Abstract

Synthesis and in vitro testing of a prodrug designed for the controlled delivery of the anticancer drug camptothecin within pancreatic cancer cells are reported. Our study reveals a non-conventional pharmacokinetic release characterized by an exponential pattern before reaching the half-life (t1/2) and a linear pattern thereafter. The release mechanism was triggered either by hydrolytic enzymes and/or by the acid microenvironment of cancer cells.

Abstract

Berries represent an important source of bioactive compounds, among which, (poly)phenols are the main constituents. (Poly)phenols have been shown to have important biological activities and in particular, to take a relevant role in mediating the modulation of gut microbiota and human health. Gut microbiota constitute significant players in the metabolism of ingested (poly)phenols and consequently in their biological effects. However, the evidence regarding the mutual contribution between (poly)phenol intake and gut microbiota activity in humans is still unclear. The aim of the current systematic review was to collect and assess results from randomized clinical trials addressing the role of berries in the modulation of the gut microbiota composition and how this can contribute to the metabolism of berry (poly)phenols. When information was available, the interaction of berry-gut microbiota on human health was evaluated. A total of 16 trials were included and analyzed. Overall, berries and their (poly)phenols could represent a driver for the modulation of intestinal microbes. The gut microbiota catabolize (poly)phenols by producing bioactive metabolites which can positively affect both the composition of commensal microbiota and also the host health.

Abstract

Introduction: Over the years, disease registers have been increasingly considered a source of reliable and valuable population studies. However, the validity and reliability of data from registers may be limited by missing data, selection bias or data quality not adequately evaluated or checked. This study reports the analysis of the consistency and completeness of the data in the Italian Multiple Sclerosis and Related Disorders Register. Methods: The Register collects, through a standardized Web-based Application, unique patients. Data are exported bimonthly and evaluated to assess the updating and completeness, and to check the quality and consistency. Eight clinical indicators are evaluated. Results: The Register counts 77,628 patients registered by 126 centres. The number of centres has increased over time, as their capacity to collect patients. The percentages of updated patients (with at least one visit in the last 24 months) have increased from 33% (enrolment period 2000–2015) to 60% (enrolment period 2016–2022). In the cohort of patients registered after 2016, there were ≥ 75% updated patients in 30% of the small centres (33), in 9% of the medium centres (11), and in all the large centres (2). Clinical indicators show significant improvement for the active patients, expanded disability status scale every 6 months or once every 12 months, visits every 6 months, first visit within 1 year and MRI every 12 months. Conclusions: Data from disease registers provide guidance for evidence-based health policies and research, so methods and strategies ensuring their quality and reliability are crucial and have several potential applications.

However, the validity and reliability of data from registers may be limited by missing data, selection bias or data quality not adequately evaluated or checked.This study reports the analysis of the consistency and completeness of the data in the Italian Multiple Sclerosis and Related Disorders Register.MethodsThe Register collects, through a standardized Web-based Application, unique patients.Data are exported bimonthly and evaluated to assess the updating and completeness, and to check the quality and consistency. Eight clinical indicators are evaluated.ResultsThe Register counts 77,628 patients registered by 126 centres. The number of centres has increased over time, as their capacity to collect patients.The percentages of updated patients (with at least one visit in the last 24 months) have increased from 33% (enrolment period 2000-2015) to 60% (enrolment period 2016-2022). In the cohort of patients registered after 2016, there were >= 75% updated patients in 30% of the small centres (33), in 9% of the medium centres (11), and in all the large centres (2).Clinical indicators show significant improvement for the active patients, expanded disability status scale every 6 months or once every 12 months, visits every 6 months, first visit within 1 year and MRI every 12 months.ConclusionsData from disease registers provide guidance for evidence-based health policies and research, so methods and strategies ensuring their quality and reliability are crucial and have several potential applications.

Abstract

Background A Covid-19 outbreak developed in Lombardy, Veneto and Emilia-Romagna (Italy) at the end of February 2020. Fear of an imminent saturation of available ICU beds generated the notion that rationing of intensive care resources could have been necessary. Results In order to evaluate the impact of Covid-19 on the ICU capacity to manage critically ill patients, we performed a retrospective analysis of the first 2 weeks of the outbreak (February 24–March 8). Data were collected from regional registries and from a case report form sent to participating sites. ICU beds increased from 1545 to 1989 (28.7%), and patients receiving respiratory support outside the ICU increased from 4 (0.6%) to 260 (37.0%). Patients receiving respiratory support outside the ICU were significantly older [65 vs. 77 years], had more cerebrovascular (5.8 vs. 13.1%) and renal (5.3 vs. 10.0%) comorbidities and less obesity (31.4 vs. 15.5%) than patients admitted to the ICU. PaO2/FiO2 ratio, respiratory rate and arterial pH were higher [165 vs. 244; 20 vs. 24 breath/min; 7.40 vs. 7.46] and PaCO2 and base excess were lower [34 vs. 42 mmHg; 0.60 vs. 1.30] in patients receiving respiratory support outside the ICU than in patients admitted to the ICU, respectively. Conclusions Increase in ICU beds and use of out-of-ICU respiratory support allowed effective management of the first 14 days of the Covid-19 outbreak, avoiding resource rationing.

