Abstract The current analytical study is dedicated to the boundary layer regime where heat and mass transfer rates are ruled by natural convection. A rectangular enclosure filled with a combination of an arbitrary buoyancy ratio has an Oseen-linear solution, and the position of Beavers and Joseph's condition is employed at the porous fluid interface. Thermal radiation's interaction with a porous lining influences overall heat transfer in a system. Porous linings and radiation are employed in many applications, such as furnaces, insulation, heat exchangers, solar energy collecting and storage, and heat control in electronics. The effect of slip and radiation is to increase the flow rate because of the reduction in friction at the surface. It indicates the fact that temperature and concentration are rapidly lowering. As the slip parameter and radiation parameter increase, the heat and mass transport increase due to the rise in velocity. The Nusselt and Sherwood numbers reach their maximum when the radiation parameter, Rayleigh number, and slip parameter are increased. The findings of the Nusslet number and Sherwood numbers are related to the finite situations of the slip parameter tending to infinity, the radiation parameter going to zero and the angle 90°.

A good night sleep is required for wellbeing of every organism. Humans though considered as an intelligent among living beings, seldom understand the importance of sleep impulse and ignore it. This ignorance has paved way for many health issues. Impulses are generated for maintenance of body and must be managed at proper time. Sleep impulse is essential for various brain functions, production of various hormone like growth hormone, cognitive power, good immune response etc. Today in highly competitive world human have forgotten his body needs. Earlier handful of persons suffered from sleep disturbance unlike today, where sleep disturbance is very common factor almost in every household. This article is an attempt to pull in the importance of sleep impulse and the ill effects if not attended on time.

BACKGROUND The US Preventive Services Task Force (USPSTF) endorses routine screening for genetic risk of breast and/or ovarian cancer as a component of primary health care. Implementation of this recommendation may prove challenging, especially in clinics serving disadvantaged communities. METHODS The authors tested the feasibility of implementing the USPSTF mandate at a federally qualified health center (FQHC) to identify women who were eligible for genetic counseling (GC). A 12-month usual-care phase was followed by a 12-month intervention phase, during which time cancer genetic risk assessment (CGRA) was systematically performed for all women aged 25 to 69 years who presented for an annual examination. Women who were eligible for GC were recruited to participate in the study. RESULTS After initiating CGRA, 112 women who were eligible for GC consented to study participation, and 56% of them received a referral for GC from their primary care physician. A subgroup of 50 participants were seen by the same primary care physician during both the usual-care and intervention phases. None of these patients was referred for GC during usual care, compared with 64% after the initiation of CGRA (P

