1. About a Large Botryoid Rhabdomyosarcoma in a Little Girl: Management Difficulties and Literature Review

Abstract

Background. Rhabdomyosarcoma (RMS) is a rare high-grade malignant tumor and the most common soft-tissue sarcoma, which occurs in young girl over 5 years old. Multimodality treatment associating with surgery, chemotherapy, and/or radiotherapy culminate in a >70% overall 5-year survival. This is the first case reported in 30 years of practice in Côte d’Ivoire, low- and middle-income country (LMIC). Objective. To summarize clinical data, the significant alternative chemotherapy efficiency and difficulties related to the prognosis evaluation in an LMIC. Case. A 2-year-old girl had been examined for a large mass in the vulvar region and clitoris. We carried out a biopsy for histopathologist exam. This allows pathologic, genetic, and biological characterization of nonmetastatic botryoid rhabdomyosarcoma. A multidisciplinary team decision of neoadjuvant chemotherapy was retained combining vincristine, cyclophosphamide, and actinomycin D or alternatively with Adriamycin. After 3 weeks of chemotherapy, significant volumetric reduction of tumor was observed. Yet a surgical removal was proposed but not performed because the patient has no longer consulted our medical center and was lost to follow-up. Therefore, we cannot assess the long-term evolution and prognosis. Conclusion. Embryonal RMS (ERMS) of clitoris is a rare malignant tumor of infant. Histology and immunohistochemistry are essential for diagnostic but unavailable in our context. We want to emphasize on the difficulties encountered in treatment and prognosis assessment. The primary free surgical removal of the vulva with adjuvant chemotherapy and/or radiotherapy must then be implemented in our practice.

Abstract

Lord of the Flies is an anti-utopic (dystopic) novel by the English author William Golding, who wrote about how human’s primitive aspect can peep out by portraying kids whose ages vary from 5 to 12 on a deserted island. That kids are chosen as the characters of the novel makes it possible to observe in an empirical way the human nature in its nakedness far away from civilization and what it offers with its norms, education and social intitutions. The island, as a space, is significant for it offers a clear vision of how primitive instincts can appear when there is no outside interference. In our article, first we have made a theoretical discussion on primitive aspect of humans and then analysed how they might become savage in a setting away from civilization. In our analysis, we have found out that human nature is likely to transgress into savagery in a setting devoid of laws, moral norms and social institutions. The reason why we have chosen such a topic is directly related to a need to better understand the recent devastating violance and hatred, which ravage our world and close geography in particular.

Abstract

The Gene Ontology (GO) knowledgebase (http://geneontology.org) is a comprehensive resource concerning the functions of genes and gene products (proteins and noncoding RNAs). GO annotations cover genes from organisms across the tree of life as well as viruses, though most gene function knowledge currently derives from experiments carried out in a relatively small number of model organisms. Here, we provide an updated overview of the GO knowledgebase, as well as the efforts of the broad, international consortium of scientists that develops, maintains, and updates the GO knowledgebase. The GO knowledgebase consists of three components: (1) the GO-a computational knowledge structure describing the functional characteristics of genes; (2) GO annotations-evidence-supported statements asserting that a specific gene product has a particular functional characteristic; and (3) GO Causal Activity Models (GO-CAMs)-mechanistic models of molecular "pathways" (GO biological processes) created by linking multiple GO annotations using defined relations. Each of these components is continually expanded, revised, and updated in response to newly published discoveries and receives extensive QA checks, reviews, and user feedback. For each of these components, we provide a description of the current contents, recent developments to keep the knowledgebase up to date with new discoveries, and guidance on how users can best make use of the data that we provide. We conclude with future directions for the project.

