Abstract

Introduction:Twin pregnancies have a high risk of extreme preterm birth (PTB) at less than 28 weeks of gestation, which is associated with increased risk of neonatal morbidity and mortality. Currently there is a lack of effective treatments for women with a twin pregnancy and a short cervix or cervical dilatation. A possible effective surgical method to reduce extreme PTB in twin pregnancies with an asymptomatic short cervix or dilatation at midpregnancy is the placement of a vaginal cerclage. Methods and analysis: We designed two multicentre randomised trials involving eight hospitals in the Netherlands (sites in other countries may be added at a later date). Women older than 16 years with a twin pregnancy at <24 weeks of gestation and an asymptomatic short cervix of ≤25 mm or cervical dilatation will be randomly allocated (1:1) to both trials on vaginal cerclage and standard treatment according to the current Dutch Society of Obstetrics and Gynaecology guideline (no cerclage). Permuted blocks sized 2 and 4 will be used to minimise the risk of disbalance. The primary outcome measure is PTB of <28 weeks. Analyses will be by intention to treat. The first trial is to demonstrate a risk reduction from 25% to 10% in the short cervix group, for which 194 patients need to be recruited. The second trial is to demonstrate a risk reduction from 80% to 35% in the dilatation group and will recruit 44 women. A cost-effectiveness analysis will be performed from a societal perspective. Ethics and dissemination: This study has been approved by the Research Ethics Committees in the Netherlands on 3/30/2023. Participants will be required to sign an informed consent form. The results will be presented at conferences and published in a peer-reviewed journal. Participants will be informed about the results.

9. Bruker2nifti: Magnetic Resonance Images converter from Bruker

Abstract

In clinical and pre-clinical research involving medical images, the first step following a Magnetic Resonance Imaging dataset acquisition, usually entails the conversion of image data from the native scanner format to a format suitable for the intended analysis. The proprietary Bruker ParaVision software currently does not provide the tools for conversion of the data to suitable and open formats for research, such as nifti (Cox, Robert W and Ashburner, John and Breman, Hester and Fissell, Kate and Haselgrove, Christian and Holmes, Colin J and Lancaster, Jack L and Rex, David E and Smith, Stephen M and Woodward, Jeffrey B and others 2004), for which most of the available tools for medical image analysis are implemented. For this purpose we have designed and developed bruker2nifti, a pip-installable Python tool provided with a Graphical User Interface to convert from the native MRI Bruker format to the nifti format, without any intermediate step through the DICOM standard formats (Mildenberger, Eichelberg, and Martin 2002). Bruker2nifti is intended to be a tool to access the data structure and to parse all parameter files of the Bruker ParaVision format into python dictionaries, to select the relevant information to fill the Nifti header and data volume. Abstract

A dichloromethane extract from leaves of Searsia pyroides potentiated gamma aminobutyric acid-induced chloride currents by 171.8 ± 54% when tested at 100 µg/mL in Xenopus oocytes transiently expressing gamma aminobutyric acid type A receptors composed of α1β2γ2s subunits. In zebrafish larvae, the extract significantly lowered pentylenetetrazol-provoked locomotion when tested at 4 µg/mL. Active compounds of the extract were tracked with the aid of HPLC-based activity profiling utilizing a previously validated zebra-fish larval locomotor activity assay. From two active HPLC fractions, compounds 1-3 were isolated. Structurally related compounds 4-6 were purified from a later eluting inactive HPLC fraction. With the aid of ¹H and 13C NMR and high-resolution mass spectrometry, compounds 1-6 were identified as analogues of anacardic acid. Compounds 1-3 led to a concentration-dependent decrease of pentylenetetrazol-provoked locomotion in the zebrafish larvae model, while 4-6 were inactive. Compounds 1-3 enhanced gamma aminobutyric acid-induced chloride currents in Xenopus oocytes in a concentration-dependent manner, while 4-6 only showed marginal enhancements of gamma aminobutyric acid-induced chloride currents.