A field experiment was conducted at Maddur taluk, Mandya to study the influence of different organic manures along with incineration ash as a source of potassium on growth and yield of maize. The characterization of manures indicated that the major, secondary and micronutrients were higher in the pressmud compost (PMC) compared to FYM. The incinerated ash (IA) was used as an alternative source of potassium fertilizer as it is having higher total K (12.34 %) content. The results revealed that application of PMC irrespective of the level of K applied through IA recorded greater influence on increasing the growth and yield of maize compared to FYM. Application of PMC + 100 per cent NP + 125 per cent K through IA recorded higher plant height (33.54, 205.1 and 222.7 cm) at 30, 60 days after sowing (DAS) and at harvest, respectively and higher number of leaves at 90 DAS in maize. However, at 90 DAS, higher plant height and higher number of leaves plant-1 (8.433, 15.10 and 14.49 at 30 DAS, 90 DAS and at harvest, respectively) was recorded with FYM + 100 per cent NPK (150:75:40). Significantly higher test weight (31.88 g) was recorded with the application of PMC + 100 per cent NP + 125 per cent K through IA. Similarly, significantly higher maize grain (8473, 8956 and 8715 kg ha-1 during 2019-20, 2020-21 and pooled mean, respectively) and stover yield (12125, 11974 and 12050 kg ha-1 during 2019-20, 2020-21 and pooled mean, respectively) were recorded in the treatment supplied with PMC + 100 per cent NP + 125 per cent K through IA. Therefore, application of 100 per cent NP and PMC along with 125 per cent K through IA could be a viable option in increasing maize yield in and around the areas of sugar industry. Background Improving survival and extending the longevity of life for all populations requires timely, robust evidence on local mortality levels and trends. The Global Burden of Disease 2015 Study (GBD 2015) provides a comprehensive assessment of all-cause and cause-specific mortality for 249 causes in 195 countries and territories from 1980 to 2015. These results informed an in-depth investigation of observed and expected mortality patterns based on sociodemographic measures. Methods We estimated all-cause mortality by age, sex, geography, and year using an improved analytical approach originally developed for GBD 2013 and GBD 2010. Improvements included refinements to the estimation of child and adult mortality and corresponding uncertainty, parameter selection for under-5 mortality synthesis by spatiotemporal Gaussian process regression, and sibling history data processing. We also expanded the database of vital registration, survey, and census data to 14 294 geography-year datapoints. For GBD 2015, eight causes, including Ebola virus disease, were added to the previous GBD cause list for mortality. We used six modelling approaches to assess cause-specific mortality, with the Cause of Death Ensemble Model (CODEm) generating estimates for most causes. We used a series of novel analyses to systematically quantify the drivers of trends in mortality across geographies. First, we assessed observed and expected levels and trends of cause-specific mortality as they relate to the Socio-demographic Index (SDI), a summary indicator derived from measures of income per capita, educational attainment, and fertility. Second, we examined factors affecting total mortality patterns through a series of counterfactual scenarios, testing the magnitude by which population growth, population age structures, and epidemiological changes contributed to shifts in mortality. Finally, we attributed changes in life expectancy to changes in cause of death. We documented each step of the GBD 2015 estimation processes, as well as data sources, in accordance with Guidelines for Accurate and Transparent Health Estimates Reporting (GATHER). Findings Globally, life expectancy from birth increased from 61.7 years (95% uncertainty interval 61.4-61.9) in 1980 to 71.8 years (71.5-72.2) in 2015. Several countries in sub-Saharan Africa had very large gains in life expectancy from 2005 to 2015, rebounding from an era of exceedingly high loss of life due to HIV/AIDS. At the same time, many geographies saw life expectancy stagnate or decline, particularly for men and in countries with rising