BACKGROUND: There is no guideline regarding whether patients treated with intravenous corticosteroids for acute severe ulcerative colitis (ASUC) should be monitored in the hospital after transitioning to oral steroids. Abstract

The study of thermal properties of frozen salt solutions representative of ice layers in Jovian moons is crucial to support the JUpiter ICy moons Explorer (JUICE) (ESA) and Europa Clipper (NASA) missions, which will be launched in the upcoming years to make detailed observations of the giant gaseous planet Jupiter and three of its largest moons (Ganymede, Europa, and Callisto), due to the scarcity of experimental measurements. Therefore, we have conducted a set of experiments to measure and study the thermal conductivity of macroscopic frozen salt solutions of particular interest in these regions, including sodium chloride (NaCl), magnesium sulphate (MgSO$_4$), sodium sulphate (Na$_2$SO$_4$), and magnesium chloride (MgCl$_2$). Measurements were performed at atmospheric pressure and temperatures from 0 to -70$^{\circ}$C in a climatic chamber. Temperature and calorimetry were measured during the course of the experiments. An interesting side effect of these measurements is that they served to spot phase changes in the frozen salt solutions, even for very low salt concentrations. A small sample of the liquid salt-water solution was set aside for the calorimetry measurements. These experiments and the measurements of thermal conductivity and calorimetry will be valuable to constrain the chemical composition, physical state, and temperature of the icy crusts of Ganymede, Europa, and Callisto.

Se trata de un paciente de sexo masculino de 89 años de edad, portador de prótesis mitral, intervenido hace 20 años, acude a urgencias por dolor abdominal y vómitos. Se realiza una tomografía computarizada (TC) toraco-abdominal que confirma la presencia de eventración subxifoidea con sufrimiento parietal, requiriendo resección intestinal y eventroplastia con malla intraperitoneal. La eventración subxifoidea constituye una patología infrecuente, que tiene lugar, de forma mayoritaria, en pacientes que han precisado intervenciones del área cardíaca, o hepatobiliopancreática. Su diagnóstico se basa en la clínica y examen físico. Esta patología supone un reto para el cirujano dada la complejidad quirúrgica de la región anatómica donde aparece, pues las estructuras condrocostales adyacentes dificultan la sutura y eventroplastia. En casi todas las series la reparación ha sido electiva, siendo la cirugía de urgencia por incarceración algo anecdótico. La técnica sugerida sería la eventroplastia preperitoneal con malla retroxifoidea irreabsorbible, sin embargo, ésta se encuentra condicionada a los hallazgos intraoperatorios, especialmente a la estrangulación.

Abstract

La hernia inguinal supone la patología de pared abdominal más frecuente hoy en día. En el saco herniario podemos encontrar diferentes estructuras, como grasa preperitoneal, intestino delgado, colon o incluso la vejiga, resultando inusual la presencia de la trompa de Falopio y el ovario. El contenido tubo-ovárico supone un hallazgo frecuente en población pediátrica, mientras que resulta un hecho extremadamente raro en mujeres en edad reproductiva o menopáusica, siendo imprescindible su tratamiento con el fin de evitar complicaciones relacionadas con la fertilidad y la viabilidad de los anejos. Con el objetivo de revisar la bibliografía descrita al respecto dado la infrecuencia que supone este hallazgo en mujeres adultas, se presenta el siguiente caso clínico de forma resumida, con la consiguiente revisión de la literatura disponible. Se trata de una paciente mujer de 50 años, con antecedentes de mioma uterino, intervenida de forma programada de hernioplastia inguinal derecha, observando contenido tubo-ovárico en el saco herniario, en la cual se realiza hernioplastia inguinal según la técnica de Lichtenstein con preservación de anejos sin complicaciones postoperatorias. Como conclusiones finales, consideramos que los hallazgos intraoperatorios de contenido tubo-ovárico en la cirugía de hernia inguinal en mujeres adultas supone un hecho infrecuente, cuyo conocimiento y manejo resulta necesario para el cirujano general, precisando de un índice de alta sospecha con el fin de evitar complicaciones y otorgar el tratamiento quirúrgico más óptimo para el bienestar de los pacientes.