Abstract

Background and purpose: In relapsing-remitting multiple sclerosis patients (RRMS) disability progressively accumulates over time. To compare the cumulative probability of 6-month confirmed disability-worsening events using a fixed baseline or a roving Expanded Disability Status Scale (EDSS) reference, in a real-world setting. Methods: A cohort of 7964 RRMS patients followed for 2 or more years, with EDSS scores recorded every 6 months, was selected from the Italian Multiple Sclerosis Register. The overall probability of confirmed disability-worsening events and of confirmed disability-worsening events unrelated to relapse was evaluated using as reference a fixed baseline EDSS score or a roving EDSS score in which the increase had to be separated from the last EDSS assessment by at least 6 or 12 months. Results: Using a fixed baseline EDSS reference, the cumulative probability of 6-year overall confirmed disability-worsening events was 33.2%, and that of events unrelated to relapse was 10.9% (33% of overall confirmed disability-worsening events). Using a roving EDSS, the proportions were respectively 35.2% and 21.3% (61% of overall confirmed disability-worsening events). Conclusions: In a real-world setting, roving EDSS reference scores appear to be more sensitive for detecting confirmed disability-worsening events unrelated to relapse in RRMS patients.

Abstract

The contribution of oxic methane production to greenhouse gas emissions from lakes is globally relevant, yet uncertainties remain about the levels up to which methanogenesis can counterbalance methanotrophy by leading to CH4 oversaturation in productive surface waters. Here, we explored the biogeochemical and microbial community variation patterns in a meromictic soda lake, in the East African Rift Valley (Kenya), showing an extraordinarily high concentration of methane in oxic waters (up to 156 µmol L−1). Vertical profiles of dissolved gases and their isotopic signature indicated a biogenic origin of CH4. A bloom of Oxyphotobacteria co-occurred with abundant hydrogenotrophic and acetoclastic methanogens, mostly found within suspended aggregates promoting the interactions between Bacteria, Cyanobacteria, and Archaea. Moreover, aggregate sedimentation appeared critical in connecting the lake compartments through biomass and organic matter transfer. Our findings provide insights into understanding how hydrogeochemical features of a meromictic soda lake, the origin of carbon sources, and the microbial community profiles, could promote methane oversaturation and production up to exceptionally high rates., Fazi et al. report on an extraordinarily high biogenic methane concentration detected in the surface water of Lake Sonachi, Kenya. Using gas chromatography and microbiome profiling, they determine that these high concentrations are associated with cyanobacterial blooms and help provide insight to methanogenesis in meromictic soda lakes.

Abstract

A severe boron pollution has been recognized in the shallow aquifer of Isola di Castelluccio (central-eastern Tuscany, Italy) since 2009. Previous investigations showed that the high boron concentrations (up to 57 mg L-1) were related to an anthropogenic source. It was likely due to current and/or past industrial and artisanal activities, which were using B-compounds during their productive cycles. However, a univocal source of boron has so far not been recognized. In this work, we present and discuss new geochemical and isotopic data related to an extensive campaign (June 2016) carried out in 33 domestic well and piezometer waters. In addition, the results from a three-weekly to monthly monitoring survey (from August 2014 to December 2019) on selected waters were used to recognize signals in response to dry and wet periods or related to the presence of the recently installed hydraulic barrier. To verify the efficiency of the hydraulic barrier a numerical model of the shallow aquifer was performed by using the MODFLOW computer program. This study has allowed to identify where the source of contamination is likely located. This implies that specific investigations can now be carried out to definitively remove the anthropogenic source of boron.

