Exacerbation detection is based on 12 symptom-related yes-or-no questions and the measurement of peripheral capillary oxygen saturation (SpO2), forced expiratory volume in one second (FEV1), and body temperature. Automated treatment advice is based on a decision model built by clinical expert panel opinion and Bayesian network modeling. The current paper describes the validity of ACCESS. Methods: We performed secondary analyses on data from a 3-month prospective observational study in which patients with COPD registered respiratory symptoms daily on diary cards and measured SpO2, FEV1, and body temperature. We examined the validity of the most important treatment advice of ACCESS, ie, to contact the health care professional, against symptom- and event-based exacerbations. Results: Fifty-four patients completed 2,928 diary cards. One or more of the different pieces of ACCESS advice were provided in 71.7% of all cases. We identified 115 symptom-based exacerbations. Cross-tabulation showed a sensitivity of 97.4% (95% CI 92.0–99.3), specificity of 65.6% (95% CI 63.5–67.6), and positive and negative predictive value of 13.4% (95% CI 11.2–15.9) and 99.8% (95% CI 99.3–99.9), respectively, for ACCESS’ advice to contact a health care professional in case of an exacerbation. Conclusion: In many cases (71.7%), ACCESS gave at least one self-management advice to lower symptom burden, showing that ACCES provides self-management support for both day-to-day symptom variations and exacerbations. High sensitivity shows that if there is an exacerbation, ACCESS will advise patients to contact a health care professional. The high negative predictive value leads us to conclude that when ACCES does not provide the advice to contact a health care professional, the risk of an exacerbation is very low. Thus, ACCESS can safely be used in patients with COPD to support self-management in case of an exacerbation. 14. Performance of Immersed Tunnels subjected to Settlements: Noordtunnel Case Study

Abstract

In the next decades, Rijkswaterstaat will renovate many existing immersed tunnels so they can meet their intended lifespan in a good condition. Ongoing settlements could be a threat to the tunnel structural performance, as deformation may cause structural damage. Currently, immersed tunnels are subjected to greater and more uneven settlements than considered in the design. The issue of these ongoing settlements is mainly concentrated in the segment joints, which have typically been executed by a spigot and socket structure to prevent differential settlements. Significant deformation of the tunnel structure could result in concrete cracking, which entails the risks of structural connection loss, tunnel misalignment, and leakages (leading to durability reduction and damage to installations). This could impact the reliability of the structure over time, and consequently affect the availability of the road network. Therefore, the key objective of this research is how to assess the structural reliability and tunnel availability, and to investigate in which way the performance of existing immersed tunnel structures could be optimised given the ongoing settlements. Assessing the residual lifespan is beyond the scope, but optimising the tunnel performance during the remaining operational life is the main motivation for this research project. The research methodology consists of a parametric structural model, which determines the tunnel reliability and availability by using a Monte Carlo approach. The Noordtunnel case is used to demonstrate the research methodology, as this tunnel showed excessive settlements and a major renovation will be performed in the coming years. Additionally, the maintenance and renovation strategy for the segment joints in immersed tunnels are examined, to optimise the performance throughout the residual lifespan., Concrete Structures

Abstract

Introduction: Studies that assessed the efficacy of pre-operative immune checkpoint blockade (ICB) in locally advanced urothelial cancer of the bladder showed encouraging pathological complete response rates, suggesting that a bladder-sparing approach may be a viable option in a subset of patients. Chemoradiation is an alternative for radical cystectomy with similar oncological outcomes, but is still mainly used in selected patients with organ-confined tumors or patients ineligible to undergo radical cystectomy. We propose to sequentially administer ICB and chemoradiation to patients with (locally advanced) muscle-invasive bladder cancer. Methods: The INDIBLADE trial is an investigator-initiated, single-arm, multicenter phase 2 trial. Fifty patients with cT2-4aN0-2M0 urothelial bladder cancer will be treated with ipilimumab 3 mg/kg on day 1, ipilimumab 3 mg/kg plus nivolumab 1 mg/kg on day 22, and nivolumab 3 mg/kg on day 43 followed by chemoradiation. The primary endpoint is the bladder-intact event-free survival (BI-EFS). Events include: local or distant recurrence, salvage cystectomy, death and switch to platinum-based chemotherapy. We will also evaluate the potential of multiparametric magnetic resonance imaging of the bladder to identify non-responders, and we will assess the clearance of circulating tumor DNA as a biomarker for ICB treatment response. Discussion: This is the first trial in which the efficacy of induction combination ICB followed by chemoradiation is being evaluated to provide bladder-preservation in patients with (locally advanced) urothelial bladder cancer. Abstract