Abstract

Biogas and hydrogen (H2) are breaking through as alternative energy sources in road transport, specifically for heavy-duty vehicles. Until a public network of service stations is deployed for such vehicles, the owners of large fleets will need to build and manage their own refueling facilities. Fleet refueling management and remote monitoring at these sites will become key business needs. This article describes the construction of a prototype system capable of solving those needs. During the design and development process of the prototype, the standard industry protocols involved in these installations have been considered, and the latest expertise in information technology systems has been applied. This prototype has been essential to determine the Strengths, Challenges, Opportunities and Risks (SCOR) of such a system, which is the first step of a more ambitious project. A second stage will involve setting up a pilot study and developing a commercial system that can be widely installed to provide a real solution for the industry.

27. Catálogo de las plantas vasculares de Río Muni (Guinea Ecuatorial): análisis florístico, diversidad, endemicidad y estado de amenaza

Abstract

Se presenta un catálogo actualizado de las plantas vasculares de la región continental de Guinea Ecuatorial (Río Muni). El catálogo es fruto de la compilación de especímenes de herbario (6,850), registros de especies de la literatura botánica (7,985) y bases de datos en línea (10,109 registros de GBIF y 8,897 de RAINBIO). Se elaboró una base de datos de 23,517 registros georreferenciados realizando la actualización nomenclatural y estandarización de los nombres de localidades de todas estas fuentes. El catálogo comprende 2707 taxones (2598 especies, 81 subespecies y 28 variedades) incluidos en 1,020 géneros y 178 familias. El 90.6% de los taxones se consideran nativos, el 1.17% introducidos y el 5.96% naturalizados. Las 10 familias más diversas son Rubiaceae (294 especies), Fabaceae (290), Orchidaceae (168), Poaceae (105), Euphorbiaceae (87), Apocynaceae (85), Cyperaceae (79), Annonaceae (68), Acanthaceae (65) y Melastomataceae (61), que comprenden el 49.22% de las especies de Río Muni. Solo 11 especies pueden considerarse endémicas de Río Muni; este bajo número refleja la ausencia de barreras naturales en el territorio. El número de taxones amenazados (VU, EN y CR) es de 134 (5.02% del total evaluado) de los cuales 43 se encuentran en riesgo de extinción, al estar dentro de las categorías de En Peligro o En Peligro Crítico. Cinco especies restringidas al Golfo de Guinea se consideran amenazadas: tres En Peligro (Grossera angustifolia, Polyscias aequatoguineensis y Rhipidoglossum montealenense), y dos En Peligro Crítico (Asplenium carvalhoanum y Macropodiella uoroensis), por lo que deberían considerarse prioritarias en los planes de gestión y conservación.

