Abstract

We present the results of a photometric CCD monitoring of the gravitational lens system UM 673, that took place from 1995 to 2000. In total, the doubly-imaged quasar was observed in the R-band during 29 photometric nights, using optical telescopes with dimensions in the range 0.6 m to 1.3 m. We detected a significant variability in the total light of the UM 673 system, that is, in the light of the two QSO images plus the lensing galaxy. With respect to the magnitude of the gravitational lens system at its discovery, in 1986, UM 673 was 0.3 magnitude brighter. Furthermore, our December 1996 measurements show that between November 1995 and October 1997 the system became even brighter, reaching a magnitude difference of 0.5 magnitude with respect to its discovery value. Abstract

A description is given of the activities of an international working group created with the aim of obtaining both photometric and astrometric observations of visual double and multiple stars with angular separations in the range of one to fifteen arcseconds, that formed part of the HIPPARCOS Input Catalogue. The scientific aims and realisations of this European network are given. About 50 observational missions have been carried out in both hemispheres according to a pre-defined protocol. We describe the general and specifically designed methods used for the reduction of large amounts of CCD observations of double stars and give an outline of the results already presented and soon expected., Comment: 10 pages, one postscript figure. Abstract

Item does not contain fulltext BACKGROUND: Present theory is that uncomplicated and complicated appendicitis are different entities. Recent studies suggest it is safe to delay surgery in patients with uncomplicated appendicitis. We hypothesize that patients with complicated appendicitis are at higher risk for postoperative complications when surgery is delayed. METHODS: Data was used from the multicenter, prospective SNAPSHOT appendicitis study of 1975 patients undergoing surgery for suspected appendicitis. Adult patients (≥ 18 years) who underwent appendectomy for appendicitis were included in this study. The primary outcome was the difference in postoperative complications between patients with complicated appendicitis who were operated within and after 8 h after hospital presentation. Secondary outcomes were the incidence of both uncomplicated and complicated appendicitis in relationship to delay of appendectomy. Follow-up was 30 days. A multivariable analysis was performed. RESULTS: Of 1341 adult patients with appendicitis, 34.3% had complicated appendicitis. In patients with complicated appendicitis, 22.8% developed a postoperative complication compared to 8.2% for uncomplicated appendicitis (P 8 h) increased the complication rate in patients with complicated appendicitis (28.1%) compared to surgery within 8 h (18.3%; P = 0.01). Multivariate analysis showed a delay in surgery as an independent predictor for a postoperative complication in patients with complicated appendicitis (OR 1.71; 95%CI 1.01-2.68, P = 0.02). CONCLUSION: In-hospital delay of surgery (> 8 h) in patients with complicated appendicitis is associated with a higher risk of a postoperative complication. It is important that we recognize and treat these patients early.

Abstract

Purpose For the diagnosis of acute appendicitis, the combination of clinical and laboratory variables achieves high diagnostic accuracy. Nevertheless, appendicitis can present with normal laboratory tests of inflammation. The aim of this study was to investigate the incidence of normal inflammatory markers in patients operated for acute appendicitis. Methods This is an analysis of data from a prospective, multicentre SNAPSHOT cohort study of patients with suspected acute appendicitis. Only patients with histopathologically proven acute appendicitis were included. Adult patients with acute appendicitis and normal preoperative inflammatory markers were explored further in terms of abdominal complaints, preoperative imaging results and intraoperative assessment of the degree of inflammation and compared to those with elevated inflammatory markers. Results Between June and July 2014, 1303 adult patients with histopathologically proven acute appendicitis were included. In only 23 of 1303 patients (1.8%) with proven appendicitis, both preoperative white blood cell count and C-reactive protein levels were normal. Migration of pain was reported less frequently in patients with normal inflammatory markers compared to those with elevated inflammatory marker levels (17.4% versus 43.0%, p = 0.01). Characteristics like fever, duration of symptoms and localized peritonitis were comparable. Only 4 patients with normal inflammatory markers (0.3% overall) had complicated appendicitis at histopathological evaluation. Conclusion Combined normal WBC and CRP levels are seen in about 2 per 100 patients with confirmed acute appendicitis and can, although rarely, be found in patients with complicated appendicitis.

Abstract

Introduction In patients treated with an appendectomy for acute appendicitis, the specimen is generally sent for histological evaluation. In an era of increasing non-operative treatment for acute appendicitis, it is important to know the incidence, the diagnostic accuracy, and treatment consequences of appendicular neoplasms that are found in acute appendicitis. We hypothesize that pre- and intra-operative parameters might predict an appendicular neoplasm. Methods Data was used from our previous prospective observational cohort study. All patients undergoing surgery for suspected acute appendicitis were included. The primary outcome was the incidence of appendicular neoplasms in patients operated for acute appendicitis. Secondary outcomes were pre-operative diagnostics and imaging outcomes, intra-operative surgical judgment, and postoperative management and outcome. Possible predictors of an appendicular neoplasm were identified and used in multivariable logistic regression. Patients with an appendicular neoplasm were followed for 3 years after initial appendectomy. Results A total of 1975 patients underwent surgery for suspected acute appendicitis and in 98.3% (1941/1975) the appendix was removed. In 1.5% (30/1941) of these patients, an appendicular neoplasm was found. Among the malignant neoplasms, the majority were grade 1 neuroendocrine tumors (NET) in 65% (13/20). On pre-operative imaging, there was no suspicion of malignancy. In three cases, there was an intra-operative suspicion of malignancy. Multivariable analysis showed only age as an independent predictor for appendicular neoplasms. No recurrent or new malignancy was found during follow-up. Discussion The incidence of appendicular neoplasm in patients undergoing an acute appendectomy is very low and clinical risk factors could not be identified.

