1. Low Hepatitis C Virus Prevalence among Men Who Have Sex with Men Attending Public Health Services in The Netherlands

Abstract

The hepatitis C virus (HCV) prevalence is high among men who have sex with men (MSM) with HIV in the Netherlands. Large reductions in HCV incidence among MSM with HIV, however, have occurred since treatment with direct-acting antivirals. Over the years, a broader understanding of the HCV epidemic has shown that HCV infections are not solely restricted to MSM with HIV, but they also occur among HIV-negative MSM. Currently, HCV testing among HIV-negative MSM is only provided for PrEP users and is not part of routine sexually transmitted infection (STI) screening among HIV-negative MSM who are not using PrEP. In this study, we screened 1885 HIV-negative MSM who did not participate in a PrEP program, with over 1966 STI screening visits at four different public health clinic sites. Among the 1885 MSM, only one person had a new HCV infection, resulting in a 0.05% (95% confidence interval 0.0–0.3) incidence. Based on our findings, we can conclude that systematic HCV testing at STI clinics may not yield significant benefits for this particular population.

Abstract

Veel Nederlanders gebruiken lichtgevende schermen in de avond: het gebruik is het hoogst onder adolescenten (13-18 jaar) en volwassenen. Vaak neemt het gebruik in de avond ook aanzienlijke tijd in beslag (meer dan twee uur). Dit onderzoek bevestigt eerdere bevindingen dat frequent of langdurig schermgebruik in de avond samenhangt met verstoorde slaap. Bewustwording van het gebruik is dan ook belangrijk, voornamelijk wanneer een computer, smartphone of tablet in het uur voor het slapen gaan wordt gebruikt. Dit blijkt uit onderzoek van het RIVM, in samenwerking met het Amsterdam Medisch Centrum, het Nederlands Herseninstituut en Lifelines. Hierin is voor het eerst in Nederland het schermgebruik van kinderen en adolescenten (8-18 jaar) in de avond uitgebreid in kaart gebracht in samenhang met slaap. Het blijkt dat de groep die dagelijks of langdurig gebruikmaakt van een of meerdere schermen (computer, smartphone en/of tablet) tot 40 minuten korter slaapt dan de groep die dit niet, of korter, doet. De kortere slaapduur komt voornamelijk doordat deze kinderen en adolescenten later gaan slapen. Van de kinderen (8-13 jaar) gebruikt 22% dagelijks een scherm in de avond. Onder adolescenten (13-18 jaar) is dit 83%. Adolescenten die dagelijks een scherm gebruiken in het uur voor het slapen, hebben meer slaapklachten, zoals later in slaap vallen, korter slapen en 's nachts wakker worden. Ook vermelden zij meer symptomen van slaaptekort overdag, zoals moeite om wakker te blijven. Deze klachten verminderden bij adolescenten die als experiment een week lang geen scherm gebruikten of tijdens het schermgebruik een oranje bril droegen die het blauwe licht blokkeert. In de afgelopen jaren zijn steeds meer lichtgevende schermen ontwikkeld: niet alleen tv's maar ook computers, laptops, tablets en smartphones. De recent ontwikkelde schermen zenden meer blauw licht uit dan de traditionele bronnen, omdat zij gebruikmaken van led-technologie. Bekend is dat blauw licht invloed heeft op onze biologische klok, en daarmee de slaap kan verstoren. Door een structureel slaaptekort kunnen mensen zich slechter concentreren en minder goed presteren. Ook kunnen gezondheidsproblemen ontstaan. Vervolgonderzoek is nodig om te bepalen of beschikbare (ingebouwde) blauwlichtfilters op apparaten de effecten op slaap kunnen verminderen.

12. Sexual, addiction and mental health care needs among men who have sex with men practicing chemsex - a cross-sectional study in the Netherlands

Abstract

Drug use during sex ('chemsex') has been associated with sexually transmitted infections (STIs) and mental health harms. Little quantitative evidence exists on the health care needs of MSM practicing chemsex from a patient perspective. This study assessed self-perceived benefits and harms and the needs for professional counselling among MSM practicing chemsex. In 2018, 785 MSM were recruited at nine Dutch STI clinics and 511 (65%) completed the online questionnaire. Chemsex was defined as using cocaine, crystal meth, designer drugs, GHB/GBL, ketamine, speed and/or XTC/MDMA during sex <6 months. Chemsex was reported by 41% (209/511), of whom 23% (48/209) reported a need for professional counselling. The most reported topic to discuss was increasing self-control (52%, 25/48). Most MSM preferred to be counselled by sexual health experts (56%, 27/48). The need for professional counselling was higher among MSM who engaged in chemsex ≥2 times per month (30% vs. 17%, p = 0.03), did not have sex without drugs (sober sex) in the past three months (41% vs. 20%, p = 0.04), experienced disadvantages of chemsex (28% vs. 15%, p = 0.03), had a negative change in their lives due to chemsex (53% vs. 21%, p = 0.002), and/or had an intention to change chemsex behaviours (45% vs. 18%, p < 0.001). Our study shows that almost one in four MSM practicing chemsex expressed a need for professional counselling on chemsex-related issues. STI healthcare providers should assess the need for professional counselling in MSM practicing chemsex, especially in MSM with above mentioned characteristics, such as frequent users.