BACKGROUND No new agent has improved overall survival in patients with unresectable or metastatic urothelial carcinoma when added to first-line cisplatin-based chemocontlctd-therapy. METHODS In this phase 3, multinational, open-label trial, we randomly assigned patients with previously untreated unresectable or metastatic urothelial carcinoma either to receive intravenous nivolumab (at a dose of 360 mg) plus gemcitabine-cisplatin (nivolumab combination) every 3 weeks for up to six cycles, followed by nivolumab (at a dose of 480 mg) every 4 weeks for a maximum of 2 years, or to receive gemcitabine-cisplatin alone every 3 weeks for up to six cycles. The primary outcomes were overall and progression-free survival. The objective response and safety were exploratory outcomes. RESULTS A total of608 patients underwent randomization (304 to each group). At a median follow-up of 33.6 months, overall survival was longer with nivolumab-combination therapy than with gemcitabine -cisplatin alone (hazard ratio for death, 0. 78; 95% confidence interval [CI], 0.63 to 0.96; P=0.02); the median survival was 21.7 months (95% CI, 18.6 to 26.4) as compared with 18.9 months (95% CI, 14.7 to 22.4), respectively. Progression-free survival was also longer with nivolumabcombination therapy than with gemcitabine-cisplatin alone (hazard ratio for progression or death, 0.72; 95% CI, 0.59 to 0.88; P=0.001). The median progression-free survival was Z9 months and 7.6 months, respectively. At 12 months, progression-free survival was 34.2% and 21.890, respectively. The overall objective response was 5Z6°/0 (complete response, 21.7°/0) with nivolumab-combination therapy and 43.1% (complete response, 11.8°/0) with gemcitabine-cisplatin alone. The median duration of complete response was 3Z1 months with nivolumabcombination therapy and 13.2 months with gemcitabine-cisplatin alone. Grade 3 or higher adverse events occurred in 61.8% and 51.7% of the patients, respectively. CONCLUSIONS Combination therapy with nivolumab plus gemcitabine-cisplatin resulted in significantly better outcomes in patients with previously untreated advanced urothetial carcinoma than gemcitabine-cisplatin alone. Abstract

Colorectal cancer (CRC) remains an important cause of cancer related death worldwide. Early stage disease is often still curable but the availability of curative therapies for metastasized CRC is limited. Our aim was to gain a better understanding of the underlying growth dynamics of this disease to hopefully unravel new therapeutic avenues that will eventually improve patient outcomes. Therefore, we have investigated the fundamental growth dynamics of CRC during initiation, progression and growth, in both a qualitative as quantitative fashion. The work presented in this thesis, demonstrates that different selection dynamics underlie the multistep CRC evolution and the existence of an intimate relationship between the CRC cells and their environment.

Abstract

Employing a mixed-method approach to quantitative data from the Queensland Police Gazette and qualitative evidence from newspaper archives and government reviews of women’s gaols, this chapter studies women’s imprisonment in Queensland, Australia, at the end of the nineteenth century. It describes the profiles of men and women committed to prison in Queensland from 1880–1899, and the extent to which men and women recidivated. In spite of a number of methodological caveats, women were more likely to be (chronic) recidivists than men during the late nineteenth century in Queensland. This chapter argues that this can be explained in terms of their different social and economic disadvantages and vulnerabilities, related to their stigmatization, policing and institutionalization.

Abstract

Substantial amounts of nutrients are lost from soils through leaching. These losses can be environmentally damaging, causing groundwater eutrophication and also comprise an economic burden in terms of lost agricultural production. More intense precipitation events caused by climate change will likely aggravate this problem. So far it is unresolved to which extent soil biota can make ecosystems more resilient to climate change and reduce nutrient leaching losses when rainfall intensity increases. In this study, we focused on arbuscular mycorrhizal (AM) fungi, common soil fungi that form symbiotic associations with most land plants and which increase plant nutrient uptake. We hypothesized that AM fungi mitigate nutrient losses following intensive precipitation events (higher amount of precipitation and rain events frequency). To test this, we manipulated the presence of AM fungi in model grassland communities subjected to two rainfall scenarios: moderate and high rainfall intensity. The total amount of nutrients lost through leaching increased substantially with higher rainfall intensity. The presence of AM fungi reduced phosphorus losses by 50% under both rainfall scenarios and nitrogen losses by 40% under high rainfall intensity. Thus, the presence of AM fungi enhanced the nutrient interception ability of soils, and AM fungi reduced the nutrient leaching risk when rainfall intensity increases. These findings are especially relevant in areas with high rainfall intensity (e.g. such as the tropics) and for ecosystems that will experience increased rainfall due to climate change. Overall, this work demonstrates that soil biota such as AM fungi can enhance ecosystem resilience and reduce the negative impact of increased precipitation on nutrient losses. Abstract

Soil life and particularly arbuscular mycorrhizal fungi (AMF) play a fundamental role in ecosystem functioning and are crucial drivers of vegetation dynamics and efficient nutrient use. This study investigated the effect of liquid manure application and aerial irrigation on litter decomposition and AMF colonization in mountain meadowlands. Six intensity levels of irrigation and fertilization were experimentally tested in eleven meadows located in the Swiss Alps. For litter decomposition, a novel teabag approach was followed. AMF root colonization was assessed from field samples and a trap culture experiment. Decomposition measured with tea bag burials was higher under maximal management intensity levels. AMF colonization was 22% lower in high-input compared to low-input plots in the field while in the trap cultures colonization levels depended on the functional plant species group. Our findings shed light on the impact of management intensification in mountain hay meadows on belowground functioning. Ultimately, the findings of this study will help to develop pragmatic management recommendations to farmers that would allow acceptable yields while preserving functional diversity.
