Abstract

BackgroundTens of millions of children are exposed to Mycobacterium tuberculosis globally every year; however, there are no contemporary estimates of the risk of developing tuberculosis in exposed children. The effectiveness of contact investigations and preventive therapy remains poorly understood.MethodsIn this systematic review and meta-analysis, we investigated the development of tuberculosis in children closely exposed to a tuberculosis case and followed for incident disease. We restricted our search to cohort studies published between Jan 1, 1998, and April 6, 2018, in MEDLINE, Web of Science, BIOSIS, and Embase electronic databases. Individual-participant data and a pre-specified list of variables were requested from authors of all eligible studies. These included characteristics of the exposed child, the index case, and environmental characteristics. To be eligible for inclusion in the final analysis, a dataset needed to include: (1) individuals below 19 years of age; (2) follow-up for tuberculosis for a minimum of 6 months; (3) individuals with household or close exposure to an individual with tuberculosis; (4) information on the age and sex of the child; and (5) start and end follow-up dates. Studies assessing incident tuberculosis but without dates or time of follow-up were excluded. Our analysis had two primary aims: (1) estimating the risk of developing tuberculosis by time-period of follow-up, demographics (age, region), and clinical attributes (HIV, tuberculosis infection status, previous tuberculosis); and (2) estimating the effectiveness of preventive therapy and BCG vaccination on the risk of developing tuberculosis. We estimated the odds of prevalent tuberculosis with mixed-effects logistic models and estimated adjusted hazard ratios (HRs) for incident tuberculosis with mixed-effects Poisson regression models. The effectiveness of preventive therapy against incident tuberculosis was estimated through propensity score matching. The study protocol is registered with PROSPERO (CRD42018087022).FindingsIn total, study groups from 46 cohort studies in 34 countries-29 (63%) prospective studies and 17 (37%) retrospective-agreed to share their data and were included in the final analysis. 137 647 tuberculosis-exposed children were evaluated at baseline and 130 512 children were followed for 429 538 person-years, during which 1299 prevalent and 999 incident tuberculosis cases were diagnosed. Children not receiving preventive therapy with a positive result for tuberculosis infection had significantly higher 2-year cumulative tuberculosis incidence than children with a negative result for tuberculosis infection, and this incidence was greatest among children below 5 years of age (19·0% [95% CI 8·4-37·4]). The effectiveness of preventive therapy was 63% (adjusted HR 0·37 [95% CI 0·30-0·47]) among all exposed children, and 91% (adjusted HR 0·09 [0·05-0·15]) among those with a positive result for tuberculosis infection. Abstract

Introduction: We integrate results of a project developed in order to increase the application of the realistic approach to community-based interventions for the prevention and control of vector-borne diseases. Methods: They were performed secondary analyzes of data from the implementation of a community-based intervention for dengue prevention (La Lisa, 2004- 2007). They were systematically analyzed all papers on community-based interventions in vector-borne diseases published in peer- reviewed journals (2006-2016). Besides, they were made theoretical-methodological contributions for the inclusion of the realistic approach in the evaluation of community-based interventions. Results. It was proven that capacity-buil- ding and organization processes of local actors at different levels lead to community work, and consequently, to an increased problem-solving ability and to the vector reduction. Me- chanism for success of such processes in the Cuban context were: intrinsic and extrinsic motivation, team spirit and self-perceived efficacy of individual and organizational actors. When analyzing the existing scientific literature, they were identified gaps in the reporting on what works, for whom and in what context for community-based intervention due to limited understanding of its functioning principles and the scarce application of a realistic approach to evaluation. It was demonstrated feasibility of community-based intervention in the preven- tion of trypanosomiasis in the Republic of Congo; it was contributed to the development of a multi-country protocol of mixed methods to assess the efficacy of a community-based inter- vention in the reduction of dengue infection in Brazil. 24. Lung cancer occurrence after an episode of tuberculosis: a systematic review and meta-analysis

Abstract

INTRODUCTION: People with tuberculosis experience long-term health effects beyond cure, including chronic respiratory diseases. We investigated whether tuberculosis is a risk factor for subsequent lung cancer. METHODS: We searched PubMed, Scopus, Cochrane, Latin American and Caribbean Health Sciences Literature and the Scientific Electronic Library Online for cohort and case-control studies providing effect estimates for the association between tuberculosis and subsequent lung cancer. We pooled estimates through random-effects meta-analysis. The study was registered in PROSPERO (CDR42020178362). RESULTS: Out of 6240 records, we included 29 cohort and 44 case-control studies. Pooled estimates adjusted for age and smoking (assessed quantitatively) were hazard ratio (HR) 1.51 (95% CI 1.30-1.76, I(2)=81%; five studies) and OR 1.74 (95% CI 1.42-2.13, I(2)=59%; 19 studies). The occurrence of lung cancer was increased for 2 years after tuberculosis diagnosis (HR 5.01, 95% CI 3.64-6.89; two studies), but decreased thereafter. Most studies were retrospective, had moderate to high risk of bias, and did not control for passive smoking, environmental exposure and socioeconomic status. Heterogeneity was high. CONCLUSION: We document an association between tuberculosis and lung cancer occurrence, particularly in, but not limited to, the first 2 years after tuberculosis diagnosis. Some cancer cases may have been present at the time of tuberculosis diagnosis and therefore causality cannot be ascertained. Prospective studies controlling for key confounding factors are needed to identify which tuberculosis patients are at the highest risk, as well as cost-effective approaches to mitigate such risk.