Abstract

Measurements of some psychological variables were obtained from 27 patients with a genital herpes infection and compared with those from 12 patients with a gonorrhea infection. The measurements referred to the period before and during the infection. Evidence was found that during the infection patients were more anxious, more sexually inhibited, more bitter towards their partners, and had more psychological complaints than before the infection. Generally there was no difference between the two groups of patients, with one exception: patients with genital herpes judged themselves as having fewer psychological complaints prior to the disease than did patients with gonorrhea.

Abstract

Background Asymptomatic bacteriuria is frequent in kidney transplant recipients (KTRs). However, there is no consensus on diagnosis or management. We conducted a European survey to explore current practice related to the diagnosis and management of asymptomatic bacteriuria in adult KTRs. Methods A panel of experts from the European Renal Association-European Dialysis Transplant Association/Developing Education Science and Care for Renal Transplantation in European States working group and the European Study Group for Infections in Compromised Hosts of the European Society of Clinical Microbiology and Infectious Diseases designed this cross-sectional, questionnaire-based, self-administered survey. Invitations to participate were e-mailed to European physicians involved in the care of KTRs. Results Two hundred and forty-four participants from 138 institutions in 25 countries answered the survey (response rate 30%). Most participants [72% (176/244)] said they always screen for asymptomatic bacteriuria in KTRs. Six per cent (15/240) reported never treating asymptomatic bacteriuria with antibiotics. When antimicrobial treatment was used, 24% of the participants (53/224) said they would start with empirical antibiotics. For an episode of asymptomatic bacteriuria caused by a fully susceptible microorganism and despite no contraindications, a majority of participants (121/223) said they would use a fluoroquinolone (n = 56), amoxicillin/clavulanic acid (n = 38) or oral cephalosporins (n = 27). Conclusions Screening for and treating asymptomatic bacteriuria are common in KTRs despite uncertainties around the benefits and harms. In an era of antimicrobial resistance, further studies are needed to address the diagnosis and management of asymptomatic bacteriuria in these patients.

RIVM rapport:Veel Nederlanders gebruiken lichtgevende schermen in de avond: het gebruik is het hoogst onder adolescenten (13-18 jaar) en volwassenen. Vaak neemt het gebruik in de avond ook aanzienlijke tijd in beslag (meer dan twee uur). Dit onderzoek bevestigt eerdere bevindingen dat frequent of langdurig schermgebruik in de avond samenhangt met verstoorde slaap. Bewustwording van het gebruik is dan ook belangrijk, voornamelijk wanneer een computer, smartphone of tablet in het uur voor het slapen gaan wordt gebruikt. Dit blijkt uit onderzoek van het RIVM, in samenwerking met het Amsterdam Medisch Centrum, het Nederlands Herseninstituut en Lifelines. Hierin is voor het eerst in Nederland het schermgebruik van kinderen en adolescenten (8-18 jaar) in de avond uitgebreid in kaart gebracht in samenhang met slaap. Het blijkt dat de groep die dagelijks of langdurig gebruikmaakt van een of meerdere schermen (computer, smartphone en/of tablet) tot 40 minuten korter slaapt dan de groep die dit niet, of korter, doet. De kortere slaapduur komt voornamelijk doordat deze kinderen en adolescenten later gaan slapen. Van de kinderen (8-13 jaar) gebruikt 22% dagelijks een scherm in de avond. Onder adolescenten (13-18 jaar) is dit 83%. Adolescenten die dagelijks een scherm gebruiken in het uur voor het slapen, hebben meer slaapklachten, zoals later in slaap vallen, korter slapen en 's nachts wakker worden. Ook vermelden zij meer symptomen van slaaptekort overdag, zoals moeite om wakker te blijven. Deze klachten verminderden bij adolescenten die als experiment een week lang geen scherm gebruikten of tijdens het schermgebruik een oranje bril droegen die het blauwe licht blokkeert. In de afgelopen jaren zijn steeds meer lichtgevende schermen ontwikkeld: niet alleen tv's maar ook computers, laptops, tablets en smartphones. De recent ontwikkelde schermen zenden meer blauw licht uit dan de traditionele bronnen, omdat zij gebruikmaken van led-technologie. Bekend is dat blauw licht invloed heeft op onze, Use of light-emitting screens during the evening is very common among Dutch adults (=18 years), adolescents (13-18 years) and children (8-13 years). This use frequently occurs during a long period of time during the evening (over 2 hours), in particular by adolescents. The current study further shows, in line with previous research, that frequent or long term use of light-emitting screens during the evening is associated with disturbed sleep. More awareness is needed, in particular regarding use of computers, smartphones and/or tablet from one hour until going to sleep. This is evident from research conducted by RIVM in collaboration with Amsterdam Medical Centre, Netherlands Institute for Neuroscience, and Lifelines. In this study, use of light-emitting screens among Dutch children and adolescents was elaborately investigated. Results show that children and adolescents who daily or on average for more than 2 hours per evening use an light-emitting screen (computer, smartphone, and/or tablet) sleep up to 40 minutes shorter compared to children who do not daily use an screen or for a shorter duration. Among children 22% uses a screen daily, whereas this is 83% among adolescents. Adolescents who use an light-emitting screen daily in the hour before going to sleep, have more sleep complaints such as: a longer latency to fall asleep, shorter sleep duration and more frequent waking up during the night. They also have more symptoms of sleep deprivation during the day, such as having trouble staying awake. These complaints were reduced among adolescents who, during an experimental study, were not using screens for one week during the evening, or where using orange tinted glasses that block blue light. Abstract