29. Matrix Cauterization With Silver Nitrate in the Treatment of Ingrown Toenails in Children: Pilot Study

Abstract

Background: Although partial onychectomy with chemical matricectomy has been described as the treatment of choice, there is sparse evidence in the literature regarding the use of silver nitrate for matricectomy. Our aim is to describe the effectiveness of silver nitrate for matrix cauterization after partial onychectomy. Methods: A prospective observational study was performed on patients with ingrown toenails stage 2–3 who underwent partial onychectomy with silver nitrate chemical matricectomy during 2018–2019 in our institution. All patients were evaluated in the outpatient clinic on the 7th and 30th post-operative day and a telephone evaluation was performed every 6 months after the surgical procedure to date. Results: One hundred and twenty-three patients, who underwent 231 partial onychectomies with silver nitrate chemical matricectomy were included, with a median follow-up of 21 months (interquartile range, 12–29). The procedure had an effectiveness of 95.3%, with only 11 recurrences (4.7%) reported so far on follow-up. Postoperative infections were observed in 4 patients (1.7%). Adverse effects, such as pain and postoperative drainage, were irrelevant in most patients. Conclusions: Silver nitrate matricectomy after partial onychectomy is an effective and safe alternative for the treatment of ingrown toenail in children, with scarce postoperative morbidity and low recurrence rate. Resumen: Antecedentes: Aunque la onicectomía parcial con matricectomía química ha sido descrita como tratamiento de elección en los casos de uñas encarnadas en los dedos de los pies, existe escasa evidencia en la literatura en cuanto al uso de nitrato de plata para matricectomía. Nuestro objetivo es describir la efectividad del nitrato de plata para cauterización de la matriz tras onicectomía parcial. Métodos: Se realizó un estudio observacional prospectivo en pacientes con uñas encarnadas en los dedos de los pies estadio 2-3, sometidos a onicectomía parcial con matricectomía con nitrato de plata durante los años 2018 y 2019 en nuestra institución. Se evaluó a todos los pacientes en la clínica ambulatoria el 7.° y 30.° días postoperatorios, realizándose una evaluación telefónica cada 6 meses, a contar desde la fecha de la intervención quirúrgica. Resultados: Se incluyó a 123 pacientes, a quienes se realizaron 231 onicectomías parciales con matricectomía química con nitrato de plata, con un seguimiento medio de 21 meses (rango intercuartílico: 12-29). El procedimiento tuvo una efectividad del 95,3%, con solo 11 recidivas (4,7%) reportadas hasta el seguimiento. Se observaron infecciones postoperatorias en 4 pacientes (1,7%). Los efectos adversos, tales como dolor y el flujo postoperatorio, fueron irrelevantes en muchos pacientes. Conclusiones: La matricectomía con nitrato de plata tras onicectomía parcial es una alternativa efectiva y segura para el tratamiento de las uñas encarnadas en niños, con escasa morbilidad postoperatoria y baja tasa de recidiva.

Abstract

Abstract Aims The criteria for patients with heart failure (HF) and improved ejection fraction (HFimpEF) are a baseline left ventricular ejection fraction (LVEF) ≤40%, a ≥10‐point increase from baseline LVEF, and a second LVEF measurement >40%. We aimed to (i) assess patients with HF and reduced LVEF (HFrEF) at baseline and compare quality of life (QoL) changes between those that fulfilled and those that did not fulfil the HFimpEF criteria 1 year later and (ii) assess the prognostic role of QoL in patients with HFimpEF. Methods We reviewed data from a prospective registry of real‐world outpatients with HF that were assessed for LVEF and QoL at a first visit to the HF clinic and 1 year later. QoL was evaluated with the Minnesota Living with Heart Failure Questionnaire (MLWHFQ). The primary prognostic endpoint was the composite of all‐cause death or HF hospitalization. Results Baseline and 1‐year LVEF and MLWFQ scores were available for 1040 patients with an initial LVEF ≤40% (mean age, 65.2 ± 11.7 years; 75.9% men). The main aetiology was ischaemic heart disease (52.9%), and patients were mostly in New York heart Association Classes II (71.1%) and III (21.6%). At baseline, the mean LVEF was 28.5% ± 7.3, and the mean MLWHFQ score was 30.2 ± 19.5. After 1 year, the mean LVEF increased to 38.0% ± 12.2, and the MLWHFQ scores improved to 17.4 ± 16.0. Abstract

This paper presents a synchronism system that has been designed to facilitate the generation of ultrasonic images of pieces with complex geometry through the use of industrial robots. Modern robotic manipulators and, more specifically, industrial robotic arms integrated with server computers, sensors and actuators have revolutionized the way automated non-destructive testing is performed. Currently there are commercial industrial robots that have the precision, speed and repetitiveness in their movements that make them suitable for use in numerous non-destructive testing inspections whose designs are carried out by small and medium sized specialized companies. Automatic ultrasonic inspection of complex parts remains one of the most difficult challenges according to the specific and increasingly exigent demands of the markets. The closed configuration of these complex robotic arms makes it difficult to maintain adequate synchronism between the movement of the robot and the acquisition of the data, making it difficult to generate ultrasonic images consistent with the geometry of the part. This is a serious problem in the inspection of aerospace components where high quality is necessary to assess the condition of the inspected component. In this paper, we present an autonomous independent external system that provides control signals to synchronize the ultrasound system with the robot trajectories without needing to access its position in real time. A methodology to obtain the timing pattern for a given part inspected with a given robotic system will also be presented here.

