2. Decision-making frameworks for controversial public decision processes: A comparative analysis with a case study

Earlier approaches and most current practices of project evaluation are still driven mainly by technical and economic concerns and do not incorporate environmental or social issues. Therefore, they have failed to account for sustainability concerns in the evaluation of major projects with environmental impacts. In this paper we develop a comparative analysis of three new decision-making frameworks for the assessment of the sustainability of these projects. We analyse similitudes and differences, regarding their theoretical base, objectives, overarching issues and operational implementation, aiming to reveal the main issues of the evaluation process. We also present a case study, with the critical analysis of the ex-post evaluation process of the Alqueva dam multipurpose project based on the comparative analysis of the methodological frameworks presented. The aim is to illustrate the relevance of these methodologies for the improvement of the evaluation processes vis à vis more traditional practices. FCT

Neste artigo procede-se a uma síntese dos modelos formais de interpretação do princípio da precaução, das suas virtualidades, limitações e acções tendentes a ultrapassá-las. O conceito de precaução apresenta grande relevância na regulação ambiental na União Europeia. Por um lado, e apesar da natureza algo vaga da legislação, o princípio da precaução tem merecido desenvolvimentos recentes no âmbito de quadros e modelos de interpretação económica e da sua aplicação. Há, por outro lado, a necessidade de quadros reguladores para a implementação operacional do princípio da precaução na tomada de decisão, isto é, de clarificar conceitos e procedimentos de gestão adequados à natureza dos ris os ambientais. Importa, por isso, conhecer as abordagens e os modelos económicos mais relevantes com o objectivo de procurar conhecer o seu contributo para o debate sobre a precaução no contexto da gestão dos riscos ambientais discutir a sua relevância prática em termos da decisão pública.

This thesis aims to address the practical and institutional problems in the development of farmers' microcredit to ultimately contribute to achieve sustainable development. The author, based on his long-term work experience in rural finance, analyzes the current situation of microcredit in rural areas and presents a set of mechanisms for risk management and scientific pricing of farmers’ in the framework of the construction of an iterative model with the support of statistical analysis. To be specific, based on improving farmers' scheduled repayment rate and a study of farmers' microcredit factors, criteria are formulated for selecting credible borrowers among farmers; Making full use of the decisive role of the market, an effective risk management model is gradually formulated that helps to foster a credit-based financial service environment in rural areas. Besides, software engineering is utilized to optimize and control farmers' microcredit risk management and scientific pricing of credit products. The study suggests that the farmers' microcredit risk control mechanisms established can effectively enable farmers' microcredit financial institutions to function as a main market player, while overcoming theoretical and practical difficulties in microcredit for farmers. Four suggestions are also presented. First, ensure that farmers enjoy the "right of approval" when applying for loans; second, transfer the "right of pricing" loan interests to farmers; third, transfer the "right of salary distribution" and risk control of loans to loan officers; and fourth, entrust the "management right" of loans to the system. Esta tese tem como objetivo abordar os problemas práticos e institucionais no desenvolvimento do microcrédito dos agricultores para, finalmente, contribuir para alcançar o desenvolvimento sustentável. O autor, com base em sua experiência do trabalho de longo prazo em finanças rurais, analisa a situação atual do microcrédito em áreas rurais e apresenta um conjunto de mecanismos para a gestão do risco e a determinação científica de preços para o microcrédito dos agricultores no quadro da construção de um modelo interativo com o apoio da análise estatística. Mais especificamente, com base na melhoria da previsão da taxa de reembolso do microcrédito pelos agricultores e em um estudo dos fatores condicionantes desse reembolso, são formulados critérios para selecionar mutuários confiáveis entre os agricultores; Fazendo pleno uso do papel decisivo do mercado, é formulado gradualmente um modelo eficaz de gestão de risco que ajuda a promover nas áreas rurais um ambiente favorável de serviços financeiros baseados no crédito. Além disso, a engenharia de software é utilizada para otimizar e controlar a gestão de riscos de microcrédito dos agricultores e a determinação científica dos preços dos produtos de crédito. O estudo demonstra que o mecanismo de controle de risco de microcrédito dos agricultores assim estabelecido pode efetivamente permitir que as instituições financeiras de microcrédito funcionem como um participante principal do mercado, ultrapassando dificuldades que a teoria e a prática têm revelado no microcrédito para os agricultores. Quatro sugestões são ainda apresentadas. Em primeiro lugar, garantir que os agricultores desfrutem do "direito de aprovação" quando solicitam empréstimos. Em segundo lugar, transferir para os agricultores "direito de determinação do preço" da taxa de juro do empréstimo. Em terceiro lugar, passar o "direito de distribuição salarial" e o controle de risco dos empréstimos para os agentes de crédito. Em quarto lugar, confiar ao sistema o "direito de gestão" dos empréstimos.

