Abstract Background Echocardiography is widely used to evaluate left ventricular (LV) diastolic function in patients suspected of heart failure. For patients in sinus rhythm, a combination of several echocardiographic parameters can differentiate between normal and elevated LV filling pressure with good accuracy. However, there is no established echocardiographic approach for the evaluation of LV filling pressure in patients with atrial fibrillation. The objective of the present study was to determine if a combination of several echocardiographic and clinical parameters may be used to evaluate LV filling pressure in patients with atrial fibrillation. Results In a multicentre study of 148 atrial fibrillation patients, several echocardiographic parameters were tested against invasively measured LV filling pressure as the reference method. No single parameter had sufficiently strong association with LV filling pressure to be recommended for clinical use. Based on univariate regression analysis in the present study, and evidence from existing literature, we developed a two-step algorithm for differentiation between normal and elevated LV filling pressure, defining values ≥ 15 mmHg as elevated. The parameters in the first step included the ratio between mitral early flow velocity and septal mitral annular velocity (septal E/e’), mitral E velocity, deceleration time of E, and peak tricuspid regurgitation velocity. Patients who could not be classified in the first step were tested in a second step by applying supplementary parameters, which included left atrial reservoir strain, pulmonary venous systolic/diastolic velocity ratio, and body mass index. ObjectivesTo determine the risk of mortality and need for aortic valve replacement (AVR) in patients with low-flow low-gradient (LFLG) aortic stenosis (AS).MethodsA longitudinal multicentre study including consecutive patients with severe AS (aortic valve area [AVA] < 1.0 cm2) and normal left ventricular ejection fraction (LVEF). Patients were classified as: high-gradient (HG, mean gradient ≥ 40 mmHg), normal-flow low-gradient (NFLG, mean gradient < 40 mmHg, indexed systolic volume (SVi) > 35 ml/m2) and LFLG (mean gradient < 40 mmHg, SVi ≤ 35 ml/m2).ResultsOf 1,391 patients, 147 (10.5%) had LFLG, 752 (54.1%) HG, and 492 (35.4%) NFLG. Echocardiographic parameters of the LFLG group showed similar AVA to the HG group but with less severity in the dimensionless index, calcification, and hypertrophy. The HG group required AVR earlier than NFLG (p < 0.001) and LFLG (p < 0.001), with no differences between LFLG and NFLG groups (p = 0.358). Overall mortality was 27.7% (CI 95% 25.3–30.1) with no differences among groups (p = 0.319). The impact of AVR in terms of overall mortality reduction was observed the most in patients with HG (hazard ratio [HR]: 0.17; 95% CI: 0.12–0.23; p < 0.001), followed by patients with LFLG (HR: 0.25; 95% CI: 0.13–0.49; p < 0.001), and finally patients with NFLG (HR: 0.29; 95% CI: 0.20–0.44; p < 0.001), with a risk reduction of 84, 75, and 71%, respectively.ConclusionsParadoxical LFLG AS affects 10.5% of severe AS, and has a lower need for AVR than the HG group and similar to the NFLG group, with no differences in mortality. AVR had a lower impact on LFLG AS compared with HG AS. Therefore, the findings of the present study showed LFLG AS to have an intermediate clinical risk profile between the HG and NFHG groups.

Background: Wild-type transthyretin amyloidosis is a systemic disease with predominantly cardiac symptoms. The aim of this study was to assess the short-term prognosis of these patients through contractile reserve measured by stress echocardiography, given the usefulness that this parameter has demonstrated in other populations. We considered major events as death from any cause and hospitalization for heart failure. Material and methods: We conducted a study with a 1-year follow-up in 11 patients who were proposed to undergo a stress echocardiogram, with the follow-up as usual according to their doctor. We excluded pacemaker wearers, patients with permanent atrial fibrillation, those incapable of exertion at low loads, and those with poor acoustic windows. Results: We found that contractile reserve estimated by right ventricular free wall longitudinal strain is correlated with a lower rate of death (all of them cardiovascular deaths) and hospitalizations for heart failure. Conclusions: Contractile reserve assessed by right ventricular free wall longitudinal strain is a predictor of major events in patients with wild-type transthyretin cardiac amyloidosis.

