Cancer-associated cachexia (CAC) is a major contributor to morbidity and mortality in individuals with non-small cell lung cancer. Key features of CAC include alterations in body composition and body weight. Here, we explore the association between body composition and body weight with survival and delineate potential biological processes and mediators that contribute to the development of CAC. Computed tomography-based body composition analysis of 651 individuals in the TRACERx (TRAcking non-small cell lung Cancer Evolution through therapy (Rx)) study suggested that individuals in the bottom 20th percentile of the distribution of skeletal muscle or adipose tissue area at the time of lung cancer diagnosis, had significantly shorter lung cancer-specific survival and overall survival. This finding was validated in 420 individuals in the independent Boston Lung Cancer Study. Individuals classified as having developed CAC according to one or more features at relapse encompassing loss of adipose or muscle tissue, or body mass index-adjusted weight loss were found to have distinct tumor genomic and transcriptomic profiles compared with individuals who did not develop such features. Primary non-small cell lung cancers from individuals who developed CAC were characterized by enrichment of inflammatory signaling and epithelial-mesenchymal transitional pathways, and differentially expressed genes upregulated in these tumors included cancer-testis antigen MAGEA6 and matrix metalloproteinases, such as ADAMTS3. In an exploratory proteomic analysis of circulating putative mediators of cachexia performed in a subset of 110 individuals from TRACERx, a significant association between circulating GDF15 and loss of body weight, skeletal muscle and adipose tissue was identified at relapse, supporting the potential therapeutic relevance of targeting GDF15 in the management of CAC. ispartof: NATURE MEDICINE vol:29 issue:4 ispartof: location:United States status: Published online

Metastatic disease is responsible for the majority of cancer-related deaths1. We report the longitudinal evolutionary analysis of 126 non-small cell lung cancer (NSCLC) tumours from 421 prospectively recruited patients in TRACERx who developed metastatic disease, compared with a control cohort of 144 non-metastatic tumours. In 25% of cases, metastases diverged early, before the last clonal sweep in the primary tumour, and early divergence was enriched for patients who were smokers at the time of initial diagnosis. Simulations suggested that early metastatic divergence more frequently occurred at smaller tumour diameters (less than 8 mm). Single-region primary tumour sampling resulted in 83% of late divergence cases being misclassified as early, highlighting the importance of extensive primary tumour sampling. Polyclonal dissemination, which was associated with extrathoracic disease recurrence, was found in 32% of cases. Primary lymph node disease contributed to metastatic relapse in less than 20% of cases, representing a hallmark of metastatic potential rather than a route to subsequent recurrences/disease progression. Metastasis-seeding subclones exhibited subclonal expansions within primary tumours, probably reflecting positive selection. Lung cancer is the leading cause of cancer-associated mortality worldwide1. Here we analysed 1,644 tumour regions sampled at surgery or during follow-up from the first 421 patients with non-small cell lung cancer prospectively enrolled into the TRACERx study. This project aims to decipher lung cancer evolution and address the primary study endpoint: determining the relationship between intratumour heterogeneity and clinical outcome. In lung adenocarcinoma, mutations in 22 out of 40 common cancer genes were under significant subclonal selection, including classical tumour initiators such as TP53 and KRAS. We defined evolutionary dependencies between drivers, mutational processes and whole genome doubling (WGD) events. Despite patients having a history of smoking, 8% of lung adenocarcinomas lacked evidence of tobacco-induced mutagenesis. These tumours also had similar detection rates for EGFR mutations and for RET, ROS1, ALK and MET oncogenic isoforms compared with tumours in never-smokers, which suggests that they have a similar aetiology and pathogenesis. Large subclonal expansions were associated with