1. Choice of antipsychotic treatment by European psychiatry trainees: are decisions based on evidence?

Abstract

Abstract Background Little is known about the factors influencing treatment choice in psychosis, the majority of this work being conducted with specialists (consultant) in psychiatry. We sought to examine trainees' choices of treatment for psychosis if they had to prescribe it for themselves, their patients, and factors influencing decision-making. Methods Cross-sectional, semi-structured questionnaire-based study. Results Of the 726 respondents (response rate = 66%), the majority chose second-generation antipsychotics (SGAs) if they had to prescribe it for themselves (n = 530, 93%) or for their patients (n = 546, 94%). The main factor influencing choice was perceived efficacy, 84.8% (n = 475) of trainees stating this was the most important factor for the patient, and 77.8% (n = 404) stating this was the most important factor for their own treatment. Trainees with knowledge of trials questioning use of SGAs (CATIE, CUtLASS, TEOSS) were more likely to choose second-generation antipsychotics than those without knowledge of these trials (χ2 = 3.943; p = 0.047; O.R. = 2.11; 95% C.I. = 1.0-4.48). Regarding psychotherapy, cognitive behavioural therapy (CBT) was the most popular choice for self (33.1%; n = 240) and patient (30.9%; n = 224). Trainees were significantly more likely to prefer some form of psychotherapy for themselves rather than patients (χ2 = 9.98; p < 0,002; O.R. = 1.54; 95% CIs = 1.18-2.0). Conclusions Trainees are more likely to choose second-generation antipsychotic medication for patients and themselves. Despite being aware of evidence that suggests otherwise, they predominantly base these choices on perceived efficacy.

Abstract

Background: Delirium is frequently unrecognised. EEG shows slower frequencies (i.e. below 4 Hz) during delirium, which might be useful in improving delirium recognition. We studied the discriminative performance of a brief single-channel EEG recording for delirium detection in an independent cohort of patients. Methods: In this prospective, multicentre study, postoperative patients aged ≥60 yr were included (n=159). Before operation and during the first 3 postoperative days, patients underwent a 5-min EEG recording, followed by a video-recorded standardised cognitive assessment. Two or, in case of disagreement, three delirium experts classified each postoperative day based on the video and chart review. Relative delta power (1–4 Hz) was based on 1-min artifact-free EEG. The diagnostic value of the relative delta power was evaluated by the area under the receiver operating characteristic curve (AUROC), using the expert classification as the gold standard. Results: Experts classified 84 (23.3%) postoperative days as either delirium or possible delirium, and 276 (76.7%) non-delirium days. The AUROC of the relative EEG delta power was 0.75 [95% confidence interval (CI) 0.69–0.82]. Exploratory analysis showed that relative power from 1 to 6 Hz had significantly higher AUROC (0.78, 95% CI 0.72–0.84, P=0.014). Conclusions: Delirium/possible delirium can be detected in older postoperative patients based on a single-channel EEG recording that can be automatically analysed. This objective detection method with a continuous scale instead of a dichotomised outcome is a promising approach for routine detection of delirium. Clinical trial registration: NCT02404181.

Abstract

The bicentennial celebrations in 2015 of the Battle of Waterloo have rekindled public interest in the history and heritage of this historical event. The commemoration also led to a massive increase in publications. Many of these are based on new sources (ego-documents, archaeological finds, and archival sources) discovered in the last decades. These new sources have allowed historians to move away from official accounts and towards a view from below. This approach has added significantly to our understanding of the battle, but in some cases the new sources have caused confusion and led to new historiographical controversies, both on a national and transnational level. At the same time historians in and outside academia are moving beyond national histories of the battle to more balanced and transnational accounts. Recent interest in heritage and memory has resulted in new research on the appropriation of Waterloo by various interested parties. At the same time it is to be regretted that academic historians almost exclusively focus on heritage and memory and have been neglecting the story of the battle. As a result they have been largely absent during the commemorations. Public interest in Waterloo has been mostly served by popular historical publications and reenactment events. Topics covered include the appropriation of the battles by the British, Belgian and Dutch troops' role in preventing the French from taking the strategically important crossroads at Quatre-Bras, and French Emperor Napoleon Bonaparte's acknowledgement that the Battle of Waterloo was already lost at Quatre-Bras.