6. The effects of methyltransferase genes on human haematopoietic stem cell function

Abstract

The highly structured and hierarchical haematopoietic system is defined by the existence of a pool of haematopoietic stem cells (HSCs) at the apex, which possess both multipotent differentiation and self-renewal properties. The HSC pool is heterogenous in terms of self-renewal and differentiation capacity, and HSCs with various genetic alterations lie intermingled with wild type (WT) HSCs. Clonal expansion of HSCs is an ineluctable feature of advancing age, and age related clonal haematopoiesis (ARCH) is associated with an increased risk of haematological malignancy, atherosclerosis and cardiovascular disease. The functional effects of these clonal expansions remain to be thoroughly investigated in primary human HSCs. In this thesis, I have focused on 2 methyltransferase genes, PRMT5 and DNMT3A, which are both epigenetic modulators of HSC function. Pharmacological inhibition of PRMT5 in healthy human cord blood (hCB) haematopoietic stem and progenitor cells (HSPCs) leads to reduced HSPC proliferation and a relative differentiation block at the CD34+ progenitor level in a dose dependent manner. While PRMT5 has been identified by others as an important therapeutic target in the context of haematological malignancies, my work indicates its clinical applicability may be limited by the likely toxicity of PRMT5 inhibition on HSPCs, which could lead to significant myelosuppression. Mutations in the gene DNMT3A are the most common in ARCH and are associated with haematological and non-haematological diseases with significant morbidity and mortality. The R882 mutation is the most common within DNMT3A, especially in the context of haematological disease. I used single cell biology tools to obtain functional data from 12 primary human samples from ARCH individuals and acute myeloid leukaemia (AML) patients. A total of 2990 single HSC/multi-potent progenitor (MPP) derived colonies from DNMT3A mutant HSCs/MPPs and internal WT control HSC/MPPs from within each individual were compared. DNMT3A R882 mutation confers an intrinsic differentiation phenotype, with DNMT3A R882 mutant HSC/MPPs displaying more efficient differentiation towards neutrophils and less efficient monocyte differentiation in vitro compared to internal WT control HSC/MPPs in 6 independent individuals. No overall lineage bias is observed in DNMT3A R882 mutant HSC/MPPs, and no selective differentiation advantage. RNA sequencing of single HSC/MPP derived colonies containing mature monocytes confirmed that DNMT3A R882 mutant HSC/MPPs produce less mature monocytes, and are marked by upregulation of metabolic and inflammatory gene pathways. Overall, these functional phenotypes provide critical insight into the inherent functional effects of DNMT3A R882 mutation at the HSC/MPP level in humans, beyond the previously described transcriptional priming towards more quiescent HSCs and increased self-renewal phenotypes. Further mechanistic understanding will be required to elucidate the link between the myeloid differentiation phenotypes observed in my study and the inflammatory conditions associated with ARCH.

Abstract

The emotional workplace has been explored through several perspectives (e.g. Hochschild, 1983; Bolton, 2005) and within several different contexts (e.g. James, 1992; Ashforth and Humphrey, 1993; Morris and Feldman, 1996; Fineman, 2003; Ward and McMurray, 2016). Although several contributions exist on emotions within organisations, missing from these debates is the exploration of how institutional logics shape the performance and management of emotional labour on the frontline. There is also limited knowledge on how workers navigate their emotional labour process in light of heterogeneous logics. In this light, both Hochschild's (1983) and Bolton's (2005) analyses have been considered and extended in order to re-ignite debates of the emotional workplace. This thesis examines whether a lens provided by the institutional logics perspective develops our understanding of emotional labour by showing how competing institutional forces can be considered as shaping emotional aspects of work. The empirical focus of this thesis is on the context of British medicine. Despite research evidence pointing towards the importance of health/wellbeing for healthcare professionals, emotional aspects of the medical labour process are worryingly overlooked for junior doctors (Boorman, 2009; Vijendren et al, 2015). A fundamental aspect of medicine is too often neglected by both academics and practitioners: the emotional labour in medicine. Accordingly, this thesis explores the emotional context of medical and surgical work as a means of extending theoretical debates and addressing an important empirical focus. An ethnographic exploration into the work experiences of junior doctors offers a promising case study to explore theorising of emotional labour. An ethnographic case study approach, through an interpretivist philosophy, provides important insights into the medical labour process, allowing for immersion within the institutional context and the gathering of junior doctor narratives. I conducted 40 semi-structured interviews with junior doctors across the training grade spectrum practicing within both medical and surgical services - including a handful of consultants to help contextualise the junior doctor data. In addition to interviewing, I undertook ethnographic observations - within ward areas, training sessions/breakout rooms, specialist clinics and operation theatres. This allowed the space and scope to explore the institutional context, associated workloads and pace of work processes in medicine. Two hospitals and a treatment centre within Wilton Trust informed this PhD - constituting a rich and insightful dataset. In this thesis, I first aimed to uncover how institutional logics can be understood as shaping the performance of emotional labour. Changes to the political economy of the UK and associated public service reforms have brought conflicting institutional logics into the frontline of public service work. Given the shift towards neoliberalism and the political focus on the efficiency of services (e.g. Bolton, 2002), I found that multiple, competing institutional logics shape important aspects of a junior doctors' labour process, and thus the texture of their emotional labour with patients and related others. Logics of bureaucratic rationality, professionalism, consumerism and an orientation towards the community were navigated by the doctors routinely in this study - doctors were found to choose, resist and negotiate these influences depending on work contexts and situational demands. Understanding the significance of the institutional logics perspective offers the first contribution to knowledge. Secondly, I aimed to understand types of emotional labour performed by the doctors considering the identified institutional influences. This thesis, therefore, helps to bring recent work locating emotions within institutional theory (e.g. Voronov and Vince, 2012) and extant literature on emotional labour in order to examine characteristic forms of emotional labour within medicine - this offers the second contribution to knowledge. Thirdly, I aimed to explore the means by which emotions were managed on (and off) the job with patients, colleagues and others. I found that doctors tended to share work experiences and relied mainly on collective processes in order to manage the pains and pleasures of medical/surgical work. This offers the third (empirical) contribution of the thesis.

