HIV During the COVID Era: Two Complementary Constructs Associated With Health-Related Quality of Life.

Abstract

Background: Resilience is defined as an individual's positive adaptation to stressors. The COVID-19 pandemic represents a generalized stressor which may affect differently people living with HIV (PLWH). The objective of this study was to characterize resilience in PLWH with particular regarding the identification of frailty-resilience phenotypes, which may differently affect health-related quality of life (HR-QoL)., Methods: This was an observational study of PLWH attending Modena HIV Metabolic Clinic. Frailty was assessed in 2019, before the onset of the COVID-19 pandemic by using 37-Item frailty index ranging from 0 to 1. The frailty index score was categorized as fit (<0.25) or frail (>0.25). In January 2021, PLWH were offered to complete a set of electronic questionnaires including the CD-RISC-25 for resilience and EQ-5D5L and SF-36 for HR-QoL. Resilience was defined as CD-RISC-25 score >75.7 (ranging from 0 to 100)., Results: Of 800 PLWH reached by mail, 575 (72%) completed the questionnaires. The median age and HIV duration were 54.5 and 24.3 years, respectively. Impaired resilience was associated with loneliness [odds ratio (OR = 2.39; 1.20 to 4.76, P < 0.001)]. Predictors for EQ-5D5L <89.7% were the phenotypes "frail/nonresilient" [OR = 5.21, 95% confidence interval (CI): 2.62 to 10.33] and "fit/nonresilient" (OR = 5.48, 95% CI: 2.8 to 10.74). Predictors for SF-36 <64.40 were the phenotypes "frail/nonresilient" (OR = 7.43, 95% CI: 2.57 to 21.22) and "fit/nonresilient" (OR = 6.27, 95% CI: 2.17 to 18.16). Both models were corrected for age, sex, HIV duration, and nadir CD4., Conclusions: Resilience characterizes the well-being of PLWH during the COVID-19 crisis. 34. Metabolic-Associated Fatty Liver Disease Is Highly Prevalent in the Postacute COVID Syndrome.

Abstract

Background: A proposal has recently been advanced to change the traditional definition of nonalcoholic fatty liver disease to metabolic-associated fatty liver disease (MAFLD), to reflect the cluster of metabolic abnormalities that may be more closely associated with cardiovascular risk. Long coronavirus disease 2019 (COVID-19) is a smoldering inflammatory condition, characterized by several symptom clusters. This study aims to determine the prevalence of MAFLD in patients with postacute COVID syndrome (PACS) and its association with other PACS-cluster phenotypes., Methods: We included 235 patients observed at a single university outpatient clinic. The diagnosis of PACS was based on ≥1 cluster of symptoms: respiratory, neurocognitive, musculoskeletal, psychological, sensory, and dermatological. The outcome was prevalence of MAFLD detected by transient elastography during the first postdischarge follow-up outpatient visit. The prevalence of MAFLD at the time of hospital admission was calculated retrospectively using the hepatic steatosis index., Results: Of 235 patients, 162 (69%) were men (median age 61). The prevalence of MAFLD was 55.3% at follow-up and 37.3% on admission ( P  < .001). Insulin resistance (odds ratio [OR] = 1.5; 95% confidence interval [CI], 1.14-1.96), body mass index (OR = 1.14; 95% CI, 1.04-1.24), and the metabolic syndrome (OR = 2.54; 95% CI, 1.13-5.68) were independent predictors of MAFLD. The number of PACS clusters was inversely associated with MAFLD (OR = 0.86; 95% CI, .76-0.97). Thirty-one patients (13.2%) had MAFLD with no other associated PACS clusters. All correlations between MAFLD and other PACS clusters were weak., Conclusions: Metabolic-associated fatty liver disease was highly prevalent after hospital discharge and may represent a specific PACS-cluster phenotype, with potential long-term metabolic and cardiovascular health implications., (© The Author(s) 2022. Abstract