We present the updated International Union against Sexually Transmitted Infections guideline for the management of non-gonococcal urethritis in men. This guideline recommends confirmation of urethritis in symptomatic men before starting treatment. It does not recommend testing asymptomatic men for the presence of urethritis. All men with urethritis should be tested for Chlamydia trachomatis and Neisseria gonorrhoeae and ideally M. genitalium using a NAAT as this is highly likely to improve clinical outcomes. If a NAAT is positive for gonorrhoea, a culture should be performed before treatment. In view of the increasing evidence that azithromycin 1 g may result in the development of antimicrobial resistance in Mycoplasma genitalium azithromycin 1 g is no longer recommended as first line therapy, which should be doxycycline 100 mg bd for 7 days. If azithromycin is to be prescribed an extended of 500 mg, then 250 mg daily for 4 days is to be preferred over 1 g stat. In men with persistent NGU, M. genitalium NAAT testing is recommended if not previously undertaken, as is Trichomonas vaginalis NAAT testing in populations where T. vaginalis is detectable in >2% of symptomatic women.

Abstract

OBJECTIVE: To describe the results of HIV-surveillance activities in the Netherlands between 1987 and 2001. DESIGN: Descriptive. METHOD: Data were obtained from HIV-surveillance at STI-clinics, laboratory-surveillance in the region Arnhem, surveillance among injecting drug users, the AIDS-notification, STI-registration and the Amsterdam cohort studies on HIV/AIDS. RESULTS: In the Netherlands, the highest HIV-prevalences were found among injecting drug users (1-26%) and homo- and bisexual men (0-17%). In these high-risk populations, an increase in HIV-prevalence and--incidence, respectively, was found among injecting drug users in Heerlen and homosexual men (> 35 years of age) in Amsterdam. The HIV-prevalence was lower among heterosexuals in the Netherlands (0-2%). However, in certain local populations an increase was seen. In both Amsterdam and Rotterdam, the HIV-prevalence was higher in individuals tested anonymously than in those tested by name. CONCLUSION: Local increases in HIV-infections have been observed recently, in both high- and medium-risk populations

Abstract

Objective: Study the reproducibility of wet smear interpretation of clue cells, lactobacillary grades and leukocyte dominance with conventional bright light and phase contrast microscopy. Study design: Sets of vaginal specimens were taken from unselected consecutive women attending an outpatient gynaecology clinic. Air-dried vaginal fluid on a microscope slide was rehydrated with isotonic saline before examination by six independent international investigators. Some investigators initially used a conventional bright light microscope, followed by phase contrast technique. Results: Using phase contrast microscopy, an excellent inter-observer agreement was obtained among all investigators for clue cells detection (Kappa values from 0.69 to 0.94) and lactobacillary grades (Kappa 0.73-0.93). When conventional light microscopes were used, poor agreement was obtained for these criteria (Kappa index 0.37-0.72 and 0.80, respectively), but switching to phase contrast microscopy by the same investigators, improved Kappa to 0.83-0.85 and 0.88, respectively. The inter-observer agreement for estimation of the leukocyte/epithelial cell ratio (Kappa index 0.17-0.67) was poor, irrespective of the type of microscopy applied. Intra-observer agreement of clue cell detection and lactobacillary grading was also found to be excellent if phase contrast microscopy was used (Kappa 0.87-0.93), and poor with conventional bright light microscopy (Kappa 0.45-0.66). Conclusion: Clue cells and the lactobacillary grades are reliably identified by phase contrast microscopy in wet smears, with excellent intra- and inter-observer reproducibility agreement, and better than when simple bright light microscopy was used. Evaluation of leukocyte grading, on the other hand, was inconsistent among the different microscopists, irrespective of the type of microscope used. We propose to grade the leukocytes in a different way than searching for leukocyte dominance over epithelial cells, namely by counting them