Abstract

Background: Multiple sclerosis (MS) is characterized by phenotypical heterogeneity, partly resulting from demographic and environmental risk factors. Socio-economic factors and the characteristics of local MS facilities might also play a part. Methods: This study included patients with a confirmed MS diagnosis enrolled in the Italian MS and Related Disorders Register in 2000–2021. Patients at first visit were classified as having a clinically isolated syndrome (CIS), relapsing–remitting (RR), primary progressive (PP), progressive-relapsing (PR), or secondary progressive MS (SP). Demographic and clinical characteristics were analyzed, with centers’ characteristics, geographic macro-areas, and Deprivation Index. We computed the odds ratios (OR) for CIS, PP/PR, and SP phenotypes, compared to the RR, using multivariate, multinomial, mixed effects logistic regression models. Results: In all 35,243 patients from 106 centers were included. The OR of presenting more advanced MS phenotypes than the RR phenotype at first visit significantly diminished in relation to calendar period. Females were at a significantly lower risk of a PP/PR or SP phenotype. Older age was associated with CIS, PP/PR, and SP. The risk of a longer interval between disease onset and first visit was lower for the CIS phenotype, but higher for PP/PR and SP. The probability of SP at first visit was greater in the South of Italy. Discussion: Differences in the phenotype of MS patients first seen in Italian centers can be only partly explained by differences in the centers’ characteristics. The demographic and socio-economic characteristics of MS patients seem to be the main determinants of the phenotypes at first referral.

Abstract

Wetlands are hotspots of CH4 emissions to the atmosphere, mainly sustained by microbial decomposition of organic matter in anoxic sediments. Several knowledge gaps exist on how environmental drivers shape CH4 emissions from these ecosystems, posing challenges in upscaling efforts to estimate global emissions from waterbodies. In this work, CH4 and CO2 diffusive fluxes, along with chemical and isotopic composition of dissolved ionic and gaseous species, were determined from two wetlands of Tuscany (Italy): (i) Porta Lake, a small wetland largely invaded by Phragmites australis reeds experiencing reed die-back syndrome, and (ii) Massaciuccoli Lake, a wide marsh area including open-water basins and channels affected by seawater intrusion and eutrophication. Both wetlands were recognized as net sources of CH4 to the atmosphere. Our data show that the magnitude of CH4 diffusive emission was controlled by CH4 production and consumption rates, being mostly governed by (i) water temperature and availability of labile carbon substrates and (ii) water column depth, wind exposure and dissolved O2 contents, respectively. This evidence suggests that the highest CH4 diffusive fluxes were sustained by reed beds, providing a large availability of organic matter supporting acetoclastic methanogenesis, with relevant implications for global carbon budget and future climate models.

Abstract

Volcanoes are currently to be regarded as natural sources of air pollutants. Climatic and environmental forcing of large volcanic eruptions are well known, although gases emitted through passive degassing during periods of quiescence or hydrothermal activity can also be highly dangerous for the environment and public health. Based on compositional and isotopic data, a survey on the spatial distribution in air of the main volatile compounds of carbon (CO2 and CH4) and sulfur (H2S and SO2) emitted from the fumarolic field of Pisciarelli (Campi Flegrei, Pozzuoli, Naples), a hydrothermal area where degassing activity has visibly increased since 2009, was carried out. The main goals of this study were (i) to evaluate the impact on air quality of these natural manifestations and (ii) inquire into the behavior of the selected chemical species once released in air, and their possible use as tracers to distinguish natural and anthropogenic sources. Keeling plot analysis of CO2 and CH4 isotopes revealed that the hydrothermal area acts as a net source of CO2 in air, whilst CH4 originated mainly from anthropogenic sources. Approaching the urban area, anthropogenic sources of CO2 increased and, at distances greater than 800 m from the Pisciarelli field, they prevailed over the hydrothermal signal. While hydrothermal CO2 simply mixed with that in the atmospheric background, H2S was possibly affected by oxidation processes. Therefore, SO2 measured in the air near the hydrothermal emissions had a secondary origin, i.e. generated by oxidation of hydrothermal H2S. Anthropogenic SO2 was recognized only in the furthest measurement site from Pisciarelli. Finally, in the proximity of a geothermal well, whose drilling was in progress during our field campaign, the H2S concentrations have reached values up to 3 orders of magnitude higher than the urban background, claiming the attention of the local authorities.

Abstract

The past decade has seen extraordinary increase in worldwide availability of and access to several large multiple sclerosis (MS) databases and registries. MS registries represent powerful tools to provide meaningful information on the burden, natural history, and long-term safety and effectiveness of treatments. Moreover, patients, physicians, industry, and policy makers have an active interest in real-world observational studies based on register data, as they have the potential to answer the questions that are most relevant to daily treatment decision-making. In 2014, the Italian MS Foundation, in collaboration with the Italian MS clinical centers, promoted and funded the creation of the Italian MS Register, a project in continuity with the existing Italian MS Database Network set up from 2001. Main objective of the Italian MS Register is to create an organized multicenter structure to collect data of all MS patients for better defining the disease epidemiology, improving quality of care, and promoting research projects in high-priority areas. The aim of this article is to present the current framework and network of the Italian MS register, including the methodology used to improve the quality of data collection and to facilitate the exchange of data and the collaboration among national and international groups.

