Abstract

Dans un monde qui se percoit en croissance infinie, peut-on aborder la question des limites, plus particulierement des limites therapeutiques ? Qu’elles soient d’origine corporelle, psychique, institutionnelle ou existentielle, les limites auxquelles nous sommes confrontees sont incontournables. Mais elles ne sont pas pacifiees. Elles restent de l’ordre du conflit et c’est sur ce champ de bataille qu’il faut s’aventurer. Le medecin semble le mieux place pour construire un rapport aux limites. Mais sa formation, les orientations de la medecine, le fonctionnement institutionnel avec l’objectif d’accroissement de la quantite de la vie par la production d’actes medicoeconomiques viennent mettre a mal l’evidence de cette fonction. Dans ce contexte, l’enjeu est plutot de construire une responsabilite collective avec la determination de differents niveaux de responsabilite: 1) un niveau medical, s’appuyant sur des reperes cliniques et une sagesse pratique; 2) un niveau collectif avec l’instauration d’une deliberation pluridisciplinaire comprenant le patient, son entourage et l’equipe soignante; 3) un niveau disciplinaire avec la formulation de recommandations nationales permettant de baliser collectivement les prises de decision; 4) un niveau societal et politique avec une mediatisation de choix politiques clairs et coherents en assumant publiquement les orientations prises et le rapport a la limite. En conclusion, le medecin peut construire un rapport aux limites en exercant trois fonctions. La premiere est de l’ordre de l’« accompagnement ». La deuxieme fonction est de l’ordre de la « mediation ». La troisieme fonction est plus collective. Il s’agit d’etre un des createurs d’une culture de service, d’un « ethos » contenant partiellement la destructuration et l’angoisse du patient. Ce positionnement langagier ouvre, sans pretendre le combler, le questionnement sur le sens ou non-sens de la condition humaine mise a mal par cette confrontation ineluctable aux diverses limites.

Abstract

The coexistence of dual hematological neoplasms is very rare. Sequential or synchronous neoplasms in hematology are an uncommon and complex clinical situation. The aim of the Hemo2 study was to describe the clinical characteristics and analyze the outcome of these patients. We performed a retrospective review of all patients diagnosed with sequential or synchronous hematological malignancies in the university hospital of Tours, between 2007 and 2018. We identified 49 patients in our study, with a prevalence of 0.89%. Sequential and synchronous combinations were found in 36 (73%) and 13 (27%) patients, respectively. One patient presented three sequential neoplasms. The median cumulative incidence was 6 years (95% CI 3-7). Among all neoplasms diagnosed (n = 99), we found 79 lymphoid neoplasms (LNs) (80%) and 20 myeloid neoplasms (MNs) (20%). Sex ratio was 1.88 with 65% of males and 35% of females. The most common LNs were Hodgkin lymphoma (n = 16; 16%) and multiple myeloma (n = 11; 11%). The most frequent MN was essential thrombocythemia (n = 5; 5%). The most common combination was Hodgkin lymphoma and follicular lymphoma in five (10%) patients. The overall survival from the first diagnosis (OS1) at 5 years was 82.4% (95% CI 72.1-94.3). The median overall survival from the second diagnosis (OS2) was 98 months (95% CI 44-NR) and 5-year OS2 was 58.7% (95% CI 45.5-75.7). Median progression-free survival from the second diagnosis (PFS) was 47 months (95% CI 27-NR) with 5-year PFS of 49% (95% CI 35.9-67). OS and PFS did not statistically differ between synchronous and sequential dual neoplasms. In this cohort, that the death relative risk (RR) was significantly lower if the second neoplasm appeared after more than 4 years following the first diagnosis (OR 0.37 (95% CI 0.16-0.90)). The Hemo2study confirmed the rarity of dual hematological neoplasms. In this cohort, HL and FL were the most frequent combinations. Our results may support that synchronous and sequential dual neoplasms bear the same prognosis. Further studies are needed to better characterize these uncommon clinical situations. Abstract

Background. Bispecific antibodies (BsAb) are a new effective treatment for relapsed/refractory multiple myeloma (RRMM). Although these immunotherapies are well tolerated, infectious complications emerged as a predominant issue, mainly with anti-BCMA BsAb. Recent guidelines were presented to prevent and manage these complications based on clinical trials safety signals. In this study, we aimed to report incidence and characteristics of infectious complications in patients treated with anti-BCMA and anti-GPRC5D BsAb.

Abstract

BACKGROUND. Donor cell engraftment is a prerequisite of successful allogeneic hematopoietic stem cell transplantation. Based on peripheral blood analyses, it is characterized by early myeloid recovery and T and B cell lymphopenia. However, cellular networks associated with bone marrow engraftment of allogeneic human cells have been poorly described. METHODS. Mass cytometry and CITE-Seq analyses were performed on bone marrow cells 3 months after transplantation in patients with acute myelogenous leukemia. RESULTS. Mass cytometric analyses in 26 patients and 20 healthy controls disclosed profound alterations in myeloid and B cell progenitors, with a shift toward terminal myeloid differentiation and decreased B cell progenitors. Unsupervised analysis separated recipients into 2 groups, one of them being driven by previous graft-versus-host disease (R2 patients). We then used single-cell CITE-Seq to decipher engraftment, which resolved 36 clusters, encompassing all bone marrow cellular components. Hematopoiesis in transplant recipients was sustained by committed myeloid and erythroid progenitors in a setting of monocyte-, NK cell-, and T cell-mediated inflammation. Gene expression revealed major pathways in transplant recipients, namely, TNF-α signaling via NF-κB and the IFN-γ response. The hallmark of allograft rejection was consistently found in clusters from transplant recipients, especially in R2 recipients. CONCLUSION. Bone marrow cell engraftment of allogeneic donor cells is characterized by a state of emergency hematopoiesis in the setting of an allogeneic response driving inflammation. FUNDING.