mortality from war or interpersonal violence. From 2005 to 2015, male life expectancy in Syria dropped by 11.3 years (3.7-17.4), to 62.6 years (56.5-70.2). Total deaths increased by 4.1% (2.6-5.6) from 2005 to 2015, rising to 55.8 million (54.9 million to 56.6 million) in 2015, but age-standardised death rates fell by 17.0% (15.8-18.1) during this time, underscoring changes in population growth and shifts in global age structures. The result was similar for non-communicable diseases (NCDs), with total deaths from these causes increasing by 14.1% (12.6-16.0) to 39.8 million (39.2 million to 40.5 million) in 2015, whereas age-standardised rates decreased by 13.1% (11.9-14.3). Globally, this mortality pattern emerged for several NCDs, including several types of cancer, ischaemic heart disease, cirrhosis, and Alzheimer's disease and other dementias. By contrast, both total deaths and age-standardised death rates due to communicable, maternal, neonatal, and nutritional conditions significantly declined from 2005 to 2015, gains largely attributable to decreases in mortality rates due to HIV/AIDS (42.1%, 39.1-44.6), malaria (43.1%, 34.7-51.8), neonatal preterm birth complications (29.8%, 24.8-34.9), and maternal disorders (29.1%, 19.3-37.1). Progress was slower for several causes, such as lower respiratory infections and nutritional deficiencies, whereas deaths increased for others, including dengue and drug use disorders. Age-standardised death rates due to injuries significantly declined from 2005 to 2015, yet interpersonal violence and war claimed increasingly more lives in some regions, particularly in the Middle East. In 2015, rotaviral enteritis (rotavirus) was the leading cause of under-5 deaths due to diarrhoea (146 000 deaths, 118 000-183 000) and pneumococcal pneumonia was the leading cause of under-5 deaths due to lower respiratory infections (393 000 deaths, 228 000-532 000), although pathogen-specific mortality varied by region. Globally, the effects of population growth, ageing, and changes in age-standardised death rates substantially differed by cause. Our analyses on the expected associations between cause-specific mortality and SDI show the regular shifts in cause of death composition and population age structure with rising SDI. Country patterns of premature mortality (measured as years of life lost [YLLs]) and how they differ from the level expected on the basis of SDI alone revealed distinct but highly heterogeneous patterns by region and country or territory. Ischaemic heart disease, stroke, and diabetes were among the leading causes of YLLs in most regions, but in many cases, intraregional results sharply diverged for ratios of observed and expected YLLs based on SDI. Communicable, maternal, neonatal, and nutritional diseases caused the most YLLs throughout sub-Saharan Africa, with observed YLLs far exceeding expected YLLs for countries in which malaria or HIV/AIDS remained the leading causes of early death. Interpretation At the global scale, age-specific mortality has steadily improved over the past 35 years; this pattern of general progress continued in the past decade. Progress has been faster in most countries than expected on the basis of development measured by the SDI. Against this background of progress, some countries have seen falls in life expectancy, and age-standardised death rates for some causes are increasing. Despite progress in reducing age-standardised death rates, population growth and ageing mean that the number of deaths from most non-communicable causes are increasing in most countries, putting increased demands on health systems. While the historical benefits of hydrogen peroxide on wounds and wound healing have recently been questioned, physicians have started to explore its other potential medicinal benefits. We present a case of a 14-year-old girl who presented to our urology unit with macroscopic haematuria and clot retention. Ultrasonography confirmed a large organised intravesical blood clot. Despite numerous attempts, manual bladder irrigation was unsuccessful and caused significant discomfort to the patient. Her clot retention was relieved after 4 irrigation cycles with a 3 % hydrogen peroxide solution. She experienced no complications or side effects post intravesical instillation of hydrogen peroxide.