Abstract

Background and aim: An increase of vaccine hesitancy has spread worldwide and lead to reduction in coverage rates. The trivalent Measles-Mumps-Rubella (MMR) vaccine has been one of the most targeted leading due to possible (but never proven) adverse effects. This resulted in an increase of measles cases. The aim of the study is to analyze the 24-months coverage rates for the MMR vaccine in Emilia-Romagna Region (RER) between 2007 and 2018 and to correlate any significant changes to index events. Methods: Official aggregate data on vaccination coverage at 24-month provided by the RER and the Italian Ministry of health were analyzed and discussed. Results: From 2012 to 2015 a significant reduction in vaccination rates has been registered. In the following years an increase was recorded temporarily related to national and regional laws. Conclusion: The mandatory vaccination strategies seem to be in the short period effective in RER to counteract the growing population neegative attitudes towards vaccination and mitigate vaccine hesitancy.

Abstract

La coltivazione del pero in Emilia-Romagna ha una lunga storia che nasce dalla vocazionalità regionale per questa coltura. Questo stretto rapporto ha permesso lo sviluppo di un ricco germoplasma locale. La riscoperta delle antiche varietà di germoplasma non deve essere vista solo in riferimento al loro indubbio valore culturale, legato agli usi e alle tradizioni delle popolazioni che hanno coltivato queste antiche varietà. Bisogna ricordare che l’importanza delle cultivar locali risiede anche nell’elevata variabilità genetica che le caratterizza e ha permesso loro di adattarsi alle specifiche condizioni ambientali presenti in regione. Pur essendo spesso prive delle caratteristiche qualitative delle moderne pere che dominano i mercati, queste antiche varietà di pero dell’Emilia-Romagna meritano di essere preservate dall’erosione genetica anche per i caratteri che possono essere reintrodotti nei programmi di miglioramento genetico quali, ad esempio, la precocità di raccolta, la serbevolezza, la polpa rossa del frutto, la resilienza a stress ambientali in anni caratterizzati da profondi mutamenti climatici.

Abstract

With this study a nine-year hiatus (May 2010-April 2019) in the quantification of the CO2 content of Lago Albano by our working group has been resolved through the acquisition and analysis from two new field campaigns. Based on a CO2 budget analysis the dynamics of CO2 degassing throughout the past thirty years (1989-2019) is detailed and quantified. The decreasing CO2 content (expressed as dissolved inorganic carbon, DIC) in the lake, since the co-seismic CO2 input during the 1989-1990 seismic swarm beneath Colli Albani volcano, was accelerated at lake bottom layers (-140 m to bottom, near -160 m) in the 4-5 years after the swarm, continued afterwards at lower depths (-125 to -95 m), and seems to have reached steady-state conditions during recent years. The peculiar lake basin morphology has control on the degassing dynamics. The low chemical gradients detected during the April 2019 survey have induced near-zero degassing conditions, and arguably stopped the gas-self lifting process: Lago Albano might not become CO2-free in the future. This finding has implications for gas hazard when the next seismic swarm will hit the area. The updated degassing model also takes into account the lake level drop, and hence the volume decrease of Lago Albano, caused by excessive well pumping for anthropic purposes. This volume decrease appears to have a destabilizing effect on the degassing dynamics, which renders Lago Albano’s gas release less predictable in the future. Enhanced gas surveys (high-frequency and fine-scale spatial measurements) are needed to shed light on how Lago Albano degasses in this quiescent stage during the Anthropocene. A submersible infra-red detector to directly measure in-lake dissolved CO2 concentrations, applied satisfactorily during this study, is an adapted instrument to do so.

Abstract

Little is known about the quantity and quality of dissolved organic matter (DOM) in waters from continental geothermal systems, with only a few reports available from the Yellowstone US National Park. In this study, we explored the chemodiversity of DOM in water samples collected from two geothermal hot springs from the Kenyan East African Rift Valley, a region extremely rich in fumaroles, geysers, and spouting springs, located in close proximity to volcanic lakes. The DOM characterization included in-depth assessments performed by negative electrospray ionization Fourier-transform ion cyclotron resonance mass spectrometry (FT-ICR-MS). Reduced, saturated and little aromatic DOM compounds were dominant in the hot spring waters collected from either the Ol Njorowa gorge (ON) or the south shore of the soda-saline Lake Elementaita (ELM). Oxygen-poor and sulfur-bearing DOM molecules prevailed in ON, probably reflecting abiotic sulfurization from sulfide-rich geofluids. Nitrogen-bearing aliphatic and protein-like molecules were abundant in ELM, possibly perfusing through the organic-rich sediments of the adjacent Lake Elementaita. Notably, the heat-altered DOM of ancient autochthonous derivation could represent an overlooked source of aliphatic organic carbon for connected lentic environments, with a potential direct impact on nutrient cycling in lakes that receive geothermal water inputs.