Abstract

OBJETIVO: explorar las percepciones sobre la participación comunitaria en la prevención del dengue durante la investigación formativa de un proyecto comunitario. MATERIAL Y MÉTODOS: Estudio conducido en tres áreas de salud de Santiago de Cuba durante el año 2000. Se combinan técnicas cualitativas y cuantitativas para explorar opiniones de los profesionales de salud, líderes comunitarios y una muestra aleatoria de población. RESULTADOS: Los profesionales identifican la no participación de la comunidad y sus propias limitaciones para involucrarla activamente en la prevención. Los líderes refieren falta de motivación, y la población vincula la eliminación del vector con el uso de tecnologías. Se perciben barreras en la aceptación de las actividades del programa de control y en general la "participación" es interpretada como "colaboración". CONCLUSIONES: La población ha trasferido responsabilidad del control del Aedes aegypti al sector salud. Se evidencia la necesidad de unificar conceptos sobre la participación comunitaria y conciliar intereses entre usuarios y proveedores de los servicios para lograr una real movilización social.

43. Recurrent TB: a systematic review and meta-analysis of the incidence rates and the proportions of relapses and reinfections

Abstract

BACKGROUND: A recurrent tuberculosis (TB) episode results from exogenous reinfection or relapse after cure. The use of genotyping allows the distinction between both. METHODS: We did a systematic review and meta-analysis, using four databases to search for studies in English, French and Spanish published between 1 January 1980 and 30 September 2020 that assessed recurrences after TB treatment success and/or differentiated relapses from reinfections using genotyping. We calculated person years of follow-up and performed random-effects model meta-analysis for estimating pooled recurrent TB incidence rates and proportions of relapses and reinfections. We performed subgroup analyses by clinical-epidemiological factors and by methodological study characteristics. FINDINGS: The pooled recurrent TB incidence rate was 2.26 per 100 person years at risk (95% CI 1.87 to 2.73; 145 studies). Heterogeneity was high (I(2)=98%). Stratified pooled recurrence rates increased from 1.47 (95% CI 0.87 to 2.46) to 4.10 (95% CI 2.67 to 6.28) per 100 person years for studies conducted in low versus high TB incidence settings. Background HIV prevalence, treatment drug regimen, sample size and duration of follow-up contributed too. The pooled proportion of relapses was 70% (95% CI 63% to 77%; I²=85%; 48 studies). Heterogeneity was determined by background TB incidence, as demonstrated by pooled proportions of 83% (95% CI 75% to 89%) versus 59% (95% CI 42% to 74%) relapse for studies from settings with low versus high TB incidence, respectively. INTERPRETATION: The risk of recurrent TB is substantial and relapse is consistently the most frequent form of recurrence. Notwithstanding, with increasing background TB incidence the proportion of reinfections increases and the predominance of relapses among recurrences decreases.

Abstract

Objetivo. Confirmar la presencia de brote de dengue en la ciudad de Santa Cruz, Bolivia, así como identificar el denguevirus causal, estimar la tasa de ataque y determinar la proporción de infecciones sintomáticas. Material y métodos. En marzo de 1997 se realizó una encuesta seroepidemiológica con muestreo aleatorio en un distrito céntrico de la ciudad. Se obtuvo información sobre episodios de enfermedad aguda, antecedentes de cuadro febril reciente y muestras de sangre venosa. Se determinó la presencia de IgM antidengue con el método MAC ELISA y se procedió a la tipificación del virus con tecnología de reacción en cadena de la polimerasa. Resultados. Se detectaron anticuerpos IgM en 6.5% de los adultos (IC95% 3.4-9.6) y 5.1% de los niños (IC 95% 2.0-8.2). El virus circulante fue identificado como dengue serotipo 2, genotipo Jamaica. Menos de la mitad de los niños infectados tuvieron una infección sintomática, contra casi 90% de los adultos. Conclusiones. La tasa de ataque estimada es compatible con una epidemia de dengue en Santa Cruz. La introducción del serotipo 2 - Jamaica en el país aumenta el riesgo de dengue hemorrágico.
