Abstract

Background: Vascular inflow occlusion (VIO) during liver resections (Pringle manoeuvre) can be applied to reduce blood loss, however may at the same time, give rise to ischemia-reperfusion injury (IRI). The aim of this study was to assess the characteristics of hepatic microvascular perfusion during VIO in patients undergoing major liver resection. Methods: Assessment of hepatic microcirculation was performed using a handheld vital microscope (HVM) at the beginning of surgery, end of VIO (20 minutes) and during reperfusion after the termination of VIO. The microcirculatory parameters assessed were: functional capillary density (FCD), microvascular flow index (MFI) and sinusoidal diameter (SinD). Results: A total of 15 patients underwent VIO; 8 patients showed hepatic microvascular perfusion despite VIO (partial responders) and 7 patients showed complete cessation of hepatic microvascular perfusion (full responders). Functional microvascular parameters and blood flow levels were significantly higher in the partial responders when compared to the full responders during VIO (FCD: 0.84±0.88 vs. 0.00±0.00 mm/mm2 , P<0.03, respectively, and MFI: 0.69–0.22 vs. 0.00±0.00, P<0.01, respectively). Conclusions: An interpatient heterogeneous response in hepatic microvascular blood flow was observed upon VIO. This may explain why clinical strategies to protect the liver against IRI lacked consistency

Abstract

Ethics and dissemination: The protocol is approved by the ethics committee at the coordinating centre (Academic Medical Center (AMC) Amsterdam) and by the local Institutional Review Board at the participating centres. Data will be presented at international conferences and published in peer-reviewed journals. Conclusions: This pilot study will determine the safety and efficacy of IRE in the prostate. It will show the radiological and histopathological effects of IRE ablations and it will provide data to construct an accurate treatment planning tool for IRE in prostate tissue. Introduction: Current surgical and ablative treatment options for prostate cancer have a relatively high incidence of side effects, which may diminish the quality of life. The side effects are a consequence of procedure-related damage of the blood vessels, bowel, urethra or neurovascular bundle. Ablation with irreversible electroporation (IRE) has shown to be effective in destroying tumour cells and harbours the advantage of sparing surrounding tissue and vital structures. The aim of the study is to evaluate the safety and efficacy and to acquire data on patient experience of minimally invasive, transperineally image-guided IRE for the focal ablation of prostate cancer. Methods and analysis: In this multicentre pilot study, 16 patients with prostate cancer who are scheduled for a radical prostatectomy will undergo an IRE procedure, approximately 30 days prior to the radical prostatectomy. Data as adverse events, side effects, functional outcomes, pain and quality of life will be collected and patients will be controlled at 1 and 2 weeks post-IRE, 1 day preprostatectomy and postprostatectomy. Prior to the IRE procedure and the radical prostatectomy, all patients will undergo a multiparametric MRI and contrast-enhanced ultrasound of the prostate. The efficacy of ablation will be determined by whole mount histopathological examination, which will be correlated with the imaging of the ablation zone.

16. Hemodynamic optimization in perioperative- and intensive care patients: A special focus on blood pressure management

Abstract

This dissertation focusses on the hemodynamic optimization of perioperative and intensive care patients, with special attention to blood pressure. Part I examines the definition and management of hypotension, and its association with morbidity and mortality. A high variability in threshold definitions of hypotension is found in the literature. However, we found one and the same definition is being used in most ICU’s worldwide, regardless of patient category. ICU physicians and nurses indicated that hypotension occurs in most patients, and is an underdiagnosed and largely preventable phenomenon. In most ICU’s, no protocol to treat hypotension is available, and more than half of the respondents indicated preferring to have one in the future. In a systematic review, we show exposure to hypotension during surgery increases the risk of morbidity and mortality, and that progressive severity of the hypotension threshold used in studies resulted in increasing strength of associations. Part II focuses on the prediction and optimization of hemodynamic changes. We show that the intraoperative employment of an algorithm predicting hypotension, combined with proactive treatment guidance, results in a reduction in the amount of intraoperative hypotension. Moreover, it might also reduce the total percentage of time spent in hypotension after surgery, indicating a potential vicious circle of hemodynamic instability. Finally, we investigated whether a difference in immediate hemodynamic response following transcatheter aortic valve implantation could be predictive for differences in outcome. We found that a larger immediate reduction in left ventricular ejection time resulted in greater increase in health related quality of life.

17. Haemodynamics: Modern applications of basic physiological concepts

Abstract

Effective haemodynamics require sufficient myocardial contractility, and adequate filling and tonus of the circulatory system. Perioperatively, in the operating theatre, at the postoperative anaesthesia care unit and at the intensive care unit (ICU), patients often need support of their circulatory system. Maintaining stable haemodynamics is challenging and at present our treatments are mostly reactive. Furthermore, selecting the correct treatment modality can be difficult as a proper pathophysiological diagnosis of the cause of the haemodynamic instability might be inapparent. In Part I of this thesis we primarily focus on patients admitted to the ICU and aim to implement a physiological theory on venous return in clinical practice. In Part II we focus on prediction and prevention of haemodynamic instability (defined as intraoperative hypotension) with the use of machine learning.

18. Transfusion-associated circulatory overload: A survey among Dutch intensive care fellows.

Abstract

Objectives Transfusion-associated circulatory overload (TACO) is a severe pulmonary transfusion reaction and leading cause of transfusion-related morbidity and mortality in Europe. TACO is of particular importance in critically ill patients, since they often receive blood transfusions and have multiple risk factors for TACO. This study investigates transfusion practices in patients at risk of developing TACO, and furthermore knowledge concerning risk factors, diagnoses and treatment strategies among Dutch intensive care unit (ICU) fellows. Material and methods An unannounced paper-based survey was conducted among Dutch ICU fellows during an educational conference. The survey consisted of 16 multiple and open choice questions. Results Of all 65 Dutch ICU fellows 56.8% completed the survey; of respondents 88.9% identified the correct constellation of symptoms for TACO. In total, 29.7% of the respondents are aware they are obligated to report TACO cases to the blood bank. Major risk factors for TACO that respondents identified were reduced left ventricular function, infusion volume and infusion rate. In a non-emergency setting, 45.9% of fellows start red blood cell transfusion with 2 units or more. Transfusion rates exceeded national guidelines in 15.4% of fictitious cases. TACO is treated with furosemide by 94.5% of the fellows, however goals of the therapy varied greatly. Conclusion Dutch ICU fellows are knowledgeable of TACO symptoms, risk factors and treatment, however knowledge on reporting and transfusion practice in the setting of at risk patients for TACO should be improved.