Abstract

The terrestrial environment is an important contributor of microplastics (MPs) to the oceans. Urban streams, strictly interwoven in the city network and to the MPs’ terrestrial source, have a relevant impact on the MP budget of large rivers and, in turn, marine areas. We investigated the fluxes (items/day) of MPs and natural fibers of Mugnone Creek, a small stream crossing the highly urbanized landscape of Florence (Italy) and ending in the Arno River (and eventually to the Tyrrhenian Sea). Measurements were done in dry and wet seasons for two years (2019–2020); stream sediments were also collected in 2019. The highest loads of anthropogenic particles were observed in the 2019 wet season (109 items/day) at the creek outlet. The number of items in sediments increased from upstream (500 items/kg) to urban sites (1540 items/kg). Fibers were the dominant shape class; they were mostly cellulosic in composition. Among synthetic items, fragments of butadiene-styrene (SBR), indicative of tire wear, were observed. Domestic wastewater discharge and vehicular traffic are important sources of pollution for Mugnone Creek, especially during rain events. The study of small creeks is of pivotal importance to limit the availability of MPs in the environment.

Abstract

Red wood ants are ecologically important species in Europe that form large colonies. Their nest mounds are characterized by stable microclimatic conditions, that are favourable to the development of rich invertebrate and microbial communities. Through their respiration processes, all these inhabitants contribute to the total gas emissions of the mounds. Quantifications of red wood ant mounds CO2 production are only available from Northern and Central Europe, and the Alps, where these ants are common. During the second half of the last century some species were transplanted from the Alps to southernmost sites, where they were not present, to be employed as biocontrol agents. No information on the contribution of these low-latitudes populations to the local forest CO2 production is available. The microbial communities living within red wood ant mounds are also poorly known. In this study, we investigated the CO2 gas emissions and the microbiome of the mounds of an introduced population of the red wood ant Formica paralugubris in a Southern Europe montane forest. We found that ant mounds produced more CO2 than the forest soil, and that their CO2 efflux as well as internal concentration were higher during summer than winter, with a lighter CO2 carbon isotopic signature in summer than winter, likely due to an increased ant activity. Moreover, the top part of the mound was characterised by higher CO2 efflux and lower CO2 internal concentration compared to the bottom, probably due to its internal structure and conditions. The isotopic signature of the mound material was similar between summer and winter, suggesting a metabolic similarity of the microbial communities. Also, we estimated the ants' relative contribution to the total mound CO2 production to be 83%, whereas the microbiota CO2 contribution was estimated at 17%. Finally, the mound microbiome composition varied between summer and winter, though no seasonal difference in the diversity indexes or beta-diversity was found. Our results demonstrate the impacts of the introduced red wood ants on the carbon dynamics of the recipient ecosystem.

Abstract

Background: Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage ≥3 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage ≥3 CKD in a large cohort of patients affected by T1DM. Methods: A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage ≥3 CKD (eGFR < 60 mL/min/1.73 m2) or eGFR reduction > 30% from baseline was evaluated. Results: The mean estimated GFR was 98 ± 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR < 90 ml/min/m2 were independent risk factors for stage ≥3 CKD and renal function worsening. When compared to patients with eGFR > 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions: Albuminuria and eGFR reduction represent independent risk factors for incident stage ≥3 CKD in T1DM patients. The simultaneous occurrence of reduced eGFR and albuminuria have a synergistic effect on renal function worsening.

Abstract

Landfills for solid waste disposal release to the atmosphere a large variety of volatile organic compounds (VOCs). Bacterial activity in landfill cover soils can play an important role in mitigating VOC emission. In order to evaluate the effects of degradation processes and characterize VOCs composition in landfill cover soil, gases from 60 sites and along 7 vertical profiles within the cover soil were collected for chemical and isotopic analysis at two undifferentiated urban solid waste disposal sites in Spain: (i) Pinto (Madrid) and (ii) Zurita (Fuerteventura, Canary Islands). The CO2/CH4 ratios and δ13C-CO2 and δ13C-CH4 values were controlled by either oxidation or reduction processes of landfill gas (LFG). VOCs were dominated by aromatics, alkanes and O-substituted compounds, with minor cyclics, terpenes, halogenated and S-substituted compounds. Degradation processes, depending on both (i) waste age and (ii) velocity of the uprising biogas through the soil cover, caused (i) an increase of degradation products (e.g., CO2, O-substituted compounds) and (ii) a decrease of degradable components (e.g., CH4, alkanes, alkylated aromatics, cyclic and S-substituted compounds). Terpenes, halogenated compounds, phenol and furans were unaffected by degradation processes and only depended on waste composition. These results highlight the fundamental role played by microbial activity in mitigating atmospheric emissions of VOCs from landfills. Nevertheless, the recalcitrant behaviour shown by compounds hazardous for health and environment remarks the importance of a correct landfill management that has to be carried out for years after the waste disposal activity is completed, since LFG emissions can persist for long time.