Abstract

Dental development defects (DDDs) are quantitative and/or qualitative alterations produced during odontogenesis that affect both primary and permanent dentition. The etiology remains unknown, being associated with prenatal, perinatal, and postnatal factors. The aims were to identify the possible etiological factors, as well as the prevalence of DDDs in the primary and permanent dentition in a pediatric population. Two hundred twenty-one children between 2 and 15 years of age, patients of the master’s degree in Pediatric Dentistry of the Complutense University of Madrid, were reviewed. DDDs were observed in 60 children. Next, a cross-sectional, case-control study was carried out (60 children in the control group and 60 children in the case group). The parents or guardians completed a questionnaire aimed at identifying associated etiological factors. The prevalence of DDDs in patients attending our master’s program in both dentitions was 27.15%. Otitis, tonsillitis, high fevers, and medication intake stood out as the most relevant postnatal factors among cases and controls. The permanent maxillary right permanent central incisor and the primary mandibular right second molar were the most affected; there were no differences in relation to gender. One out of three children who presented DDDs in the primary dentition also presented DDDs in the permanent dentition. Prenatal and postnatal etiological factors showed a significant relationship with DDD alterations, considered risk factors for DDDs in both dentitions.

Abstract

The paper evaluates the sensitivity of sonic tomography imaging of historic masonry structures to variations in stress level and damage. Six stone masonry walls with different geometries representative of historical typologies were constructed by a professional mason. A laboratory campaign was carried out, subjecting the walls to cyclic uniaxial compression tests. During the test, an automated sonic tomography system was used to inspect the wall under loading cycles of increasing amplitude, which led to obtain tomographic images during loading and compare them under different stress level and damage condition. The use of robotic systems was proved essential to carry out the sonic inspections simultaneously to the compression tests. Results show that sonic wave prop- agation is not only sensitive to damage level, but also to the stress state. Thus, sonic tomography has the potential to be used to measure damage and stress level of masonry components over time, for example during renovation works on an existing construction.

Tal que, a otros muchos, sin duda, con el inicio de los confinamientos decretados para intentar contener la actual pandemia, nos vino a la cabeza El séptimo sello (1957) de Bergman, relacionando los acontecimientos del tiempo en que se desarrolla la película con los que vivíamos y vivimos actualmente: la epidemia de Peste Negra que asolaba Suecia en el siglo XIV de un lado y la pandemia por Covid-19 de otro, sindemias ambas como algunos prefieren llamarlas. Tomando como disculpa la obra de Ingmar Bergman nuestra pretensión es hacer algunas comparaciones entre la peste medieval y la pandemia actual aportando algunas reflexiones y opiniones personales surgidas muchas de ellas de nuestra propia experiencia durante estos largos meses.

Abstract

Background: Clinical trials have demonstrated the efficacy and safety of ustekinumab in Crohn’s disease (CD). However, more data are necessary on the effectiveness of ustekinumab in bio-naïve patients in real-life studies. Objectives: The aim of our study was to evaluate the effectiveness and safety of ustekinumab in patients with CD refractory or intolerant to conventional therapy and without previous exposure to biological drugs. Design: We performed a nationwide, observational, retrospective, multicentre study including patients with CD, in which ustekinumab was used as the first biological drug. Methods: The corticosteroid-free clinical and biological response and remission were analysed at weeks 16, 24, 52 and 72. Clinical remission was defined as Harvey–Bradshaw index ⩽ 4 and biological remission as a faecal calprotectin (FC)
