Abstract

Background: The relationship between morbid obesity and long-term patient outcomes after primary total hip arthroplasty (THA) has been understudied. The purpose of this study was to determine the association between morbid obesity and 10-year complications (revision surgery, reoperation, dislocation) and mortality in patients undergoing primary THA. Methods: We conducted a population-based cohort study of patients aged 45-74 years who underwent primary THA for osteoarthritis between 2002 and 2007 using Ontario administrative health care databases. Patients were followed for 10 years. We estimated risk ratios (RRs) of mortality, reoperation, revision and dislocation in patients with body mass index (BMI) greater than 45 kg/[m.sup.2] (morbidly obese patients) compared with patients with a BMI of 45 kg/[m.sup.2] or less (nonmorbidly obese patients). Results: There were 22251 patients in the study cohort, of whom 726 (3.3%) were morbidly obese. Morbid obesity was associated with higher 10-year risk of death (RR 1.38, 95% confidence interval [CI] 1.18-1.62). Risks of revision (RR 1.43, 95% CI 0.96-2.13) and dislocation (RR 2.38, 95% CI 1.38-4.10) were higher in morbidly obese men than in nonmorbidly obese men; there were no associations between obesity and revision or dislocation in women. Risk of reoperation was higher in morbidly obese women than in nonmorbidly obese women (RR 1.59, 95% CI 1.05-2.40); there was no association between obesity and reoperation in men. Conclusion: Morbidly obese patients undergoing primary THA are at higher risk of long-term mortality and complications. There were differences in complication risk by sex. The results of this study should inform perioperative counselling of patients considering THA. Contexte: Le lien entre l'obesite morbide et les issues a long terme des patients ayant subi une arthroplastie totale primaire de la hanche (ATH) est sous-etudie. Cette etude visait a caracteriser l'association entre l'obesite morbide et les complications (chirurgie de revision, reintervention, dislocation) et la mortalite sur 10 ans chez les patients ayant subi une ATH. Methodes: Nous avons mene une etude de cohorte basee sur la population aupres de patients de 45 a 74 ans atteints d'arthrose ayant subi une ATH primaire entre 2002 et 2007 en utilisant les bases de donnees administratives en sante de l'Ontario. Les patients ont ete suivis pour une periode de 10 ans. Nous avons estime des rapports de risque (RR) pour la mortalite, la reintervention, la chirurgie de revision et la dislocation chez les patients ayant un indice de masse corporelle (IMC) de plus de 45 kg/[m.sup.2] (obesite morbide) en comparaison avec les patients ayant un IMC de 45 kg/[m.sup.2] ou moins. Resultats: L'etude de cohorte comptait 22251 patients, dont 726 (3,3%) etaient atteints d'obesite morbide. L'obesite morbide a ete associee a un risque de mortalite sur 10 ans accru (RR 1,38; intervalle de confiance [IC] de 95% 1,18-1,62). Le risque de chirurgie de revision (RR 1,43; IC de 95% 0,96-2,13) et de dislocation (RR 2,38; IC de 95% 1,38-4,10) etait plus eleve chez les hommes atteints d'obesite morbide que chez les autres hommes; aucune association n'a ete observee entre l'obesite et la chirurgie de revision ou la dislocation chez les femmes. Abstract

BackgroundKnee pain is one of the major sources of pain and disability in developed countries, particularly in aging populations, and is the primary indication for total knee arthroplasty (TKA) in patients with osteoarthritis (OA).ObjectivesTo determine the presence of myofascial pain in OA patients waitlisted for TKA and to determine whether their knee pain may be alleviated by trigger point injections.MethodsFollowing ethics approval, 25 participants were recruited from the wait list for elective unilateral primary TKA at the study centre. After providing informed consent, all participants were examined for the presence of active trigger points in the muscles surrounding the knee and received trigger point injections of bupivacaine. Assessments and trigger point injections were implemented on the first visit and at subsequent visits on weeks 1, 2, 4 and 8. Outcome measures included the Timed Up and Go test, Brief Pain Inventory, Centre for Epidemiologic Studies Depression Scale, State-Trait Anxiety Inventory and Short-Form McGill Pain Questionnaire.ResultsMyofascial trigger points were identified in all participants. Trigger point injections significantly reduced pain intensity and pain interference, and improved mobility. All participants had trigger points identified in medial muscles, most commonly in the head of the gastrocnemius muscle. An acute reduction in pain and improved functionality was observed immediately following intervention, and persisted over the eight-week course of the investigation.ConclusionAll patients had trigger points in the vastus and gastrocnemius muscles, and 92% of patients experienced significant pain relief with trigger point injections at the first visit, indicating that a significant proportion of the OA knee pain was myofascial in origin. Further investigation is warranted to determine the prevalence of myofascial pain and whether treatment delays or prevents TKA.