Abstract

Little is known about the quantity and quality of dissolved organic matter (DOM) in waters from continental geothermal systems, with only a few reports available from the Yellowstone US National Park. In this study, we explored the chemodiversity of DOM in water samples collected from two geothermal hot springs from the Kenyan East African Rift Valley, a region extremely rich in fumaroles, geysers, and spouting springs, located in close proximity to volcanic lakes. The DOM characterization included in-depth assessments performed by negative electrospray ionization Fourier-transform ion cyclotron resonance mass spectrometry (FT-ICR-MS). Reduced, saturated and little aromatic DOM compounds were dominant in the hot spring waters collected from either the Ol Njorowa gorge (ON) or the south shore of the soda-saline Lake Elementaita (ELM). Oxygen-poor and sulfur-bearing DOM molecules prevailed in ON, probably reflecting abiotic sulfurization from sulfide-rich geofluids. Nitrogen-bearing aliphatic and protein-like molecules were abundant in ELM, possibly perfusing through the organic-rich sediments of the adjacent Lake Elementaita. Notably, the heat-altered DOM of ancient autochthonous derivation could represent an overlooked source of aliphatic organic carbon for connected lentic environments, with a potential direct impact on nutrient cycling in lakes that receive geothermal water inputs.

Abstract

Background: A Covid-19 outbreak developed in Lombardy, Veneto and Emilia-Romagna (Italy) at the end of February 2020. Fear of an imminent saturation of available ICU beds generated the notion that rationing of intensive care resources could have been necessary. Results: In order to evaluate the impact of Covid-19 on the ICU capacity to manage critically ill patients, we performed a retrospective analysis of the first 2 weeks of the outbreak (February 24–March 8). Data were collected from regional registries and from a case report form sent to participating sites. ICU beds increased from 1545 to 1989 (28.7%), and patients receiving respiratory support outside the ICU increased from 4 (0.6%) to 260 (37.0%). Patients receiving respiratory support outside the ICU were significantly older [65 vs. 77 years], had more cerebrovascular (5.8 vs. 13.1%) and renal (5.3 vs. 10.0%) comorbidities and less obesity (31.4 vs. 15.5%) than patients admitted to the ICU. PaO2/FiO2 ratio, respiratory rate and arterial pH were higher [165 vs. 244; 20 vs. 24 breath/min; 7.40 vs. 7.46] and PaCO2 and base excess were lower [34 vs. 42 mmHg; 0.60 vs. 1.30] in patients receiving respiratory support outside the ICU than in patients admitted to the ICU, respectively. Conclusions: Increase in ICU beds and use of out-of-ICU respiratory support allowed effective management of the first 14 days of the Covid-19 outbreak, avoiding resource rationing.

Abstract

Background and purpose: In relapsing-remitting multiple sclerosis patients (RRMS) disability progressively accumulates over time. To compare the cumulative probability of 6-month confirmed disability-worsening events using a fixed baseline or a roving Expanded Disability Status Scale (EDSS) reference, in a real-world setting. Methods: A cohort of 7964 RRMS patients followed for 2 or more years, with EDSS scores recorded every 6 months, was selected from the Italian Multiple Sclerosis Register. The overall probability of confirmed disability-worsening events and of confirmed disability-worsening events unrelated to relapse was evaluated using as reference a fixed baseline EDSS score or a roving EDSS score in which the increase had to be separated from the last EDSS assessment by at least 6 or 12 months. Results: Using a fixed baseline EDSS reference, the cumulative probability of 6-year overall confirmed disability-worsening events was 33.2%, and that of events unrelated to relapse was 10.9% (33% of overall confirmed disability-worsening events). Using a roving EDSS, the proportions were respectively 35.2% and 21.3% (61% of overall confirmed disability-worsening events). Conclusions: In a real-world setting, roving EDSS reference scores appear to be more sensitive for detecting confirmed disability-worsening events unrelated to relapse in RRMS patients.