Abstract

The Gene Ontology Consortium (GOC) provides the most comprehensive resource currently available for computable knowledge regarding the functions of genes and gene products. Here, we report the advances of the consortium over the past two years. The new GO-CAM annotation framework was notably improved, and we formalized the model with a computational schema to check and validate the rapidly increasing repository of 2838 GO-CAMs. In addition, we describe the impacts of several collaborations to refine GO and report a 10% increase in the number of GO annotations, a 25% increase in annotated gene products, and over 9,400 new scientific articles annotated. As the project matures, we continue our efforts to review older annotations in light of newer findings, and, to maintain consistency with other ontologies. As a result, 20 000 annotations derived from experimental data were reviewed, corresponding to 2.5% of experimental GO annotations. The website (http://geneontology.org) was redesigned for quick access to documentation, downloads and tools. To maintain an accurate resource and support traceability and reproducibility, we have made available a historical archive covering the past 15 years of GO data with a consistent format and file structure for both the ontology and annotations.

Abstract

Abstract Objective Survival of patients with advanced laryngeal squamous cell carcinoma (LSCC) remains poor and management protocols warrant further development. We thus investigated treatment and outcome‐related factors for LSCC in Stockholm, Sweden. Methods In a retrospective setting, 520 patients with LSCC diagnosed during 2000–2014, were included. Data on stage, treatment, and outcome were correlated with recurrence‐free and overall survival (RFS and OS, respectively). Results Five‐year OS for all patients was 65%. Five‐year RFS for T1a, T1b, T2, T3, and T4 glottic LSCC was 90%, 91%, 77%, 47%, and 80%, respectively. The corresponding figures for T1, T2, T3, and T4 supraglottic LSCC were 64%, 66%, 64%, and 86%. Conclusion Patients with a T3 glottic LSCC had unexpectedly poor survival, especially when compared with patients with a T4 tumor. Patients with T4 disease were primarily treated with laryngectomy and postoperative radiotherapy (RT)/chemoradiotherapy (CRT), while most patients with T3 LSCC were treated with RT/CRT.

Background: As per Ayurvedic classics, Rasāyana is a term used for unique therapeutic measures intended to improve cognition, memory, physical strength and delay the degenerative processes. Ayush Rasayana A & B are developed from extracts of herbs pertaining rasayana property. Aims & Objectives: To assess the safety and effectiveness of 'Ayush Rasayana A and B' in improving physical endurance, quality of life and cognition of elderly. Materials and Methods: This is an open label, single arm, multi-centre study among 256 apparently healthy elderly between 60-75 years. Ayush Rasayan A was given for 6 days and Ayush Rayana B for 180 days. The effect of the trial drug was seen on functional capacity, quality of life and cognitive function and, safety of the intervention was assessed through haematological and biochemical tests, clinical examination and incidence of adverse events. Abstract

Summary Acute promyelocytic leukaemia (APL), defined by the t(15;17)(q24;q21) translocation, accounts for 5%–10% of paediatric acute myeloid leukaemia cases. All‐trans retinoic acid (ATRA) and arsenic trioxide (ATO) are key treatments, though ATO access varies. We evaluated treatment, complications and survival in 50 UK paediatric APL patients diagnosed between 2014 and 2021. All patients received ATRA and most received ATO. Event‐free survival was lower in high‐risk patients (85% vs. 100%, p = 0.03), and those not receiving ATO at diagnosis. All relapsed patients could be salvaged with ATO. Addressing ATO availability and consistent funding is crucial to ensure timely treatment and improve outcomes. [ABSTRACT FROM AUTHOR]