Abstract

We present Vmagnitudes, $({\it B}-{\it V})$and $({\it U}-{\it B})$colours of the components of 111 visual double stars and their relative position measurements ρ and θ. The primaries are of spectral class A; the angular separation is in the $2\hbox{$.\!\!^{\prime\prime}$}5-8^{\prime\prime}$range. The observations were made at the 61-cm reflector of University of Toronto Southern Observatory, Chile. The obtained photometric results are of a moderate quality for absolute values ($0.04^{\rm m}$(mag) in Vand about $0.02^{\rm m}$in the $({\it B}-{\it V})$and $({\it U}-{\it B})$colour indices) and more precise in $\Delta {\it V}$and $\Delta ({\it U}-{\it B})$, $\Delta ({\it B}-{\it V})$($0.023^{\rm m}$and $0.015^{\rm m}$, respectively). We find that about 60% of secondaries are also of spectral type A; 56% of pairs have $\Delta({\it B}-{\it V})$less than $0.1^{\rm m}$. The two-colour diagram reveals significant offset of objects from the curve of unreddened Main Sequence stars. A comprehensive survey of stars in the close vicinity to the objects is also presented. The final astrometric accuracy of $\approx 0\hbox{$.\!\!^{\prime\prime}$}01$in ρ and $0\hbox{$.\!\!^{\circ}$}1$in θprovides additional measurements for study of relative motion of the components.

Abstract

We present angular separations, position angles and $\Delta V$instrumental magnitude differences of 83 visual double stars with angular separations around two arcseconds. Our observations were made by means of a CCD detector attached to the Kryonerion 1.2 m telescope. 12 of them are common proper motion stars and their components should therefore be stars of common origin.

Abstract

Differences $\Delta V$of magnitude and $\Delta (B{-}V)$and $\Delta (U{-}B)$colours, as well as separations and position angles of 42 relatively close double stars are presented. The selection criteria of our sample are: the relatively small separation (between 2.5 and 5 arcseconds), magnitude difference under 1.25 mag, and mainly G-type primaries. The CCD observations, performed at the 90 cm Dutch telescope at ESO, were made using the Bessel U, Band Vfilters; the astrometry was done in the Vfilter only. From the analysis of the photometric data we conclude that 16 binaries in our sample have components with practically the same Teff, since they have almost the same colours; one binary has components with identical characteristics. We also note the good internal accuracy of the astrometric CCD measurements.

Abstract

We present the first observations of the initiation of a coronal mass ejection (CME) seen on the disk of the Sun. Observations with the EIT experiment on SOHO show that the CME began in a small volume and was initially associated with slow motions of prominence material and a small brightening at one end of the prominence. Shortly afterward, the prominence was accelerated to about 100 km s-1and was preceded by a bright loop-like structure, which surrounded an emission void, that traveled out into the corona at a velocity of 200–400 km s-1. These three components, the prominence, the dark void, and the bright loops are typical of CMEs when seen at distance in the corona and here are shown to be present at the earliest stages of the CME. The event was later observed to traverse the LASCO coronagraphs fields of view from 1.1 to 30 R⊙. Of particular interest is the fact that this large-scale event, spanning as much as 70 deg in latitude, originated in a volume with dimensions of roughly 35" (2.5 x 104km). Further, a disturbance that propagated across the disk and a chain of activity near the limb may also be associated with this event as well as a considerable degree of activity near the west limb.

Abstract

Abstract Background Currently, there is no consensus opinion regarding the optimal procedure of choice in super-super-morbid obesity (Body mass index, BMI > 60 kg/m2). Roux-en-Y gastric bypass (RYGB) is associated with failure to achieve or maintain 50% excess weight loss (EWL) or BMI < 35 in approximately 15% of patients. Also, percent EWL is significantly less after 1-year in the super-super-obese group as compared with the less obese group and many patients are still technically considered to be obese (lowest post-surgical BMI > 35) following RYGB surgery in this group. The addition of adjustable gastric band (AGB) to RYGB has been reported as a revisional procedure but this combined bariatric procedure has not been explored as a primary operation. Methods In a primary laparoscopic RYGB, an AGB is drawn around the gastric pouch through a small opening between the blood vessels on the lesser curve and the gastric pouch. The band is then fixed by suturing the gastric remnant to the gastric pouch both above and below the band to prevent slippage. Results Between November 2009 and March 2010, 6 consecutive super-super-obese patients underwent a primary laparoscopic adjustable banded Roux-en-Y gastric bypass procedure at our institution. One male patient (21 years, BMI 70 kg/m²) developed a pneumonia postoperatively. No other postoperative complications were observed. Conclusion To the best of our knowledge, this is the first series of patients that underwent a laparoscopic adjustable banded RYGB as a primary operation for the super-super obese in the indexed literature. With the combined procedure, a sequential action mechanism for weight loss is to be expected. The restrictive, malabsorptive and hormonal working mechanism of the RYGB will induce weight loss from the start reaching a stabilised plateau of weight after 12 - 18 months. At that time, filling of the band can be started resulting in further gastric pouch restriction and increased weight loss. Moreover, besides improving the results of total weight loss, a gradual filling of the band can as well prevent the RYGB patient from weight regain if restriction would fade away with time.
