Abstract

The role of flux-limited thermal conduction on the fusion performance of the uniaxially-driven targets studied by Derentowicz et al.; Jour. Tech. Phys. 18, 465 (1977) and Jour. Tech. Phys. 25, 135 (1977), is explored as part of a wider effort to understand and quantify uncertainties in ICF systems sharing similarities with First Light Fusion's projectile-driven concept. We examine the role of uncertainties in plasma microphysics and different choices for the numerical implementation of the conduction operator on simple metrics encapsulating the target performance. The results indicate that choices which affect the description of ionic heat flow between the heated fusion fuel and the gold anvil used to contain it are the most important. The electronic contribution is found to be robustly described by local diffusion. The sensitivities found suggest a prevalent role for quasi-nonlocal ionic transport, especially in the treatment of conduction across material interfaces with strong gradients in temperature and conductivity. We note that none of the simulations produce neutron yields which substantiate those reported by Derentowicz et al. Jour. Tech. Abstract

The aim of the study was to analyze the effect of polymorphic variants previously associated with arterial hypertension (AH) in Genome Wide Association Studies (GWASs) in/next to genes and locuses CYP7A1 and PLEKHA7 on the development of coronary artery disease (CAD) in Bulgarian patients. A hundred and nine consecutive patients with angiographically documented CAD were studied. The genotyping was done with 7900 HT Fast Real-Time PCR (Applied Biosystems) with TaqMan® method. The control group consisted of 192 healthy population controls, selected from the bio- bank of the Molecular Medicine Center. SPSS and PLINK were used for the statistical analysis with level of significance < 0.05 and confidence interval 95%. The mean age of the studied patients was 63.71 ± 9.35 years; 35 (35%) females. Previous myocardial infarction (MI) had 38(38%); one-vessel – 39 (39%); two-vessel – 28 (28%); three-vessel disease – 34 (34%); 43 (43%) were with diabetes mellitus; 92 (92%) – with arterial hypertension (AH); 77 (77%) – with dyslipidemia; 42 (42%) were smokers; 25 (25%) were obese. We did not find any significant association between CAD and poly- morphism rs11191548 near CYP17A1 and only a tendency for genotype of rs381815 in PLEKHA7 (p = 0.06; OR 0.64; CI 0.40-1.02 for CAD) under dominant model. This is of practical importance both for studying the genetic aspects of CAD in the future and for enlargement of the current database.

Abstract

The main result is that the qc-scalar curvature of a seven dimensional quaternionic contact Einstein manifold is a constant. In addition, we characterize qc-Einstein structures with certain flat vertical connection and develop their local structure equations. Finally, regular qc-Ricci flat structures are shown to fibre over hyper-Kahler manifolds.

Abstract

There is certain discordance between the functional (hemodynamic) and imaging methods for assessing the characteristics of the atherosclerotic plaques. It would be of practical value to find if there was a correlation between neurotrophins -- brain-derived growth factor (BDNF), neuronal growth factor (NGF) and carotid plaque morphology. Aim: To make a first attempt to combine biochemical and morphological markers to better assess the risk of carotid plaques. Material and Methods: Thirteen consecutive patients (Eastern European -- Caucasian race, 9 males and 4 females; mean age 73.08±7.65 years) with angiographically proven significant carotid atherosclerotic stenoses, referred for carotid stenting were evaluated. Plaque morphology was evaluated with intravascular ultrasound (IVUS) and virtual histology (VH). Serum concentrations of BDNF and NGF were tested and their results compared with a control group of 17 individuals without documented clinically significant atherosclerotic disease, carotid pathology, or cardiovascular risk factors. Results: The patients with significant carotid stenoses had lower mean BDNF values than the reference group (Eastern European, Caucasian race). There was a negative correlation between luminal diameter and BDNF (p=0.049; r=-0.605), and positive correlation between: total Ca2+ area (mm2) and BDNF (r=0.681, p=0.021), proximal Ca2+area and BDNF (p=0.007, r=0.784), fibro-fatty area and total calcium area and NGF (p=0.001). The results of carotid IVUS and VH measurements and serum BDNF and NGF concentrations in patients with carotid stenoses compared to patients without carotid artery disease are discussed. Both groups were free of acute vascular incident in other vascular territories. Conclusion: Preliminary results from a small case series study with intravascular ultrasound and serum levels of BDNF and NGF established a potentially meaningful clinical association between carotid atherosclerosis and neurotrophins, necessitating future expansion of the studies. Abstract