Background Malnutrition is associated with poor prognosis in several cardiovascular diseases. However, its prognostic impact in patients undergoing transcatheter edge‐to‐edge mitral valve repair (TEER) is not well known. This study sought to assess the prevalence, clinical associations, and prognostic consequences of malnutrition in patients undergoing TEER. Methods and Results A total of 892 patients undergoing TEER from the international MIVNUT (Mitral Valve Repair and Nutritional Status) registry were studied. Malnutrition status was assessed with the Controlling Nutritional Status score. The association of nutritional status with mortality was analyzed with multivariable Cox regression models, whereas the association with heart failure admission was assessed by Fine‐Gray models, with death as a competing risk. Resumen: Introducción: en los procedimientos de extracción de electrodos, la vía femoral se usa cuando la vía superior ha fracasado. Objetivo: describir la incidencia, el éxito, las complicaciones y los predictores de uso de la vía femoral. Métodos: se realizó un análisis de la incidencia del uso de la vía femoral y los factores predictores en pacientes a quienes se les realizó extracción de electrodos entre noviembre de 2011 y noviembre de 2017. Resultados: se incluyeron 85 pacientes, con edad media de 62,36 ± 11,15 años. El 38,9% de los dispositivos eran marcapasos. Se extrajeron 135 electrodos, 59,3% de fijación pasiva. La mediana de tiempo desde el implante fue de 102 (60-174) meses. Se empleó la ruta femoral en el 25,9% de los procedimientos. Se obtuvo éxito clínico en el 92,9% de los pacientes. La extracción no fue exitosa en el 22,7% de los procedimientos cuando se usó la vía femoral, en comparación con el 1,6% cuando se usó la vía superior (p 0,004). La extracción no fue completa en el 36,4% de los procedimientos cuando se empleó la vía femoral en comparación con el 9,5% por vía superior (p 0,007).Los factores que predijeron el empleo de la ruta femoral fueron la presencia de electrodos de fijación pasiva [OR IC 95% 13,69 (3,06-62,5) p 0,001] y el tiempo desde el implante del electrodo [OR IC 95% por cada 10 meses 1,04 (1,00-1,09) p 0,044]. Conclusiones: se empleó la ruta femoral en el 25,9% de los procedimientos. No fue eficaz en el 22,7% de las intervenciones. Los factores que predijeron su utilización fueron la presencia de electrodos de fijación pasiva y el tiempo desde el implante del electrodo. Abstract: Introduction: The femoral route is used in electrode removal procedures when the upper route has failed. Objective: To describe the incidence, success rate, complications and predictive factors for the use of the femoral route. Methods: An analysis was performed on the incidence of use of the femoral route and the predictive factors in patients in whom electrode removal was carried out between November 2011 and November 2017. Results: The study included 85 patients with a mean age of 62.36 ± 11.5 years. Pacemakers made up 38.9% of the devices. A total of 135 electrodes, 59.3% of passive fixation, were removed. The median time since the implant was 102 (60-174) months. The femoral route was used in 25.9% of the procedures. Clinical success was achieved in 92.9% of the patients. The removal was not successful in 22.7% of the procedures when the femoral route was used, compared to 1.6% when the upper route was used (P = .004). The removal was not completed in 36.4% of the procedures when the femoral route was used, compared to 9.5% with the upper route (P = .007).The factors that predicted the use of the femoral route were the presence of passive-fixation electrodes (OR = 13.69: 95% CI; 3.06 – 62.5, P = .001), and the time since the electrodes were implanted (OR = 1.04, 95% CI; 1.00 – 1.09, P = .044, for every 10 months). Conclusions: The femoral route was employed in 25.9% of the procedures. It was not effective in 22.7% of the interventions. The factors that predicted its use were the presence of passive-fixation electrodes and the time since the electrode implant.

RESUMEN La insuficiencia tricuspídea se asocia de manera independiente con un aumento de la mortalidad. Investigaciones recientes demuestran que la cirugía aislada de la válvula tricúspide presenta la mortalidad quirúrgica más alta dentro de los procedimientos valvulares. Las terapias percutáneas han surgido como una alternativa a la cirugía en pacientes seleccionados de alto riesgo quirúrgico. El dispositivo MitraClip en posición tricuspídea ha sido el más empleado. El propósito de este artículo es revisar los diferentes dispositivos transcatéter para el tratamiento de la insuficiencia tricuspídea, con especial énfasis en el MitraClip. Se describirán las vistas básicas de la exploración por imagen y el procedimiento paso a paso para la inserción del clip. Este documento pretende ser una guía para la sistematización de la evaluación de la válvula tricúspide y los pasos del implante del dispositivo para asegurar el éxito del procedimiento.