Abstract

This study was aimed to test a new methodological approach to carry out measurements of gaseous elemental mercury (GEM) diffusively emitted from soils in hydrothermal-volcanic environments. This method was based on the use of a static closed-chamber (SCC) in combination with a Lumex® RA-915M analyzer that provides GEM measurements in a wide range of concentrations (from 2 to 50,000 ng m-3). Gas samples were collected at fixed time intervals from the SCC positioned on the ground (time-series samples). The Lumex® inlet port was equipped with a three-way Teflon valve allowing the free entrance of air through a carbon trap, in order to: (i) prevent disturbance to the Lumex® operative flow rate (10 L min-1) during the injection of the gas samples from the SCC and (ii) minimize the instability of the baseline signal induced by possible variations of GEM concentrations in air. In the lab, known amounts of GEM, pipetted from a vessel containing an Hg-saturated air in equilibrium with liquid mercury at 27 °C, were injected in the Lumex® via the modified inlet port to construct a calibration curve. The latter was used to calculate the amount of GEM in the SCC (KSCC) from the corresponding GEM concentrations measured by the Lumex® analyzer. The KSCC values of the time-series samples were proportionally increasing with the GEM fluxes (φGEM), thus φGEM values were computed according to the following equation: φGEM=(dKSCC/dt)/A, where A is the basal area of the SCC and dt is the time interval of the time-series sampling. Up to 214 φGEM measurements were carried out at Solfatara crater (Campi Flegrei, southern Italy), a hydrothermally altered tuff cone characterized by an anomalous diffuse soil emission of GEM-rich geogenic gases. The measured φGEM values varied up to 4 orders of magnitude, from 1,296, corresponding to the sensitivity of the method at the selected sampling time interval (1 min), to 1,957,500 ng m-2 day-1, and were consistent with those recently measured in this crater using a different method. In the field, 10 replicates were carried out in 5 selected sites, allowing to demonstrate that the proposed method has a high reproducibility (RDS < 4%). The φGEM and φCO2 values, the latter being measured in the same 214 sites by using the accumulation chamber method, showed a low correlation, although both gases were originated from the same deep source. This suggests that GEM and CO2 soil fluxes are differently affected by environmental parameters, such as soil humidity and temperature, which have a strong effect on the release of GEM from the soil, whereas they do not play a significant role in the diffuse degassing of CO2. The measured fluxes were used to compute the CO2 and GEM total outputs (402 and 5.41 × 10-6 t day-1, respectively) from the study area (92,000 m2) and to construct contour maps showing the spatial distribution of the φCO2 and φGEM values. By modifying the geometry of SCC and the time interval of the sampling series, the proposed method can be applied to the measurements of GEM soil fluxes in other geological systems and man-made environments. Abstract

Mount Etna, located in eastern Sicily, is the largest stratovolcano in Europe and one of the most intensely degassing volcanoes of the world (Allard et al., 1991; Gerlach, 1991). In particular, previous estimates highlighted that Mt Etna emits about 1.6 % of global H2O fluxes from arc volcanism (Aiuppa et al., 2008) and 10 % of global average volcanic emission of CO2 and SO2 (D’Alessandro et al., 1997; Caltabiano et al., 2004). Furthermore, Gauthier and Le Cloarec (1998) underscored that Mt. Etna is an important source of volcanic particles, having a mass flux of particle passively released from the volcano during non-eruptive period estimated between 7 to 23 tons/day (Martin et al., 2008; Calabrese et al., 2011). In general, Etna is considered to be still under evolution and rather ‘friendly’, which, along with the above, makes it a favorable natural laboratory to study volcanic geochemistry. During the previous years, 6 field campaigns and the first 2 editions of the “Etna International Training School of Geochemistry. Science meets practice” were carried out at Mt. Etna and hosted in the Volcanological Observatory of Pizzi Deneri, a picturesque building located at the base of the N-E Crater at an altitude of 2850 m a.s.l, on the rim of the Ellittico caldera. The basic idea was to share scientific knowledge and experiences in a multidisciplinary community, using local resouces with a low-cost organisation. The “Etna International Training School of Geochemistry, 2018. Science meets practice” is addressed to senior graduate students, postdoctoral researchers, fellows, and newly appointed assistant professors, aiming to bring together the next generation of researchers active in studies concerning the geochemistry and the budget of volcanic gases. Direct sampling of high-to-low temperature fumaroles, plume measurement techniques (using CO2/SO2 sensors such as Multi-GAS instruments, MAX-DOAS instruments and UV SO2 cameras, alkaline traps and particle filters) and measurement of diffuse soil gas fluxes of endogenous gases (CO2, Hg0, CH4 and light hydrocarbons) are introduced during brief theoretical sessions before being directly applied in the field. An active participation of the students to the fieldwork is always requested. Hence, the teaching approach includes frontal lesson, practical demonstrations and field applications. “Etna International Training School of Geochemistry, 2017. Science meets practice” was partially funded by EGU. Highlights of the 2016 and 2017 summer school will be presented and a new summer school for 2018 is announced. . .