Em Moçambique, o consumo energético cresceu 56% entre 2002 e 2008, para tal tendo contribuído o alargamento da base da população com acesso a energia, fruto da aposta do Governo na electrificação nacional e do crescimento das indústrias de consumo intensivo de energia. E a maior estabilidade macroeconómica e política que o País alcançou determinou o crescimento económico do País, com impactos no rendimento disponível (crescimento do PIB) e na modernização do tecido empresarial público e privado (em particular os serviços), cada vez mais exigente em termos de utilização de energia. De forma a expandir e atrair o sector privado para a indústria de electricidade de modo sustentável, as tarifas devem reflectir os custos de fornecimento de serviços pelas empresas, isto é serem competitivas e adaptadas ao mercado, visto que constituem a principal fonte de financiamento para a expansão do sector de energia elétrica. In Mozambique, energy consumption grew 56% between 2002 and 2008, largely due to the expansion of the population with access to electricity, the government's investment in national electrification and the growth of energy – intensive industries. The macroeconomic and political stability determined the country's economic growth, with significant impacts on disposable income and on the modernization of the public and private business (including services), increasingly demanding in terms of energy use. In order to expand and attract the private sector to the electricity industry in a sustainable manner, tariffs must reflect the costs of providing services for companies, i.e. be competitive and market driven, since they are the main source of funding for the expansion of the electricity sector.

Purpose: Several studies have been carried out, and there is no classification for proximal humeral fractures (PHF) exempted from variability in interpretation and with questioned reliability. In the present study, we investigated the 'absolute diagnostic reliability' of the most currently used classifications for PHFs on a single anterior-posterior X-ray shoulder image., Methods: Six orthopaedic surgeons, with varying levels of experience in shoulder pathology, evaluated radiographs from 30 proximal humeral fractures, according to the 'absolute reliability' criteria. Each of the observers rated each fracture according to Neer, Müller/AO and Codman-Hertel's classification systems., Results: The overall inter-observer agreement (κ) has been 0.297 (CI95% 0.280 to 0.314) for the Neer's classification system, 0.206 (CI95% 0.193 to 0.218) for the Müller/AO classification system, and 0.315 (CI95% 0.334 to 0.368) for the Codman-Hertel classification system. We found loss of agreement in Neer's classification as the study progressed, low agreement in the AO classification, and stable values in the different evaluations with the best degree of agreement for Codman-Hertel classification, with a moderate agreement in the second evaluation among the six evaluators., Conclusion: The Neer, AO, and Hertel-Codman classification systems for PHF with a single radiographic projection have a difficult interpretation for orthopaedic surgeons of varying levels of experience, and therefore substantial agreements are not obtained.

- Abstract
The push-pull solvatochromic pyrene derivatives PA and PK have been applied to the study of model membrane vesicles, cells and purified human serum lipoproteins, using both confocal fluorescence microscopy and fluorescence spectroscopy. These polarity-sensitive probes provide information similar to that obtained by Laurdan or Prodan, i.e. mainly lipid order in biomembranes, but they have the essential advantage of being excitable by a standard 405 nm laser light, bypassing the use of multiphoton excitation. In addition, they are brighter and much more photostable than those dimethylamino naphthalene derivatives. Our results with model membrane spectroscopy (multilamellar vesicles) and with microscopy (giant unilamellar vesicles) showed the capacity of PA and PK to report differently on liquid-disordered, liquid-ordered and gel phase bilayers. Moreover, a ratiometric parameter, the Red/Blue Intensity Ratio (RBIR) could be used for inter-domain, inter-vesicle and even inter-technique comparison, and the appropriate microscopy-spectroscopy conversion coefficients could be estimated. In studies at the cellular level, PA probe stained almost exclusively the plasma membrane of red blood cells, revealing its high degree of lipid order. Using Chinese Hamster Ovary cells PA was shown to be an excellent probe for the detection of cytoplasmic lipid droplets, superior to Nile Red in that PA provides simultaneously a detailed information of membrane order in the whole cell, in which the lipid droplets appear with a very good contrast. Moreover, spectrofluorometric data of PA-stained serum lipoproteins indicated an essentially identical value of RBIR for lipid droplets and for high-density lipoproteins., (Copyright © 2020 Elsevier B.V. Sphingolipids consist of a sphingoid base N-linked to a fatty acyl chain. Among them, sphingomyelins (SM) are major components of mammalian cells, while ceramide (Cer) plays an important role as a lipid second messenger. We have performed a quantitative lipidomic study of Cer and SM species in different mammalian tissues (adipose tissue, liver, brain and blood serum of human, mice, rat and dog), as well as in cell cultures of mammalian origin (primary hepatocytes, immortalized MDCK cells, mice melanoma b16 cells, and mice primary CD4 + T lymphocytes) using an ultra-high performance liquid chromatography coupled to time-of-flight mass spectrometry (UHPLC-ToF-MS)-based platform. The data have been compared with published, in general semi-quantitative, results from 20 other samples, with good agreement. The sphingoid base was predominantly d18-1 sphingosine (2-amino-4-octadecene-1,3-diol) in all cases. The fatty acid composition of SM was clearly different from that of Cer. In virtually all samples the most abundant Cer species were those containing C24:0 and C24:1 in their N-acyl chains, while the main species contained in SM was C16:0. 14. Clearly Detectable, Kinetically Restricted Solid-Solid Phase Transition in cis-Ceramide Monolayers.