positive subclonal selection. Patients with tumours harbouring recent subclonal expansions, on the terminus of a phylogenetic branch, had significantly shorter disease-free survival. Subclonal WGD was detected in 19% of tumours, and 10% of tumours harboured multiple subclonal WGDs in parallel. Subclonal, but not truncal, WGD was associated with shorter disease-free survival. Copy number heterogeneity was associated with extrathoracic relapse within 1 year after surgery. These data demonstrate the importance of clonal expansion, WGD and copy number instability in determining the timing and patterns of relapse in non-small cell lung cancer and provide a comprehensive clinical cancer evolutionary data resource. ispartof: NATURE vol:616 issue:7957 ispartof: location:England status: Published online

TRACERx Consortium - Abstract
Lung adenocarcinomas (LUADs) display a broad histological spectrum from low-grade lepidic tumors through to mid-grade acinar and papillary and high-grade solid, cribriform and micropapillary tumors. How morphology reflects tumor evolution and disease progression is poorly understood. Whole-exome sequencing data generated from 805 primary tumor regions and 121 paired metastatic samples across 248 LUADs from the TRACERx 421 cohort, together with RNA-sequencing data from 463 primary tumor regions, were integrated with detailed whole-tumor and regional histopathological analysis. Tumors with predominantly high-grade patterns showed increased chromosomal complexity, with higher burden of loss of heterozygosity and subclonal somatic copy number alterations. Individual regions in predominantly high-grade pattern tumors exhibited higher proliferation and lower clonal diversity, potentially reflecting large recent subclonal expansions. Co-occurrence of truncal loss of chromosomes 3p and 3q was enriched in predominantly low-/mid-grade tumors, while purely undifferentiated solid-pattern tumors had a higher frequency of truncal arm or focal 3q gains and SMARCA4 gene alterations compared with mixed-pattern tumors with a solid component, suggesting distinct evolutionary trajectories. Clonal evolution analysis revealed that tumors tend to evolve toward higher-grade patterns. The presence of micropapillary pattern and 'tumor spread through air spaces' were associated with intrathoracic recurrence, in contrast to the presence of solid/cribriform patterns, necrosis and preoperative circulating tumor DNA detection, which were associated with extra-thoracic recurrence. International audience; Purpose: An adaptive proton therapy workflow using cone-beam computed tomography (CBCT) isproposed. It consists of an online evaluation of a fast range-corrected dose distribution based on avirtual CT (vCT). This may be followed by more accurate offline dose recalculation on the vCTwhich may trigger a rescan CT (rCT) for replanning.Methods and Materials: The workflow was tested retrospectively on twenty consecutive lung cancerpatients. A diffeomorphic Morphons algorithm was used to generate the lung vCT, by deforming theaverage planning CT (pCT) onto the CBCT. An additional correction step was applied to account foranatomical modifications that cannot be modeled by deformation alone. A set of clinical indicators forreplanning were generated based on water equivalent thickness (WET) and dose statistics, andcompared to those obtained on a rCT. The fast dose approximation consisted of warping the initialplanned dose onto the vCT based on changes in WET. Potential under/over-ranges were assessed asvariation in WET at the target’s distal surface.Results: The range-corrected dose from the vCT reproduced similar clinical indicators as the rCT.The workflow performed well under different clinical scenarios: atelectasis, lung reinflation anddifferent types of tumor response. Between vCT and rCT, we found a difference in the measured 95%percentile of the over-ranges distribution of 3.4±2.7mm. The limitations of the technique consisted ofinherent uncertainties of deformable registration and drawbacks of CBCT imaging. The correctionstep was adequate when gross errors occurred but could not recover subtle anatomical or densitychanges in tumors with complex topology.Conclusions: A proton therapy workflow based on CBCT provided similar clinical indicators as rCTon lung patients with considerable anatomical changes.