The US Preventive Services Task Force (USPSTF) endorses routine screening for genetic risk of breast and/or ovarian cancer as a component of primary health care. Implementation of this recommendation may prove challenging, especially in clinics serving disadvantaged communities. The authors tested the feasibility of implementing the USPSTF mandate at a federally qualified health center (FQHC) to identify women who were eligible for genetic counseling (GC). A 12-month usual-care phase was followed by a 12-month intervention phase, during which time cancer genetic risk assessment (CGRA) was systematically performed for all women aged 25 to 69 years who presented for an annual examination. Women who were eligible for GC were recruited to participate in the study. After initiating CGRA, 112 women who were eligible for GC consented to study participation, and 56% of them received a referral for GC from their primary care physician. A subgroup of 50 participants were seen by the same primary care physician during both the usual-care and intervention phases. None of these patients was referred for GC during usual care, compared with 64% after the initiation of CGRA (P .001). Only 16% of referred participants attended a GC session.Implementing USPSTF recommendations for CGRA as a standard component of primary health care in FQHCs is feasible and improves referral of minority women for GC, but more work is needed to understand the beliefs and barriers that prevent many underserved women from accessing cancer genetic services.

Incineration ash is a by-product of sugar industry produced during cogeneration process to produce heat and electrical energy by burning concentrated spent wash at higher temperature in a multi-fuel boiler along with supporting fuel bagasse or coal was used to study its effect on growth and yield of paddy (KRH-4). The present study was conducted in sandy clay soil imposed with graded levels of incineration ash and FYM (5 and 10 t ha-1) in different combinations along with recommended dose of fertilizers during Kharif 2017 consisting of seven treatments, each with three replications and laid out in a randomized complete block design. The incineration ash had high pH and EC with appreciable amounts of plant nutrients i.e., K (14.25%), Ca (1.80%), Mg (2.80%), S (3.94%) and micro nutrients. The results of the experiment indicated that the number of tillers at 60 days after transplanting, yield attributes like number of tillers per hill (17.67), number of productive tillers per hill (12.93), number of filled grains per panicle (317), grain yield (9056 kg ha-1), straw yield (10154 kg ha-1) and uptake of major nutrients like N, P and K by both grain and straw were found to be significantly higher with application of recommended dose of fertilizers along with 10 t ha-1 of incineration ash and 10 t ha-1 of FYM. Thus, combined application of incineration ash and FYM along with recommended dose of fertilizers found to increase the growth, yield and nutrient uptake in paddy. [ABSTRACT FROM AUTHOR]

Medicine (General) ,R5-920 - Abstract
Temperature monitoring is essential for assessing neonates and providing appropriate neonatal thermal care. Thermoneutrality is defined as the environmental temperature range within which the oxygen and metabolic consumptions are minimum to maintain normal body temperature. When neonates are in an environment below thermoneutral temperature, they respond by vasoconstriction to minimise heat losses, followed by a rise in metabolic rate to increase heat production. This condition, physiologically termed cold stress, usually occurs before hypothermia. In addition to standard axillary or rectal temperature monitoring by a thermometer, cold stress can be detected by monitoring peripheral hand or foot temperature, even by hand-touch. However, this simple method remains undervalued and generally recommended only as a second and lesser choice in clinical practice. This review presents the concepts of thermoneutrality and cold stress and highlights the importance of early detection of cold stress before hypothermia occurs. The authors suggest systematic clinical determination of hand and foot temperatures by hand-touch for early detection of physiological cold stress, in addition to monitoring core temperature for detection of established hypothermia, particularly in low-resource settings.

Diseases of the genitourinary system. Urology ,RC870-923 - Abstract
Cytomegalovirus (CMV), an AIDS defining disease, has a high seroprevalence in the general population, while symptomatic infections occur mostly in immunocompromised individuals. Symptomatic CMV infections commonly include pneumonia, encephalitis, retinitis and colitis, while urinary tract involvement is a rare entity. We present a rare case of massive macroscopic haematuria due to CMV haemorrhagic cystitis in a 29-year-old woman in her second trimester of pregnancy. She was treated with intravenous Ganciclovir after initial resuscitation, and her symptoms promptly resolved. Timely diagnosis and treatment of symptomatic CMV infection is necessary to prevent associated morbidity, and this is especially significant during pregnancy in order to prevent foetal transmission. Both our patient and her baby remained symptom free at the 6-month follow-up post-delivery. Clinicians should have a high index of suspicion to biopsy the bladder urothelium of at-risk patients who present with haemorrhagic cystitis and have non-specific cystoscopy findings as histopathological analysis is the mainstay of diagnosing CMV-cystitis.

Flapping wing Micro Air Vehicles (MAVs) are a class of bio-inspired autonomous flying robots that have stringent weight and size restrictions, making them attractive for many military as well as civilian applications. In this work, a novel material is proposed for both the structure and the flapping actuator of the MAV. The said material is a kind of piezoelectric fiber composite, called Macro Fiber Composite (MFC). MFC is made by combining piezoelectric fibers in epoxy-based polymer materials and electroded in an inter-digitated manner. The focus of this paper is to address the challenge of designing suitable power electronics for this piezocomposite actuator. The requirements of a light weight, high voltage (−500V to 1500V) drive circuit drawing power from low voltage sources, such as on-board batteries, are addressed. The paper presents the detailed design and simulation of such a drive circuit for MAV applications. The designed circuit first generates a DC voltage of 1500V from 5V DC by using a voltage multiplier and a fly back converter. This high voltage DC signal is then converted to a high voltage AC signal (in the required range of −500V to 1500V AC) by using a H-bridge inverter circuit. PWM signals are used for proper amplitude control of the output signal. The designed circuit is simulated in MATLAB Simulink and the desired results are obtained. The entire design is done to meet the stringent weight requirement of MAV and the circuit can effectively drive the piezoelectric composite actuator.