Introduction: Fractional fl ow reserve (FFR) implies the ratio of the maximal hyperemic myocardial blood fl ow in the case of a pathologically changed coronary artery to the maximal hyperemic myocardial blood fl ow in the case of a healthy coronary artery. Aims: The aim of the current study is to perform an economic evaluation and budget impact analysis of measuring FFR as a medical activity in Bulgaria. Material and Methods: For the purpose of the current analysis, two models using Microsoft Excel and TreeAge Pro were developed to evaluate the cost-effectiveness of the FFR-guided percutaneous coronary intervention (PCI) compared to the classic angiography-guided PCI in patients with ischemic heart disease (IHD) with one-vessel coronary artery disease (CAD) or multivessel CAD. The analysis will focus on the health perspective and the payer perspective - National Health Insurance Fund (NHIF). Results: The medico-diagnostic activity related to measuring FFR in patients with multi-vessel coronary artery disease is shown to be a cost-effective therapeutic approach in Bulgaria compared to the angiography-guided PCI (ICER = 50 456 BGN/QALY) with a cost-effectiveness threshold of 51 510 BGN/QALY. FFR- guided PCI strategy in one-vessel coronary artery disease patients is a cost-saving approach (-853 BGN) when compared to the angiography-guided PCI (4 150 BGN). Conclusion: Budget impact analysis revealed that the FFR-guided PCI strategy is a cost-saving alternative approach to the angiographyguided PCI. The savings of the NHIF during the fi rst year of reimbursement of FFR would be -1,1 million BGN and could reach -1,6 million BGN in the third year.

They have cumulative effect in target organ damage. The precise correlation between cardiovascular risk factors and cognitive impairment, as well as assessing the extent to which they may affect cognitive functioning, is difficult to ascertain in everyday clinical practice. Quick, specific, and sensitive neuropsychological tests may be useful in screening for, and the prophylaxis of, target organ damage in hypertensive patients.Methods: We gathered full anamnesis, performed physical examination, laboratory screening and echocardiography. These variables were observed at office and home for all patients, For half of the patients, 24-hour ambulatory blood pressure monitoring and neuropsychological testing using Montreal Cognitive Assessment (MoCA), Mini Mental State Examination (MMSE), Geriatric Depression Scale, and the 4-instrumental activities of daily living scale were undertaken.Results: For a period of 2 years, 931 patients were included after applying the inclusion and exclusion criteria. Abstract

Although disinfection is key to infection control, the colonization patterns and resistomes of hospital-environment microbes remain underexplored. We report the first extensive genomic characterization of microbiomes, pathogens and antibiotic resistance cassettes in a tertiary-care hospital, from repeated sampling (up to 1.5 years apart) of 179 sites associated with 45 beds. Deep shotgun metagenomics unveiled distinct ecological niches of microbes and antibiotic resistance genes characterized by biofilm-forming and human-microbiome-influenced environments with corresponding patterns of spatiotemporal divergence. Quasi-metagenomics with nanopore sequencing provided thousands of high-contiguity genomes, phage and plasmid sequences (>60% novel), enabling characterization of resistome and mobilome diversity and dynamic architectures in hospital environments. Phylogenetics identified multidrug-resistant strains as being widely distributed and stably colonizing across sites. Comparisons with clinical isolates indicated that such microbes can persist in hospitals for extended periods (>8 years), to opportunistically infect patients. These findings highlight the importance of characterizing antibiotic resistance reservoirs in hospitals and establish the feasibility of systematic surveys to target resources for preventing infections., Spatiotemporal characterization of microbial diversity and antibiotic resistance in a tertiary-care hospital reveals broad distribution and persistence of antibiotic-resistant organisms that could cause opportunistic infections in a healthcare setting.