Objectives: This research aims to study the relationship between personality traits and periodontal clinical outcomes by taking into account the level of anxiety and depression, periodontal health and oral hygiene behaviour of patients affected with gingivitis or moderate periodontitis requiring periodontal therapy., Material and Methods: The periodontal data of 40 systemically healthy patients affected by gingivitis or moderate periodontitis were collected at baseline and 18 months later. The psychological variables, dental awareness and adherence intent of the patients were assessed through questionnaires, and only those patients that exhibited a higher degree of compliance were included in the study. The personality traits (cluster A: paranoid, schizoid, schizotypal; cluster B: borderline, antisocial, narcissistic, and histrionic; cluster C: avoidant, dependent, and obsessive-compulsive) and the level of anxiety and depression of the patients were assessed. Patients were instructed with oral hygiene measures and were treated with periodontal therapy., Results: Clusters A and B showed a consistent tendency for reduced levels of oral hygiene (increased full-mouth plaque score - FMPS). The results from cluster B were found to be significantly related to deep periodontal pockets at baseline. On the contrary, cluster C seemed to be linked to clinically better indices, particularly in terms of full-mouth-bleeding-score and pocket depth, both at baseline and 18 months later. The results collected from clusters B and C were directly correlated with anxiety, depression and FMPS. Moreover, anxiety was directly correlated with the patient's need for professional oral-care., Conclusions: Personality traits appear to play a significant role in determining the therapeutic outcomes of periodontal therapy in themselves. Thus, it is ideal for several important psychological, affective or behavioural factors to be associated with various personality traits so as to orient the outcome of periodontal therapy.

Abstract

Unlabelled: Impact of shiftwork on sleep and psychological health., Objective: The purpose of the present study was to evaluate the impact of a rapid shiftwork system on sleep parameters and on psychopathological status among a sample of nurses. We hypothesize that in this type of shiftwork system, the rapid and continuous rotation of shifts doesn't lead to a lasting alteration of circadian rhythm but to a transitory increase of psychological disturbances after the night shift., Methods: The population studied was the permanently employed staff of a public hospital in Modena. We recruited fifty-five workers. Participation was entirely voluntary. Every subject was given a self-administered questionnaire divided into three sections. Section I consisting in a base-line evaluation (sociodemographic data, use of psychotropic substances, General Health Questionnaire and Maslash Burnout Inventory). Sections II and III providing an evaluation of sleep and psychopathological parameters (Sleep Diary and Symptom Questionnaire) in two different moments during the shiftwork: respectively after the night following the night shift (corresponding to the end of the shift cycle), and after the night following the last day off (corresponding to the beginning of a new shift cycle)., Results: There were no significative differences in sleep parameters between the two moments of assessment. Regarding psychopathological features we found a reduction of SQ-Somatization (P = 0.002), SQ-Anxiety (p = 0.006) and SQ-Hostility (p = 0.075) subscale score at the end of the shift cycle., Conclusions: We confirm this type of shiftwork as not causing alterations of sleep parameters. We explain the improvement in psychopathological symptoms noted at the end of the shift cycle with the favourable psychological attitude of workers in the perspective of time off. The present data suggest that the psychological attitude of workers may play an important role in the clinical expression of psychopathological features.

Abstract

Susceptibility to penicillin, vancomycin, imipenem, streptomycin, kanamycin and gentamicin was tested in 130 clinical isolates of Enterococcus spp. by an agar dilution method. Penicillin resistance (MIC > 8 mg/l) was only observed among strains of Enterococcus faecium and Enterococcus raffinosus. Thirty-nine percent of the penicillin-resistant enterococci showed low-level resistance to at least one of the three aminoglycosides tested (gentamicin, kanamycin and streptomycin). Six Enterococcus strains (5 E. faecium and 1 E. raffinosus) with low-level resistance to gentamicin and different MICs for penicillin were tested for antibiotic synergy using time-killing curves. When penicillin concentrations equal to or higher than the MICs were used, synergism was established, even when highly penicillin-resistant strains (MIC > 200 mg/l) were tested. No synergy was observed when penicillin concentrations were below the MICs.

Abstract

Reflex sympathetic dystrophy is a well known syndrome in adults, but often misdiagnosed in children. We describe a case occurred in a 12 years old girl. Psychological factors played a major role in the mechanism of the disease. Early recognition and prompt management allow complete recovery without sequelae.