ABSTRACT Severe tricuspid regurgitation is independently associated with an increased mortality. Recent studies show that surgery for isolated tricuspid valve (TV) is still associated with the highest surgical risk of all valve procedures. Percutaneous treatments for the management of tricuspid regurgitation have become an alternative to surgery in select patients of high surgical risk. To this day, the most widely used device is MitraClip in the tricuspid position. We reviewed the different devices used today for transcatheter treatments paying special attention to MitraClip. We describe the basic standpoints on transesophageal echocardiography screening and step-by-step procedure for the implantation of the clip. This document stands as a guide to systematize tricuspid valve assessments and the steps involved in device implantation to achieve successful procedures.

La insuficiencia mitral (IM) es muy prevalente en Europa a pesar de la reducción de la fiebre reumática. El desarrollo de las técnicas de reparación mitral introducido por Alain Carpentier ha cambiado el pronóstico y el manejo de los pacientes con IM. Las técnicas de imagen son fundamentales para la evaluación del tipo de lesión anatómica, etiología, mecanismo, cuantificación, así como para valorar la posibilidad de éxito de la reparación. La ecocardiografía transesófagica (ETE) bidimensional se emplea de forma rutinaria para planear la reparación mitral. Sin embargo, la ETE tiene una serie de dificultades en relación a las relaciones anatómicas y las anomalías morfológicas de la válvula mitral (VM). La recientemente introducida ecocardiografía tridimensional (3D) nos ha permitido entender la anatomía funcional de la VM, la fisiopatología de las lesiones mitrales y, en especial, de la insuficiencia mitral funcional (IMF). En la actualidad, la ecotransesofágico 3D (ETE-3D) supone una herramienta imprescindible, tanto para el diagnóstico de lesiones mitrales como para la monitorización en el quirófano de cirugía cardíaca y de los procedimientos percutáneos en hemodinámica. Permite evaluar online todas las estructuras cardíacas y ofrece nuevos planos como la «vista de cirujano» para valorar la morfología de la VM en una sola adquisición. El objetivo de este artículo es revisar la aportación de la ETE, así como la ETE-3D en el conocimiento de la anatomía funcional de la VM, sus aplicaciones clínicas y sus implicaciones terapéuticas.

Existen pocas actuaciones quirúrgicas en la enfermedad cardiovascular cuya contribución a la calidad y duración de la vida del paciente sea tan clara como la corrección de la valvulopatía aórtica. En los países desarrollados, la estenosis aórtica (EAo) es la enfermedad valvular de mayor prevalencia. Al ser una manifestación del envejecimiento, esta valvulopatía es cada vez más frecuente, en tanto que la esperanza de vida de la población aumenta. La EAo grave sintomática es fatal si no se trata quirúrgicamente, por el contrario, si se interviene en su debido momento, el pronóstico es excelente. Existe controversia en distintos temas relacionados con la EAo, como en el manejo médico de la enfermedad, el momento de la intervención en pacientes asintomáticos y en aquellos con enfermedad muy avanzada, y el papel del tratamiento percutáneo. La incidencia de IAo clínicamente significativa aumenta con la edad, con un pico en la cuarta-sexta décadas. La prevalencia de la misma en el estudio Framingham fue del 4,9%, con regurgitación moderada o grave en el 0,5%. En los pacientes jóvenes la primera opción de tratamiento a considerar debe ser la reparación aórtica, seleccionando cuidadosamente la técnica apropiada dependiendo del análisis de la válvula y de la raíz aórtica. El campo de los procedimientos intervencionistas quirúrgicos y/o percutáneos para el tratamiento de la valvulopatía aórtica ha crecido de forma marcada en los últimos años, y es preciso que tanto pacientes como especialistas cardiólogos y cirujanos cardiovasculares estén al tanto de estas nuevas opciones.