Design, Setting, and ParticipantsThis is a nonprespecified, post hoc analysis of GLOBAL LEADERS, a randomized, open-label superiority trial comparing 2 antiplatelet treatment strategies after PCI. The trial included 130 secondary/tertiary care hospitals in different countries, with 15991 unselected patients with stable coronary artery disease or ACS undergoing PCI. Patients had outpatient visits at 1, 3, 6, 12, 18, and 24 months after index procedure. InterventionsThe experimental group received aspirin plus ticagrelor for 1 month followed by 23-month ticagrelor monotherapy; the reference group received aspirin plus either clopidogrel (stable coronary artery disease) or ticagrelor (ACS) for 12 months, followed by 12-month aspirin monotherapy. In this analysis, we examined the clinical outcomes occurring between 31 days and 365 days after randomization, specifically in patients with ACS who, within this time frame, were assigned to receive either ticagrelor alone or ticagrelor and aspirin. Main Outcomes and MeasuresThe primary outcome was the composite of all-cause death or new Q-wave myocardial infarction. ResultsOf 15968 participants, there were 7487 patients with ACS enrolled; 3750 patients were assigned to the experimental group and 3737 patients to the reference group. Between 31 and 365 days after randomization, the primary outcome occurred in 55 patients (1.5%) in the experimental group and in 75 patients (2.0%) in the reference group (hazard ratio [HR], 0.73; 95% CI, 0.51-1.03; P=.07); investigator-reported Bleeding Academic Research Consortium-defined bleeding type 3 or 5 occurred in 28 patients (0.8%) in the experimental group and in 54 patients (1.5%) in the reference arm (HR, 0.52; 95% CI, 0.33-0.81; P=.004). Conclusions and RelevanceBetween 1 month and 12 months after PCI in ACS, aspirin was associated with increased bleeding risk and appeared not to add to the benefit of ticagrelor on ischemic events. These findings should be interpreted as exploratory and hypothesis generating; however, they pave the way for further trials evaluating aspirin-free antiplatelet strategies after PCI. Abstract

Chemical and lipid composition of species of wild edible mushrooms (Morchella escu- lenta) grown in Bulgaria was examined. Morchella esculenta is rich in carbohydrates (78.33 g 100 g-1 dw), followed by proteins (11.31 g 100 gg-1 dw), ash (7.81 g 100 gg-1 dw) and fat (2.54 g 100 g-1 dw). Also moisture (89.41%), and total energy (1618.04 kJ 100 g-1 dw) were calculated. The content of Saturated fatty acids (SFA) consisted of 32.3%. Unsaturated fatty acids (UFA) in the oil from mushroom are 67.7% and the content of monounsaturated fatty acids (MUFA) consisted of 9.0%. On the other hand, the amount of monounsaturated fatty acids (PUFA) was lower 58.7%. Based on the results, it can be concluded that the examined species of mushrooms is a potential source of phospholipids and fatty acid, and can be used as a functional food or as food additives in many products. Phosphatidic acids were the main components in lipids from Morchella esculenta (19.6%). The content of the other phospholipids in the oils from Morchella esculenta was from 2.7 to 19.5%. Abstract

The effects of cooking methods (steaming, pressure-cooking, microwaving, frying and boiling) on total phenolic contents of Morchella esculenta mushrooms were evaluated. The best cooking method was microwaving with losses up to 2%, and the worst -- boiling with losses amounting to 50%. The results of the study provided some information on the effects of different cooking methods on the phenolic compounds of Morchella esculenta mushrooms, which might encourage the food industry recommending particular cooking methods. Phenolic profiles of raw (control) and cooked mushrooms were analysed. Seven acids were detected in control sample, among which gallic acid amount was the greatest (1.28 ± 0.1 μg g-1), and that of catechin was the least (0.11 ± 0.02 μg g-1). Seven phenolic acids were detected after application of microwaving and steaming methods. After boiling, acids were only four, with highest amount of vanillic acid (0.26 ± 0.03 μg g-1), and lowest -- of protocatechuic acid (0.09 ± 0.02 μg g-1). [ABSTRACT FROM AUTHOR]