Abstract

Background: Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage ≥3 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage ≥3 CKD in a large cohort of patients affected by T1DM. Methods: A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage ≥3 CKD (eGFR < 60 mL/min/1.73 m2) or eGFR reduction > 30% from baseline was evaluated. Results: The mean estimated GFR was 98 ± 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR < 90 ml/min/m2 were independent risk factors for stage ≥3 CKD and renal function worsening. When compared to patients with eGFR > 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions: Albuminuria and eGFR reduction represent independent risk factors for incident stage ≥3 CKD in T1DM patients. The simultaneous occurrence of reduced eGFR and albuminuria have a synergistic effect on renal function worsening.

Abstract

The link between carbon dioxide diffuse degassing and its structural control was examined in the Caviahue – Copahue Volcanic Complex, over an area of approximately 10 km 2 that encompasses five hydrothermal sites, namely, Anfiteatro, Copahue town, Las Maquinitas I, Las Maquinitas II and Las Máquinas. Both geochemical and structural approaches were applied in order to investigate the structural scenario releasing hydrothermal fluids in this area. Also, the first isotopic analysis of soil CO 2 degassing in the Andean region is presented. The geochemical analysis shows well-defined CO 2 diffuse degassing structures in Copahue town, Las Maquinitas I, Las Maquinitas II and Las Máquinas hydrothermal sites. Soil diffuse CO 2 is fed by multiple sources: a deep, magmatic – hydrothermal, source, a biogenic source and a mixed source. The structural analysis indicates the presence of an extensional regime that gives place to three structural trends, consisting of NE-SW trending normal faults, NW-SE sinistral strike-slip faults and ENE-WSW right lateral strike-slip faults. The comparison between diffuse degassing structures directions and faults directions suggests that there is a structural control on diffuse degassing. Diffuse CO 2 is released through an early extensional setting, and it is emitted through normal faults, strike-slip faults and through areas with higher structural damage, such as relay areas between fault segments and terminations of individual fault segments. Abstract

Onshore and offshore mud volcanism in the Makran accretionary prism is related to convergence of the Arabian and Eurasian plates. This study describes the chemical and isotopic composition of hydrocarbon-rich fluids from four active on-shore mud volcanoes located along the Makran coast (southern Iran), namely Borborok, Ain, Napag and Sand Mirsuban (Makran coast, southern Iran), as well as Pirgel mud volcano (SE Iran) that is located between the Taftan and Bazman igneous volcanoes. The main aim was to provide insights into the source region (s) of gases and waters discharged from these systems and the secondary processes controlling their chemical features. The four on-shore mud volcanoes emitted CH4-dominated gases, with significant concentrations of C2+ alkanes suggesting a dominant thermogenic origin, as confirmed by their delta C-13-CH4 values. Carbon dioxide was present at relatively low concentrations (0.78-2.33%) with an isotopic signature (delta C-13-CO2 from -34.2 to -11.1 parts per thousand vs. V-PDB) in the range of that typical of thermogenic gases. Hence, the geochemical features of these mud volcanoes point to the occurrence of a deep gas source rich in hydrocarbons, although the occurrence of an exploitable gas reservoir has to be confirmed by geophysical measurements and detailed geostructural surveys. Gas chemistry from Pirgel mud volcano completely differs with respect to that of the previous ones, since the former emits gases dominated by CO2 and showing relatively high R/Ra values (approximate to 1.6), suggesting a significant fluid contribution from the nearby volcanic systems. Moreover, waters from the on-shore mud volcanoes showed a Na-Cl composition, typically associated with mud volcanism, whereas those discharged from Pirgel were Na-HCO3-type and rich in chemical species typical of hydrothermal fluids such as As. Waters from the on-shore volcanoes were characterized by a strong delta O-18-positive shift and high B and Li concentrations, likely indicating clay mineral dehydration and long-term water-rock interaction. Such geochemical features were also shown by the waters from Pirgel, where the high concentrations of B and Li were possibly related to volcanic source. Estimated temperatures for the on-shore mud volcanoes estimated using the Mg-Li and Mg-K geothermometers range from 84 to 165 degrees C, corresponding to 3-7 km depth.