Background and Aims: Cardiovascular diseases are the leading causes of morbidity and mortality in chronic kidney disease (CKD) patients. Our aim was to derive predictors of cardiac morbidity, mortality, cardiac complications and to develop/validate a scoring tool in patients with CKD undergoing non-cardiac surgery. Methods: A prospective observational multicentre study was done on 770 patients with CKD. The primary outcome (“Event”) was one or more than one of sudden cardiac death, pulmonary oedema, acute coronary syndrome, arrhythmia and 30-day mortality. Secondary outcome was hypertension and hypotension. Predictors of cardiac risk were identified. A scoring tool was developed on the 2018 dataset and was validated on the 2019 dataset. Results: The overall incidence of cardiac events was 290 (37.66%) whereas the incidence of major adverse cardiac and cerebrovascular events was 15.04%. Mortality due to cardiac cause was 13 (1.68%). On multivariate regression analysis, seven perioperative variables had significant association with increased risk of events: age > 65 years (P = 0.004), metabolic equivalents (METS) ≤4 (P≤0.032), emergency surgery (P =0.032), mean arterial pressure >119 (P = 0.001), echocardiographic scoring (P = 0.054), type of anaesthesia (P ≤ 0.0001) and type of surgery (P = 0.056). Using these variables, a risk stratification tool was developed. C statistics showed favourable predictive accuracy (0.714) and the model showed good calibration. Conclusion: This risk scoring tool based on preoperative variables will help to predict the risk of events in high-risk CKD patients undergoing non-cardiac surgery. This will help in better counselling and optimisation.

OBJECTIVE To assess the effect of yoga on anthropometry, blood pressure, glycemic control, and oxidative stress in type 2 diabetic patients on standard care in comparison with standard care alone. RESEARCH DESIGN AND METHODS The study involved 123 patients stratified according to groups with microvascular complications, macrovascular complications, and peripheral neuropathy and without complications and assigned to receive either standard care or standard care along with additional yoga for 3 months. RESULTS In comparison with standard care alone, yoga resulted in significant reduction in BMI, glycemic control, and malondialdehyde and increase in glutathione and vitamin C. There were no differences in waist circumference, waist-to-hip ratio, blood pressure, vitamin E, or superoxide dismutase in the yoga group at follow-up. CONCLUSIONS Yoga can be used as an effective therapy in reducing oxidative stress in type 2 diabetes. Yoga in addition to standard care helps reduce BMI and improve glycemic control in type 2 diabetic patients.

Pediatrics ,RJ1-570 - Abstract
Background Neonatal hypothermia has been widely regarded as a major contributory factor to neonatal mortality and morbidity in low-resource settings. The high prevalence of potentially preventable hypothermia today urges an investigation into why neonates still become hypothermic despite awareness of the problem and established thermal care guidelines. This study aimed to explore the gaps in knowledge and practices of neonatal thermal care among healthcare workers in low-resource settings. Methods A cross-sectional, questionnaire-based survey was performed online among healthcare workers in low-resource settings. We applied a purposive and snowballing sampling method to recruit participants through a two-round international online survey. Questionnaires were developed using themes of neonatal thermal care extracted from existing neonatal care guidelines. Results 55 neonatal care professionals participated in the first-round survey and 33 in the second. Almost all participants (n=44–54/55) acknowledged the importance of the WHO’s warm chain to keep a neonate warm. However, fewer participants (n=34–46/55) responded to practice them. When asked about cold stress, defined as a condition in which neonates are below optimum environmental temperature and using more oxygen and energy while maintaining normal body temperature, 15 out of 55 participants answered that checking extremity temperatures by hand touch was useless. Some participants reported concern about the extremity temperature’s inaccuracy compared with core temperature. Opinions and preferences for rewarming methods differed among participants, and so did the availability of warming equipment at their institutions. Conclusion An inadequate understanding of cold stress underestimates the potential benefits of extremity temperatures and leads to missed opportunities for the timely prevention of hypothermia. The current thermal care guidelines fail to highlight the importance of monitoring cold stress and intervening before hypothermia occurs. Therefore, we urge introducing the concept of cold stress in any neonatal thermal care guidelines.

