The geographical location and differences in tumor biology significantly change the management of gastric cancer. The prevalence of gastric cancer ranks fifth and sixth among men and women, respectively, in Turkey. The international guidelines from the Eastern and Western countries fail to manage a considerable amount of inconclusive issues in the management of gastric cancer. The uncertainties lead to significant heterogeneities in clinical practice, lack of homogeneous data collection, and subsequently, diverse outcomes. The physicians who are professionally involved in the management of gastric cancer at two institutions in Istanbul, Turkey, organized a consensus meeting to address current problems and plan feasible, logical, measurable, and collective solutions in their clinical practice for this challenging disease. The evidence-based data and current guidelines were reviewed. The gray zones in the management of gastric cancer were determined in the first session of this consensus meeting. The second session was constructed to discuss, vote, and ratify the ultimate decisions. The identification of the T stage, the esophagogastric area, imaging algorithm for proper staging and follow-up, timing and patient selection for neoadjuvant treatment, and management of advanced and metastatic disease have been accepted as the major issues in the management of gastric cancer. The recommendations are presented with the percentage of supporting votes in the results section with related data.

This is a case of 52 years old male patient with chronic obstructive pulmonary disease (COPD) who developed semi-invasive pulmonary aspergillosis while on pegylated-interferon alpha-2a and for chronic hepatitis C infection. This complication is very rarely seen during interferon therapy for other any purposes. Pulmonary aspergillosis in particular group of chronic hepatitis C patients under immunosupressive therapy should also be in mind and interdisciplinary medical cooperation might be needed during the treatment for immune-suppressed patients. Pulmonary aspergillosis is also discussed from many aspects in the light of the literature.

In previous studies of proton pump inhibitors may have effects on systems other than the stomach has not been investigated sufficiently. Özellikle yoğun bakım şartlarında tedavi edilen hastalarda kullanılması ile ortaya çıkan çok önemli komplikasyonların, mide pH sının yükselmesi ile sekonder gelişen bakteri kolonizasyonundaki artmaya bağlı olabileceği öne sürülmüştür. Ancak bu komplikasyonların mukozal immün sistemde olabilecek bir defekt veya yetmezlik sonucu olabileceğine dair yapılmış herhangi bir çalışma bulunmamaktadır. Bu nedenle çalışmamızda, mukozal immün sistemin komponentlerinden biri olan paneth hücreleri üzerine, proton pompası inhibitörlerinin olası etkilerinin araştırılması amaçlandı. Ayrıca, antibiyotik uygulamaları ile ortamda bakterilerin bulunması ve paneth hücrelerinden antimikrobiyal madde salınımı arasında ilişkinin ortaya konması planlandı. Bu amaçla 42 adet Wistar Albino sıçan, altı ayrı gruba ayrılarak; ayrı gruplarda H2 reseptör blokeri, proton pompası inhibitörü ve bu gruplardan her birinde, antibiyotik ile birlikte verildi. Işık mikroskop ile yapılan histolojik incelemeler sonucunda, pantoprazolün ince barsak paneth hücreleri üzerine etkili olduğu ve böylece mukozal immün sistemin bir komponenti olan antibiyotik peptitlerin degranülasyonunu engellediği saptandı. Mukozal direnci düşürecek bu etkinin; yakın konumlu enterokromafin hücreler, mast hücreleri ve ileum lamina propriası lökositlerini de dolaylı veya doğrudan etkilediği bulundu. Genel yaygın bilgiye karşın, pantoprazolün mide pariyetal hücreleri dışındaki proton pompası içeren hücrelere de etkiyebileceği düşünüldü. Paneth hücrelerinde gözlenen etkinin, gerek barsak ve diğer mukozal savunma sistemlerine olası etkilerinin gerekse sistemik etkilerinin incelenebilmesi yolunda ileri çalışmalara öncülük edebileceği öngörüldü.

Duodenal adenocarcinomas account for 0.5% of all gastrointestinal malignancies. These malignancies are generally characterized by blood loss and self-limited bleeding. We report adenocarcinoma of the duodenum causing recurrent, clinically significant bleeding. Case: A 59-year-old male was admitted to hospital with recurrent melena. History revealed an 8 kg weight loss in 1.5 months, and signs of anemia were observed on physical examination. Hb: 7.5 g/dl, MCV: 69.7, and sedimentation rate of 6 mm/h were detected in laboratory examination. An actively bleeding, ulcero-vegetan mass involving the anterior wall of the bulbus and second part of the duodenum was observed in endoscopy. Since active bleeding continued, Whipple operation was applied in the Surgery Department. Pathologic examination of the surgical samples showed stage III, primary adenocarcinoma of the duodenum arising from villous adenoma, Primer duodenal adenokarsinomlar tüm gastrointestinal malinitelerin %0.5'lik kısmını oluşturur. Endoskopide, bulbusun ön duvarı ve duodenum 2. kısmını içine alan aktif kanamalı ülserovejetan kitle lezyonu gözlendi. Kanamanın devam etmesi nedeniyle cerrahiye devredilen hastaya Whipple operasyonu uygulandı. Patolojik inceleme sonucu evre III villöz adenomdan kaynaklanan primer duodenal adenokarsinom tespit edildi.

In this studry 32 hyperglycemic patients with type II DM were treated with diet (7 patients), with gliclazide, an oral antidiabetic agent (10 patients), or with insulin (15 patients). The levels of liporoteins, apolipoprotein A and B and LCAT enzyme are measured before the therapy and 3 weeks after the achievement of glycemic control. Treatment types are compared with each other and with control group (10 normal persons) triglyseride (TG) and VLDL levels which were found to be high in all of the patients decreased in all groups to levels similar to control group. Genellikle 1-3 cm boyutların da olan lezyonun tanısı obtrüksiyona bağlı yakınmalarla veya rastlantısal olarak konulmaktadır. Yazımızda anemiye neden olan dev ileal inflamatuar fibroid polip olgusu sunulmuştur. Olgu: 53 yaşında erkek hasta son haftalarda gelişen güçsüzlük ve renk sol gunluğu yakınması ile başvurdu. Fizik muayene ve laboratuar incelemeleri ile demir eksikliği anemisi tanısı konuldu. Anemi etiyolojisi araştırılırken ya pılan kolonoskopide terminal ileumda valv’den yaklaşık 20 cm proksimalde 7-8 cm boyutunda pedinküle polipoid lezyon mevcuttu. Hastaya laparosko pik segmenter ince barsak rezeksiyonu yapıldı. Lezyonun patolojik incele mesinde iltihabi fibroid polip saptandı. Sonuç :İnflamatuar fibroid polip demir eksikliği anemisine yol açabilen poli poid nitelikteki oluşumların ayırıcı tanısında göz önünde bulundurulmalıdır.

Iıntroduction and Aim: Inflammatory Fibroid Polyp is a rare benign polipoid lesion of the Gastrointestinal tract commonly found in the stomach, however, it can also be found elsewhere throughout the gastrointestinal tract. The lesion is typically 1-3 cm in size, generally found as an obstructive lesion, however, may also be an isolated incidental finding during endoscopy. We presented here a case with giant inflammatory fibroid polyp causing anemia. Case: A 53 year old male presented to our hospital with a chief complaint of weakness and pallor. Subsequently, physical exam and laboratory evaluation were consistent iron deficiency anemia which prompted colonoscopic investigation that revealed a 7 cm pedunculated polyp in the ileum approximately 20cm proximal to the ileo-cecal valve. As a result, a laparoscopic segmental resection of the ileum was performed and the histopathology confirmed the diagnosis of Inflammatory Fibroid Polyp.