Abstract

The Canadian Collaborative Acute Pain Initiative, established in 2002, is a voluntary, multidisciplinary consortium of acute pain health professionals from across Canada whose goal is to improve acute pain management through discussion and consensus. The group met in January 2002 to define strategic areas related to the treatment of acute pain. The areas identified were: the definition of pain; the epidemiology of pain; the concept of an 'ideal' acute pain management service; education; therapeutic options; symptom management; and research and safety. In November 2002, a second meeting was held to develop objectives and recommendations for the management of acute pain based on the defined areas. The outcome of these discussions is summarized in this paper.

37. Health-related quality of life of Canadian forces veterans after transition to civilian life

Abstract

OBJECTIVES: Describe health-related quality of life (HRQoL) of former Canadian Forces (CF) men and women in uniform (Veterans) after transition to civilian life, and compare to age- and sex-adjusted Canadian norms. METHODS: The 2010 Survey on Transition to Civilian Life was a national computer-assisted telephone survey of CF Regular Force personnel who released during 1998-2007. HRQoL was assessed using the SF-12 Physical (PCS) and Mental (MCS) Component Summary scores. Descriptive analysis of HRQoL was conducted for socio-demographic, health, disability and determinants of health characteristics. RESULTS: Mean age was 46 years (range 20-67). Compared to age- and sex-adjusted Canadian averages, PCS (47.3) was low and MCS was similar (52.0). PCS and MCS were variably below average for middle age groups and lowest for non-commissioned ranks, widowed/divorced/separated, 10-19 years of service, physical and mental health conditions, disability, dissatisfaction with finances, seeking work/not working, low social support and difficulty adjusting to civilian life. Among Veterans Affairs Canada clients, 83% had below-average physical PCS, 49% had below-average MCS, and mean PCS (38.2) was significantly lower than mean MCS (48.3). CONCLUSIONS: HRQoL varied across a range of biopsychosocial factors, suggesting possible protective factors and vulnerable subgroups that may benefit from targeted interventions. These findings will be of interest to agencies supporting Veterans in transition to civilian life and to researchers developing hypotheses to better understand well-being in Canadian Veterans. METHODE: L'Enquete sur la transition a la vie civile de 2010 etait une enquete telephonique nationale assistee par ordinateur menee aupres du personnel de la force reguliere des Forces canadiennes ayant ete libere entre 1998 et 2007. Nous avons evalue la QVLS a l'aide des cotes sommaires physiques (CSP) et mentales (CSM) du questionnaire SF-12. Nous avons procede a l'analyse descriptive de la QVLS selon les caracteristiques sociodemographiques, de la sante, de l'invalidite et des determinants de la sante. RESULTATS: L'age moyen des repondants etait de 46 ans (20-67 ans). Comparativement aux moyennes canadiennes ajustees selon l'age et le sexe, la CSP (47,3) etait inferieure et la CSM etait semblable (52,0). Les CSP et CSM etaient variablement inferieures a la moyenne dans les groupes d'age moyen; elles etaient les plus faibles pour le grade des sousofficiers, les personnes veuves, divorcees ou separees, les personnes ayant entre 10 et 19 ans d'anciennete, les personnes ayant des troubles medicaux physiques et mentaux, les personnes handicapees, les personnes insatisfaites sur le plan financier, les personnes cherchant du travail ou sans travail, les personnes ayant peu de soutien social et celles ayant du mal a s'ajuster a la vie civile. Parmi la clientele du ministere des Anciens Combattants du Canada, 83 % avaient une CSP inferieure a la moyenne, 49 % avaient une CSM inferieure a la moyenne, et la CSP moyenne (38,2) etait significativement inferieure a la CSM moyenne (48,3). CONCLUSION: La QVLS varie en fonction d'un eventail de facteurs biopsychosociaux, ce qui suggere l'existence de facteurs de protection et de sous-groupes vulnerables qui pourraient beneficier d'interventions ciblees. Ces constatations interesseront les organismes d'appui aux anciens combattants en transition vers la vie civile et les chercheurs qui elaborent des hypotheses pour mieux comprendre le bien-etre des anciens combattants canadiens.