Objective: To define the optimized inter-injection interval of injectable testosterone undecanoate (TU) treatment for hypogonadal and transmen based o n individual dose titration in routine clinical practice. Design and methods: A prolective observational study of consecutive TU injections in men undergoing testosterone replacement therapy for pathological hypogonadism or masculinization of female-to-male transgender (transmen) subject to individual dosing titration to achieve a stable replacement regimen. Results: From 2006 to 2019, 6899 injections were given to 325 consecuti ve patients. After excluding the 6-week loading dose, 6300 injections were given t o 297 patients who had at least three and a median of 14 injections. The optimal injec tion interval (mean of last three injection intervals) had a median of 12.0 weeks (int erquartile range 10.4–12.7 weeks). The interval was significantly influenced by ag e and body size (body surface area, BSA) but not by diagnosis or trough serum LH, FSH , and SHBG. Longer (≥14 weeks; 68/297, 23%), but not shorter (≤10 weeks; 22/297, 7.4%), intervals were weakly correlated with age but not diagnosis or other cova riables. Low blood hemoglobin increased with trough serum testosterone to reach pl ateau once testosterone was about 10 nmol/L or higher. Conclusion: Optimal intervals between TU injection after individual titrat ion resulted in the approved 12-week interval in 70% of patients with only m inor influence for clinical application of BSA and not of trough serum LH, FSH, an d SHBG. Individually optimized inter-injection interval did not differ between men wi th primary or secondary hypogonadism or transmen.

Advanced Technologies are transforming the Automotive industry and the pace of innovation is accelerating at a breakneck speed. Autonomous Vehicles (AVs) incorporate many different systems and technologies and their increased computer functionality and connectivity lead to enormous cybersecurity risk. The aim of this research is to explore the significant factors that influence cyber threats on AVs and to examine their level of importance. Partial Least Squares path modeling was preferred for research studies for its flexible modeling and identifying key drivers. The data analysis was carried out using ADANCO 2.0. to develop and evaluate the structural model and the causal relationships between the variables. Correlation of in-vehicular network vulnerabilities with trust and the correlation between the “workload of the driverless system” with cyber-attacks and cyber threats to AVs are two relations but have not been touched upon in previous studies. In this research, a modified framework is proposed based on the Cyber Cycle and integrated model of Diamond Model of Intrusion Analysis with the Active Cyber Defense Cycle.

Intraocular pressure (IOP) measurements vary with different tonometers in irregular corneas. The purpose of our study was to determine the comparability of the ocular blood flow tonometer (OBF) and Tonopen with the Goldmann applanation tonometer (GAT) in measuring IOP in post-keratoplasty eyes. The IOP was measured using the OBF tonometer, GAT, and Tonopen in 69 post-keratoplasty eyes by two observers. The same instruments were used, and the sequence of measurements was randomized. Pachymetry to measure corneal thickness was done in all patients.Interobserver variation for IOP measured with GAT was statistically significant ( p = 0.039). Two-way analysis of variance (ANOVA), however, did not demonstrate any statistical difference between measurements with the three tonometers and two observers. Corneal thickness did not appear to have an effect on measurements with any of the tonometers. There was no significant difference in the overall accuracy of the OBF tonometer over the Tonopen versus the GAT. The OBF tonometer can be a useful alternative in measuring IOP in post-keratoplasty eyes.