Abstract

The development of integrated micro‐power sources is mainly driven by innovative investigations in materials chemistry, battery designs, and microfabrication processes. Here, a new technique is described for the development of coaxial wire‐shaped Li‐ion batteries by successfully adopting the unidirectional helical winding method starting from a twisted Cu fabric. Such a coaxial wire battery is examined by scanning electron microscopy and characterized by electrochemical techniques. These investigations reveal that the proposed approach is definitely suitable for achieving wire batteries showing high energy storage capacity while being highly flexible and stretchable without compromising the whole electrochemical performance even under mechanical deformations. The full cell based on the Li4Ti5O12/LiCoO2 system delivers an outstanding linear capacity of 137 µAh cm−1 at a 0.1C rate with nearly 100% coulombic efficiency and is further capable of being operated at various kinetics. It is also shown that such a coaxial wire battery can be used to power different electronic devices, which is of significance for the development of autonomous wearable applications like electronic textiles [ABSTRACT FROM AUTHOR]

42. Exploring Factors Influencing the Internal Displacement Decisions Amidst Natural Disasters in Bangladesh.

Abstract

Bangladesh, a densely populated country in the world's largest coastal delta, is seriously threatened by the ongoing onslaught of major natural disasters brought on by climate change. The main perpetrators, cyclonic storms and floods cause millions of displaced people each year and increase the vulnerability of communities already facing devastation from the environment and poverty due to these disasters. This study explores the complex patterns of internal displacement brought on by cyclonic storms and floods in Bangladesh from 2008 to 2022. The study identifies the patterns, trends, vulnerable districts, and influencing factors of displacement by analysing data from multiple sources. Every year, an average of 6.14 million people are displaced by floods, a persistent problem. Gaibandha is the district most at risk, having seen 13 floods in the previous 15 years. Throughout the study period, Bhola experienced eight strikes from cyclonic storms that decimated the southern coast. These natural disasters force people to move from environmentally susceptible to safe areas. The disaster's intensity or frequency and some economic, social, and political factors influence this decision to displace. The study also identifies factors that influence displacement decisions after catastrophic disasters. Additionally, it emphasises the importance of understanding displacement patterns and influencing factors to make informed decisions about displacement in such situations. [ABSTRACT FROM AUTHOR]

Abstract

BackgroundDuring the second wave of the COVID-19 pandemic in India, the Ministry of Ayush conducted a community study to provide therapeutic care to patients with asymptomatic, mild, and moderate COVID-19 in home isolation based on the empirical evidence generated on the efficacy of AYUSH-64 in COVID-19.ObjectiveTo document disease characteristics, care-seeking behavior, and outcomes in patients with asymptomatic, mild, or moderate COVID-19 in home isolation who used AYUSH-64 for COVID-19.MethodsCross-sectional analysis of the data generated through a community study conducted in India from 08 May to 31 August 2021 was performed to study the disease characteristics, care-seeking behavior during home isolation, clinical outcomes, adverse events, and the association between various risk factors and clinical recovery during the study period. The data were collected through semi-structured questionnaires, available in electronic data collection format at the baseline, 7, 14, and 21 days. A logistic regression was performed to explore the relationship between relevant variables and clinical recovery.ResultsData from 64,642 participants were analyzed for baseline assessment, and final analysis was done for 49,770 participants. The mean age of the enrolled participants was 38.8 ± 11.7 years, and 8.4% had co-morbidities. AYUSH-64 was utilized as an add-on to the standard care by 58.3% of participants. Comparable clinical outcomes were observed in participants utilizing AYUSH-64 either as a standalone or as an add-on to standard care, in terms of clinical recovery, disease progression, the requirement for oxygen supplementation, hospitalization, ICU admission, and need for ventilator support. Younger age, having no co-morbidities or substance abuse, and having been vaccinated were associated with early clinical recovery than those who were older and not vaccinated.ConclusionsThe study findings suggest that AYUSH-64 use, either standalone or as an adjunct to standard care, in asymptomatic, mild, or moderate COVID-19 is associated with good clinical outcomes. Ayush services and interventions can be effectively integrated into the mainstream public health architecture to serve public health goals.