Sphingosine [(2 S,3 R,4 E)-2-amino-4-octadecene-1,3-diol] is the most common sphingoid base in mammals. Ceramides are N-acyl sphingosines. Numerous small variations on this canonical structure are known, including the 1-deoxy, the 4,5-dihydro, and many others. However, whenever there is a Δ4 double bond, it adopts the trans (or E) configuration. We synthesized a ceramide containing 4 Z-sphingosine and palmitic acid ( cis-pCer) and studied its behavior in the form of monolayers extended on an air-water interface. cis-pCer acted very differently from the trans isomer in that, upon lateral compression of the monolayer, a solid-solid transition was clearly observed at a mean molecular area ≤44 Å
2 ·molecule-1 , whose characteristics depended on the rate of compression. The solid-solid transition, as well as states of domain coexistence, could be imaged by atomic force microscopy and by Brewster-angle microscopy. Atomistic molecular dynamics simulations provided results compatible with the experimentally observed differences between the cis and trans isomers. The data can help in the exploration of other solid-solid transitions in lipids, both in vitro and in vivo, that have gone up to now undetected because of their less obvious change in surface properties along the transition, as compared to cis-pCer.- Published

- Abstract
The balance between the efficacy and safety of anticoagulant therapy in patients aged >/=90 years with venous thromboembolism (VTE) is uncertain. RIETE is an ongoing, prospective registry of consecutive patients with acute, objectively confirmed, symptomatic VTE. We evaluated the efficacy and safety of anticoagulant therapy during the first three months in all patients aged >/=90 years. In addition, we tried to identify those at a higher risk for VTE. Of 21,873 patients enrolled from March 2001 to February 2008, 610 (2.8%) were aged >/=90 years. Of these, 307 (50%) presented with pulmonary embolism (PE), 240 (39%) had immobility >/=4 days, and 271 (44%) had abnormal creatinine levels. During the first three months of therapy, 140 patients aged >/=90 years (23%) died. Of these, 45 (32%) died of PE (34 of the initial episode, 11 of recurrent PE), 18 (13%) had fatal bleeding. Recent immobility >/=4 days was the most common risk factor for VTE (240 of 610 patients, 39%), but only 54 of them (22%) had received thromboprophylaxis. The most frequent causes for immobility were senile dementia, acute infection, trauma or decompensated heart failure. The duration of immobility was <4 weeks in 126 patients (52%), and most of them were bedridden at home. In conclusion, one in every four VTE patients aged >/=90 years died during the first three months of therapy. Of these, one in every three died of PE, one in every eight had fatal bleeding. Identifying at-risk patients may help to prevent some of these deaths.

- Abstract
Human herpes virus 8 (HHV8) was discovered in 1994 in the biopsy of a Kaposi's sarcoma in a patient with AIDS. Since then it has been identified in all variants of Kaposi's sarcoma and in another two rare disorders: multicentric Castleman's disease and primary body-cavity based lymphomas. The case discusses a 68 year old, HIV-negative male patient, presenting Kaposi's sarcoma for one year and being monitored by dermatology, who presented for weakness, anorexia and fever. On examination, he was found to have adenitis of the lymph nodes in his neck, underarm and groin. A biopsy on one of the swellings led to findings characteristic of multicentric plasma cell variant Castleman's disease. Blood tests for HHV8 and HIV were carried out, resulting positive and negative respectively (IgG anti-HHV8 positive, title 1/640, indirect immunofluorescence). PCR amplification showed HHV8 in peripheral blood. Patient received 8 cycles of CHOP and rituximab, leading to complete disappearance of the adenitis and general symptoms, with no worsening of his Kaposi's sarcoma. Patient remained in complete remission for 10 months after treatment. This paper discusses the case of a HIV-, HHV8+ patient, diagnosed with classic Kaposi's sarcoma, who developed multicentric plasma cell variant Castleman's disease. The coincidence of two or more HHV8-related illnesses in a HIV-negative patient has rarely been described in medical literature. Treatment with rituximab combined with CHOP chemotherapy was effective in this case, and no worsening of the patient's KS was observed.