Clouds play a vital role in Earth’s water cycle and the energy balance of the climate system; understanding them and their composition is crucial in comprehending the Earth–atmosphere system. The dataset “Understanding Clouds from Satellite Images” contains cloud pattern images downloaded from NASA Worldview, captured by the satellites divided into four classes, labeled Fish, Flower, Gravel, and Sugar. Semantic segmentation, also known as semantic labeling, is a fundamental yet complex problem in remote sensing image interpretation of assigning pixel-by-pixel semantic class labels to a given picture. In this study, we propose a novel approach for the semantic segmentation of cloud patterns. We began our study with a simple convolutional neural network-based model. We worked our way up to a complex model consisting of a U-shaped encoder-decoder network, residual blocks, and an attention mechanism for efficient and accurate semantic segmentation. Being an architecture of the first of its kind, the model achieved an IoU score of 0.4239 and a Dice coefficient of 0.5557, both of which are improvements over the previous research conducted in this field.

Environment-responsive-cum-site-specific delivery of therapeutic drugs into tumor cells is a foremost challenge for chemotherapy. In the present work, Moringa oleifera gum–based pH-responsive nanogel (MOGN) was functionalized as a doxorubicin (DOX) carrier. It was synthesized via free radical polymerization through the γ-irradiation method using acrylamide and N,N’-MBA followed by hydrolysis, sonication, and ultracentrifugation. The swelling behavior of MOGN as a function of pH was assessed using a gravimetric method that revealed its superabsorbent nature (365.0 g/g). Furthermore, MOGN showed a very high loading efficiency (98.35 %L) of DOX by MOGN. In vitro release studies revealed that DOX release from DOX-loaded MOGN was 91.92% at pH 5.5 and 12.18% at 7.4 pH, thus favorable to the tumor environment. The drug release from nanogel followed Korsmeyer–Peppas model at pH 5.5 and 6.8 and the Higuchi model at pH 7.4. Later, the efficient DOX release at the tumor site was also investigated by cytotoxicity study using Rhabdomyosarcoma cells. Thus, the synthesized nanogel having high drug loading capacity and excellent pH-triggered disintegration and DOX release performance in a simulated tumor environment could be a promising candidate drug delivery system for the targeted and controlled release of anticancer drugs.

We appreciate the thoughtful comments by Sperandei (1) on our article (2). Following are the requested clarifications. Sperandei’s first question was on how we could use paired t test and the Wilcoxon signed rank test for analyzing unpaired variables between two groups even though these tests are designed for paired variables. We would like to respond that we had used paired t test …

Aim: The objective is to evaluate the effect of intravenous (i.v.) administration of bisphosphonate (zoledronic acid) therapy on the jaw bone density and incidence of any other bony patholog. Settings and Design: Observational – prospective study. Materials and Methods: A total of 57 patients having a history of bony metastasis (excluding the jaw bone) were enrolled following the inclusion/exclusion criteria. Each patient received six doses of 4 mg i.v. bisphosphonate once a month. Multidetector computed tomography (MDCT) of jawbones for each patient was performed before the start of therapy (baseline) and subsequently at 6 and 12 months. Bone density was assessed at 24 predetermined sites (8 sites in maxilla and 16 sites in mandible) and any pathological change in either of the jaw bones was noted. Statistical Analysis Used: Shapiro–Wilk test ,Pearson's Chisquare test and repeated measures analysis of variance. Results: The result showed no statistically significant increase in mean bone density over a period of 1 year in maxilla and mandible. However, a significant increase in bone density was observed from 6 months to 1 year in mandibular anterior cancellous bone. The detailed observation of each MDCT scan showed no pathological change in either of the jaw bones during the study period. Conclusion: The administration (i.v.) of six doses of 4 mg bisphosphonate did not lead to a significant change in bone density over a period of 1 year.