31. Toward adaptive radiotherapy for head and neck patients: Uncertainties in dose warping due to the choice of deformable registration algorithm

- Abstract
Purpose: The aims of this work were to evaluate the performance of several deformable image registration (DIR) algorithms implemented in our in-house software (NiftyReg) and the uncertainties inherent to using different algorithms for dose warping. Methods: The authors describe a DIR based adaptive radiotherapy workflow, using CT and conebeam CT (CBCT) imaging. The transformations that mapped the anatomy between the two time points were obtained using four different DIR approaches available in NiftyReg. These included a standard unidirectional algorithm and more sophisticated bidirectional ones that encourage or ensure inverse consistency. The forward (CT-to-CBCT) deformation vector fields (DVFs) were used to propagate the CT Hounsfield units and structures to the daily geometry for "dose of the day" calculations, while the backward (CBCT-to-CT) DVFs were used to remap the dose of the day onto the planning CT (pCT). Data from five head and neck patients were used to evaluate the performance of each implementation based on geometrical matching, physical properties of the DVFs, and similarity between warped dose distributions. Geometrical matching was verified in terms of dice similarity coefficient (DSC), distance transform, false positives, and false negatives. The physical properties of the DVFs were assessed calculating the harmonic energy, determinant of the Jacobian, and inverse consistency error of the transformations. Dose distributions were displayed on the pCT dose space and compared using dose difference (DD), distance to dose difference, and dose volume histograms. Results: All the DIR algorithms gave similar results in terms of geometrical matching, with an average DSC of 0.85 +/- 0.08, but the underlying properties of the DVFs varied in terms of smoothness and inverse consistency. When comparing the doses warped by different algorithms, we found a root mean square DD of 1.9% +/- 0.8% of the prescribed dose (pD) and that an average of 9% +/- 4% of voxels within the treated volume failed a 2% pD DD-test (DD2%-pp). Larger DD2%-pp was found within the high dose gradient (21% +/- 6%) and regions where the CBCT quality was poorer (28% +/- 9%). The differences when estimating the mean and maximum dose delivered to organs-at-risk were up to 2.0% pD and 2.8% pD, respectively. Conclusions: The authors evaluated several DIR algorithms for CT-to-CBCT registrations. In spite of all methods resulting in comparable geometrical matching, the choice of DIR implementation leads to uncertainties in dose warped, particularly in regions of high gradient and/or poor imaging quality. Mestrado em Marketing O presente trabalho de projecto tem como principal objectivo fundamentar as bases para a elaboração de um Guia de Cliente, enquanto ferramenta de comunicação na Meeting Industry, no âmbito da prestação de serviços de gestão de eventos num negócio business-to-business. O guia foi desenhado para o Centro de Congressos de Lisboa, enquanto produto de uma estratégia de marketing e comunicação assente no marketing de relacionamento, focalização no cliente e nas suas necessidades, bem como na gestão efectiva da rede de marketing, fundamental à integração dos intervenientes no processo de organização de eventos e no sucesso do produto final. Este trabalho parte de um enquadramento teórico para uma aplicação estratégica, terminando num plano táctico e respectiva operacionalização. Optou-se por efectuar uma revisão bibliográfica, bem como uma recolha de informação em estudos e relatórios de associações relacionadas com a Meeting Industry, de modo a fundamentar devidamente os antecedentes e respectiva justificação. O Guia do Cliente, enquanto documento final, exigiu uma pesquisa profunda em diversas áreas do conhecimento, num esforço de antecipação das necessidades, dúvidas e questões dos clientes e potenciais clientes do Centro de Congressos de Lisboa. Não sendo o projecto em si, assume-se como o produto do trabalho de projecto agora apresentado. This project report discusses the bases used for the development of a Customer Guide as a tool of communication in the Meeting Industry, under the services marketing and the business-to-business framework. The guide was designed for the Lisbon Congress Centre, as a product of a Marketing Strategy based on communication and relationship marketing, focused on the customers and their needs and on the effective management of the marketing network, relevant for the integration of all the parts in the process of organizing events and for the success of the final product. This report presents a theoretical framework for a strategic application that resulted in a tactical plan and its implementation. There was the need to carry out a literature review and collect information from studies and reports produced by associations related to the Meeting Industry, in order to provide the background and justification for the project. The Customer Guide, as the final document, required careful consideration of various areas of knowledge in an effort to anticipate the needs, concerns and questions from customers and potential customers of the Lisbon Congress Centre. The guide not being the project itself is assumed to be the product of the project hereby reported.
